Clinical guidelines for late-onset Pompe disease

meramente paliativo. El desarrollo de la terapia de sustitución con la enzima α-glucosidasa recombinante humana ha
constituido el primer tratamiento especÃfico para esta enfermedad. El objetivo de esta guÃa es servir de referencia en el
manejo de la variedad de inicio tardÃo de la enfermedad de Pompe, es decir, la que aparece después del primer año de
vida. En la guÃa, un grupo de expertos españoles hace recomendaciones especÃficas en cuanto a diagnóstico, seguimiento
y tratamiento de esta enfermedad. En cuanto al diagnóstico, el método de la muestra en sangre seca es imprescindible
como primer paso para el diagnóstico de la enfermedad de Pompe, y el diagnóstico de confirmación de la enfermedad de
Pompe debe realizarse mediante un estudio de la actividad enzimática en muestra lÃquida en linfocitos aislados o mediante
el análisis mutacional del gen de la alfa-glucosidasa. En cuanto al tratamiento de la enfermedad con terapia de sustitución
enzimática, los expertos afirman que es eficaz en la mejorÃa o estabilización de la función motora y pulmonar, y debe

was merely palliative. The development of a recombinant human alpha-glucosidase enzymatic replacement therapy has
become the first specific treatment for this illness. The aim of this guide is to serve as reference for the management of the
late-onset Pompe disease, the type of Pompe disease that develops after one year of age. In the guide a group of Spanish
experts make specific recommendations about diagnosis, follow-up and treatment of this illness. With regard to diagnosis,
the dried blood spots method is essential as the first step for the diagnosis of Pompe disease. The confirmation of the
diagnosis of Pompe disease must be made by means of an study of enzymatic activity in isolated lymphocytes or a
mutation analysis of the alpha-glucosidase gene. With regard to treatment with enzymatic replacement therapy, the
experts say that is effective improving or stabilizating the motor function and the respiratory function and it must be
introduced when the first symptoms attributable to Pompe disease appea

