    Ziel dieser Arbeit ist es, der Frage nach dem Verhältnis von „spektakulärer“ Architektur im Museumsbau und der Darstellung und Ausstellung der Kunst im Museum nachzugehen. Exemplarisch diskutiert wird das Thema anhand einer Analyse der Neuen Staatsgalerie Stuttgart von James Frazer Stirling. Die Staatsgalerie wurde ausgewählt, da sie zur Zeit ihrer Entstehung kontrovers diskutiert und die Frage nach der „Abgehobenheit der Architektur“ gegenüber der darin ausgestellten Kunst bei diesem Gebäude dabei intensiv debattiert wurde. Auch eignet sich die Neue Staatsgalerie als exemplarisches Museum zur Analyse und Diskussion, da die architekturgeschichtliche Rezeption des Museums die Staatsgalerie als einen der wichtigsten Bauten der Postmoderne in Deutschland klassifiziert, sich also an der Staatsgalerie nicht nur das Verhältnis von Architektur und Kunst, sondern auch der Übergang von Moderne zu Postmodernde reflektieren lassen. Die Neue Staatsgalerie Stuttgart wird anhand ihres Entstehungszusammenhangs, der architektonischen Gestaltung im Inneren und im Äußeren, sowie der im Zeitverlauf und mit verschiedenen Direktoren wechselnden Inszenierungsweisen der Ausstellungen analysiert. Dabei wird die Staatsgalerie auch in den Kontext des Museumsbaus ihrer Zeit gestellt und mit ausgewählten anderen Museumsarchitekturen verglichen

    Effectiveness of Dietary Interventions in the Treatment of Endometriosis: a Systematic Review

    A patients’ increasing interest in dietary modifications as a possible complementary or alternative treatment of endometriosis is observed. Unfortunately, the therapeutic potential of dietary interventions is unclear and to date no guidelines to assist physicians on this topic exist. The aim of this study, therefore, was to systematically review the existing studies on the effect of dietary interventions on endometriosis. An electronic-based search was performed in MEDLINE and COCHRANE. We included human and animal studies that evaluated a dietary intervention on endometriosis-associated symptoms or other health outcomes. Studies were identified and coded using standard criteria, and the risk of bias was assessed with established tools relevant to the study design. We identified nine human and 12 animal studies. Out of the nine human studies, two were randomized controlled trials, two controlled studies, four uncontrolled before-after studies, and one qualitative study. All of them assessed a different dietary intervention, which could be classified in one of the following principle models: supplementation with selected dietary components, exclusion of selected dietary components, and complete diet modification. Most of the studies reported a positive effect on endometriosis; they were however characterized by moderate or high-risk bias possibly due to the challenges of conducting dietary intervention trials. According to the available level of evidence, we suggest an evidence-based clinical approach for physicians to use during consultations with their patients. Further well-designed randomized controlled trials are needed to accurately determine the short-term and long-term effectiveness and safety of different dietary interventions

    Safety of 80% vs 30–35% fraction of inspired oxygen in patients undergoing surgery: a systematic review and meta-analysis

    Background: Evidence-based guidelines from the World Health Organization (WHO) have recommended a high (80%) fraction of inspired oxygen (FiO2) to reduce surgical site infection in adult surgical patients undergoing general anaesthesia with tracheal intubation. However, there is ongoing debate over the safety of high FiO2. We performed a systematic review to define the relative risk of clinically relevant adverse events (AE) associated with high FiO2. Methods: We reviewed potentially relevant articles from the WHO review supporting the recommendation, including an updated (July 2018) search of EMBASE and PubMed for randomised and non-randomised controlled studies reporting AE in surgical patients receiving 80% FiO2 compared with 30–35% FiO2. We assessed study quality and performed meta-analyses of risk ratios (RR) comparing 80% FiO2 against 30–35% for major complications, mortality, and intensive care admission. Results: We included 17 moderate–good quality trials and two non-randomised studies with serious-critical risk of bias. No evidence of harm with high FiO2 was found for major AE in the meta-analysis of randomised trials: atelectasis RR 0.91 [95% confidence interval (CI) 0.59–1.42); cardiovascular events RR 0.90 (95% CI 0.32–2.54); intensive care admission RR 0.93 (95% CI 0.7–1.12); and death during the trial RR 0.49 (95% CI 0.17–1.37). One non-randomised study reported that high FiO2 was associated with major respiratory AE [RR 1.99 (95% CI 1.72–2.31)]. Conclusions: No definite signal of harm with 80% FiO2 in adult surgical patients undergoing general anaesthesia was demonstrated and there is little evidence on safety-related issues to discourage its use in this population

