    Estimating the change in asymptotic direction due to secular changes in the geomagnetic field

    The concept of geomagnetic optics, as described by the asymptotic directions of approach, is extremely useful in the analysis of cosmic radiation data. However, when changes in cutoff occur as a result of evolution in the geomagnetic field, there are corresponding changes in the asymptotic cones of acceptance. A method is introduced of estimating the change in the asymptotic direction of approach for vertically incident cosmic ray particles from a reference set of directions at a specific epoch by considering the change in the geomagnetic cutoff

    Probable detection of solar neutrons by ground-level neutron monitors during STIP interval 16

    The third solar neutron event detected by Earth-orbiting spacecraft was observed during STIP Interval XVI. The solar flare beginning at 2356 UT on 24 April l984 produced a variety of emissions including gamma rays and solar neutrons. The neutrons were observed by the SMM satellite and the neutron-decay protons were observed on the ISEE-3 spacecraft. Between 0000 and 0010 UT on 25 April an increase of 0.7 and 1.7 percent was recorded by neutron monitors at Tokyo (Itabashi) and Morioka, Japan. These stations were located about 42 degrees from the sub-solar point, and consequently, these is approximately 1400 grams of atmosphere between the incident neutrons at the top of the atmosphere and their detection on the Earth's surface. Nevertheless, the time coincidence of a small increase in the total counting rate of two independent neutron monitors indicates the presence of solar neutrons with energies greater than 400 MeV at the top of the Earth's atmosphere. The small increases in the counting rate emphasize the difficulty in identifying similar events using historical neutron monitor data

    Toxic effects of indocyanine green, infracyanine green, and trypan blue on the human retinal pigmented epithelium

    Background: Indocyanine green, infracyanine green, and trypan blue are frequently used as aids to visualize structures removed during vitreoretinal surgery. But they may have toxic effects on the retina. We therefore compared the acute and chronic toxicities of these stains on cultured human retinal pigmented epithelial (RPE) cells using clinically relevant concentrations and an identical experimental setup for each agent. Methods: Monolayers of RPE cells were incubated with various concentrations of indocyanine green, infracyanine green (each at 0.005%, 0.05%, and 0.5%) or trypan blue (0.05%, 0.06%, 0.1%, 0.15%, and 0.5%) for 5min (acute exposure) or 6 days (chronic exposure). Using the propidium iodide assay, acute cytotoxicity was monitored at 15-min intervals for up to 3h. Chronic cytotoxicity was assessed by monitoring cell calcein esterase activity, cell proliferation, and cell morphology (viability) after 6 days of exposure. Results: Indocyanine and infracyanine green induced acute and chronic toxicities at a concentration above 0.05%. Trypan blue evoked no acute toxicity, but it was chronically cytotoxic at all tested concentrations. Conclusions: Despite thorough rinsing after application, significant amounts of the not sufficiently water soluble indocyanine and infracyanine green are retained after surgery by the eye. Trypan blue, being more water-soluble than ICG, is probably retained to the least degree. This circumstance is fortunate given that trypan blue exhibits a chronic cytotoxicity comparable to ICG at all clinically relevant concentrations.     Time extended production of neutrons during a solar flare

    The most energetic neutral emissions expected from solar flares are gamma rays (10 MeV) from relativistic primary and secondary electron bremsstrahlung,from approx 0 meson decay, and from neutrons ( 50 MeV). Bremsstrahlung photon energies extend to that of the highest energy electron present, but the shape of the pi sup 0 gamma ray spectrum, peaking at 69 MeV, does not depend strongly on the proton spectrum above threshold, which is approx. 292 MeV for meson production on protons. The highest energy neutrons observed indicate directly the highest energy ions which interact at the Sun, and the presence or absence of anergy cutoff in the acceleration process. Die therapeutischen Überlegungen sollten die Komorbiditäten und potenzielle Behinderungen des Urinabflusses miteinschließen. In der Behandlung stehen Chinolone, Breitspektrumpenizilline und Cephalosporine der 3. Generation im Vordergrund. Bei den ökonomischen Überlegungen sollte v. a. der höhere Preis von i.v.-Antibiotika im Vergleich zu per os mit einfliessen. Urinary tract infection (UTI) is the most common infection in hospitalized adults. Nosocomial UTIs are mainly associated with the use of urinary catheters. Thus, the decision for catheterization should be made carefully and catheters removed in time. In order to prevent unnecessary antibiotic use in patients with urinary catheters correct diagnosis is crucial. Chinolones, broad-spectrum penicillins and third-generation cephalosporins are the mainstay of therapy. Comorbidities should be considered and potential obstructions of urinary flow removed. Economically important are the normally higher prices of i.v. antibiotics compared to oral use

