    THE RELATIONSHIP OF POSTTRAUMATIC STRESS DISORDER WITH CHILDHOOD TRAUMAS, PERSONALITY CHARACTERISTICS, DEPRESSION AND ANXIETY SYMPTOMS IN PATIENTS WITH DIAGNOSIS OF MIXED ANXIETY-DEPRESSION DISORDER

    Background: The purpose of the current study is to investigate the relationship of symptoms of Posttraumatic Stress Disorder (PTSD) to childhood trauma and personality characteristics among patients with the diagnosis of mixed anxiety-depression disorder. Subjects and methods: A total of 130 patients who were admitted to the psychiatry outpatient and were diagnosed with mixed anxiety-depression disorder were conveniently sampled in the current study. In order for the researchers to complete the assessment, the patients with a history of at least one traumatic experience (40.8%, n=53) were asked to complete a socio-demographic form, the Beck Anxiety Inventory and Beck Depression Inventory, the Childhood Trauma Questionnaire-Short Form, the Posttraumatic Stress Disorder Checklist-Civilian Version, and the Eysenck Personality Questionnaire Revised/Abbreviated Form. Results: One hundred thirty patients participated in the current study. In the study, 40.8 % of the patients (n=53) reported that they had been exposed to at least one traumatic event during their lifetime. Patients with a history of at least one traumatic experience (40.8%, n=53) were separated into two groups: those with a diagnosis of PTSD (n=21) (39.6%) and those without PTSD (n=32) (60.4%) (cut off score is 50 and above). The mean comparisons concluded that the group with PTSD and the group without PTSD were significantly different from each other in terms of depression and anxiety symptom severity, neuroticism, and history of physical abuse and neglect. Spearman’s correlation analysis revealed that PTSD severity was significantly and positively correlated with neuroticism, and with depression and anxiety symptom severity, as the scores were significantly and negatively correlated with extraversion. The two-step logistic analysis first revealed the severity of the depression symptom, and secondly, physical abuse significantly predicted PTSD in the PTSD group. Conclusions: In patients with anxiety and/or depressive disorder, PTSD is one of the most frequently-observed comorbid psychiatric disorders. The current study revealed that a history of childhood trauma, neuroticism, and introversion can be considered risk factors for PTSD among patients with mixed anxiety-depression disorder in adulthood. Therefore, it should be taken into consideration that those neurotic and introverted patients with childhood trauma history could be more at risk for PTSD

    Heart rate turbulence analysis in female patients with fibromyalgia

    OBJECTIVE: Fibromyalgia is characterized by diffuse musculoskeletal pain and discomfort. There are several reports regarding autonomic nervous system dysfunction in patients with fibromyalgia. Heart rate turbulence is expressed as ventriculophasic sinus arrhythmia and has been considered to reflect cardiac autonomic activity. Heart rate turbulence has been shown to be an independent and powerful predictor of sudden cardiac death in various cardiac abnormalities. The aim of this study is to determine whether heart rate turbulence is changed in female patients with fibromyalgia compared with healthy controls. METHODS: Thirty-seven female patients (mean age, 40±11 years) with fibromyalgia, and 35 age- and sex-matched healthy female control subjects (mean age, 42±9 years) were included. Twenty-four hours of ambulatory electrocardiography recordings were collected for all subjects, and turbulence onset and turbulence slope values were automatically calculated. RESULTS: The baseline clinical characteristics of the two groups were similar. There were no significant differences in turbulence onset and turbulence slope measures between patients and control subjects (turbulence onset: −1.648±1.568% vs. −1.582±1.436%, p ϝ 0.853; turbulence slope: 12.933±5.693 ms/RR vs. 13.639±2.505 ms/RR, p ϝ 0.508). Although body mass index was negatively correlated with turbulence slope (r ϝ −0.258, p ϝ 0.046), no significant correlation was found between body mass index and turbulence onset (r ϝ 0.228, p ϝ 0.054). CONCLUSION: To the best of our knowledge, this is the first study to evaluate heart rate turbulence in patients with fibromyalgia. It appears that heart rate turbulence parameters reflecting cardiac autonomic activity are not changed in female patients with fibromyalgia

    Relationship of Internet addiction with impulsivity and severity of psychopathology among Turkish university students

