    Profil Ekg Pasien Hipertensi Di Poliklinik Jantung

    Pendahuluan: Sebanyak 31,7% penduduk Indonesia memiliki hipertensi. Hipertensi dapat mengakibatkan Perubahan pada anatomi dan atau fungsi jantung yang dapat menyebabkan morbiditas dan atau mortalitas. Perubahan itu sendiri dapat dideteksi dengan elektrokardiografi (EKG). Berdasarkan alasan tersebut, tujuan dari studi ini adalah mengetahui profil EKG pasien hipertensi di Poliklinik Jantung RSUD Dr. Soetomo Surabaya.Metodologi: Studi ini merupakan penelitian deskriptif observasional. Sampel penelitian didapatkan melalui data primer dan data sekunder dengan menggunakan teknik sampel total selama periode 1-31 Maret 2015. Data yang terkumpul diolah berdasarkan jenis kelamin, usia dan ada tidaknya hipertrofi ventrikel kiri jantung.Hasil: Terdapat 140 pasien hipertensi selama periode 1-31 Maret 2015. Prevalensi hipertensi didapatkan sama antara pria dan wanita. Prevalensi rentang usia tertinggi pasien hipertensi adalah usia 51-60 tahun. Dua Hasil EKG menunjukkan hipertrofi ventrikel kiri, dimana kedua hasil tersebut terdapat pada pria usia 55 tahun dan 61 tahun, dan 138 hasil EKG menunjukkan selain hipertrofi ventrikel kiri, seperti aritmia dan fibrilasi atrial.Diskusi: Hasil yang diperolah dan telah diolah menunjukkan hasil yang sama antara pria dan wanita, dimana menurut beberapa penelitian yang telah ada terdapat perbedaan antara pria dan wanita.     Cloning and Expression of hGAD65 Gene in E. Coli BL21

    The aim of this study is to construct the hGAD65 gene and to identify the hGAD65 clone by using PCR & RFLP. The samples were derived from normal person & DM patient’s blood. Blood DNA was isolated by salting out method and then amplified by PCR with a pair of specific primer, GAD65-F-BamH1-807 & GAD65-R-Xho1-945. The PCR-product was cloned into vector pET-28a and the pET28a-hGAD65-clone was transformed into E.coli BL21 competent cells. The pET28a-hGAD65-clone was confirmed by PCR and RFLP by BamH1 & XhoI. The PCR product of pET28a-hGAD65-clone was one band of 159bp and has two bands 5.3 kb and 159 bp by RFLPwith both restriction enzymes. The GAD65 protein is expressed in 65kD of pET28a-hGAD65-clone. PET28a-hGAD65-clone was able to recognize by gold standard monoclonal antibody specifically. These results indicated that the hGAD65 gene inserted into pET28a properly and provided the GAD65 protein expression. Key words: hGAD65, PCR, pET-28a, RFL

    Analgesic effects of adding lidocaine to morphine pumps after orthopedic surgeries

    Background: Opiate is used in patient-controlled intravenous analgesia pumps (PCIA) for controlling pain in post-surgical patients. Other drugs are remarkably added to opioid pumps to enhance quality, lengthen analgesia, and reduce side effects. Lidocaine, a local anesthetic which inhibits sodium channels, has anesthetic and analgesic effects when injected locally or intravenously. The objective of this study is to evaluate the analgesic effects of adding lidocaine 1 to different doses of morphine via IV pump to patient-controlled analgesia (PCA) after orthopedic surgeries. Materials and Methods: In a randomized clinical trial, 60 patients who had undergone orthopedic surgery of lower extremities were divided into three equal groups to control postoperative pain. Intravenous pump with 5 ml/h flow rate was used as the analgesic method. The solution consisted of lidocaine 1 plus 20 mg morphine for the first group, lidocaine 1 plus 10 mg morphine for the second group, and only 20 mg morphine for the third group (control group). Patients were checked every 12 h, and Visual Analog Scale (VAS), extra opioid doses, nausea/vomiting, and sedation scale were examined. Results: Pain score was lower in the first group compared to the other two groups. Mean VAS was 2.15 ± 0.2, 2.75 ± 0.2, and 2 ± 0.25 on the first day and 1.88 ± 0.1, 2.74 ± 0.3, and 2.40 ± 0.3 on the second day, respectively, in the three groups and the difference was statistically significant (P < 0.01 and -;0.05, respectively). Also, 10 of patients in the first group needed extra opioid doses, while this figure was 30 in the second group and 25 in the third group (P < 0.01). Nausea/vomiting and sedation scores were not statistically different among the three groups. Conclusion: Compared to lidocaine 1 plus 10 mg morphine or 20 mg morphine alone in PCIA, adding lidocaine 1 to 20 mg morphine decreases the pain score and opioid dose after orthopedic surgeries without having side effects

    Vibrational spectroscopy and DFT calculations of 1,​3-​dibromo-​2,​4,​6-​trimethylbenzene: Anharmonicity, coupling and methyl group tunneling

    International audienceThe Raman, IR and INS spectra of 1,3-dibromo-2,4,6-trimethylbenzene (DBMH) were recorded in the 80-3200 cm-1 range. The molecular conformation and vibrational spectra of DBMH were computed at the MPW1PW91/LANL2DZ level. Except for the methyl 2 environment, the agreement between the DFT calculations and the neutron diffraction structure is almost perfect (deviations < 0.01 Å for bond lengths, < 0.2° for angles). The frequencies of the internal modes of vibration were calculated with the harmonic and anharmonic approximations; the later method yields results that are in remarkable agreement with the spectroscopic data, resulting in a confident assignment of the vibrational bands. Thus, no scaling is necessary. The coupling, in phase or anti-phase, of the motions of symmetrical C-Br and C-Me bonds is highlighted. Our DFT calculations suggest that the torsion of methyl groups 4 and 6 is hindered in deep wells, whereas methyl group 2 is a quasi-free rotor. The failure of the calculations to determine the frequencies of the methyl torsional modes is explained as follows: DFT does not consider the methyl spins and assumes localization of the protons, whereas the methyl groups must be treated as quantum rotors

