    The Primary Solid Waste Storage Gaps Experienced By Nairobi Households

    This study identifies and analyses the solid waste management service gaps and situations in these different socio-economic setups. The objectives of this study were: (i) To assess households’ current bin types; (ii) To assess households’ desired bin types; and (iii) To identify the bin gaps experienced by households. A total of 430 households were selected through stratified sampling from dwellings with different social status, as represented by estate to give the quantitative data. The qualitative methods such as key informant interviews, observation surveys, and secondary data were also used. All the 8 divisions in Nairobi were represented, in a ratio proportionate to households from slums, low income, and middle income estates. There is a bin gap of about 15% households needing bucket; 26% excess plastics; and a deficit of 12% households needing but short of metal bins; and a 6% deficit of the 5-10litre bins; a 13% excess of bins less than 5 litres; 1.6% deficit of 10-15 litre bins, and 14 % deficit of households needing bins larger than 20 litres. 30% of households have heavy to very heavy bins. Entrepreneurs should improve availability of storage bins of correct specification (size, material and make) as preferred by various households. Correct specifications of garbage bins should be designed to satisfy these household needs.Key Words: Household Storage, Solid Waste Management; Garbage Bin

    Reason and Sagacity in Africa: Odera Oruka’s Contribution to Philosophy

    Commentators on the four trends in contemporary African philosophy as enunciated by H. Odera Oruka frequently focus on the merits and demerits of each trend. However, many of them are obblivious to the way in which sagacity emancipates African philosophy by putting reason in its rightful pivotal position. This article argues that while the professional philosophers accused ethno-philosophers of doing disservice to African philosophy, they too stand accused of the same. This is due to the fact that both ethno-philosophy and professional philosophy function within the Western grid and therefore in the interest and service of the Western world. Philosophic sagacity, the article argues, discards the undesirable elements of ethno-philosophy and professional philosophy, while retaining desirable ones, namely, the Africanness in ethno-philosophy and the objectivity in professional philosophy. Because philosophic sagacity is African and objective, it is a desired tool of change in Africa. It can, for example, be used to address negative aspects of ethnicity that bedevil Africa. There lies the most important contribution by H. Odera Oruka to philosophy in general and African philosophy in particular.     Spatial and temporal dynamics of malaria transmission in rural western Kenya

    ABSTRACT: BACKGROUND: Understanding the impact of reducing Plasmodium falciparum malaria transmission requires estimates of the relationship between health outcomes and exposure to infectious mosquitoes. However, measures of exposure such as mosquito density and entomological inoculation rate (EIR) are generally aggregated over large areas and time periods, biasing the outcome-exposure relationship. There are few studies examining the extent and drivers of local variation in malaria exposure in endemic areas. METHODS: We describe the spatio-temporal dynamics of malaria transmission intensity measured by mosquito density and EIR in the KEMRI/CDC health and demographic surveillance system using entomological data collected during 2002-2004. Geostatistical zero inflated binomial and negative binomial models were applied to obtain location specific (house) estimates of sporozoite rates and mosquito densities respectively. Model-based predictions were multiplied to estimate the spatial pattern of annual entomological inoculation rate, a measure of the number of infective bites a person receive per unit of time. The models included environmental and climatic predictors extracted from satellite data, harmonic seasonal trends and parameters describing space-time correlation. RESULTS: Anopheles gambiae s.l was the main vector species accounting for 86% (n=2309) of the total collected mosquitoes with the remainder being Anopheles funestus. Sixty eight percent (757/1110) of the surveyed houses had no mosquitoes. Distance to water bodies, vegetation and day temperature were significantly associated with mosquito density. Overall annual point estimates of EIR were 6.7, 9.3 and 9.6 infectious bites per annum for 2002, 2003 and 2004 respectively. Monthly mosquito density and EIR varied over the study period peaking in May during the wet season. The predicted and observed densities and EIR showed a strong seasonal and spatial pattern over the study area. CONCLUSIONS: Spatio-temporal maps of malaria transmission intensity obtained in this study are not only useful in understanding variability in malaria epidemiology over small areas but also provides a high resolution exposure surface that can be used to analyse the impact of malaria exposure on mortalit

    EVALUATION OF HERBS AS POTENTIAL DRUGS

    Herbal drugs have been used since ancient times as medicines for the treatment of a wide range of diseases, for both human and livestock. A study conducted in the Lake Victoria Basin Kenya revealed vast knowledge and reliance on traditional medicine as source of healthcare. The study documented 34 medicinal plant species distributed among 21 botanical families and 34 genera, used in the management of human ailments. The highest numbers of species were from the families Asteraceae and Leguminosae. The most commonly harvested plant parts were leaves (46.51%) and roots (34.88%). The most common growth forms utilised were herbs (40.54%) followed by shrubs (27.03%). The major methods of herbal drug preparation were concoction (31.03%) and decoction (24.14%) administered mainly through oral and dermal routes, (64.29%) and (32.14%) respectively. The use of herbal drugs as mixtures was reported to be a common practice by the herbal practitioners; 57.14% of the preparations were dispensed as mixtures while 42.86% of the preparations composed single plants. A rich knowledge of medicinal plants was recognized and phytochemical and bioactivity analyses of these herbal plants are recommended to determine their safety and efficacy

    Effectiveness of conditional cash transfers (Afya credits incentive) to retain women in the continuum of care during pregnancy, birth and the postnatal period in Kenya: a cluster-randomised trial

