    Value of Brain Natriuretic Peptide in Predicting Prognosis of Coronary Artery Disease in Myocardial Infarction

    Background: Brain natriuretic peptide (BNP) is an important predictor of outcomes in patients with heart failurebut the prognostic value of BNP elevation in patients with myocardial infarction (MI) is not completely defined.This study aims to identify the prognostic value of BNP in patients with MI.Materials and Methods: We studied patients with MI who were hospitalized in the Coronary Care Unit of ImamHossein Hospital. Patients' demographic data, past medical and drug history besides echocardiography report andBNP levels were documented during the hospital stay and echocardiography was repeated after 3 months.Results: This prospective observational cross-section study was done between January 2018 through January2019. During the study period, 124 patients were recruited. There was significant negative correlation betweenBNP levels and ejection fraction (P=0.001), systolic blood pressure (P=0.012), diastolic blood pressure(P=0.003) and ratio between early mitral inflow velocity and early diastolic mitral annular velocity (E/e')(P=0.03) and EF in follow up (P=0.001). The correlation between BNP levels with infarction location (P=0.40),arterial involvement in the left main coronary artery (P=0.15), left anterior descending artery (P=0.53), leftcircumflex artery (P= 0.97), right coronary artery (P=0.50) and hospital stay (P=0.66) were not significant.Conclusion: BNP is a valuable marker for predicting prognosis in patients with the acute coronary syndrome.Also, it could be considered as a prognostic long-term marker for predicting the EF of patients with AMI

    Social Phobia and its Association with Academic Performance among Student of Kerman University of Medical Sciences, Iran

    Background & Objective: Public speaking is a common academic activity. Social phobia is associated with fear of public speaking which affects educational achievement. However, few researches have examined the prevalence of social phobia among university students in Iran. The aim of this study was to evaluate the prevalence of social phobia among students of Kerman University of Medical Sciences, Iran, and its impact on their academic performance. Methods: Social Phobia Inventory (SPIN), Brief Version of Negative Evaluation Scale, and a questionnaire assessing demographic characteristics and academic performance were applied to 700 randomly selected students from Kerman University of Medical Sciences. Results: The prevalence of social phobia among the university students was 40.6%. Grade Point Average (GPA) was not significantly different between the students with social phobia comparing to students without social phobia. Yet, students with social phobia believed that fear of speaking in public had negative effects on their academic performance. In this study, only 4.1% of students with social phobia have been visited by psychiatrists. Conclusion: Preventive strategies are recommended to reduce the under-recognition and the adverse impact of social phobia on academic performance of university students.     Challenges and perspectives in continuous glucose monitoring

    Diabetes is a global epidemic that threatens the health and well-being of hundreds of millions of people. The first step in patient treatment is to monitor glucose levels. Currently this is most commonly done using enzymatic strips. This approach suffers from several limitations, namely it requires a blood sample and is therefore invasive, the quality and the stability of the enzymatic strips vary widely, and the patient is burdened by performing the measurement themselves. This results in dangerous fluctuations in glucose levels often going undetected. There is currently intense research towards new approaches in glucose detection that would enable non-invasive continuous glucose monitoring (CGM). In this review, we explore the state-of-the-art in glucose detection technologies. In particular, we focus on the physical mechanisms behind different approaches, and how these influence and determine the accuracy and reliability of glucose detection. We begin by reviewing the basic physical and chemical properties of the glucose molecule. Although these play a central role in detection, especially the anomeric ratio, they are surprisingly often overlooked in the literature. We then review state-of-the art and emerging detection methods. Finally, we survey the current market for glucometers. Recent results show that past challenges in glucose detection are now being overcome, thereby enabling the development of smart wearable devices for non-invasive continuous glucose monitoring. These new directions in glucose detection have enormous potential to improve the quality of life of millions of diabetics, as well as offer insight into the development, treatment and even prevention of the disease

    Autism and heritable bone fragility: A true association?

