Land Consumption, Ecosystem Services and Urban Planning Policies: Preliminary Results of Research Undertaken in the Tuscany Region
In the contemporaneity, the issues of land or soil consumption and of the protection of areas that, within the urban areas, provide ecosystem services (ESs) is becoming increasingly important also in relation of the 2030 Sustainable Development Goals. The concept of "Ecosystem Service" appears, in this respect, a fruitful support to define the land consumption effects on the loss of functionality and of settlement quality. Following this considerations the paper presents the first results of a research developed in Tuscany and commissioned by the Regional Government. The research aims to measure the loss of ESs in connection with land use/land cover transformations, and to verify the contribution of soil consumption to these variations. The research use methodologies for elaborating of the geographical data required for territorial governance, LUCL 2010/2016 and Land Cover Flow (LCF) model, and the theoretical model of the “Capacity matrix” to provide ecosystem services
A challenging case of fever of unknown origin (FUO)
Introduction. Fever of unknown origin (FUO) is challenging for physicians as there are more than 200 differential diagnosis of FUO. The diagnosis often requires numerous non-invasive and invasive procedures and sometimes the etiology remains unknown. Clinical case. Our patient is a 24-year-old Caucasian female under contraceptive vaginal ring treatment who presented the first time to the emergency department with fever (38,5°C) and stomach ache. Haematological parameters showed thrombocytosis (728000/mmc) and anemia (10,2 g/dl). Ultrasonography of the abdomen revealed a 4 cm hepatic angioma. She was diagnosed a gastropathy and treated with PPI. Then she was studied deeper because of elevated inflammation markers, persistent thrombocytosis and low-grade fewer combined with fatigue and stomach ache. Despite multiple blood and urine cultures, serology, autoimmune serology, bone marrow biopsy, echocardiography, PET/CT, total body CT, brain MRI scan, transvaginal sonography and gynecological examination no clinical focus could be identified. Esophagogastroduodenoscopy was negative but the histological examination was suggestive of mild celiac disease. Thus, this diagnosis was unlikely because serologic tests and haplotypes DQ2 and DQ8 were negative. Abdomen RMI was suggestive of a 4 cm hepatic adenoma. After surgical resection of the adenoma platelet count normalized and fever disappeared. Conclusions. Hepatic adenoma is an uncommon liver tumor associated with use of oral contraceptive and can be a rare cause of FUO. MRI scan can be a useful tool to detect this tumor
Quality of life measures in Italian children with atopic dermatitis and their families
<p>Abstract</p> <p>Background</p> <p>The impact of atopic dermatitis (AD) on children's quality of life (QoL) in US and European countries is relatively well known, though rarely evaluated in the Italian population. Moreover, the association between child age and QoL has not been enough investigated, even though few studies detected a worse QoL in youngest AD children. The aim of the study was to evaluate the QoL in an Italian sample of atopic children and their families, also exploring a possible association with child age.</p> <p>Methods</p> <p>60 AD children aged between 1-12 years and their mothers completed specific QoL questionnaires (IDQoL/CDLQI, DFI) and a clinician completed a measure of AD severity (SCORAD).</p> <p>Results</p> <p>AD severity (Objective SCORAD) significantly correlated with QoL measures. Severe AD children showed higher IDQoL/CDLQI and DFI scores compared to mild and moderate AD groups (<it>P </it>= 0.006 and <it>P </it>< 0.0005, respectively), but only DFI scores differed in these last two conditions (<it>P </it>= 0.014). DFI scores negatively correlated with children's age (<it>P </it>= 0.046), but did not differ when considering child age ranges. Multiple linear regression analyses revealed a significant association between Objective SCORAD and QoL measures.</p> <p>Conclusions</p> <p>A strong association between severe AD and poor QoL, both in children and mothers, was found in the Italian sample, in line with the international literature. Family's QoL scores were sensitively related to AD severity, more than the child's QoL, emphasising that the disease has a deep impact on the family. A significant association between age and QoL was only partially found and needs further investigation.</p

Nutraceuticals and Physical Activity as Antidepressants: The Central Role of the Gut Microbiota
