    One Instructional Leadership Team’s Journey through a Professional Learning Cycle

    Schools often set a large number of goals, making it difficult to plan, support, monitor, and match strategic levers in a systematic way. This study narrates the actions and findings of one instructional leadership team as they implement an eight week cycle of professional learning around a single focus. Scripted observations of teachers and students, achievement data, student work samples, as well as exit slips with action items recount the team members’ perspectives and experiences. The team chronicled its journey, which proved to be one of inquiry, trial and error, reflection and retooling. The study closes with highlighting the changes in both teachers’ instructional practices and school-wide improvement systems, the challenges and benefits, and the researcher’s interpretations and implications for school staffs

    Functional Mobility Testing: A Novel Method to Establish Human System Interface Design Requirements

    Across all fields of human-system interface design it is vital to posses a sound methodology dictating the constraints on the system based on the capabilities of the human user. These limitations may be based on strength, mobility, dexterity, cognitive ability, etc. and combinations thereof. Data collected in an isolated environment to determine, for example, maximal strength or maximal range of motion would indeed be adequate for establishing not-to-exceed type design limitations, however these restraints on the system may be excessive over what is basally needed. Resources may potentially be saved by having a technique to determine the minimum measurements a system must accommodate. This paper specifically deals with the creation of a novel methodology for establishing mobility requirements for a new generation of space suit design concepts. Historically, the Space Shuttle and the International Space Station vehicle and space hardware design requirements documents such as the Man-Systems Integration Standards and International Space Station Flight Crew Integration Standard explicitly stated that the designers should strive to provide the maximum joint range of motion capabilities exhibited by a minimally clothed human subject. In the course of developing the Human-Systems Integration Requirements (HSIR) for the new space exploration initiative (Constellation), an effort was made to redefine the mobility requirements in the interest of safety and cost. Systems designed for manned space exploration can receive compounded gains from simplified designs that are both initially less expensive to produce and lighter, thereby, cheaper to launch

    Use of Traditional and Novel Methods to Evaluate the Influence of an EVA Glove on Hand Performance

    The gloved hand is one of an astronaut s primary means of interacting with the environment, and any restrictions imposed by the glove can strongly affect performance during extravehicular activity (EVA). Glove restrictions have been the subject of study for decades, yet previous studies have generally been unsuccessful in quantifying glove mobility and tactility. Past studies have tended to focus on the dexterity, strength, and functional performance of the gloved hand; this provides only a circumspect analysis of the impact of each type of restriction on the glove s overall capability. The aim of this study was to develop novel capabilities to provide metrics for mobility and tactility that can be used to assess the performance of a glove in a way that could enable designers and engineers to improve their current designs. A series of evaluations were performed to compare unpressurized and pressurized (4.3 psi) gloved conditions with the ungloved condition. A second series of evaluations were performed with the Thermal Micrometeoroid Garment (TMG) removed. This series of tests provided interesting insight into how much of an effect the TMG has on gloved mobility - in some cases, the presence of the TMG restricted glove mobility as much as pressurization did. Previous hypotheses had assumed that the TMG would have a much lower impact on mobility, but these results suggest that an improvement in the design of the TMG could have a significant impact on glove performance. Tactility testing illustrated the effect of glove pressurization, provided insight into the design of hardware that interfaces with the glove, and highlighted areas of concern. The metrics developed in this study served to benchmark the Phase VI EVA glove and to develop requirements for the next-generation glove for the Constellation program

    Temperature as a Circadian Marker in Older Human Subjects: Relationship to Metabolic Syndrome and Diabetes

    Background: Circadian rhythms are characterized by approximate 24-hour oscillations in physiological and behavioral processes. Disruptions in these endogenous rhythms, most commonly associated with shift work and/or lifestyle, are recognized to be detrimental to health. Several studies have demonstrated a high correlation between disrupted circadian rhythms and metabolic disease. The aim of this study was to determine which metabolic parameters correlate with physiological measures of circadian temperature amplitude (TempAmp) and stability (TempStab). Methods: Wrist skin temperature was measured in 34 subjects (ages 50 to 70, including lean, obese, and diabetic subjects) every 10 minutes for 7 consecutive days. Anthropometric measures and fasting blood draws were conducted to obtain data on metabolic parameters: body mass index, hemoglobin A1C, triglycerides, cholesterol, high-density lipoprotein, and low-density lipoprotein. A history of hypertension and current blood pressure was noted. Results: Analysis of the data indicated a substantial reduction in TempAmp and TempStab in subjects with metabolic syndrome (three or more risk factors). To determine the impact of individual interdependent metabolic factors on temperature rhythms, stepwise multilinear regression analysis was conducted using metabolic syndrome measurements. Interestingly, only triglyceride level was consistently correlated by the analysis. Triglyceride level was shown to contribute to 33% of the variability in TempAmp and 23% of the variability in TempStab. Conclusion: Our results demonstrate that elevated triglycerides are associated with diminished TempAmp and TempStab in human subjects, and triglycerides may serve as a primary metabolic predictor of circadian parameters

