Infective endocarditis caused by Gemella morbillorum is a very rare occurrence and only a few cases have been reported in the literature so far. We describe a case of G. morbillorum endocarditis in a 31-year-old Pakistani woman who had a congenitally bicuspid aortic valve. She had also undergone repair of a large aortic aneurysm 1 year before this presentation. She presented to our institution with a 1.5-month history of fever, exertional dyspnea and generalized edema. A transthoracic echocardiogram showed two large, echogenic and mobile vegetations (7 x 15 mm and 8 x 10 mm) attached to the right coronary cusp and non-coronary cusp. Blood cultures of the Patient grew pan-sensitive G. morbillorum. The Patient fulfilled the Duke\u27s criteria for infective endocarditis. She was successfully treated with ceftriaxone and gentamycin therapy. At two years\u27 follow up she has remained well. This case represents a rare cause of infectious endocarditis that was successfully managed with medical treatment alone in a Patient with the predisposing risk factor of a bicuspid aortic valve

Business Model Canvas to Create and Capture AI-enabled Public Value

    Get PDF
The compatibility between the business model and AI-enabled value creation is paramount for the sustainability of organizations. The public sector lags the private sector in the race to AI readiness and adoption. Although the concept of the business model for the public sector has previously been discussed, we found a lack of evidence for the process of adaption of the business model as a value creation and capture tool from commercial motives to public value motives. This paper adapts the conventional business model canvas for the public sector as it pertains to the design and development of AI systems. Employing a design-science research approach, we postulate five design principles that public agencies must follow to design and deploy AI-enabled public services

    Get PDF
This quantitative study aims to assess the degree to which self-efficacy (the sense of their abilities by students) enables students to overcome their academic stress. A sample of 174 men and women university students (age range of 18-23 and 23-29) of the Islamia University of Bahawalpur participated in the research; data was obtained by convenience sampling technique. The mode of collecting data was online i.e. via emails and social apps like WhatsApp etc. Demographic information such as age, name, gender, socioeconomic status, family system and education was taken, besides, The General Self-Efficacy scale (GSE) (Schwarzer & Jerusalem, 1995) and Opinions of Scholastic Pressure (PAS) scale (Bedewy & Gabriel) were utilized. Study findings suggest that there is a negative correlation between self-efficacy and academic stress which means, if an individual's self-efficacy declines, academic stress will increase and vice versa. The results have also shown that Self-efficacy is a strong predictor of academic stress. Implications were given for future studies on the role of self-efficacy in helping students overcome their stress and improve their academic and other fields of life success

Mostly cases underwent amputation on elective basis. These cases were admitted in the ward. All necessary investigations were carried out and they were evaluated for anesthesia fitness. On emergency basis operation was done in emergency department and elective amputations were done in main operation theater. A written consent was taken from all patients and also from the medical superintendant of the hospital for conducting this study. A proforma was designed for documenting presenting complaints, cause of amputation, total hospital stay, Outcome and complications after operation. After discharge from the ward these cases were called for follow-up and stump was examined for necrosis, infection or hematoma etc. All data was analyzed using Microsoft office and SPSS version 2007. Results were calculated in the form of frequencies and expressed via tables and graphs. Results: Out of 75 cases 28(37.3%) cases underwent amputation of lower limbs on emergency basis and 47() cases were operated on elective basis. Ages of these cases was ranging from 30-75 years with mean age of 45.7 years. Mostly cases were above 40 years of age. There were 11(14.7%) cases with age range of 30-40 years, 17 with 41-50 years, 28 between 51-60 years, 7 cases between 61-70 years and 12 cases with age above 70 years. There were both male and female populations in this study group. There were 60(80%) males and 15(20%) female patients. It was seen that in most of the cases right limb was involved more than left limb. In 72% cases right lower limb and in 28% cases left lower limb amputation was done. There were different causes of amputation such as in 32 cases diabetes was cause, in 18 cases trauma, in 20 cases acute vascular insufficiency and in 5 cases infection was cause of lower limb amputation. Leading cause was diabetic foot. Most common complication seen after operation was wound infection seen in 23 cases, wound hematoma in 6 cases and stump necrosis in 3 cases was found. Out of 75 pos operative cases 50 were discharged, 15 were referred, 7 cases left against medical advice and 3 cases died due to septicemia and multi organ failure.    Conclusion: Lower limb amputation is most commonly performed amputation.
