Roaring Trains and Ringing Bells: A Stylistic Analysis of Soundscape in Charles Dickens's Dombey and Son

In my essay I investigate Charles Dickens's innovative use of language combined with a specific reference to the acoustic environment, which characterizes most of his works and defines his pervasive style. Rhythms, alliterations, the use of anaphora and onomatopoeia, and the variations of register in Dombey and Son constitute the most visible and 'audible' examples of the relationship between language and soundscape, as I explain in the first part of my work. Later, I explore how, in Dombey and Son, Dickens surprises and displaces the reader by using internal deviation and omitting the normally expected clues of context and coherence. I analyse these linguistic and stylistic aspects in the chapters that refer to the railway boom and the sounds of trains and ringing bells that accompany the troubled misadventures of one of the novel's main characters

The synchronous occurrence of squamous cell carcinoma and gastrointestinal stromal tumor (GIST) at esophageal site

<p>Abstract</p> <p>Background</p> <p>Esophageal squamous cell carcinoma is a relative common malignancy with a very poor prognosis, even adopting an integrated and multidisciplinary approach. According to the literature, gastrointestinal stromal tumors (GISTs) rarely originate from the esophagus. Moreover there are not reports of synchronous occurrence of squamous cell carcinoma and GIST at esophageal site.</p> <p>Case presentation</p> <p>We describe a case of a 74 year old patient who underwent surgery for squamous cell carcinoma of the lower third of the esophagus with an incidental pathologic diagnosis of a concomitant GIST in the thoracic tract.</p> <p>Conclusion</p> <p>In literature there is no evidence of concomitant squamous carcinoma and GIST of the thoracic esophagus, even if esophageal GISTs are sometimes described. The occasional finding of this neoplastic lesion underlines the importance of a carefully pathological diagnosis for its identification. Surgery, followed by a multidisciplinary approach remains the first-line treatment in both squamous and stromal neoplasm.</p

A rare case of endometriosis in Turner's syndrome

Endometriosis is defined by the presence of functional endometrial tissue outside the uterine cavity and musculature. It has a prevalence rate as high as 35e50% in women experiencing pain or infertility [1] and sometimes has a peculiar and rare onset [2,3]. Endometriosis is a common disease in menstruating women [4,5] but has also been reported in postmenopausal or surgically castrated women on hormone replacement therapy (HRT) [6]. The common denominator of all such cases is exposure to female hormones [7]. However, endometriosis, a common and important clinical problem in women of reproductive age, has rarely been described in prepubertal girls. In patients with Turner's syndrome or other ovarian dysgenesis, endometriosis is very rare. Some cases are subclinical, and endometriosis is an incidental finding during routine examination. Although endometriosis was described in the medical literature at the end of the 19th century, and the first theories regarding its hystogenesis were developed at the beginning of the 20th century, the real pathogenesis of endometriosis remains unknown. Most studies about the etiology of endometriosis claim that the main possible causes of endometriosis are probably multifactorial. Three theories of histogenesis have been proposed. (1) The metastatic theory [8] proposes the transplantation of endometrial tissue via retrograde menstrual implantation, vascular/lymphatic spread, and intraoperative implantation. (2) The coelomic metaplastic theory [9] suggests that the germinal epithelium of the ovary can be transformed by metaplasia into endometrium.