    OBJECTIVES: Given high maternal and child mortality rates, we assessed the impact of conditional cash transfers (CCTs) to retain women in the continuum of care (antenatal care (ANC), delivery at facility, postnatal care (PNC) and child immunisation). DESIGN: We conducted an unblinded 1:1 cluster-randomised controlled trial. SETTING: 48 health facilities in Siaya County, Kenya were randomised. The trial ran from May 2017 to December 2019. PARTICIPANTS: 2922 women were recruited to the control and 2522 to the intervention arm. INTERVENTIONS: An electronic system recorded attendance and triggered payments to the participant's mobile for the intervention arm (US4.5),andphonecreditforthecontrolarm(US4.5), and phone credit for the control arm (US0.5). Eligibility criteria were resident in the catchment area and access to a mobile phone. PRIMARY OUTCOMES: Primary outcomes were any ANC, delivery, any PNC between 4 and 12 months after delivery, childhood immunisation and referral attendance to other facilities for ANC or PNC. Given problems with the electronic system, primary outcomes were obtained from maternal clinic books if participants brought them to data extraction meetings (1257 (50%) of intervention and 1053 (36%) control arm participants). Attendance at referrals to other facilities is not reported because of limited data. RESULTS: We found a significantly higher proportion of appointments attended for ANC (67% vs 60%, adjusted OR (aOR) 1.90; 95% CI 1.36 to 2.66) and child immunisation (88% vs 85%; aOR 1.74; 95% CI 1.10 to 2.77) in intervention than control arm. No intervention effect was seen considering delivery at the facility (90% vs 92%; aOR 0.58; 95% CI 0.25 to 1.33) and any PNC attendance (82% vs 81%; aOR 1.25; 95% CI 0.74 to 2.10) separately. The pooled OR across all attendance types was 1.64 (1.28 to 2.10). CONCLUSIONS: Demand-side financing incentives, such as CCTs, can improve attendance for appointments. However, attention needs to be paid to the technology, the barriers that remain for delivery at facility and PNC visits and encouraging women to attend ANC visits within the recommended WHO timeframe.     Health Care-Seeking Behavior During Childhood Diarrheal Illness: Results of Health Care Utilization and Attitudes Surveys of Caretakers in Western Kenya, 2007-2010

    We interviewed caretakers of 1,043 children 20,000 children on five separate subsequent occasions (May of 2009 to December 31, 2010) to assess healthcare seeking patterns for diarrhea. Diarrhea prevalence during the preceding 2 weeks ranged from 26% at baseline to 4–11% during 2009–2010. Caretakers were less likely to seek healthcare outside the home for infants (versus older children) with diarrhea (adjusted odds ratio [aOR] = 0.33, confidence interval [CI] = 0.12–0.87). Caretakers of children with reduced food intake (aOR = 3.42, CI = 1.37–8.53) and sunken eyes during their diarrheal episode were more likely to seek care outside home (aOR = 4.76, CI = 1.13–8.89). Caretakers with formal education were more likely to provide oral rehydration solution (aOR = 3.01, CI = 1.41–6.42) and visit a healthcare facility (aOR = 3.32, CI = 1.56–7.07). Studies calculating diarrheal incidence and healthcare seeking should account for seasonal trends. Improving caretakers' knowledge of home management could prevent severe diarrhea

    Sperm Proteasomes Degrade Sperm Receptor on the Egg Zona Pellucida during Mammalian Fertilization

    Despite decades of research, the mechanism by which the fertilizing spermatozoon penetrates the mammalian vitelline membrane, the zona pellucida (ZP) remains one of the unexplained fundamental events of human/mammalian development. Evidence has been accumulating in support of the 26S proteasome as a candidate for echinoderm, ascidian and mammalian egg coat lysin. Monitoring ZP protein degradation by sperm during fertilization is nearly impossible because those few spermatozoa that penetrate the ZP leave behind a virtually untraceable residue of degraded proteins. We have overcome this hurdle by designing an experimentally consistent in vitro system in which live boar spermatozoa are co-incubated with ZP-proteins (ZPP) solubilized from porcine oocytes. Using this assay, mimicking sperm-egg interactions, we demonstrate that the sperm-borne proteasomes can degrade the sperm receptor protein ZPC. Upon coincubation with motile spermatozoa, the solubilized ZPP, which appear to be ubiquitinated, adhered to sperm acrosomal caps and induced acrosomal exocytosis/formation of the acrosomal shroud. The degradation of the sperm receptor protein ZPC was assessed by Western blotting band-densitometry and proteomics. A nearly identical pattern of sperm receptor degradation, evident already within the first 5 min of coincubation, was observed when the spermatozoa were replaced with the isolated, enzymatically active, sperm-derived proteasomes. ZPC degradation was blocked by proteasomal inhibitors and accelerated by ubiquitin-aldehyde(UBAL), a modified ubiquitin protein that stimulates proteasomal proteolysis. Such a degradation pattern of ZPC is consistent with in vitro fertilization studies, in which proteasomal inhibitors completely blocked fertilization, and UBAL increased fertilization and polyspermy rates. Preincubation of intact zona-enclosed ova with isolated active sperm proteasomes caused digestion, abrasions and loosening of the exposed zonae, and significantly reduced the fertilization/polyspermy rates after IVF, accompanied by en-mass detachment of zona bound sperm. Thus, the sperm borne 26S proteasome is a candidate zona lysin in mammals. This new paradigm has implications for contraception and assisted reproductive technologies in humans, as well as animals
