    Assessment of Background Ionizing Radiation Dose Levels in Quarry Sites Located in Ebonyi State, Nigeria

    The study presents a radiometric survey of Background Ionizing Radiation (BIR) dose levels in ten quarry sites located in Ebonyi State, Nigeria. In-situ BIR dose rate measurements, by means of nuclear radiation survey meter, at 1 m above ground level were carried out at the excavation section (ES) and quarrying section (QS) of the investigated quarry sites. The obtained results indicated dose rates ranging from from 0.14 to 0.18 \u3bcSv/h with mean of 0.15\ub10.01 \u3bcSv/h at the ES and 0.16 to 0.19 \u3bcSv/h with mean value of 0.18\ub10.01 \u3bcSv/h at the QS. While the values obtained at the QS are respectively higher than those measured at the ES, they are all higher than the worldwide average value of 84 nSv/h signifying BIR elevated environments. The estimated mean annual effective dose (AED) and excess lifetime cancer risk (ELCR) are 0.27\ub10.03 mSv/y and 0.94 710\u20133 respectively at the ES and 0.31\ub10.02 mSv/y and 1.07 710\u20133 at the QS. The obtained AED values for all the sites are well above the outdoor worldwide average value of 0.07 mSv/y but lower than the International Commission on Radiological Protection recommended permissible limits of 1.0 mSv/y for the general public. Generally, the BIR levels of the quarry sites are within acceptable limits and no immediate radiological health threat may be derived from the current levels. However, long-term health effects due to continuous exposure to low-level radiation doses may manifested in future over a lifetime exposure of 70 years as indicated by the ELCR values

    SUITABILITY OF BURNT AND CRUSHED COW BONES AS PARTIAL REPLACEMENT FOR FINE AGGREGATE IN CONCRETE

    The suitability of burnt and crushed cow bones (BCCB) as partial replacement for fine aggregate in concrete was studied. The percentages of replacements of fine aggregates of 0, 10, 20, 30, 40 and 50%, respectively of BCCB were tested considering 1: 2: 4 and 1: 11/2 :3 concrete mix ratios. The cow bones were burnt for 50 minutes up to 92oC before being crushed. Ninety-six (96) concrete cubes of 1: 2: 4 mix ratio and ninety-six (96) concrete cubes of 1 : : 3 mix ratio measuring 150x150x150mm were tested for the compressive strength at 7, 14, 21 and 28 days respectively. The research revealed that the BCCB acted as a retarder in the concrete. Water-cement ratio increased with the increase in the percentage of the BCCB. The mixes of 1:2:4 and 1::3 at 28 days curing yielded average compressive strengths in N/mm2 ranging from 16.49 - 24.29 and 18.71 - 29.73, respectively. For the mix ratios of 1:2:4 and 1:: 3 at 28 days curing age,  it was observed that increase in the BCCB content beyond 40 and 50%, respectively resulted to the reduction of the average compressive strength below recommended minimum strength for use of concrete in structural works.http://dx.doi.org/10.4314/njt.v36i3.

    The effect of health sector industrial actions on TB and TB/HIV case finding in Ogun State, Nigeria: Is Public-Private Mix a viable solution?

    Background: Though industrial disputes are a global phenomenon, there is a rising concern of its frequent occurrence in Sub-Saharan Africa. Objective: To assess the effect of industrial actions embarked upon by the health workers during the year 2014 on Tuberculosis (TB) notification in Ogun State, Nigeria. Methods: A retrospective review of TB notification in 2013 and 2014 was conducted. Quarterly TB case notification, the proportions of Human Immunodeficiency Virus (HIV) test, Co-trimoxazole (CPT) uptake and Antiretroviral Therapy (ART) uptake in the years 2013 and 2014 were compared using the Epi-info software. Results: There was a decline in the proportion of TB cases reported by the public sector health services and an increase in the proportion of TB cases reported by the private health facilities during the period of industrial disputes in the public health sector (doctors and non-doctors) (p = 0.001). Compared to the year 2013, the proportion of presumptive TB cases tested for HIV declined significantly during the period of the strike actions by the non-doctors but not during the strike actions by doctors in 2014. There was no difference in the uptake of Co-trimoxazole (p = 0.456 and 0.511) and Anti-retroviral Therapy (p = 0.192 and 0.544) by TB/HIV co-infected patients during the strike actions by the doctors and the non-doctors respectively. Conclusion: This study demonstrated the importance of Public-Private–Mix for TB case finding efforts in the developing countries, where there are incessant strike actions by health workers in the public sector

    Dental Care Utilization and Satisfaction of Residential University Students

    Aim: The objective of this study was to provide information on the level of utilization and satisfaction of residential university students with the dental services provided by the dental clinic of a teaching hospital. Volunteers and Material: A stratified sampling technique was used to recruit volunteers from the outpatient clinic of the Obafemi Awolowo University Teaching Hospital Complex, Ile-Ife, Nigeria. Information was collected by a self-administered questionnaire composed of questions that measure the level of utilization and satisfaction with the dental services provided. Questionnaires were provided to 650 randomly chosen students residing in the University hostels. There were 39 refusals, and 6 incomplete questionnaires were discarded. This left a sample size of 605 volunteers. Results: Forty seven students (7.8%) indicated that they visited the dental hospital within the last 12 months. Males and females utilized the dental services equally, and utilization increased with age and the number of years spent on campus. Anticipation of painful dental treatment, high dental charges, long waiting times and being too busy for a dental visit were cited as the most important impediments to seeking dental treatment. Females expressed greater satisfaction with the services. Conclusion: Dental service utilization among the students was found to be low. Oral health awareness campaigns, improving the quality of the services, and shortening the waiting time are expected to increase service utilization and satisfaction.     Prevalence of intestinal parasitic infections among HIV patients in Benin City, Nigeria