Ventilación no invasiva en cifoescoliosis: estudio comparativo entre aspirador volumétrico y soporte de presión (BIPAP)
El tratamiento de elección de los pacientes con insuficiencia respiratoria crónica secundaria a cifoescoliosis grave es la administración de ventilación no invasiva domiciliaria. El propósito de nuestro estudio ha sido comparar, en un estudio prospectivo, cruzado y aleatorizado, la evolución clÃnica, gasométrica y funcional tras un mes de tratamiento domiciliario con los dos tipos de respirador, asà como evaluar las alteraciones que presentan durante el sueño estos enfermos. Para ello hemos incluido en el estudio a 10 pacientes con insuficiencia respiratoria crónica secundaria a afección restrictiva por cifoescoliosis. Se asignó a cada paciente el primer dispositivo de manera aleatoria y tras un mes de tratamiento en su domicilio se realizó una nueva evaluación clÃnica, funcional y polisomnográfica con el respirador. El mismo protocolo se aplicó con el segundo dispositivo, dejando entre uno y otro 10 dÃas de descanso. En la polisomnografÃa basal observamos un sueño fragmentado, con disminución de las fases profundas de sueño NREM y de sueño REM, asà como un patrón respiratorio con frecuencias muy elevadas que coincidÃan con importantes desaturaciones. En todos los enfermos se objetivó una mejorÃa clÃnica y gasométrica con ambos dispositivos, sin diferencias estadÃsticas entre ellos. En los registros polisomnográficos se observó una disminución significativa en el porcentaje de tiempo transcurrido con saturación de oxÃgeno por debajo del 90% con ambos respiradores respecto al registro basal. En nueve de los 10 pacientes se observó una mejor adaptación y tolerancia a la presión de soporte respecto al respirador volumétrico. En conclusión, en nuestra experiencia la ventilación no invasiva en los pacientes con insuficiencia respiratoria crónica secundaria a cifoescoliosis es igualmente efectiva administrada con respirador volumétrico o con soporte de presión (BIPAP). La respuesta subjetiva y la adaptación al dispositivo parece ligeramente superior para el BIPAP
Practical Recommendations for Diagnosis and Management of Respiratory Muscle Weakness in Late-Onset Pompe Disease
Pompe disease is an autosomal-recessive lysosomal storage disorder characterized by progressive myopathy with proximal muscle weakness, respiratory muscle dysfunction, and cardiomyopathy (in infants only). In patients with juvenile or adult disease onset, respiratory muscle weakness may decline more rapidly than overall neurological disability. Sleep-disordered breathing, daytime hypercapnia, and the need for nocturnal ventilation eventually evolve in most patients. Additionally, respiratory muscle weakness leads to decreased cough and impaired airway clearance, increasing the risk of acute respiratory illness. Progressive respiratory muscle weakness is a major cause of morbidity and mortality in late-onset Pompe disease even if enzyme replacement therapy has been established. Practical knowledge of how to detect, monitor and manage respiratory muscle involvement is crucial for optimal patient care. A multidisciplinary approach combining the expertise of neurologists, pulmonologists, and intensive care specialists is needed. Based on the authors’ own experience in over 200 patients, this article conveys expert recommendations for the diagnosis and management of respiratory muscle weakness and its sequelae in late-onset Pompe disease
Prospective study to evaluate quality of life in amyotrophic lateral sclerosis
Abstract Amyotrophic lateral sclerosis (ALS) is a neurodegenerative rare disease characterized by symptoms and signs in the upper and lower motor neurons, leading to progressive neuro-degeneration and muscle atrophy. Our objective was to analyse the quality of life (QoL) in patients with ALS and compare with general population and with patients with cancer. Prospective study from consecutive ALS patients in one center. In order to assess quality of life, during the first visit three questionnaires were administered: Amyotrophic Lateral Sclerosis Functional Rating Scale (ALSFRS-R), Short Form-36 (SF-36) and EuroQoL 5D (EQ-5D). We compared SF-36 of ALS patients with a reference population (n = 9151), and we compared the EQ-5D index score of ALS patients versus patients with cancer in the same area and in the same period (2015–2018). Between June 2015 and September 2017, 23 were included. The mean age was 65.1 ± 12.6 years and 56.5% were women. Compared with the general population, patients with ALS showed lowest QoL (p < 0.05) in all the dimensions, with a very important impairment in physical function (median: 0; p25-75: 0–10) and physical role (median: 0; p25-75: 0–6.25). In EQ-5D questionnaire, patients with ALS presented an EQ-5D index score of 0.21 ± 0.39 (mean ± standard deviation) with a visual analog scale (VAS) score of 0.32 ± 0.24. Compared with an oncological population, patients with ALS had a worse EQ-5D index score both clinically and statistically (0.21 ± 0.39 vs. 0.77 ± 0.27; p < 0.05). We demonstrate a poorer quality of life in patients with ALS is poor, and clinically and statistically worse than in patients with cancer or general population. New studies need to evaluate the impact of strategies in this population to improve the quality of life
Overexpression of Aquaporin-1 in lung adenocarcinomas and pleural mesotheliomas
Aquaporin-1 (AQP1) is the main water
channel responsible for water transport through many
epithelia and endothelia. The latest evidence pointed
toward an important role of this protein also in gas
permeation, angiogenesis, cell proliferation and
migration. In the present work we studied the expression
of AQP1 by immunohistochemical staining of 92 lung
biopsies from patients diagnosed with a pleuropulmonary
tumor (71 lung and 21 pleural neoplasms).
AQP1 expression was analyzed comparing the results
among the different histological patterns and against 9
control cases (5 parenchyma and 4 healthy pleura). Clear
staining of AQP1 was detected in 39 of the 92 tumors
analyzed. In parenchyma, AQP1 was more frequently
detected in primary lung adenocarcinomas (55%,
P<0.001); in contrast, small cell carcinomas were the
least AQP1 expressive tumors studied (93% of negative
staining, P<0.05). Carcinomas analyzed in pleura
(mesotheliomas and metastatic adenocarcinomas) also
revealed strong expression of AQP1. High expression of
this protein was detected in small capillaries in areas
near or surrounding the tumor, and novel intense AQP1
immunostaining was detected over thicker alveolar walls
in alveoli inside or next to the tumoral tissue regardless
of the tumor type. An important role of AQP1 in tumor
angiogenesis is sustained by the abundant expression of
this protein in the endothelia of tumor capillaries.
Further studies are necessary to elucidate the potential
pathophysiological role of this protein in pleuropulmonary
neoplasms
