Determination of βS haplotypes in patients with sickle-cell anemia in the state of Rio Grande do Norte, Brazil
βS haplotypes were studied in 47 non-related patients with sickle-cell anemia from the state of Rio Grande do Norte, Brazil. Molecular analysis was conducted by PCR/RFLP using restriction endonucleases XmnI, HindIII, HincII and HinfI to analyze six polymorphic sites from the beta cluster. Twenty-seven patients (57.5%) were identified with genotype CAR/CAR, 9 (19.1%) CAR/BEN, 6 (12.8%) CAR/CAM, 1 (2.1%) BEN/BEN, 2 (4.3%) CAR/Atp, 1 (2.1%) BEN/Atp and 1 (2.1%) with genotype Atp/Atp. The greater frequency of Cameroon haplotypes compared to other Brazilian states suggests the existence of a peculiarity of African origin in the state of Rio Grande do Norte
A Community Study of Factors Related to Poorly Controlled Asthma among Brazilian Urban Children
BACKGROUND: Asthma constitutes a serious public health problem in many regions of the world, including the city of Salvador, State of Bahia-Brazil. The purpose of this study was to analyse the factors associated with poor asthma control. METHODOLOGY/PRINCIPAL FINDINGS: Two definitions were used for asthma: 1) wheezing in the last 12 months; 2) wheezing in the last 12 months plus other asthma symptoms or asthma diagnosis ever. The definition of poorly controlled asthma was: at least one reported hospitalisation due to asthma and/or high frequency of symptoms, in the last year. Children with poorly controlled asthma (N = 187/374) were compared with wheezing children with controlled asthma regarding age, gender, atopy, parental asthma, rhinitis, eczema, exposure to second hand tobacco smoke, presence of moulds, pets and pests in the house, helminth infections and body mass index. Crude and logistic regression adjusted odds ratios were used as measures of association. There was a higher proportion of poorly controlled asthma among children with eczema (OR = 1.55; 95% CI 1.02; 2.37). The strength of the association was greater among children with eczema and rhinitis (42.6%, 53.4% and 57.7%, respectively, in children who had no rhinitis nor eczema, had only one of those, and had both (p = 0.02 for trend test). The presence of mould in the houses was inversely associated with poorly controlled asthma (OR = 0.54; 95% CI 0.34; 0.87). CONCLUSIONS/SIGNIFICANCE: Our results indicate an association between eczema and poor asthma control in this environment, but emphasize the role of various other individual and environmental factors as determinants of poor control
Primary healthcare and the construction of thematic health networks: what role can they play?
The enhancement of primary healthcare has been a core strategy for the empowerment of the Brazilian Unified Health System (SUS). Recent guidelines issued by OPAS and the Ministry of Health highlight the role it has played as a thematic communication network center, a regulating agent for the access and use of services required for comprehensive healthcare. Sponsored by PPSUS/Fapesp, this study examines the possibilities of the primary healthcare network exercising such a strategic function. Life narratives involving 15 regular users were produced in two cities of ABC Paulista, which have adopted the Family Health Strategy for the organization of their primary healthcare networks. The study presents three main findings: the primary healthcare network serves as an outpost of SUS by producing user values even for high complexity service users; the primary network is perceived is a place for simple care needs; there is shared impotence between users and teams when it comes to the network functioning as the coordinator of care, indicating that it does not possess the technological, operational and organizational material conditions or symbolic conditions (values, meanings, and representations) to be in a central position in the coordination of thematic healthcare networks.O fortalecimento da atenção básica tem sido valorizado como estratégia central para a construção do SUS. Diretrizes recentes emanadas pela OPAS e pelo MS destacam seu papel como centro de comunicação de redes temáticas, como reguladora do acesso e utilização dos serviços necessários para a integralidade do cuidado. O presente estudo, financiado com recursos PPSUS/Fapesp, problematiza as possibilidades da rede básica exercer tal função estratégica. Foram produzidas narrativas de vida de 15 usuários altamente utilizadores de serviços de saúde em dois municípios do ABC paulista, que adotaram a Estratégia de Saúde da Família para organização de suas redes básicas. Vaccination with Recombinant Microneme Proteins Confers Protection against Experimental Toxoplasmosis in Mice
