    Exceptional response to immunotherapy in association with radiotherapy in patient with breast metastasis from urothelial carcinoma: A case report

    Most common sites of metastasis of urothelial carcinoma (UC) are lungs, liver, lymph nodes and bone. Pembrolizumab, a humanized monoclonal antibody directed against programmed cell death protein-1 (PD-1), represents an effective second-line therapy for advanced UC. Radiotherapy has been shown to induce a mechanism of immunogenic cell death (ICD) resulting in immune memory and advantageous systemic effects. We present the first case of breast metastasis (BM) from a UC described in literature who had an exceptional response to second-line therapy with pembrolizumab in association with radiotherapy, showing the efficacy of combining immunotherapy and radiotherapy even in patients with atypical metastatic sites

    Crenças e atitudes em relação à fluorose dentária de jovens em região rural do Brasil

    OBJECTIVE: To understand beliefs and attitudes about fluorosis among young people living in a rural area. METHODOLOGICAL PROCEDURES: Qualitative study consisting of semi-structured interviews with 23 adolescents with dental fluorosis, 14 teachers and three health authorities in the city of São Francisco, Southeastern Brazil, in 2002. Content analysis and social representation theory were applied. ANALYSIS OF RESULTS: The organoleptic characteristics of carbonates that affect groundwater (salty flavor, whitish coloration, and turbidity) associated with negative aspects of household use of this water are considered a cause of mottled enamel. Even after contact with researchers who investigated this phenomenon and helped find a solution for this condition, the local population is still unwilling to accept fluoride as the cause of the problem and does not fully agree to use water from other sources because they are afraid of the quality of water. CONCLUSIONS: Misperceptions of the causes of dental fluorosis and water treatment costs compromise the implementation of uncontaminated surface water supplies. Health education strategies are required in parallel with solutions for securing water supply in drought-ravaged areas.OBJETIVO: Comprender creencias y actitudes con relación a la fluorosis dentaria de jóvenes residentes en región rural de Brasil. MÉTODOS: Estudio cualitativo en que fueron realizadas entrevistas semi-estructuradas con 23 adolescentes con fluorosis dentaria, 14 profesores y tres gestores públicos en el municipio de Sao Francisco, en Sureste de Brasil, en 2002. Fueron empleadas la técnica de análisis de contenido y la teoría de las representaciones sociales. RESULTADOS: Por las características organolépticas que imprime al agua (sabor salado, coloración blanquecina y turbidez) y su asociación con aspectos negativos relacionados al uso doméstico, el carbonato es considerado la causa de la fluorosis por adolescentes y profesores. Aún posterior al contacto con científicos que investigaron el fenómeno y que participaron de su solución, la población continúa resistiendo en aceptar el flúor como causa del problema y no concuerda plenamente con el uso de agua de otras fuentes por dudar de su calidad. CONCLUSIONES: Percepciones equivocadas con relación a las causas de la fluorosis dentaria y la dificultad en costear el tratamiento del agua comprometen la implantación del suplemento de agua de superficie no contaminada.     Characterizing HR3549B using SPHERE

    Aims. In this work, we characterize the low mass companion of the A0 field star HR3549. Methods. We observed HR3549AB in imaging mode with the the NIR branch (IFS and IRDIS) of SPHERE@VLT, with IFS in YJ mode and IRDIS in the H band. We also acquired a medium resolution spectrum with the IRDIS long slit spectroscopy mode. The data were reduced using the dedicated SPHERE GTO pipeline, purposely designed for this instrument. We employed algorithms such as PCA and TLOCI to reduce the speckle noise. Results. The companion was clearly visible both with IRDIS and IFS.We obtained photometry in four different bands as well as the astrometric position for the companion. Based on our astrometry, we confirm that it is a bound object and put constraints on its orbit. Although several uncertainties are still present, we estimate an age of ~100-150 Myr for this system, yielding a most probable mass for the companion of 40-50MJup and T_eff ~300-2400 K. Comparing with template spectra points to a spectral type between M9 and L0 for the companion, commensurate with its position on the color-magnitude diagram.Comment: Accepted by A&A, 13 pages, 10 Figures (Figures 9 and 10 degraded to reduce the dimension

    Multi-band high resolution spectroscopy rules out the hot Jupiter BD+20 1790b - First data from the GIARPS Commissioning

    Context. Stellar activity is currently challenging the detection of young planets via the radial velocity (RV) technique. Aims. We attempt to definitively discriminate the nature of the RV variations for the young active K5 star BD+20 1790, for which visible (VIS) RV measurements show divergent results on the existence of a substellar companion. Methods. We compare VIS data with high precision RVs in the near infrared (NIR) range by using the GIANO - B and IGRINS spectrographs. In addition, we present for the first time simultaneous VIS-NIR observations obtained with GIARPS (GIANO - B and HARPS - N) at Telescopio Nazionale Galileo (TNG). Orbital RVs are achromatic, so the RV amplitude does not change at different wavelengths, while stellar activity induces wavelength-dependent RV variations, which are significantly reduced in the NIR range with respect to the VIS. Results. The NIR radial velocity measurements from GIANO - B and IGRINS show an average amplitude of about one quarter with respect to previously published VIS data, as expected when the RV jitter is due to stellar activity. Coeval multi-band photometry surprisingly shows larger amplitudes in the NIR range, explainable with a mixture of cool and hot spots in the same active region. Conclusions. In this work, the claimed massive planet around BD+20 1790 is ruled out by our data. We exploited the crucial role of multi- wavelength spectroscopy when observing young active stars: thanks to facilities like GIARPS that provide simultaneous observations, this method can reach its maximum potential.Comment: 12 pages, 7 figure

    The Integral Field Spectrograph of SPHERE: the Planet Finder for VLT

    SPHERE is an instrument designed and built by a consortium of French, German, Italian, Swiss and Dutch institutes in collaboration with ESO. The project is currently in its Phase B. The main goal of SPHERE is to gain at least one order of magnitude with respect to the present VLT AO facility (NACO) in the direct detection of faint objects very close to a bright star, especially giant extrasolar planets. Apart from a high Strehl ratio, the instrument will be designed to reduce the scattered light of the central bright star and subtract the residual speckle halo. Sophisticated post-AO capabilities are needed to provide maximum detectivity and possibly physical data on the putative planets. The Integral Field Spectrograph (IFS), one of the three scientific channels foreseen in the SPHERE design, is a very low resolution spectrograph (R~20) which works in the near IR (0.95-1.35 micron), an ideal wavelength range for the ground based detection of planetary features. Its goal is to suppress speckle to a contrast of 10^7, with a goal of 10^8, and at the same time provide spectral information in a field of view of about 1.5 x 1.5 arcsecs^2 in proximity of the target star.     Expected performance of the ASTRI-SST-2M telescope prototype

    ASTRI (Astrofisica con Specchi a Tecnologia Replicante Italiana) is an Italian flagship project pursued by INAF (Istituto Nazionale di Astrofisica) strictly linked to the development of the Cherenkov Telescope Array, CTA. Primary goal of the ASTRI program is the design and production of an end-to-end prototype of a Small Size Telescope for the CTA sub-array devoted to the highest gamma-ray energy region. The prototype, named ASTRI SST-2M, will be tested on field in Italy during 2014. This telescope will be the first Cherenkov telescope adopting the double reflection layout in a Schwarzschild-Couder configuration with a tessellated primary mirror and a monolithic secondary mirror. The collected light will be focused on a compact and light-weight camera based on silicon photo-multipliers covering a 9.6 deg full field of view. Detailed Monte Carlo simulations have been performed to estimate the performance of the planned telescope.