    Nano dust impacts on spacecraft and boom antenna charging

    High rate sampling detectors measuring the potential difference between the main body and boom antennas of interplanetary spacecraft have been shown to be efficient means to measure the voltage pulses induced by nano dust impacts on the spacecraft body itself (see Meyer-Vernet et al, Solar Phys. 256, 463 (2009)). However, rough estimates of the free charge liberated in post impact expanding plasma cloud indicate that the cloud's own internal electrostatic field is too weak to account for measured pulses as the ones from the TDS instrument on the STEREO spacecraft frequently exceeding 0.1 V/m. In this paper we argue that the detected pulses are not a direct measure of the potential structure of the plasma cloud, but are rather the consequence of a transitional interruption of the photoelectron return current towards the portion of the antenna located within the expanding cloud

    Studi Pengaruh Pengambilan Angka Ekivalen Beban Kendaraan Pada Perhitungan Tebal Perkerasan Fleksibel Di Jalan Manado – Bitung

    Beban as yang berbeda-beda pada masing-masing kendaraan akan menimbulkan tingkat kerusakan yang berbeda-beda pula. Kumulatif beban as tersebut selama umur rencana (Wt18) merupakan salah satu data yang diperlukan dalam perhitungan tebal perkerasan. Semua beban yang diterima oleh struktur perkerasan jalan diekivalenkan kedalam beban as standar. Parameter yang setara dengan beban yang diekivalenkan dengan beban as standar adalah sebesar 8,16 ton (Equivalent Standar Axle Load). Selanjutnya pada metode Bina Marga (1987) menetapkan rumus angka ekivalen berdasarkan sumbu tunggal dan sumbu ganda, sedangkan pada metode NAASRA (1987) selain berdasarkan sumbu tunggal dan sumbu ganda juga didasarkan pada konfigurasi roda kendaraan. Sehingga perhitungan lintas ekivalen kumulatif tentunya berbeda untuk kedua metode tersebut diatas.Penelitian ini dilakukan pada ruas jalan Manado-Bitung dengan mengambil dua jenis data, yakni data distribusi berat kendaraan yang diperoleh dari jembatan timbang Wangurer dan data survey volume lalulintas. Survey dilakukan pada bulan Juni 2012 dan menghasilkan LHR sebesar 2383 kendaraan yang terdiri dari 32 % kendaraan ringan, 63 % kendaraan bus, dan 5 % kendaraan truk. Perhitungan Lintas Ekivalen Permulaan (LEP) dihitung berdasarkan LHR yang didapat dan angka ekivalen menurut metode Bina Marga maupun metode NAASRA.Hasil perhitungan menunjukkan perbedaan yang cukup berarti pada perhitungan LEP. Berdasarkan metode Bina Marga LEP sebesar 133,069 ESAL dan berdasarkan metode NAASRA LEP sebesar 215,954 ESAL, atau nilai Wt18 berdasarkan metode NAASRA adalah lebih besar 1,62 kali lipat dari nilai Wt18 berdasarkan metode Bina Marga yang tentunya menghasilkan perhitungan yang berbeda dalam mendesain tebal perkerasan. Namun, berdasarkan nilai LEP tersebut diatas pada kumulatif beban as standar (Wt18) dibawah 670000 ESAL pada metode Bina Marga hasil perhitungan tebal perkerasan adalah relatif sama. Hasil perhitungan tebal perkerasan menunjukkan perbedaan yang cukup berarti pada tebal perkerasan lapisan subbase bila Wt18 lebih besar dari 1000000 ESAL

    As Fábricas do Vale do Nabão: estudo comparativo dos sistemas construtivos e sua relação com a água

    Entre meados do século XVIII e princípio do século XX, o concelho de Tomar no vale do Rio Nabão foi ocupado por inúmeras fábricas, sendo considerada uma das regiões mais industrializadas de Portugal. Muitas dessas fábricas, entre elas a Real Fábrica de Fiação que data de 1790 (cujo Açude é classificado como Monumento de Interesse Público), ou o Conjunto Industrial da Levada de Tomar com a Moagem "A Nabantina" de 1883, utilizavam as águas do rio como força motriz para suas maquinarias e mais tarde para geração de energia. Em 1901 é instalada uma central termo e hidroelétrica no centro histórico, tornando Tomar a terceira cidade em Portugal a ter iluminação elétrica. Esta central termo e hidroelétrica, pertencente ao Conjunto Industrial da Levada, assegurava a produção de energia elétrica para abastecimento da cidade até 1954, ano em que é inaugurada a Barragem de Castelo do Bode. As fábricas testemunham diversos tipos de sistemas construtivos. Uma das primitivas, a Fábrica de Papel do Sobreirinho, é uma construção de alvenaria mista, encontrando-se atualmente em estado de ruina, sem cobertura e com paredes desprovidas de revestimento. A Fábrica de Papel de Porto Cavaleiros, e a maioria das fábricas, tem estrutura em betão armado e paredes em alvenaria de tijolo. O objetivo do estudo é analisar e comparar os sistemas construtivos deste conjunto de fábricas abandonadas, incluindo o seu embasamento e a relação com a água. O estudo dos sistemas construtivos é condição para uma correta proposta de conservação e reabilitação do conjunto histórico. É primordial o conhecimento profundo destas fábricas, que constituem o património industrial da região de forma inigualável.info:eu-repo/semantics/publishedVersio