Major depressive disorder (MDD) is a common mental illness. Evidence suggests that the gut microbiota play an essential role in regulating brain functions and the pathogenesis of neuropsychiatric diseases, including MDD. There are numerous mechanisms through which the gut microbiota and brain can exchange information in a continuous, bidirectional communication. Current research emphasizes the interexchange of signals influenced by the gut microbiota that are detected and transduced in information from the gut to the nervous system involving neural, endocrine, and inflammatory mechanisms, suggesting a relationship between oxidative stress and the pathophysiology of MDD via the hyperactivation of inflammatory responses. Potential sources of inflammation in the plasma and hippocampus of depressed individuals could stem from increases in intestinal permeability. Some nutraceuticals, such as specific probiotics, namely psychobiotics, polyphenols, carotenoids, butyrate, and prebiotics, have been demonstrated to exert an antidepressant activity, but most of them need to be metabolized and activated by gut microorganisms. By inducing changes in the gut microbiota composition, physical exercise might also exert a role in alleviating depression-like symptoms. The mutual relationships among nutraceuticals, exercise, and depression will be discussed, and the potential role of the gut microbiota as a therapeutic target to treat depression will be explored
Telerehabilitation proposal of mind-body technique for physical and psychological outcomes in patients with fibromyalgia
Fibromyalgia (FM) syndrome is characterized by the close correlation of chronic widespread pain and other non-pain related symptoms. Aim of this study was to investigate whether telerehabilitation that provides physical and psychological support services of the mind-body techniques can affect the clinical profile and pain relief of FM patients. The study included twenty-eight female FM patients, mean aged 56.61 +/- 8.56 years. All patients underwent a rehabilitation treatment (8 sessions, 1/week, 1 h/each) through Zoom platform, with the following principles of rehabilitation treatment: Anchoring to a positive emotion; listen and perceive your own body; conscious breathing; improve interoceptive awareness; relax. All patients then underwent clinical assessment of the physical distress and fear of movement for the Numeric Rating Scale (NRS); the Fatigue Assessment Scale (FAS); the Fear Avoidance Belief Questionnaire (FABQ); with measures of physical and mental disability for the Fibromyalgia Impact Questionnaire (FIQ); the 12-Items Short Form Survey; the Resilience Scale for Adults and the Coping Strategies Questionnaire-Revised. The evaluations were performed at T0 (baseline), T1 (after 8 weeks of treatment), and T2 (after 1 month of follow-up). The main finding was that telerehabilitation reduced physical and mental distress, fear, and disability (p < 0.001). Resilience and coping ability were less affected by the rehabilitative treatment. Our attempt of mind-body technique telerehabilitation has shown good results in the improvement of painful symptoms and quality of life for the FM patients but showed fewer positive impacts for the resilience and coping abilities aspects
REGIONAL MAPPING OF MYOCARDIAL HIBERNATION PHENOTYPE IN IDIOPATHIC END-STAGE DILATED CARDIOMYOPATHY
Myocardial hibernation (MH) is a well-known feature of human ischaemic cardiomyopathy (ICM), whereas its presence in human idiopathic dilated cardiomyopathy (DCM) is still controversial. We investigated the histological and molecular features of MH in left ventricle (LV) regions of failing DCM or ICM hearts. We examined failing hearts from DCM (n = 11; 41.9 ± 5.45 years; left ventricle-ejection fraction (LV-EF), 18 ± 3.16%) and ICM patients (n = 12; 58.08 ± 1.7 years; LVEF, 21.5 ± 6.08%) undergoing cardiac transplantation, and normal donor hearts (N, n = 8). LV inter-ventricular septum (IVS) and antero-lateral free wall (FW) were transmurally (i.e. sub-epicardial, mesocardial and sub-endocardial layers) analysed. LV glycogen content was shown to be increased in both DCM and ICM as compared with N hearts (P < 0.001), with a U-shaped transmural distribution (lower values in mesocardium). Capillary density was homogenously reduced in both DCM and ICM as compared with N (P < 0.05 versus N), with a lower decrease independent of the extent of fibrosis in sub-endocardial and sub-epicardial layers of DCM as compared with ICM. HIF1-α and nestin, recognized ischaemic molecular hallmarks, were similarly expressed in DCM-LV and ICM-LV myocardium. The proteomic profile was overlapping by ~50% in DCM and ICM groups. Morphological and molecular features of MH were detected in end-stage ICM as well as in end-stage DCM LV, despite epicardial coronary artery patency and lower fibrosis in DCM hearts. Unravelling the presence of MH in the absence of coronary stenosis may be helpful to design a novel approach in the clinical management of DCM
On an accidental degeneracy in the 31P{¹H} NMR chemical shifts in ruthenium diphosphine complexes
The [RuCl(bipy)(dppb)(4-pic)]PF6 complex was prepared and fully characterized. The X-ray crystal structure of this complex was determined in order to make an unambiguous distinction between the two possible positions of the 4-methylpyridine ligand (4-pic) in the compound: trans to phosphorus atom or trans to nitrogen atom. The [RuCl(bipy)(dppb)(4-pic)]PF6 complex exhibits an unusual temperature-dependent accidental degeneracy of the 31P chemical shifts in its solution NMR spectrum
