    Transcatheter closure of a fenestration in intracardiac Fontan circulation

    We present a case of tricuspid atresia and the treatment that was used in a child now aged 12. The malformation was corrected at the age of five through intracardiac Fontan circulation. Later, the child developed a right-to-left shunt due to dehiscence in the intra-atrial conduit, causing significant desaturation. This was treated by transcatheter closure with a 10 mm Amplatzer device at 8 years of age. Currently the child is asymptomatic with no residual shunts, and the position of the device is normal. The percutaneous closure of Fontan fenestrations using the Amplatzer device is a therapeutic approach that provides good results. It results in normal oxygen saturation and has low morbidity. The technique is simple, available to most cardiac catheterization laboratories, safe, and offers a reasonable cost-benefit ratio. In addition, the design of the device is suitable for this type of defect

    Antioxidant and physicochemical properties of Chrysophyllum albidum fruit at different ripening stages

    Chrysophyllum albidum (African Star Apple) fruit is an indigenous African fruit. The African Star Apple plant popularly referred to as the “alasa” tree grows widely in tropical Africa. Ripening influences biochemical processes and subsequently affects the nutritional and bioactive characteristics of the fruits. This study sampled Chrysophyllum albidum fruit of three maturity stages and determined the colour, proximate, total phenol and antioxidant characteristics of the fruits. Spectrophotometric methods were used in the determination of total antioxidant activity using 1,1-diphenyl- 2-picrylhydrazyl (DPPH) radical scavenging activity, 2,2-azinobis,3- ethylbenzothiazoline-6-sulfonic acid (ABTS) and Ferric reducing ability of plasma (FRAP) assay. Significant differences (p<0.05) were observed in the colour of the fruit across all the ripening stages. The ripe stage of the fruit recorded the highest extent of lightness among all the other ripening stages. Both the ripe and over-ripe fruits appeared reddish in colour with the ripe fruit demonstrating a higher red intensity than the over-ripe fruit. The calculated metric chroma of the Chrysophyllum albidum fruits were in the range of 2.33-21.67 for the three ripening stages. Hue angle values recorded were in the range 46.46-92.00. Browning and colour indices for fruits of all the stages of development were of range 6.40-80.30 and -14.30-16.96, respectively. Proximate analysis of the three fruit categories showed that the unripe fruits had higher carbohydrate (69.27 %), crude protein (8.17 %), and crude fat (7.99 %) content relative to fruits which were ripe and over-ripe. The over-ripe fruit had higher crude fibre (7.36 %) and ash (3.86 %) content. Total phenolic content for the unripe fruit (20μg GAE/mg) was also higher than fruits of the other ripening stages. The antioxidant ability of the Chrysophyllum albidum fruit showed that the unripe stage recorded the highest DPPH (29.24 %) and ABTS (99.09 %) radical scavenging activity. The ferric reducing antioxidant potential of the fruit at different ripening stages was significantly different (p<0.05) with the unripe stage recording the highest potential. The unripe fruits demonstrated higher proximate and antioxidant composition than the ripe and over-ripe fruits. Extracts from the unripe fruit can serve as useful nutraceuticals in functional food formulations

Based on the Stable Ecotype Model, evolution leads to the divergence of ecologically distinct populations (e.g., with different niches and/or behaviors) of ecologically interchangeable membership. In this study, pyrosequencing was used to provide deep sequence coverage of Synechococcus psaA genes and transcripts over a large number of habitat types in the Mushroom Spring microbial mat. Putative ecological species (putative ecotypes), which were predicted by an evolutionary simulation based on the Stable Ecotype Model (Ecotype Simulation), exhibited distinct distributions relative to temperature-defined positions in the effluent channel and vertical position in the upper 1 mm-thick mat layer. Importantly, in most cases variants predicted to belong to the same putative ecotype formed unique clusters relative to temperature and depth in the mat in canonical correspondence analysis, supporting the hypothesis that while the putative ecotypes are ecologically distinct, the members of each ecotype are ecologically homogeneous. Putative ecotypes responded differently to experimental perturbations of temperature and light, but the genetic variation within each putative ecotype was maintained as the relative abundances of putative ecotypes changed, further indicating that each population responded as a set of ecologically interchangeable individuals. Compared to putative ecotypes that predominate deeper within the mat photic zone, the timing of transcript abundances for selected genes differed for putative ecotypes that predominate in microenvironments closer to upper surface of the mat with spatiotemporal differences in light and O2 concentration. All of these findings are consistent with the hypotheses that Synechococcus species in hot spring mats are sets of ecologically interchangeable individuals that are differently adapted, that these adaptations control their distributions, and that the resulting distributions constrain the activities of the species in space and time