    The previous studies have found a relationship between IA and both impulsivity and psychopathology when they were considered separately. The aim of this study was to investigate the relationship of Internet addiction (IA) with impulsivity and severity of psychopathology among Turkish university students. We also wanted to control the effect of impulsivity dimensions on the relationship between IA and psychopathology. A total of 319 university students from two universities in Ankara participated to the study. Students were assessed through the Internet Addiction Scale (IAS), the Symptom Checklist-Revised (SCL-90-R) and the Barratt Impulsiveness Scale-11 (BIS-11). Correlational analyses revealed that severity of IA was related to both SCL-90-R and BIS-11 scores. Among SCL-90-R subscales, severity of obsessive-compulsive symptoms (OCS) was the only predictor for IAS score. Hierarchical regression analysis indicated that interpersonal sensitivity, additional to attentional and motor impulsiveness, was the predictor of IAS score. Although severity of IA is associated with wide range of psychopathology, particularly OCS, interpersonal sensitivity seems to be the main dimension that predict severity of IAS additional to impulsiveness (attentional and motor). Impulsivity seems to be an important construct when considering IA and its treatment among Turkish university students. (C) 2013 Elsevier Ireland Ltd. All rights reserved

    A subset of patients with acquired partial lipodystrophy developing severe metabolic abnormalities

    ###EgeUn###Purpose/Aim of the study: Acquired partial lipodystrophy (APL) is a rare disease characterized by selective loss of adipose tissue. In this study, we aimed to present a subset of patients with APL, who developed severe metabolic abnormalities, from our national lipodystrophy registry. Materials and Methods: Severe metabolic abnormalities were defined as: poorly controlled diabetes (HbA1c above 7% despite treatment with insulin more than 1 unit/kg/day combined with oral antidiabetics), severe hypertriglyceridemia (triglycerides above 500 mg/dL despite treatment with lipid-lowering drugs), episodes of acute pancreatitis, or severe hepatic involvement (biopsy-proven non-alcoholic steatohepatitis (NASH)). Results: Among 140 patients with all forms of lipodystrophy (28 with APL), we identified 6 APL patients with severe metabolic abnormalities. The geometric mean for age was 37 years (range: 27-50 years; 4 females and 2 males). Five patients had poorly controlled diabetes despite treatment with high-dose insulin combined with oral antidiabetics. Severe hypertriglyceridemia developed in five patients, of those three experienced episodes of acute pancreatitis. Although all six patients had hepatic steatosis at various levels on imaging studies, NASH was proven in two patients on liver biopsy. Our data suggested that APL patients with severe metabolic abnormalities had a more advanced fat loss and longer disease duration. Conclusions: We suggest that these patients represent a potential subgroup of APL who may benefit from metreleptin or investigational therapies as standard treatment strategies fail to achieve a good metabolic control

    Natural History Of Congenital Generalized Lipodystrophy: A Nationwide Study From Turkey

    Context: Congenital generalized lipodystrophy (CGL) is a rare autosomal recessive disorder characterized by near-total lack of body fat. Objective: We aimed to study natural history and disease burden of various subtypes of CGL. Design: We attempted to ascertain nearly all patients with CGL in Turkey. Setting: This was a nationwide study. Patients or Other Participants: Participants included 33 patients (22 families) with CGL and 30 healthy controls. Main Outcome Measure(s): We wanted to ascertain genotypes by sequencing of the known genes. Whole-body magnetic resonance imaging was used to investigate the extent of fat loss. Metabolic abnormalities and end-organ complications were measured on prospective follow-up. Results: Analysis of the AGPAT2 gene revealed four previously reported and four novel mutations (CGL1; c.144C>A, c.667_705delinsCTGCG, c.268delC, and c.316 + 1G > T). Analysis of the BSCL2 gene revealed four different homozygous and one compound heterozygous possible disease-causing mutations (CGL2), including four novel mutations (c.280C > T, c. 631delG, c. 62A > T, and c. 465-468delGACT). Two homozygous PTRF mutations (c.481-482insGTGA and c. 259C > T) were identified (CGL4). Patients with CGL1 had preservation of adipose tissue in the palms, soles, scalp, and orbital region, and had relatively lower serum adiponectin levels as compared to CGL2 patients. CGL4 patients had myopathy and other distinct clinical features. All patients developed various metabolic abnormalities associated with insulin resistance. Hepatic involvement was more severe in CGL2. End-organ complications were observed at young ages. Two patients died at age 62 years from cardiovascular events. Conclusions: CGL patients from Turkey had both previously reported and novel mutations of the AGPAT2, BSCL2, and PTRF genes. Our study highlights the early onset of severe metabolic abnormalities and increased risk of end-organ complications in patients with CGL.WoSScopu
