    Vitamin D deficiency and insufficiency in patients with chronic kidney disease

    INTRODUÇÃO: Hipovitaminose D é bem documentada em pacientes portadores de doença renal crônica (DRC). Espera-se níveis inferiores em habitantes de regiões não tropicais em relação aos habitantes de regiões tropicais, pela inferição de uma maior exposição solar e maior produção de vitamina D. OBJETIVO: Analisar os níveis séricos de vitamina D, como 25-hidroxivitamina D - 25(OH)D, de 125 pacientes brasileiros portadores de DRC em fase pré-dialítica. MÉTODOS: Foram estudados 125 pacientes (57,4 ± 16,2 anos, 78 brancos e 55,2% homens), com creatinina de 2,67 ± 1,73 mg/dL e o clearance estimado 43,7 ± 34,5 mL/min. O índice de massa corporal era de 27,4 ± 4,7 kg/m² e a circunferência abdominal de 95,0 ± 14,0 cm. O cálcio era de 9,3 ± 0,6 mg/dL, o paratormônio intacto (PTHi) 212,6 ± 221,2 pg/mL e a albumina sérica 4,2 ± 0,6 g/dL. A média de 25(OH)D era de 23,9 ± 10,7 ng/mL. RESULTADOS: Dos 125 pacientes, 92 (72,6%) apresentavam níveis de 25(OH)D < 30 ng/mL, sendo que 65 (52%) apresentavam insuficiência (15-29 ng/mL); 27 (21,5%) apresentavam deficiência (5-14 ng/mL) e apenas um paciente apresentava deficiência severa < 5 ng/mL. Não foram observadas diferenças entre os níveis de 25(OH)D nos pacientes estratificados quanto ao estágio de DRC. Os níveis de 25(OH)D foram maiores nos homens (38,1 ± 20,6 versus 22,4 ± 9,7 ng/ml; p < 0,0001), havendo também uma correlação inversa entre os níveis de 25(OH)D e de PTHi, proteinúria e circunferência abdominal, e uma correlação positiva entre 25(OH)D e cálcio total e albumina sérica. Na análise multivariada, encontrou-se apenas correlação inversa entre 25(OH)D e circunferência abdominal e PTHi. CONCLUSÃO: A despeito de a população do Brasil estar em um clima tropical, a maioria dos pacientes analisados apresentou níveis séricos subótimos de vitamina D, podendo este achado estar relacionado ao desenvolvimento de hiperparatireoidismo

    BACKGROUND: Narcolepsy is a chronic neurological syndrome with prevalence between 0.018% and 0.040% without important ethnic differences. Narcolepsy is characterized by excessive daytime sleepiness and cataplexy. The pathophysiology of the illness is not known; even so it possesses genetic marker (allele HLA DQB1 *0602) and with abnormalities in the neurotransmission of hypocretin has been described in patients with narcolepsy. OBJECTIVES: Resume news discoveries in narcolepsy and show diagnoses and treatment options. METHODS: Bibliographic review. RESULTS AND DISCUSSION: The diagnoses of narcolepsy must be done with The International Classification of Sleep Disorders criteria. The hypocretin dosage is the best exam to confirm diagnose in narcoleptic patients with typical cataplexy. The treatment is carried through with behavior actions and symptomatic drugs that promote the vigil and control the cataplexy. Differential diagnoses as schizophrenia, epilepsy, depression and others sleep disorders need to be eliminated.CONTEXTO: Narcolepsia é uma síndrome neurológica crônica com prevalência entre 0,018% e 0,040% da população, sem diferenciações étnicas importantes. Caracteriza-se por sonolência e cataplexia. A fisiopatologia da doença não é totalmente conhecida, embora possua marcador genético (alelo HLA DQB1 *0602) e anormalidades na neurotransmissão de hipocretina descritos recentemente. OBJETIVOS: Resumir as recentes descobertas na narcolepsia e expor possibilidades diagnósticas e terapêuticas. METODOLOGIA: Revisão bibliográfica. RESULTADOS E DISCUSSÃO: O diagnóstico deve seguir os critérios da Classificação Internacional de Sono. A dosagem de hipocretina é o melhor exame para confirmar narcolepsia em pacientes com cataplexia típica. O tratamento deve ser realizado com medidas comportamentais e drogas sintomáticas que promovem a vigília e controlam a cataplexia.     The perception of sleep quality in kidney transplant patients during the first year of transplantation

