    Highly effective regimen for decolonization of methicillin-resistant Staphylococcus aureus carriers

    OBJECTIVE: To evaluate the efficacy of a standardized regimen for decolonization of methicillin-resistant Staphylococcus aureus (MRSA) carriers and to identify factors influencing decolonization treatment failure. DESIGN: Prospective cohort study from January 2002 to April 2007, with a mean follow-up period of 36 months. SETTING: University hospital with 750 beds and 27,000 admissions/year. PATIENTS: Of 94 consecutive hospitalized patients with MRSA colonization or infection, 32 were excluded because of spontaneous loss of MRSA, contraindications, death, or refusal to participate. In 62 patients, decolonization treatment was completed. At least 6 body sites were screened for MRSA (including by use of rectal swabs) before the start of treatment. INTERVENTIONS: Standardized decolonization treatment consisted of mupirocin nasal ointment, chlorhexidine mouth rinse, and full-body wash with chlorhexidine soap for 5 days. Intestinal and urinary-tract colonization were treated with oral vancomycin and cotrimoxazole, respectively. Vaginal colonization was treated with povidone-iodine or, alternatively, with chlorhexidine ovula or octenidine solution. Other antibiotics were added to the regimen if treatment failed. Successful decolonization was considered to have been achieved if results were negative for 3 consecutive sets of cultures of more than 6 screening sites. RESULTS: The mean age (+/- standard deviation [SD]) age of the 62 patients was 66.2 +/- 19 years. The most frequent locations of MRSA colonization were the nose (42 patients [68%]), the throat (33 [53%]), perianal area (33 [53%]), rectum (36 [58%]), and inguinal area (30 [49%]). Decolonization was completed in 87% of patients after a mean (+/-SD) of 2.1 +/- 1.8 decolonization cycles (range, 1-10 cycles). Sixty-five percent of patients ultimately required peroral antibiotic treatment (vancomycin, 52%; cotrimoxazole, 27%; rifampin and fusidic acid, 18%). Decolonization was successful in 54 (87%) of the patie in the intent-to-treat analysis and in 51 (98%) of 52 patients in the on-treatment analysis. CONCLUSION: This standardized regimen for MRSA decolonization was highly effective in patients who completed the full decolonization treatment course

    Advanced characterization and simulation of SONNE: a fast neutron spectrometer for Solar Probe Plus

    SONNE, the SOlar NeutroN Experiment proposed for Solar Probe Plus, is designed to measure solar neutrons from 1-20 MeV and solar gammas from 0.5-10 MeV. SONNE is a double scatter instrument that employs imaging to maximize its signal-to-noise ratio by rejecting neutral particles from non-solar directions. Under the assumption of quiescent or episodic small-flare activity, one can constrain the energy content and power dissipation by fast ions in the low corona. Although the spectrum of protons and ions produced by nanoflaring activity is unknown, we estimate the signal in neutrons and γ−rays that would be present within thirty solar radii, constrained by earlier measurements at 1 AU.     Seasonal H1N1 2007 influenza virus infection is associated with elevated pre‐exposure antibody titers to the 2009 pandemic influenza A (H1N1) virus

    AbstractThe new influenza strain detected in humans in April 2009 has caused the first influenza pandemic of the 21st century. A cross‐reactive antibody response, in which antibodies against seasonal H1N1 viruses neutralized the 2009 pandemic influenza A (H1N1) virus (2009 pH1N1), was detected among individuals aged >60 years. However, factors other than age associated with such a cross‐reactive antibody response are poorly documented. Our objective was to examine factors potentially associated with elevated pre‐exposure viro‐neutralization and hemagglutination‐inhibition antibody titers against the 2009 pH1N1. We also studied factors associated with antibody titers against the 2007 seasonal H1N1 virus. One hundred subjects participating in an influenza cohort were selected. Sera collected in 2008 were analysed using hemagglutination inhibition and viro‐neutralization assays for the 2009 pH1N1 virus and the 2007 seasonal H1N1 virus. Viro‐neutralization results were explored using a linear mixed‐effect model and hemagglutination‐inhibition results using linear‐regression models for interval‐censored data. Elevated antibody titers against 2009 pH1N1 were associated with seasonal 2007 H1N1 infection (viro‐neutralization, p 0.006; hemagglutination‐inhibition, p 0.018). Elevated antibody titers were also associated with age in the viro‐neutralization assay (p <0.0001). Seasonal 2007 H1N1 infection is an independent predictor of elevated pre‐exposure antibody titers against 2009 pH1N1 and may have contributed to lowering the burden of the 2009 pH1N1 pandemic