    This study was carried out to determine the presence of intestinal parasites and their correlation with CD4+ T-cell counts and demographics among human immunodeficiency virus (HIV)-positive patients in Benin City, Nigeria. Stool specimens from 2,000 HIV-positive patients and 500 controls (HIV-negative individuals) were examined for ova, cysts, or parasites, using standard procedures. In addition, patient's blood samples were analyzed for CD4 counts by flow cytometry. An overall prevalence rate of 15.3% was observed among HIV-positive patients while 6.2% was noted among non-HIV subjects. HIV status was a significant (P<0.0001) risk factor for acquiring intestinal parasitic infections. Male gender, CD4 count <200cell/µl, and diarrhea were significantly associated with an increased prevalence of intestinal parasitic infections among HIV-positive patients. The level of education, occupation, and source of water among HIV patients significantly (P<0.0001) affected the prevalence of intestinal parasitic infections. Ascaris lumbricoides was the most predominant parasite in both HIV-positive patients and controls. A CD4 count <200 cells/µl was significantly associated with only Isospora belli and Cryptosporidium infections. The presence of pathogenic intestinal parasites such as A. lumbricoides, hookworm, Giardia intestinalis, Entamoeba histolytica, Trichuris trichiura, and Taenia species among HIV-infected persons should not be neglected. Cryptosporidium species and I. belli were the opportunistic parasites observed in this study. Routine screening for intestinal parasites in HIV-positive patients is advocated

    Increased sensitivity for detecting malaria parasites in human umbilical cord blood using scaled-up DNA preparation

    BACKGROUND: All mothers donating umbilical cord blood units to the NHS cord blood bank undergo an assessment for the likelihood of prior exposure to malaria infection. Those deemed at risk due to a history of travel to, or residence in, malaria endemic regions are screened serologically to detect anti-malaria antibodies. A positive result excludes the use of the cord blood for transplant therapy unless a risk assessment can ensure that malaria transmission is extremely unlikely. This paper details the screening of cord blood units from malaria serology positive mothers to detect malaria parasite DNA using a highly sensitive nested PCR. METHODS: Uninfected blood from a healthy volunteer was spiked with known quantities of malaria parasites and 5 millilitre and 200 microlitre aliquots were subjected to DNA extraction using QIAamp DNA maxi and DNA mini kits respectively. Nested PCR, to detect malarial SSU rRNA sequences, was performed on the purified DNA samples to determine the limit of detection for this assay with both extraction methodologies. Following assay validation, 54 cord blood units donated by mothers who were positive for anti-malaria antibodies were screened by this approach. RESULTS: When DNA was purified from 5 millilitres of blood it was possible to routinely detect as few as 50 malaria parasites per millilitre using nested PCR. This equates to a significant increase in the sensitivity of the current gold standard nucleic acid amplification technique used to detect malaria parasites (routinely performed from > 200 microlitre volumes of blood). None of the 54 donated cord blood units from serology positive mothers tested positive for malaria parasites using this scaled up DNA preparation method. CONCLUSION: Serological testing for malaria parasites may be overly conservative, leading to unnecessary rejection of cord blood donations that lack malaria parasites and which are, therefore, safe for use in stem cell therapy

    A Ranking System for Reference Libraries of DNA Barcodes: Application to Marine Fish Species from Portugal

    BACKGROUND: The increasing availability of reference libraries of DNA barcodes (RLDB) offers the opportunity to the screen the level of consistency in DNA barcode data among libraries, in order to detect possible disagreements generated from taxonomic uncertainty or operational shortcomings. We propose a ranking system to attribute a confidence level to species identifications associated with DNA barcode records from a RLDB. Here we apply the proposed ranking system to a newly generated RLDB for marine fish of Portugal. METHODOLOGY/PRINCIPAL FINDINGS: Specimens (n = 659) representing 102 marine fish species were collected along the continental shelf of Portugal, morphologically identified and archived in a museum collection. Samples were sequenced at the barcode region of the cytochrome oxidase subunit I gene (COI-5P). Resultant DNA barcodes had average intra-specific and inter-specific Kimura-2-parameter distances (0.32% and 8.84%, respectively) within the range usually observed for marine fishes. All specimens were ranked in five different levels (A-E), according to the reliability of the match between their species identification and the respective diagnostic DNA barcodes. Grades A to E were attributed upon submission of individual specimen sequences to BOLD-IDS and inspection of the clustering pattern in the NJ tree generated. Overall, our study resulted in 73.5% of unambiguous species IDs (grade A), 7.8% taxonomically congruent barcode clusters within our dataset, but awaiting external confirmation (grade B), and 18.7% of species identifications with lower levels of reliability (grades C/E). CONCLUSION/SIGNIFICANCE: We highlight the importance of implementing a system to rank barcode records in RLDB, in order to flag taxa in need of taxonomic revision, or reduce ambiguities of discordant data. With increasing DNA barcode records publicly available, this cross-validation system would provide a metric of relative accuracy of barcodes, while enabling the continuous revision and annotation required in taxonomic work