    OBJECTIVE: Poor sleep quality is one of the factors that adversely affects patient quality of life after kidney transplantation, and sleep disorders represent a significant cardiovascular risk factor. The objective of this study was to investigate the prevalence of changes in sleep quality and their outcomes in kidney transplant recipients and analyze the variables affecting sleep quality in the first years after renal transplantation. METHODS: Kidney transplant recipients were evaluated at two time points after a successful transplantation: between three and six months (Phase 1) and between 12 and 15 months (Phase 2). The following tools were used for assessment: the Pittsburgh Sleep Quality Index; the quality of life questionnaire Short-Form-36; the Hospital Anxiety and Depression scale; the Karnofsky scale; and assessments of social and demographic data. The prevalence of poor sleep was 36.7% in Phase 1 and 38.3% in Phase 2 of the study. RESULTS: There were no significant differences between patients with and without changes in sleep quality between the two phases. We found no changes in sleep patterns throughout the study. Both the physical and mental health scores worsened from Phase 1 to Phase 2. CONCLUSION: Sleep quality in kidney transplant recipients did not change during the first year after a successful renal transplantation

    When Is the Best Moment to Assess the Ankle Brachial Index: Pre- or Post-Hemodialysis?

    Background: Cardiovascular disease is an important cause of death in patients on dialysis. Peripheral arterial disease (PAD) is a prognostic factor for cardiovascular disease. The ankle brachial index (ABI) is a noninvasive method used for the diagnosis of PAD. The difference between ABI pre- and post-dialysis had not yet been formally tested, and it was the main objective of this study. Methods:The ABI was assessed using an automated oscillometric device in incident patients on hemodialysis. All blood pressure readings were taken in triplicate pre- and post-dialysis in three consecutive dialysis sessions (times 1, 2, and 3). Results: One hundred and twenty-three patients (85 men) aged 53 +/- 19 years were enrolled. We found no difference in ABI pre- and post-dialysis on the right or left side, and there was no difference in times 1, 2, and 3. In patients with a history of PAD, the ABI pre- versus post-dialysis were of borderline significance on the right side (p = 0.088). Conclusion: ABI measured pre- and post-dialysis presented low variability. The ABI in patients with a history of PAD should be evaluated with caution. The applicability of the current method in predicting mortality among patients on hemodialysis therefore needs further investigation. Copyright (C) 2012 S. Karger AG, Base

    Acute aerobic session increased anti-inflammatory cytokine (IL-10) in CKD patients / A sessão aeróbica aguda aumentou a citocina anti-inflamatória (IL-10) em pacientes com CKD

    In patients with chronic kidney disease (CKD), the presence of inflammation is a predictor of mortality. The aerobic exercise has been described to modulate the inflammatory response in general population. However, CKD patients have some limitations to performed aerobic exercise frequently. Thus, the present study evaluated the effect of an acute aerobic exercise session on markers of inflammation in non-dialytic CKD patients. MATERIALS AND METHODS: 10 stage 3 CKD patients and 10 subjects with normal kidney function performed experimental sessions of aerobic exercise on a cycle ergometer for 45 min, 50% at VO2peak. Before and after the session, blood samples were collected for the analysis of IL-6, TNF-?, IL-10, CRP, NGAL and Cystatin C.  The general linear model (GLM) test were used to evaluated the differences and interaction between group and time and Pearson's correlations were used to evaluate the association between the variables. RESULTS: Interestingly, after an aerobic exercise session, IL-10 levels increased in both group and more prominent in CKD group (p &lt;0.0001). As expected, CKD patients were more inflamed compared to the control group and IL-6 and TNF-? had a positive correlation with Cystatin-C and NGAL, markers of renal failure (p &lt; 0.001). CONCLUSION: In this study, an acute aerobic exercise session was able to increase IL-10 levels in CKD patients, suggesting that acute aerobic exercise may contribute to an anti-inflammatory response. However, it is necessary future studies to evaluate the impact of increased IL-10 and acute aerobic exercise on outcomes in patients with CKD

    Ankle-Brachial Index: A Simple Way to Predict Mortality among Patients on Hemodialysis - A Prospective Study

    Background: Ankle-brachial index (ABI) can access peripheral artery disease and predict mortality in prevalent patients on hemodialysis. However, ABI has not yet been tested in incident patients, who present significant mortality. Typically, ABI is measured by Doppler, which is not always available, limiting its use in most patients. We therefore hypothesized that ABI, evaluated by a simplified method, can predict mortality in an incident hemodialysis population. Methodology/Principal Findings: We studied 119 patients with ESRD who had started hemodialysis three times weekly. ABI was calculated by using two oscillometric blood pressure devices simultaneously. Patients were followed until death or the end of the study. ABI was categorized in two groups normal (0.9-1.3) or abnormal (&lt;0.9 and &gt;1.3). There were 33 deaths during a median follow-up of 12 months (from 3 to 24 months). Age (1 year) (hazard of ratio, 1.026; p = 0.014) and ABI abnormal (hazard ratio, 3.664; p = 0.001) were independently related to mortality in a multiple regression analysis. Conclusions: An easy and inexpensive technique to measure ABI was tested and showed to be significant in predicting mortality. Both low and high ABI were associated to mortality in incident patients on hemodialysis.
