    Pachyonychia congenita type I with severe oral leukokeratosis

    Pachyonychia Congenita (PC) is a rare autosomal dominant keratin disorder that affects a number of ectodermal structures including the nails and palmoplantar skin, and often involves the oral mucosa, tongue, larynx, teeth and hair. Clinical features are usually present at birth or early infancy. There are two main subtypes of PC. Fingernail thickening and oral keratosis are more common and severe in PC-1 and cystic lesions, hair abnormalities, natal teeth and pili torti are more common in PC-2. We report the case of a 6-year-old boy with PC-1 presenting with severe and painful oral leukokeratosis and extensive caries

    The temporal analysis of light pollution in Turkey using VIIRS data

    © 2021, The Author(s), under exclusive licence to Springer Nature B.V.Artificial Light pollution (AL) in Turkey and in Turkish observatories between 2012–2020 have been studied using the archival data of Visible Infrared Imaging Radiometer Suite (VIIRS) instrument. The astroGIS database has been used in processing the data (astrogis.org) Aksaker et al. (2020a). The total energy released to space from Turkey increased by 80% in 2019 with respect to 2012. In the span of the dataset, a steady and continuous increase has been observed throughout all cities of the country. On the other hand, Dark Sky Park locations, East and Southeast Anatolian regions and mostly rural areas around the cities kept their AL level constant. Four demographic parameters have been studied and they were found to be correlated very well with AL: Population (R≃ 0.90); GDP (R≃ 0.87); Total Power Consumption (R≃ 0.66) and Outdoor Lightening (R≃ 0.67). Contrary to countries acting to prevent AL increases, Turkey seems to be at the beginning of an era where AL will arithmetically increase throughout the country and enormous amount of energy will continuously escape to space and therefore will be wasted. Therefore, a preventive legislation, especially for invaluable astronomical site locations such as TURAG, TUG, DAG and ÇAAM where each is counted as a truly dark site due to their SQM values, has to be enacted in Turkey, in very near future

    A Case study of light pollution in France after the change in legislation

    France issued a decree to restrict and prohibit mainly outdoor lighting effective from January 1st, 2019. Effectiveness of this legislation has been evaluated in this study using GIS data which was first used in \cite{2020MNRAS.493.1204A} (so called astroGIS database - \url{astrogis.org}). A subset of Artificial Light layer of astroGIS database has been adapted for years between January 2012 and December 2019. During 2019, radiance of 1.9×1091.9 \times 10^{9} W cm2^{-2} sr1^{-1} has been released into space. Annual light pollution in France decreased by 6\% after the enactment of artificial light legislation. France continue to have potential Dark Sky Park locations for example cities like Indre, Lot, Nievre and Creuse having the lowest light pollution values. A strong correlation between population and light pollution (R0.83R\simeq 0.83) has been observed. A similar but a weak correlation can also be observed for GDP (R0.28R\simeq 0.28).     Investigation of efficacy of mitomycin-C, Sodium hyaluronate and human amniotic fluid in preventing epidural fibrosis and adhesion using a rat laminectomy model

    Study Design: A retrospective study. Purpose: The aim of this study was to evalute the effects of mitomycin-C, sodium hyaluronate and human amniotic fluid on preventing spinal epidural fibrosis. Overview of Literature: The role of scar tissue in pain formation is not exactly known, but it is reported that scar tissue causes adhesions between anatomic structures. Intensive fibrotic tissue compresses on anatomic structures and increases the sensitivity of the nerve root for recurrent herniation and lateral spinal stenosis via limiting movements of the root. Also, neuronal atrophy and axonal degeneration occur under scar tissue. Methods: The study design included 4 groups of rats: group 1 was the control group, groups 2, 3, and 4 receieved antifibrotic agents, mitomycin-C (group 2), sodium hyaluronate (group 3), and human amniotic fluid (group 4). Midline incision for all animals were done on L5 for total laminectomy. Four weeks after the surgery, the rats were sacrificed and specimens were stained with hematoxylineosin and photos of the slides were taken for quantitive assesment of the scar tissue. Results: There was no significant scar tissue in the experimental animals of groups 2, 3, and 4. It was found that there was no significant difference between drug groups, but there was a statistically significant difference between the drug groups and the control group. Conclusions: This experimental study shows that implantation of mitomycin-C, sodium hyaluronate and human amniotic fluid reduces epidural fibrosis and adhesions after spinal laminectomy in rat models. Further studies in humans are needed to determine the complications of the agents researched. © 2013 by Korean Society of Spine Surgery