Toxoplasmosis, a zoonotic disease caused by Toxoplasma gondii, is an important public health problem and veterinary concern. Although there is no vaccine for human toxoplasmosis, many attempts have been made to develop one. Promising vaccine candidates utilize proteins, or their genes, from microneme organelle of T. gondii that are involved in the initial stages of host cell invasion by the parasite. In the present study, we used different recombinant microneme proteins (TgMIC1, TgMIC4, or TgMIC6) or combinations of these proteins (TgMIC1-4 and TgMIC1-4-6) to evaluate the immune response and protection against experimental toxoplasmosis in C57BL/6 mice. Vaccination with recombinant TgMIC1, TgMIC4, or TgMIC6 alone conferred partial protection, as demonstrated by reduced brain cyst burden and mortality rates after challenge. Immunization with TgMIC1-4 or TgMIC1-4-6 vaccines provided the most effective protection, since 70% and 80% of mice, respectively, survived to the acute phase of infection. In addition, these vaccinated mice, in comparison to non-vaccinated ones, showed reduced parasite burden by 59% and 68%, respectively. The protective effect was related to the cellular and humoral immune responses induced by vaccination and included the release of Th1 cytokines IFN-γ and IL-12, antigen-stimulated spleen cell proliferation, and production of antigen-specific serum antibodies. Our results demonstrate that microneme proteins are potential vaccines against T. gondii, since their inoculation prevents or decreases the deleterious effects of the infection
The skull of Epidolops ameghinoi from the early Eocene Itaboraí fauna, southeastern Brazil, and the affinities of the extinct marsupialiform order Polydolopimorphia
The skull of the polydolopimorphian marsupialiform Epidolops ameghinoi is described
in detail for the first time, based on a single well-preserved cranium and associated left
and right dentaries plus additional craniodental fragments, all from the early Eocene
(53-50 million year old) Itaboraí fauna in southeastern Brazil. Notable craniodental
features of E. ameghinoi include absence of a masseteric process, very small
maxillopalatine fenestrae, a prominent pterygoid fossa enclosed laterally by a
prominent ectopterygoid crest, an absent or tiny transverse canal foramen, a simple,
planar glenoid fossa, and a postglenoid foramen that is immediately posterior to the
postglenoid process. Most strikingly, the floor of the hypotympanic sinus was
apparently unossified, a feature found in several stem marsupials but absent in all
known crown marsupials. "Type II" marsupialiform petrosals previously described from
Itaboraí plausibly belong to E. ameghinoi; in published phylogenetic analyses, these
petrosals fell outside (crown-clade) Marsupialia. "IMG VII" tarsals previously referred to
E. ameghinoi do not share obvious synapomorphies with any crown marsupial clade,
nor do they resemble those of the only other putative polydolopimorphians represented
by tarsal remains, namely the argyrolagids. Most studies have placed
Polydolopimorphia within Marsupialia, related to either Paucituberculata, or to
Microbiotheria and Diprotodontia. However, diprotodonty almost certainly evolved
independently in polydolopimorphians, paucituberculatans and diprotodontians, and
Epidolops does not share obvious synapomorphies with any marsupial order.
Epidolops is dentally specialized, but several morphological features appear to be
more plesiomorphic than any crown marsupial. It seems likely Epidolops that falls
outside Marsupialia, as do morphologically similar forms such as Bonapartherium and
polydolopids. Argyrolagids differ markedly in their known morphology from Epidolops
but share some potential apomorphies with paucituberculatans. It is proposed that
Polydolopimorphia as currently recognised is polyphyletic, and that argyrolagids (and
possibly other taxa currently included in Argyrolagoidea, such as groeberiids and
patagoniids) are members of Paucituberculata. This hypothesis is supported by
Bayesian non-clock phylogenetic analyses of a total evidence matrix comprising DNA
sequence data from five nuclear protein-coding genes, indels, retroposon insertions
and morphological characters: Epidolops falls outside Marsupialia, whereas
argyrolagids form a clade with the paucituberculatans Caenolestes and Palaeothentes,
regardless of whether the Type II petrosals and IMG VII tarsals are used to score
characters for Epidolops or not. There is no clear evidence for the presence of crown
marsupials at Itaboraí, and it is possible that the origin and early evolution of
Marsupialia was restricted to the "Austral Kingdom" (southern South America,
Antarctica, and Australia)
Performance evaluation of primary care services for the treatment of tuberculosis
Objective Evaluating the performance of primary care services for the treatment of tuberculosis according to the assessment referential of health services (structure/process) in Cabedelo, a port city in the state of Paraíba. Method An evaluation quantitative, cross-sectional study, in which were carried out 117 interviews with health professionals using a structured instrument. The analysis was based on the construction of indicators using a standardized value for the reduced variable (z=1). Results The structural indicators showed regular performance for the following variables: professional training, access to record instruments and coordination with other services. The process indicators related to external actions and information about the disease had unsatisfactory performance. The directly observed treatment and the flows of reference/counter-reference had regular performance. Conclusion The focused professional qualification, the fragmentation of practices and the unsystematic home care constitute obstacles for carrying out actions aimed at providing expanded, continuous and resolute care
