    Doctors\u27 perspectives on PSA testing illuminate established differences in prostate cancer screening rates between Australia and the UK: A qualitative study

    Objectives: To examine how general practitioners (GPs) in the UK and GPs in Australia explain their prostate-specific antigen (PSA) testing practices and to illuminate how these explanations are similar and how they are different. Design: A grounded theory study. Setting: Primary care practices in Australia and the UK. Participants: 69 GPs in Australia (n=40) and the UK (n=29). We included GPs of varying ages, sex, clinical experience and patient populations. All GPs interested in participating in the study were included. Results: GPs’ accounts revealed fundamental differences in whether and how prostate cancer screening occurred in their practice and in the broader context within which they operate. The history of prostate screening policy, organisational structures and funding models appeared to drive more prostate screening in Australia and less in the UK. In Australia, screening processes and decisions were mostly at the discretion of individual clinicians, and varied considerably, whereas the accounts of UK GPs clearly reflected a consistent, organisationally embedded approach based on local evidence-based recommendations to discourage screening. Conclusions: The GP accounts suggested that healthcare systems, including historical and current organisational and funding structures and rules, collectively contribute to how and why clinicians use the PSA test and play a significant role in creating the mindlines that GPs employ in their clinic. Australia’s recently released consensus guidelines may support more streamlined and consistent care. However, if GP mindlines and thus routine practice in Australia are to shift, to ultimately reduce unnecessary or harmful prostate screening, it is likely that other important drivers at all levels of the screening process will need to be addressed

    Long-term flood controls on semi-arid river form: evidence from the Sabie and Olifants rivers, eastern South Africa

    Rivers in the Kruger National Park, eastern South Africa, are characterised by bedrock-influenced ‘macrochannels’ containing variable alluvial thicknesses and riparian vegetation assemblages. Evidence from the Sabie and Olifants rivers suggests that flows up to moderate floods (<3500 m3 s-1) tend to result in net alluviation, with sediments gradually covering the underlying bedrock. More extreme floods strip alluvium and erode bedrock, effectively exerting the primary control over long-term river morphologic development. On the Olifants River, post-flood aerial LIDAR imagery reveals that the 2012 extreme flood (~14000 m3 s-1) resulted in extensive stripping of stored alluvial sediment, exposing and eroding the underlying weathered bedrock. On the Sabie River, preliminary optically stimulated luminescence ages for remnant alluvium are all less than 1000 years, highlighting typical timescales of sediment storage. Together, these results suggest that while periods of general alluviation occur on these systems, long-term river development results from extreme flood-generated bedrock erosion

    Many of the large rivers in southern Africa exhibit a strong bedrock influence, being characterised by a channel incised 10–20 m into ancient planation surfaces. Under alluviated conditions, these channels display downstream sequences of channel types, including alluvial single thread, braided, bedrock anastomosed, mixed anastomosed, and pool-rapid. This sequence of channel types has been reviewed using aerial imagery for the Sabie River, which drains a 6320km2 catchment and flows across the Lowveld of South Africa. Prior to 2000, the river exhibited a downstream sequence of channel types that broadly alternated from alluvial single thread or braided to bedrock anstomosed or mixed anastomosed, with pool-rapid types also present locally. Unconsolidated (predominantly sandy) alluvial sediments were significantly eroded by cyclone-driven extreme floods both in 2000 and 2012, exposing the underlying bedrock template along considerable lengths of the river. This bedrock-dominated state was surveyed using aerial LIDAR following the 2012 flood. Long profile data revealed the strong gradient control exerted by the bedrock anastomosed and mixed anastomosed channel types, which creates hydraulic conditions suitable for deposition in the upstream alluvial reaches. The role of these bedrock-influenced channel types on hydraulic character is also revealed in the results of 2D hydraulic modelling of moderate flood (3500 m3s-1) as the bedrock or mixed anastomosed channel type is drowned out, resulting in dramatically increased velocities along the entire river and a general stripping of unconsolidated and consolidated sediments regardless of initial channel type or location

    Interpersonal relationships are vital to our well-being. In recent years, it has become increasingly common to seek relationship help through anonymous online platforms. Accordingly, we conducted a large-scale analysis of real-world relationship help-seeking to create a descriptive overview of the nature and substance of online relationship help-seeking. By analyzing the demographic characteristics and language of relationship help-seekers on Reddit (N = 184,631), we establish the first-ever big data analysis of relationship help-seeking and relationship problems in situ among the general population. Our analyses highlight real-world relationship struggles found in the general population, extending beyond past work that is typically limited to counseling/intervention settings. We find that relationship problem estimates from our sample are closer to those found in the general population, providing a more generalized insight into the distribution and prevalence of relationship problems as compared with past work. Further, we find several meaningful associations between relationship help-seeking behavior, gender, and attachment. Notably, numerous gender differences in help-seeking and romantic attachment emerged. Our findings suggest that, contrary to more traditional contexts, men are more likely to seek help with their relationships online, are more expressive of their emotions (e.g., discussing the topic of "heartache"), and show language patterns generally consistent with more secure attachment. Our analyses highlight pathways for further exploration, providing even deeper insights into the timing, lifecycle, and moderating factors that influence who, what, why, and how people seek help for their interpersonal relationships

    Supporting the learning of deaf students in higher education: a case study at Sheffield Hallam University

    This article is an examination of the issues surrounding support for the learning of deaf students in higher education (HE). There are an increasing number of deaf students attending HE institutes, and as such provision of support mechanisms for these students is not only necessary but essential. Deaf students are similar to their hearing peers, in that they will approach their learning and require differing levels of support dependant upon the individual. They will, however, require a different kind of support, which can be technical or human resource based. This article examines the issues that surround supporting deaf students in HE with use of a case study of provision at Sheffield Hallam University (SHU), during the academic year 1994-95. It is evident that by considering the needs of deaf students and making changes to our teaching practices that all students can benefit

    Approaches to study in higher education portuguese students: a portuguese version of the Approaches and Study Skills Inventory for Students (ASSIST)

    This paper examines the validity of the Approaches and Study Skills Inventory for Students—short version (ASSIST; Tait et al. in Improving student learning: Improving students as learners, 1998), to be used with Portuguese undergraduate students. The ASSIST was administrated to 566 students, in order to analyse a Portuguese version of this inventory. Exploratory factor analysis (principal axis factor analysis followed by direct oblimin rotation) reproduced the three main factors that correspond to the original dimensions of the inventory (deep, surface apathetic and strategic approaches to learning). The results are consistent with the background theory on approaches to learning. Additionally, the reliability analysis revealed acceptable internal consistency indexes for the main scales and subscales. This inventory might represent a valuable research tool for the assessment of approaches to learning among Portuguese higher education students