    Agreement and accuracy using the FIGO, ACOG and NICE cardiotocography interpretation guidelines.

    INTRODUCTION: One of the limitations reported with cardiotocography (CTG) is the modest interobserver agreement observed in tracing interpretation. This study compared agreement, reliability and accuracy of CTG interpretation using the FIGO, ACOG and NICE guidelines. MATERIAL AND METHODS: A total of 151 tracings was evaluated by 27 clinicians from three centers where FIGO, ACOG and NICE guidelines were routinely used. Interobserver agreement was evaluated using the proportions of agreement (PA) and reliability with the kappa (k) statistic. The accuracy of tracings classified as "pathological/category III" was assessed for prediction of newborn acidemia. For all measures, 95% confidence intervals (95%CI) were calculated RESULTS: CTG classifications were more distributed with FIGO (9%, 52%, 39%) and NICE (30%, 33%, 37%) than with ACOG (13%, 81%, 6%). The category with the highest agreement was ACOG category II (PA=0.73 95%CI 0.70-76), and the ones with the lowest agreement were ACOG categories I and III. Reliability was significantly higher with FIGO (k=0.37, 95%CI 0.31-0.43), and NICE (k=0.33, 95%CI 0.28-0.39) than with ACOG (k= 0.15, 95%CI 0.10-0.21), however all represent only slight/fair reliability. FIGO and NICE showed a trend towards higher sensitivities in prediction of newborn acidemia (89% and 97% respectively) than ACOG (32%,), but the latter achieved a significantly higher specificity (95%) CONCLUSIONS: With ACOG guidelines there is high agreement in category II, low reliability, low sensitivity and high specificity in prediction of acidemia. With FIGO and NICE guidelines there is higher reliability, a trend towards higher sensitivity, and lower specificity in prediction of acidemia. This article is protected by copyright.     The long-term impact of the MEMA kwa Vijana adolescent sexual and reproductive health intervention: effect of dose and time since intervention exposure.

    BACKGROUND: Despite recent decreases in HIV incidence in many sub-Saharan African countries, there is little evidence that specific behavioural interventions have led to a reduction in HIV among young people. Further and wider-scale decreases in HIV require better understanding of when behaviour change occurs and why. The MEMA kwa Vijana adolescent sexual and reproductive health intervention has been implemented in rural Mwanza, Tanzania since 1999. A long-term evaluation in 2007/8 found that the intervention improved knowledge, attitudes to sex and some reported risk behaviours, but not HIV or HSV2 prevalence. The aim of this paper was to assess the differential impact of the intervention according to gender, age, marital status, number of years of exposure and time since last exposure to the intervention. METHODS: In 2007, a cross-sectional survey was conducted in the 20 trial communities among 13,814 young people (15-30 yrs) who had attended intervention or comparison schools between 1999 and 2002. Outcomes for which the intervention had an impact in 2001 or 2007 were included in this subgroup analysis. Data were analysed using cluster-level methods for stratified cluster-randomised trials, using interaction tests to determine if intervention impact differed by subgroup. RESULTS: Taking into account multiplicity of testing, concurrence with a priori hypotheses and consistency within the results no strong effect-modifiers emerged. Impact on pregnancy knowledge and reported attitudes to sex increased with years of exposure to high-quality intervention. CONCLUSIONS: The desirable long-term impact of the MEMA kwa Vijana intervention did not vary greatly according to the subgroups examined. This suggests that the intervention can have an impact on a broad cross-section of young people in rural Mwanza. TRIAL REGISTRATION: ClinicalTrials.gov NCT00248469