    Objectives: Osteogenesis Imperfecta (OI) is a heterogeneous condition mainly characterised by bone fragility; intelligence is reported to be normal. However, a minority of children seen also show symptomology consistent with an 'Autism Spectrum Disorder'. A joint genetics and psychology research study was undertaken to identify these patients using 'Gold Standard' research tools: Autism Diagnostic Inventory Revised (ADI-R); Autism Diagnostic Observation Schedule (ADOS) and undertake genetic analyses in them. Method: A cohort of n = 7 children with autistic traits and severe/complex OI were recruited to the study. The study was set-up to explore whether there was a genetic link between bone fragility and autism in a sub-set of patients with bone fragility identified with autism traits in our complex/severe OI clinic. This was not set-up as a prevalence study but rather an exploration of genetics in association with ADI/ADOS confirmed ASD and bone fragility. ADI& ADOS: Standardised tools were used to confirm autism diagnosis. ADI and ADOS were completed by the Clinical Psychologist; ADI comprises a 93 item semi-structured clinical review with a diagnostic algorithm diagnosing Autism; ADOS is a semi-structured assessment of socialisation, communication and play/imagination which also provides a diagnostic algorithm. Exome sequencing: In patients recruited, those that fulfilled research criteria for diagnosis of autism using above tools were recruited to trio whole exome sequencing (WES). Results: one patient had compound heterozygous variants in NBAS; one patient had a variant in NRX1; one patient had a maternally inherited PLS3 variant; all the other patients in this cohort had pathogenic variants in COL1A1/COL1A2. Conclusions: Although, not set out as an objective, we were able to establish that identifying autism had important clinical and social benefits for patients and their families in ensuring access to services, appropriate schooling, increased understanding of behaviour and support. Lay summary: It is important for clinicians looking after children with brittle bone disease, also referred to as Osteogenesis Imperfecta (OI) to be aware of early features of developmental delay/autistic traits especially with severe forms of OI as the emphasis is on their mobility and bone health. Ensuring appropriate assessment and access to services early-on will enable these patients to achieve their potential. Further investigations of genomics in bone fragility in relation to autism are required and dual diagnosis is essential for high quality clinical and educational provision

    International audienceThe significant and growing global prevalence of diabetes continues to challenge people with diabetes (PwD), healthcare providers and payers. While maintaining near-normal glucose levels has been shown to prevent or delay the progression of the long-term complications of diabetes, a significant proportion of PwD are not attaining their glycemic goals. During the past six years, we have seen tremendous advances in automated insulin delivery (AID) technologies. Numerous randomized controlled trials and real-world studies have shown that the use of AID systems is safe and effective in helping PwD achieve their long-term glycemic goals while reducing hypoglycemia risk. Thus, AID systems have recently become an integral part of diabetes management. However, recommendations for using AID systems in clinical settings have been lacking. Such guided recommendations are critical for AID success and acceptance. All clinicians working with PwD need to become familiar with the available systems in order to eliminate disparities in diabetes quality of care. This report provides much-needed guidance for clinicians who are interested in utilizing AIDs and presents a comprehensive listing of the evidence payers should consider when determining eligibility criteria for AID insurance coverage

    Adjunctive Therapies to Optimize Closed-loop Glucose Control

    Closed-loop insulin delivery systems are fast becoming the standard of care in the management of type 1 diabetes and have led to significant improvements in diabetes management. Nevertheless, there is still room for improvement for the closed-loop systems to optimize treatment and meet target glycemic control. Adjunct treatments have been introduced as an alternative method to insulin-only treatment methods to overcome diabetes treatment challenges and improve clinical and patient reported outcomes during closed-loop treatment. The adjunct treatment agents mostly consist of medications that are already approved for type 2 diabetes treatment and aim to complete the missing physiologic factors, such as the entero-endocrine system, that regulate glycemia in addition to insulin. This paper will review many of these adjunct therapies, including the basic mechanisms of action, potential benefits, side effects, and the evidence supporting their use during closed-loop treatment