    Fecopneumothorax and colopleural fistula – uncommon complications of Crohn's disease

    BACKGROUND: Colopleural fistula and fecopneumothorax are very rare complications of Crohn's disease. Fistula formation is frequent in Crohn's disease and occurs in approximately 33% of patients. On the other hand, fistulous communication between the pleural cavity and adjacent organs below the diaphragm is extremely rare. CASE PRESENTATION: We describe the case of 27 year-old female with colopleural fistula as a complication of Crohn's disease. The diagnosis was established with clinical exam, barium enema, chest X-ray, abdominal and chest CT exam. The treatment was surgical. CONCLUSION: Colopleural fistula and fecopneumothorax are rare but life treating complications of Crohn's disease. Surgical treatment is mandatory as soon as the diagnosis is established

    Coronary optical frequency domain imaging (OFDI) for in vivo evaluation of stent healing: comparison with light and electron microscopy

    Aims Coronary late stent thrombosis, a rare but devastating complication, remains an important concern in particular with the increasing use of drug-eluting stents. Notably, pathological studies have indicated that the proportion of uncovered coronary stent struts represents the best morphometric predictor of late stent thrombosis. Intracoronary optical frequency domain imaging (OFDI), a novel second-generation optical coherence tomography (OCT)-derived imaging method, may allow rapid imaging for the detection of coronary stent strut coverage with a markedly higher precision when compared with intravascular ultrasound, due to a microscopic resolution (axial ∼10-20 µm), and at a substantially increased speed of image acquisition when compared with first-generation time-domain OCT. However, a histological validation of coronary OFDI for the evaluation of stent strut coverage in vivo is urgently needed. Hence, the present study was designed to evaluate the capacity of coronary OFDI by electron (SEM) and light microscopy (LM) analysis to detect and evaluate stent strut coverage in a porcine model. Methods and results Twenty stents were implanted into 10 pigs and coronary OFDI was performed after 1, 3, 10, 14, and 28 days. Neointimal thickness as detected by OFDI correlated closely with neointimal thickness as measured by LM (r = 0.90, P < 0.01). The comparison of stent strut coverage as detected by OFDI and SEM analysis revealed an excellent agreement (r = 0.96, P < 0.01). In particular, stents completely covered by OFDI analysis were also completely covered by SEM analysis. All incompletely covered stents by OFDI were also incompletely covered by SEM. Analyses of fibrin-covered stent struts suggested that these may rarely be detected as uncovered stent struts by OFDI. Importantly, optical density measurements revealed a significant difference between fibrin- and neointima-covered coronary stent struts [0.395 (0.35-0.43) vs. 0.53 (0.47-0.57); P < 0.001], suggesting that differences in optical density provide information on the type of stent strut coverage. The sensitivity and specificity for detection of fibrin vs. neointimal coverage was evaluated using receiver-operating characteristic analysis. Conclusion The present study demonstrates that OFDI is a highly promising tool for accurate evaluation of coronary stent strut coverage, as supported by a high agreement between OFDI and light and electron microscopic analysis. Furthermore, our data indicate that optical density measurements can provide additional information with respect to the type of stent strut coverage, i.e. fibrin vs. neointimal coverage. Therefore, coronary OFDI analysis will provide important information on the biocompatibility of coronary stent
