We present a case of tricuspid atresia and the treatment that was used in a child now aged 12. The malformation was corrected at the age of five through intracardiac Fontan circulation. Later, the child developed a right-to-left shunt due to dehiscence in the intra-atrial conduit, causing significant desaturation. This was treated by transcatheter closure with a 10 mm Amplatzer device at 8 years of age. Currently the child is asymptomatic with no residual shunts, and the position of the device is normal. The percutaneous closure of Fontan fenestrations using the Amplatzer device is a therapeutic approach that provides good results. It results in normal oxygen saturation and has low morbidity. The technique is simple, available to most cardiac catheterization laboratories, safe, and offers a reasonable cost-benefit ratio. In addition, the design of the device is suitable for this type of defect

Aosdois anos e meio é novamente intervencionada,criando-se um segundo shunt de Blalock-Taussigmodificado com Impra® de 5 mm, à direita.É submetida a cirurgia correctiva, aos cincoanos de idade, realizando-se shunt de bicava-pulmonar: anastomose da veia cava superior(VCS) ao ramo direito da AP, criação de um túnelinterauricular unindo a veia cava inferior (VCI)ao ramo esquerdo da AP e encerramento daválvula pulmonar.Em Abril de 2004, já com oito anos,apresenta-se de novo cianosada, com SatO2 de83%. Ecocardiograficamente é confirmado umshunt direito-esquerdo a nível auricular, atravésde um defeito no conduto intra-auricular, deaproximadamente 8 mm de diâmetro.É realizado um cateterismo de diagnóstico.Observam-se pressões na AP inferiores àspressões venosas (pressões médias de 15mmHgno ramo esquerdo da AP, 16mmHg no ramodireito da AP, 17mmHg na VCS e 18mmHg naVCI), bem como um gradiente de 3mmHg naretirada da aurícula esquerda para o conduto. Aangiografia com o cateter através da fenestrapermite definir a mesma e o shunt direito--esquerdo. É feito posteriormente o sizing, atravésde um cateter de balão, definindo-se um orifíciode abertura de 7mm (Figura 1). É então decididaa oclusão da “fenestra”, por via percutânea.Um dispositivo de Helex® é utilizado numaprimeira tentativa de encerramento (Figura 2)mas, devido à morfologia da fenestra em túnel(Figura 1-A), tal não foi conseguido. É necessárioa sua recolha através de um cateter Basket(Figura 3). Conseguiu-se por fim a oclusão dafenestra com um dispositivo Amplatzer SeptalOccluder® de 10 mm de diâmetro (Figura 4), sobcontrolo ecocardiográfico e fluoroscópico. Não évisível fluxo residual na angiografia final (Figura 5), assegurando-se assim o encerramentototal da fenestra. A SatO2, logo após a oclusão, foide 97%.Actualmente a doente tem doze anos de idade(follow-up de três anos e meio) e encontra-setunnel, rudimentary right ventricular chamberfrom which arose a hypoplastic pulmonary artery(PA), non-restrictive ventricular septal defect andlarge atrial septal defect.At one month of life, she underwent palliativesurgery, a left-sided modified Blalock-Taussigshunt using a 4 mm Impra® graft. At 2.5 years,she was operated again to establish a right-sidedmodified Blalock-Taussig shunt with a 5 mmImpra® graft.At five years of age the patient underwentcorrective surgery to create a bicaval-pulmonaryconnection by anastomosing the superior venacava (SVC) to the right PA, creating an intra-atrialconduit linking the inferior vena cava (IVC) to theleft PA, and closing the pulmonary valve.In April 2004, at the age of eight, she againpresented with cyanosis, with SatO2 of 83%.Echocardiography confirmed an interatrial right-to-left shunt through a defect approximately 8mm in diameter in the intra-atrial conduit.Diagnostic catheterization revealed PApressures lower than venous pressures (meanpressure of 15 mmHg in the left PA, 16 mmHg inthe right PA, 17 mmHg in the SVC and 18 mmHgin the IVC), and a 3 mmHg pull-back gradientfrom the left atrium to the conduit. Angiographywith the catheter through the fenestration made itpossible to determine the extent of thefenestration and the right-to-left shunt. Sizingwith a balloon catheter revealed a 7-mm orifice(Figure 1). It was decided to close the fenestrationpercutaneously.Closure was first attempted with a Helex®device (Figure 2), but the tunnel morphology ofthe fenestration (Figure 1A) meant that this wasunsuccessful and the device had to be retrievedwith a basket catheter (Figure 3). Closure waseventually achieved using a 10 mm Amplatzer®septal occluder (Figure 4) under echo-cardiographic and fluoroscopic guidance. Noresidual flow is visible in the final angiogram(Figure 5), demonstrating that the fenestration isfully closed. Ecocar-diograficamente o dispositivo encontra-se comboa posição e integridade adequada, semqualquer shunt visível.DISCUSSÃOAs fenestras (com localização, tamanho e tipovariáveis(1)), apesar de ainda controversas(2), sãouma de muitas modificações do procedimento deFontan, surgindo com o intuito