    Loss-of-Function Variants in DRD1 in Infantile Parkinsonism-Dystonia

    The human dopaminergic system is vital for a broad range of neurological processes, including the control of voluntary movement. Here we report a proband presenting with clinical features of dopamine deficiency: severe infantile parkinsonism-dystonia, characterised by frequent oculogyric crises, dysautonomia and global neurodevelopmental impairment. CSF neurotransmitter analysis was unexpectedly normal. Triome whole-genome sequencing revealed a homozygous variant (c.110C>A, (p.T37K)) in DRD1, encoding the most abundant dopamine receptor (D1) in the central nervous system, most highly expressed in the striatum. This variant was absent from gnomAD, with a CADD score of 27.5. Using an in vitro heterologous expression system, we determined that DRD1-T37K results in loss of protein function. Structure-function modelling studies predicted reduced substrate binding, which was confirmed in vitro. Exposure of mutant protein to the selective D1 agonist Chloro APB resulted in significantly reduced cyclic AMP levels. Numerous D1 agonists failed to rescue the cellular defect, reflected clinically in the patient, who had no benefit from dopaminergic therapy. Our study identifies DRD1 as a new disease-associated gene, suggesting a crucial role for the D1 receptor in motor control

    Factors perceived to influence risky sexual behaviours among university students in the United Kingdom: a qualitative telephone interview study

    Background In the United Kingdom people under the age of 25 years are at increased risk of contracting sexually transmitted infections. Most university students belong to this age group but little is known about their sexual behaviours. The aim of the study was to explore university students’ perspectives of factors and mechanisms that influence risky sexual behaviours among university students in the United Kingdom. Methods All students at a university in a northern city of England were invited via email to participate in qualitative telephone interviews. Interviews were audio recorded and transcribed verbatim. Framework analytical approach was used. Results Twenty interviews were conducted with a diverse sample of students. The social context of university lifestyle was perceived to affect risky sex through high levels of alcohol consumption, increased sexual opportunities, liberation from moral surveillance and expectations of the stereotypical highly sexually active student. Individual and cultural differences were also perceived to account for some patterns of risky sex with older students, overseas students and religious students perceived to be less likely to engage in risky sex due to academic priorities and a tendency to be more likely to adhere to moral values. Risk denial was also a key factor that led students to engage in risky sex. Poor access to sexual health services including inconvenient opening times, lack of confidentiality and stigma were perceived to contribute to the limited use of sexually transmitted infections testing and contraceptive services. Conclusions Lifestyle, individual and structural factors seem to play an important role in influencing the risky sexual behaviours of university students. Therefore preventive interventions that focus on these factors could be very useful in this sub-population of young people. This study provides useful baseline information that helps us understand how and why some United Kingdom university students engage in risky sexual behaviours that puts them at risk of contracting sexually transmitted infections

    Impact of outpatient neuraminidase inhibitor treatment in patients infected with influenza A(H1N1)pdm09 at high risk of hospitalization: an Individual Participant Data (IPD) meta-analysis

    Background: While evidence exists to support the effectiveness of neuraminidase inhibitors (NAIs) in reducing mortality when given to hospitalized patients with A(H1N1)pdm09 virus infection, the impact of outpatient treatment on hospitalization has not been clearly established. We investigated the impact of outpatient NAI treatment on subsequent hospitalization in patients with A(H1N1)pdm09 virus infection. Methods: We assembled general community and outpatient data from 9 clinical centers in different countries collected between January 2009 and December 2010. We standardized data from each study center to create a pooled dataset and then used mixed-effects logistic regression modeling to determine the effect of NAI treatment on hospitalization. We adjusted for NAI treatment propensity and preadmission antibiotic use, including “study center” as a random intercept to account for differences in baseline hospitalization rate between centers. Results: We included 3376 patients with influenza A(H1N1)pdm09, of whom 3085 (91.4%) had laboratory-confirmed infection. Eight hundred seventy-three patients (25.8%) received outpatient or community-based NAI treatment, 928 of 2395 (38.8%) with available data had dyspnea or respiratory distress, and hospitalizations occurred in 1705 (50.5%). After adjustment for preadmission antibiotics and NAI treatment propensity, preadmission NAI treatment was associated with decreased odds of hospital admission compared to no NAI treatment (adjusted odds ratio, 0.24; 95% confidence interval, 0.20–0.30). Conclusions: In a population with confirmed or suspected A(H1N1)pdm09 and at high risk of hospitalization, outpatient or community-based NAI treatment significantly reduced the likelihood of requiring hospital admission. These data suggest that community patients with severe influenza should receive NAI treatment
