INTRODUCTION: In emergency departments, it is important to have tests that give fast results especially in the diagnosis and treatment of critically ill patients. Blood gas analysis has an important role in the diagnosis and treatment of patients applying for emergency services. On the other hand, taking samples for blood gas may differ based on clinic properties of patients. Puncture of arteries in sampling is a painful procedure. It is aimed to evaluate the differences between visual analogue pain scales of patients based on different punctured veins. METHODS: In this retrospective study, files of patients attempted to Ufuk University, Medical Faculty Dr Rıdvan Ege Education and Research Hospital, Department of Emergency Medicine between 01.01.2015– 31.12.2017 with the punctured veins process at the age of 18–65 were examined. RESULTS: A total of 84 patient files, including 55 radial and 31 femoral punctured veins, were subjected to the research. 36.4% of patients in the radial group were female, and 63.6% of them were male. In the fem- oral group, 45.2% of patients were female and 54.8% were male. Gender differences between groups were found to be statistically insignificant (p > 0.05). BMI of groups was also statistically insignificant (p > 0.05). Chronic disease distribution was found to be statistically insignificant between the groups (p > 0.05). VA score of the femoral group (6.32 ± 1.70) was significantly higher than the radial group (5.15 ± 1.98) (p < 0.05). In the sampling difficulty groups, patients whose samplings were difficult were significantly higher than other groups (p < 0.05). CONCLUSIONS: In conclusion, VA score for femoral arterial puncture is higher than a radial arterial puncture. In other words, taking blood gas with the radial arterial procedure is less painful than the femoral arterial pro- cedure. In addition, hypertension is also an important factor affecting pain after arterial puncture with VAS.

The ORC/Cdc6/MCM2-7 complex facilitates MCM2-7 dimerization during prereplicative complex formation.

The replicative mini-chromosome-maintenance 2-7 (MCM2-7) helicase is loaded in Saccharomyces cerevisiae and other eukaryotes as a head-to-head double-hexamer around origin DNA. At first, ORC/Cdc6 recruits with the help of Cdt1 a single MCM2-7 hexamer to form an 'initial' ORC/Cdc6/Cdt1/MCM2-7 complex. Then, on ATP hydrolysis and Cdt1 release, the 'initial' complex is transformed into an ORC/Cdc6/MCM2-7 (OCM) complex. However, it remains unclear how the OCM is subsequently converted into a MCM2-7 double-hexamer. Through analysis of MCM2-7 hexamer-interface mutants we discovered a complex competent for MCM2-7 dimerization. We demonstrate that these MCM2-7 mutants arrest during prereplicative complex (pre-RC) assembly after OCM formation, but before MCM2-7 double-hexamer assembly. Remarkably, only the OCM complex, but not the 'initial' ORC/Cdc6/Cdt1/MCM2-7 complex, is competent for MCM2-7 dimerization. The MCM2-7 dimer, in contrast to the MCM2-7 double-hexamer, interacts with ORC/Cdc6 and is salt-sensitive, classifying the arrested complex as a helicase-loading intermediate. Accordingly, we found that overexpression of the mutants cause cell-cycle arrest and dominant lethality. Our work identifies the OCM complex as competent for MCM2-7 dimerization, reveals MCM2-7 dimerization as a limiting step during pre-RC formation and defines critical mechanisms that explain how origins are licensed

Optical properties of GaS crystals: Combined study of temperature- dependent band gap energy and oscillator parameters

    Get PDF
Optical parameters of gallium sulfide (GaS) layered single crystals have been found through temperature-dependent transmission and room temperature reflection experiments in the wavelength range of 400-1100 nm. Experimental data demonstrates the coexistence of both optical indirect and direct transitions and the shift of the absorption edges toward lower energies by increasing temperature in the range of 10-300 K. Band gap at zero temperature, average phonon energy and electron phonon coupling parameter for indirect and direct band gap energies have been obtained from the analyses of temperature dependences of band gap energies. At high temperatures kT≫ 〈Eph〉, rates of band gap energy change have been found as 0.56 and 0.67 meV/K for Egi and Egd, respectively. Furthermore, the dispersion of refractive index has been discussed in terms of the Wemple–DiDomenico single effective oscillator model. The refractive index dispersion parameters, namely oscillator and dispersion energies, oscillator strength and zero-frequency refractive index, have been found to be 4.48 eV, 24.8 eV, 6.99×1013 m−2 and 2.56, respectively. The results of the present work will provide an important contribution to the research areas related to the characterization and optoelectronic device fabrication using GaS layered crystals

Background: High-quality chest compression is one of the key elements of resuscitation to return of spontaneous circulation. In the COVID-19 era, medical personnel should wear personal protective equipment (PPE) against aerosol generating procedures (AGP) during resuscitation. However, the use of this personal protection equipment may reduce the effectiveness of medical procedures performed. Objective: We aimed to compare chest compression quality parameters between standard manual chest compression and chest compression with TrueCPR feedback device performed by medical students wearing full personal protractive equipment against aerosol generating procedure. Methods: The study was designed as a randomized, cross-over, single-blinded simulation study. Thirty-two medical students wearing PPE-AGP performed 2-min continuous chest compression on an adult simulator with and without TrueCPR feedback device. Results: Median chest compression depth with and without TrueCPR feedback device varied and amounted to 46 (IQR; 42-53) vs. 41 (IQR; 36-45) mm (MCC vs. TrueCPR, respectively). The manual chest compression rate was 117 (IQR; 112-125) compressions per minute (CPM) and was higher than with TrueCPR feedback device - 107 (IQR; 102- 115; p = 0.017). Full chest relaxation in the manual's chest compression technique (without TrueCPR) was 33 (IQR; 26-42)% and was lower than with chest compression with TrueCPR feedback device - 58 (IQR; 40-75)% (p=0.002). Conclusions: We conclude that a TrueCPR feedback device improves chest compression quality during simulated COVID-19 resuscitation performed by medical students wearing PPE-AGP

International Specialization in Research & Development

In this paper, I examine the effects of implementing tighter Intellectual Property Rights in a model of International Trade. In my model, firms in different countries have the choice of committing their resources to introducing new products (product innovation) or to imitating and improving upon current products (process innovation). I analyze the impact of stronger patents on innovation decisions, overall welfare and the distribution of welfare among countries. I show that, depending on parameter values, firms in developed countries (North) may altogether specialize in product innovation or may attain incomplete specialization in the sense that some innovate and some imitate. Welfare analysis will depend on the degree of specialization. In the case of incomplete specialization, tighter IPRs increase the incentives for product innovation in the North but, at the same time, increase the imitation done in the South. This finding is contrary to the conventional argument that states the reverse for imitation rates. In the case of complete specialization, stronger patents do not affect the rate of product innovation but reduce the rate of imitation, and welfare is nonmonotonic in IPRs. Finally, I examine the case of Foreign Direct Investment (FDI) and predict that stronger patents will increase the FDI while lowering the wages worldwide