    Respiratory Syncytial Virus Infection in Patients with Hematological Diseases: Single-Center Study and Review of the Literature

    Background.Respiratory syncytial virus (RSV) causes significant mortality in patients with hematological diseases, but diagnosis and treatment are uncertain. Methods.We retrospectively identified RSV-infected patients with upper or lower respiratory tract infection (RTI) by culture, antigen testing, and polymerase chain reaction from November 2002 through April 2007. Patients with severe immunodeficiency (SID; defined as transplantation in the previous 6 months, T or B cell depletion in the previous 3 months, graft-versus-host disease [grade, â©Ÿ2], leukopenia, lymphopenia, or hypogammaglobulinemia) preferentially received oral ribavirin, intravenous immunoglobulin, and palivizumab. The remaining patients with moderate immunodeficiency (MID) preferentially received ribavirin and intravenous im munoglobulin. Results.We identified 34 patients, 22 of whom had upper RTI (10 patients with MID and 12 with SID) and 12 of whom had lower RTI (2 with MID and 10 with SID). Thirty-one patients were tested by polymerase chain reaction (100% of these patients had positive results; median RSV load, 5.46 log10 copies/mL), 30 were tested by culture (57% had positive results), and 25 were tested by antigen testing (40% had positive results). RSV-attributed mortality was 18% (6 patients died) and was associated with having â©Ÿ2 SID factors (P=.04), lower RTI (P=.01), and preengraftment (P=.012). Among 12 patients with MID (7 of whom received treatment), no progression or death occurred. Nine patients with SID and upper RTI received treatment (7 patients received ribavirin, intravenous immunoglobulin, and palivizumab); infection progressed to the lower respiratory tract in 2 patients, and 1 patient died. Ten patients with SID and lower RTI were treated, 5 of whom died, including 4 of 6 patients who received ribavirin, intravenous immunoglobulin, and palivizumab. The duration of RSV shedding correlated with the duration of symptoms in patients with SID but exceeded symptom duration in patients with MID (P<.05). Conclusions.Lower RTI, â©Ÿ2 SID criteria, and preengraftment are risk factors for RSV-attributed mortality. Polymerase chain reaction may optimize diagnosis and monitoring. Oral ribavirin therapy seems safe, but trials are needed to demonstrate its efficac

    The Astrobee Research Facility will maintain three identical free-flying Astrobee robots on the ISS. After the Astrobees are launched and commissioned in 2018, they will replace the SPHERES robots that have been operating on the ISS since 2006 (Fig. 2). Over the years, the SPHERES have been among the most-used payloads on the ISS, supporting dozens of experiments from a variety of guest scientists. In the next section, we'll talk about past SPHERES experiments as possible inspiration for your future research on Astrobee. Compared to SPHERES, the Astrobee robots will offer many new capabilities and will require less astronaut time to support, so we hope the new facility will be able to fly experiments much more often

    Simulation and Computation make a versatile teaching strategy, and may be an important way to motivate students and lecturers to achieve meaningful learning. Indeed, this work refers to a study whose main objective is to set the influence that a teaching approach based on the use of computer simulation would have on students’ learning, compared to the one in use today. This work involved the participation of two classes of 11th grade at a Secondary School in Lisbon, Portugal, where the main goal is to teach a specific topic to an untried student’s group. With regard to the simulation environment, it will be grounded on a Proof Theoretical approach to Knowledge Representation and Reasoning, which caters for the handling of incomplete, unknown or even self-contradictory information or knowledge

    Enhancing first year management students’ engagement: an action research project to explore the use of the Essay Feedback Checklist (EFC)

    Feedback represents one of the key factors that affect student learning. A question is raised here as to what extent engaging students with feedback and considering their voices as partners in the assessment process can improve their confidence and therefore their learning. The objective of this action research was to support the introduction of the Essay Feedback Checklist (EFC) to first year management students at a British University. This research examined the use of EFC as an interactive two-way communication tool between tutors and students. The research also aimed to help management students to be more aware of the assessment criteria thus to be more engaged in feedback. Data was collected from students and tutors before and after the use of the EFC in order to highlight their perspectives regarding the effective use of the tool. The research's findings argued that students found the EFC useful to support engagement with the feedback and in improving their confidence through receiving quality feedback to develop their work further. Tutors and students claimed that the EFC created an interactive environment in providing feedback and it helped to open a dialogue about it, but they also highlighted some drawbacks