    Precast concrete floor support and diaphragm action

    Experimental research, engineering analysis and theoretical developments comprise a study in which various interactions between ductile moment resisting frames and precast prestressed hollow core flooring have been examined. The most critical interaction tested involves support behaviour, and the ability of reinforcing details to provide control against loss of support and possible catastrophic flooring collapse under dilation effects. Plastic hinge dilation, also known as elongation or growth, is an inherent property of ductile concrete members when subjected to cyclic plastic deformations. Hence, the performance of floor support details is enveloped by the general design philosophy of seismic resisting structures. In the experimental phase, emphasis was placed on testing support construction joints from contemporary building practice, for direct comparison with special support tie details of known capabilities. The contemporary details were found to exhibit seriously flawed behaviour under monotonic and cyclic loading regimes. Corroborative experiments were undertaken to establish direct shear capacities between typical composite bond surfaces. In particular, these tests addressed the discrepancy that has emerged between direct shear and shear flow strengths. Also, the continuity response of conventional and proposed support detail types was examined. A composite section model was analysed to demonstrate the likely influence of prestressing steel on beam bending strength within a ductile frame environment. Likewise, the probable effects of prestressing steel on beam plastic hinge development were examined, but on a more theoretical basis. Other elements of theory have been presented. These mainly concern the general topic of elastic-plastic response in reinforced concrete elements. The particular focus of this work has been to demonstrate a rational basis to stiffness transition and plastic buckling analysis. The important role of stiffness degradation in dynamic analysis has also been examined. Although ductile moment resisting concrete frames have been emphasised, it is considered that the findings of this thesis are applicable to other structural systems, such as dry joint "hybrid" precast concrete frames and spring connected steel frame structures

    Mit Menschen mit Demenz forschen – ethische Reflexionen einer qualitativen Forschungspraxis zur Mobilität im öffentlichen Raum

    In diesem Beitrag reflektieren wir ethische Fragen der qualitativen Forschung mit Menschen mit Demenz anhand konkreter Projekterfahrungen und stellen theoretische Bezüge her. In dem Forschungsprojekt "Demenz in Bewegung: Studie und Handlungsempfehlungen für demenzfreundliches Unterwegssein im öffentlichen Verkehrssystem" partizipieren Menschen mit Demenz am Forschungsprozess. Im Rahmen des Forschungsprojektes wurden narrative Interviews zu Alltagserfahrungen und Mobilitätsbedürfnissen, eine Begehungsstudie mit Spaziergängen sowie eine Machbarkeitstestung von bestehenden technischen Hilfsmitteln durchgeführt. Zentrale ethische Herausforderungen zeigten sich sowohl in der theoretischen Fundierung als auch bei Vorbereitung und Durchführung der Untersuchung. Wir diskutieren den Grundsatz des informierten Einverständnisses als prozessorientierte Einwilligung (process consent) vor dem Hintergrund von Anforderungen institutionalisierter Prüfverfahren durch Ethikkommissionen. Darüber hinaus zeigen wir, dass unsere Entscheidung, "Demenz" im Gespräch mit betroffenen Menschen offen anzusprechen, im Spannungsfeld zwischen transparenter Information, dem Risiko zu verletzen und der Chance, ermächtigend zu wirken, steht

    Preoperative fibrinogen/CRP score predicts survival in upper urothelial tract carcinoma patients undergoing radical curative surgery

    Purpose Upper tract urothelial carcinoma (UTUC) represents an often aggressive malignancy associated with poor prognosis. Therefore, finding reliable prognostic biomarkers in patients undergoing curative surgery for improved risk stratification is crucial. We evaluated the prognostic value of the Fibrinogen/C-reactive protein (FC)-score in a cohort of surgically treated UTUC patients. Methods 170 patients with radiologically and histologically verified UTUC who underwent radical curative surgery between 1990 and 2020, were included. The FC-score was calculated for each patient, with patients receiving 1 point each if Fibrinogen and/or CRP levels were elevated above the 25th or 75th percentile, respectively. Patients were divided into three subgroups according to their FC-score of 0, 1 or 2 point(s). Kaplan–Meier analysis, uni- and multivariable Cox proportional hazard models were implemented. We determined cancer-specific survival (CSS) as primary endpoint, whereas overall survival (OS) and recurrence-free survival (RFS) were considered secondary endpoints. Results High FC-score (2 points) was significantly associated with adverse histological features such as vascular invasion (OR = 4.08, 95%CI 1.18–14.15, p = .0027) and tumour necrosis (OR = 6.67, 95%CI 1.35–32.96, p = 0.020). Both, uni- and multivariable Cox proportional hazard models showed the FC-score as a significant predictor for CSS (univariable analysis: FC-score = 1: HR = 1.90, 95%CI 0.92–3.93, p = 0.085 | FC-score = 2: HR = 2.86, 95%CI 1.22–6.72, p = 0.016). Furthermore, in univariable analysis, patients with higher FC-score had significantly shorter OS (FC-score = 1: HR = 1.32, 95%CI 0.70–2.49, p = 0.387 | FC-score = 2: HR = 2.19, 95%CI 1.02–4.67, p = 0.043). However, this did not prevail in multivariable analysis. Conclusion The FC-score represents a novel potential biomarker in patients with UTUC undergoing radical curative surgery