    Global Site Selection for Astronomy

    A global site selection for astronomy was performed with 1 km spatial resolution (\sim 1 Giga pixel in size) using long term and up-to-date datasets to classify the entire terrestrial surface on the Earth. Satellite instruments are used to get the following datasets of Geographical Information System (GIS) layers: Cloud Coverage, Digital Elevation Model, Artificial Light, Precipitable Water Vapor, Aerosol Optical Depth, Wind Speed and Land Use -- Land Cover. A Multi Criteria Decision Analysis (MCDA) technique is applied to these datasets creating four different series where each layer will have a specific weight. We introduce for the first time a ``Suitability Index for Astronomical Sites'' namely, SIAS. This index can be used to find suitable locations and to compare different sites or observatories. Mid-western Andes in South America and Tibetan Plateau in west China were found to be the best in all SIAS Series. Considering all the series, less than 3 \% of all terrestrial surfaces are found to be the best regions to establish an astronomical observatory. In addition to this, only approximately 10 \% of all current observatories are located in good locations in all SIAS series. Amateurs, institutions or countries aiming to construct an observatory could create a short-list of potential site locations using layout of SIAS values for each country without spending time and budget.The outcomes and datasets of this study has been made available through a web site, namely ``Astro GIS Database'' on \texttt{\url{www.astrogis.org}}.Comment: 19 Pages, 4 Figures, 7 tables, Accepted for publication in MNRA

    An evaluation of three DoE-guided meta-heuristic-based solution methods for a three-echelon sustainable distribution network

    This article evaluates the efficiency of three meta-heuristic optimiser (viz. MOGA-II, MOPSO and NSGA-II)-based solution methods for designing a sustainable three-echelon distribution network. The distribution network employs a bi-objective location-routing model. Due to the mathematically NP-hard nature of the model a multi-disciplinary optimisation commercial platform, modeFRONTIER®, is adopted to utilise the solution methods. The proposed Design of Experiment (DoE)-guided solution methods are of two phased that solve the NP-hard model to attain minimal total costs and total CO2 emission from transportation. Convergence of the optimisers are tested and compared. Ranking of the realistic results are examined using Pareto frontiers and the Technique for Order Preference by Similarity to Ideal Solution approach, followed by determination of the optimal transportation routes. A case of an Irish dairy processing industry’s three-echelon logistics network is considered to validate the solution methods. The results obtained through the proposed methods provide information on open/closed distribution centres (DCs), vehicle routing patterns connecting plants to DCs, open DCs to retailers and retailers to retailers, and number of trucks required in each route to transport the products. It is found that the DoE-guided NSGA-II optimiser based solution is more efficient when compared with the DoE-guided MOGA-II and MOPSO optimiser based solution methods in solving the bi-objective NP-hard three-echelon sustainable model. This efficient solution method enable managers to structure the physical distribution network on the demand side of a logistics network, minimising total cost and total CO2 emission from transportation while satisfying all operational constraints

    Comprehensive dissection of prevalence rates, sex differences, and blood level-dependencies of clozapine-associated adverse drug reactions

    Clozapine is often underused due to concerns about adverse drug reactions (ADRs) but studies into their prevalences are inconclusive. We therefore comprehensively examined prevalences of clozapineassociated ADRs in individuals with schizophrenia and demographic and clinical factors associated with their occurrence. Data from a multi-center study (n=698 participants) were collected. The mean number of ADRs during clozapine treatment was 4.8, with 2.4% of participants reporting no ADRs. The most common ADRs were hypersalivation (74.6%), weight gain (69.3%), and increased sleep necessity (65.9%), all of which were more common in younger participants. Participants with lower BMI prior to treatment were more likely to experience significant weight gain (>10%). Constipation occurred more frequently with higher clozapine blood levels and doses. There were no differences in ADR prevalence rates between participants receiving clozapine monotherapy and polytherapy. These findings emphasize the high prevalence of clozapine-associated ADRs and highlight several demographic and clinical factors contributing to their occurrence. By understanding these factors, clinicians can better anticipate and manage clozapine-associated ADRs, leading to improved treatment outcomes and patient well-being
