    Prosthetic joint infection. A relevant public health issue.

    Prosthetic joint infection (PJI) is a common complication of the knee and hip arthroplasty and represents a huge challenge for physicians. PJI raises serious social, economic and clinical concerns in the public health that need a comprehensive approach to better focus on proven strategies for disease prevention and treatment. History and clinical signs on joint site are useful means for suspecting PJI that need to be confirmed through major and minor diagnostic criteria. The pathogen isolation and the resulting antibiogram are crucial to guide the correct antibiotic strategy and together with surgical treatment (prosthesis revision and spacer implantation) represent the cornerstones to eradicate the infection before attempting a new arthroplasty. External fixator with removal of the spacer may be an option before performing a new arthroplasty when the infection does not heal. Arthrodesis may also be considered if the arthroplasty is contraindicated. Limb amputation is the last chance when pathogen eradication failed and might lead to life-threatening situations

    Cirrhotic Cardiomyopathy

    Cirrhotic cardiomyopathy is a clinical syndrome in patients with liver cirrhosis characterized by an abnormal and blunted response to physiologic, pathologic, or pharmacologic stress but normal to increased cardiac output and contractility at rest. As many as 50% of cirrhotic patients undergoing liver transplantation show signs of cardiac dysfunction, and 7% to 21% of deaths after orthotopic liver transplantation result from overt heart failure.     Isolated hepatocytes versus hepatocyte spheroids: in vitro culture of rat hepatocytes.

    The use of hepatocytes that express liver-specific functions to develop an artificial liver is promising. Unfortunately, the loss of specialized liver functions (dedifferentiation) is still a major problem. Different techniques, such as collagen entrapment, spherical multicellular aggregates (spheroids), and coculture of hepatocytes with extracellular matrix, have been used to improve the performance of hepatocytes in culture. The aim of this study was to compare two different models of hepatocyte isolation in culture: isolated hepatocytes (G1) and hepatocyte spheroids (60% hepatocytes, 40% nonparenchymal cells, and extracellular matrix) (G2). To test functional activity of hepatocytes, both synthetic and metabolic, production of albumin and benzodiazepine transformation into metabolites was tested. G2 showed a high albumin secretion, while a decrease after 15 days of culture in G1 was noted. Diazepam metabolites were higher in G2 than in G1 in all samples, but had statistical significance at days 14 and 21 (p < 0.01). The glycogen content, after 30 days of culture, was very low in G1 (14.2 ± 4.4%), while in G2 it was 72.1 ± 2.6% (p < 0.01). Our study confirms the effectiveness of a culture technique with extracellular matrix and nonparenchymal cells. Maintenance of a prolonged functional activity has been related to restoration of cell polarity and close cell-to-cell contact. We showed that isolated hepatocytes maintain their functional activity for a period significantly reduced, when compared to the hepatocyte spheroids. We confirmed the role of extracellular matrix as a crucial component to promote hepatocyte homeostasis, and the close link between cellular architecture and tissue-specific functions

    Correlation Analysis of Evapotranspiration, Emissivity Contrast and Water Deficit Indices: A Case Study in Four Eddy Covariance Sites in Italy with Different Environmental Habitats

    Evapotranspiration (ET) represents one of the essential processes controlling the exchange of energy by terrestrial vegetation, providing a strong connection between energy and water fluxes. Different methodologies have been developed in order to measure it at different spatial scales, ranging from individual plants to an entire watershed. In the last few years, several methods and approaches based on remotely sensed data have been developed over different ecosystems for the estimation of ET. In the present work, we outline the correlation between ET measured at four eddy covariance (EC) sites in Italy (situated either in forest or in grassland ecosystems) and (1) the emissivity contrast index (ECI) based on emissivity data from thermal infrared spectral channels of the MODIS and ASTER satellite sensors (CAMEL data-set); (2) the water deficit index (WDI), defined as the difference between the surface and dew point temperature modeled by the ECMWF (European Centre for Medium-Range Weather Forecasts) data. The analysis covers a time-series of 1 to 7 years depending on the site. The results showed that both the ECI and WDI correlate to the ET calculated through EC. In the relationship WDI-ET, the coefficient of determination ranges, depending on the study area, between 0.5 and 0.9, whereas it ranges between 0.5 and 0.7 when ET was correlated to the ECI. The slope and the sign of the latter relationship is influenced by the vegetation habitat, the snow cover (particularly in winter months) and the environmental heterogeneity of the area (calculated in this study through the concept of the spectral variation hypothesis using Rao’s Q heterogeneity index)

    Pancreatitis after percutaneous ethanol injection into HCC: a minireview of the literature

    Deaths after percutaneous ethanol injection (PEI) into hepatocellular carcinoma (HCC) may occur within a few hours to a few days following the procedure because of hemoperitoneum and haemorrhage from oesophageal varices or hepatic insufficiency. Pancreatitis has been recently reported as a rare lethal complication of intra-arterial PEI, another modality for treating HCCs. In this minireview, we analyze the literature concerning the development of acute pancreatitis after PEI. Pathogenesis of pancreatitis from opioids and ethanol is also addressed. Treatment with opioids to reduce the patient's abdominal pain after PEI in combination with the PEI itself may lead to direct toxic effects, thus favouring the development of pancreatitis