de reduzir amortalidade e a morbilidade em doentes de altorisco, ao permitir um shunt residual direito-esquerdo. Assim, as fenestras, como por exemploas intra-auriculares, durante o período deresistência vascular elevada e diminuição dofluxo pulmonar, são um modo de permitir quealgum sangue dessaturado faça bypass aospulmões, reduzindo a pressão venosa sistémica eaumentando o débito cardíaco. Sabemos que a longo prazo originam uma dessaturação per-sistente e colocam os doentes em risco deembolização (sistémica e pulmonar) (3-6). Por outrolado, em idades pediátricas deve-se considerartambém o efeito que a cianose persistente tem nocrescimento e desenvolvimento psico-motor.No nosso caso não foi realizada uma fenestradurante o acto operatório, embora o defeito noconduto posteriormente resultasse como tal.Fenestras pequenas (<3mm) geralmenteencerram espontaneamente, mas outras maislargas podem persistir e permitir volumoso shuntdireito-esquerdo(3,4,5).Assim, tal como em muitos outros Centros(3,4,5),a nossa atitude foi encerrar um shunt residualsignificativo numa doente hipoxémica, por viapercutânea. São critérios para encerramento:saturação em repouso inferior a 92% ouapresentar dessaturação significativa com oexercício(3). Com os dados hemodinâmicos obtidosno cateterismo diagnóstico, que excluíramfalência do shunt bicava-pulmonar, resolveu-sedispensar o teste de oclusão.Observámos após a oclusão da fenestra umaelevação da saturação de oxigénio para valoresnormais o que, associado ao encerramento deuma possível via de embolização, eram os nossosobjectivos.A opção percutânea deve ter em linha deconta diversos factores, tais como o tamanho e alocalização da fenestra, o diâmetro da câmaraDISCUSSIONFenestrations, which can be of differentlocations, sizes and types (1), are one of manymodifications to the Fontan procedure. Althoughcontroversial (2), they are intended to reducemortality and morbidity in high-risk patients byallowing a residual right-to-left shunt. Intra-atrialfenestrations, created at a time of increasedvascular resistance and reduced pulmonary flow,allow some desaturated blood to bypass to thelungs, thereby reducing systemic venous pressureand increasing cardiac output. In the long termthey give rise to persistent desaturation and putpatients at risk of systemic and/or pulmonaryembolism (3-6). On the other hand, it is alsoimportant to consider the effects of persistentcyanosis on growth and psychomotordevelopment.In the case presented, the fenestration was notcreated deliberately during corrective surgery,but appeared subsequently as a result of a defectin the conduit.Small fenestrations (<3 mm) usually closespontaneously, but larger ones can persist andlead to a significant right-to-left shunt (3-5).As in many other centers (3-5), our approach wastranscatheter closure of a significant residualshunt in a hypoxemic patient. Criteria for closureare resting SatO2 of less than 92% or significantdesaturation on exertion (3). Since the findings ofdiagnostic catheterization excluded failure of thebicaval-pulmonary connection, it was decided notto perform occlusion testing.After closure of the fenestration, oxygensaturation rose to normal levels, which, togetherwith closure of a possible embolization pathway,was our objective.When contemplating transcatheter closure,various factors should be borne in mind,including the size and location of the fenestration,the diameter of the intermediate chamber, and theease of positioning the device (1). This approachwill be the best one in cases of multiple or smallfenestrations or difficulty in surgical access.Transesophageal echocardiography is usefulfor measuring the fenestration, monitoringplacement of the device, and assessing residualshunts after closure (4).On the basis of published studies (2-6), it can beconcluded that the percutaneous closure ofFontan fenestrations using the Amplatzer® deviceRev Port CardiolVol. 27 Dezembro 08 / December 08intermediária e a facilidade de colocação dodispositivo(1). Opta-se por esta via de abordagemprincipalmente em casos de fenestras múltiplas,tamanho pequeno ou localizações de difícilacesso cirúrgico.O apoio da ecocardiografia transesofágica temcomo objectivos obter medições da fenestra,monitorizar a colocação do dispositivo e avaliar aincidência de shunt residual após o en-cerramento(4).Com base também em trabalhos publicados(2-6),podemos concluir que a oclusão de fenestras porvia percutânea utilizando o dispositivo deAmplatzer® é uma solução terapêutica que temtido bons resultados imediatos e a médio prazo,além de reduzida morbilidade e rarascomplicações, oferecendo diversas vantagens. Atécnica mostra-se simples, segura e com razoávelcusto-benefício, exequível na maioria doslaboratórios de hemodinâmica, assim como odesign do dispositivo adequado ao defeito.is a therapeutic approach with variousadvantages: it provides good short- and medium-term results, with low morbidity and fewcomplications.
