Critical constraints on chiral hierarchies.
Critical dynamics constrains models of dynamical electroweak symmetry breaking in which the scale of high-energy physics is far above 1 TeV. A big hierarchy requires the high-energy theory to have a second-order chiral phase transition, near which the theory is described by a low-energy effective Lagrangian with composite Higgs scalars. As scalar theories with more than one 4 coupling can have a Coleman-Weinberg instability and a first-order transition, such dynamical EWSB models cannot always support a large hierarchy. If the large-Nc Nambu Jona-Lasinio model is a good approximation to the top-condensate and strong extended technicolor models, they will not produce acceptable EWSB. © 1993 The American Physical Society

Post-Partum Pituitary Insufficiency and Livedo Reticularis Presenting a Diagnostic Challenge in a Resource Limited Setting in Tanzania: A Case Report, Clinical Discussion and Brief Review of Existing Literature.
Pituitary disorders following pregnancy are an important yet under reported clinical entity in the developing world. Conversely, post partum panhypopituitarism has a more devastating impact on women in such settings due to high fertility rates, poor obstetric care and scarcity of diagnostic and therapeutic resources available. A 37 year old African female presented ten years post partum with features of multiple endocrine deficiencies including hypothyroidism, hypoadrenalism, lactation failure and secondary amenorrhea. In addition she had clinical features of an underlying autoimmune condition. These included a history of post-partum thyroiditis, alopecia areata, livedo reticularis and deranged coagulation indices. A remarkable clinical response followed appropriate hormone replacement therapy including steroids. This constellation has never been reported before; we therefore present an interesting clinical discussion including a brief review of existing literature. Post partum pituitary insufficiency is an under-reported condition of immense clinical importance especially in the developing world. A high clinical index of suspicion is vital to ensure an early and correct diagnosis which will have a direct bearing on management and patient outcome
The Hidden Curriculum of Veterinary Education: Mediators and Moderators of Its Effects
The “hidden curriculum” has long been supposed to have an effect on students' learning during their clinical education, and in particular in shaping their ideas of what it means to be a professional. Despite this, there has been little evidence linking specific changes in professional attitudes to the individual components of the hidden curriculum. This study aimed to recognize those components that led to a change in students' professional attitudes at a UK veterinary school, as well as to identify the attitudes most affected. Observations were made of 11 student groups across five clinical rotations, followed by semi-structured interviews with 23 students at the end of their rotation experience. Data were combined and analyzed thematically, taking both an inductive and deductive approach. Views about the importance of technical competence and communication skills were promoted as a result of students' interaction with the hidden curriculum, and tensions were revealed in relation to their attitudes toward compassion and empathy, autonomy and responsibility, and lifestyle ethic. The assessment processes of rotations and the clinical service organization served to communicate the messages of the hidden curriculum, bringing about changes in student professional attitudes, while student-selected role models and the student rotation groups moderated the effects of these influences
Patient acceptance of universal screening for hepatitis C virus infection
<p>Abstract</p> <p>Background</p> <p>In the United States, about 70% of 2.9-3.7 million people with hepatitis C (HCV) are unaware of their infection. Although universal screening might be a cost-effective way to identify infections, prevent morbidity, and reduce transmission, few efforts have been made to determine patient opinions about new approaches to screening.</p> <p>Methods</p> <p>We surveyed 200 patients in August 2010 at five outpatient clinics of a major public urban medical center in Seattle, WA, with an 85.8% response rate.</p> <p>Results</p> <p>The sample was 55.3% women, median 47 years of age, and 56.3% white and 32.7% African or African-American; 9.5% and 2.5% reported testing positive for HCV and HIV, respectively. The vast majority of patients supported universal screening for HCV. When presented with three options for screening, 48% preferred universal testing without being informed that they were being tested or provided with negative results, 37% preferred testing with the chance to "opt-out" of being tested and without being provided with negative results, and 15% preferred testing based on clinician judgment. Results were similar for HIV screening.</p> <p>Conclusions</p> <p>Patients support universal screening for HCV, even if that screening involves testing without prior consent or the routine provision of negative test results. Current screening guidelines and procedures should be reconsidered in light of patient priorities.</p
Minimal Conformal Technicolor and Precision Electroweak Tests
We study the minimal model of conformal technicolor, an SU(2) gauge theory
near a strongly coupled conformal fixed point, with conformal symmetry softly
broken by technifermion mass terms. Conformal symmetry breaking triggers chiral
symmetry breaking in the pattern SU(4) -> Sp(4), which gives rise to a
pseudo-Nambu-Goldstone boson that can act as a composite Higgs boson. The top
quark is elementary, and the top and electroweak gauge loop contributions to
the Higgs mass are cut off entirely by Higgs compositeness. In particular, the
model requires no top partners and no "little Higgs" mechanism. A nontrivial
vacuum alignment results from the interplay of the top loop and technifermion
mass terms. The composite Higgs mass is completely determined by the top loop,
in the sense that m_h/m_t is independent of the vacuum alignment and is
computable by a strong-coupling calculation. There is an additional composite
pseudoscalar A with mass larger than m_h and suppressed direct production at
LHC. We discuss the electroweak fit in this model in detail. Corrections to Z
-> bb and the T parameter from the top sector are suppressed by the enhanced
Sp(4) custodial symmetry. Even assuming that the strong contribution to the S
parameter is positive and usuppressed, a good electroweak fit can be obtained
for v/f ~ 0.25, where v and f are the electroweak and chiral symmetry breaking

Changes in glacial meltwater alter algal communities in lakes of Scoresby Sund, Renland, East Greenland throughout the Holocene: abrupt reorganizations began 1000 years before present
We investigated the response of lake algal communities to changes in glacial meltwater from the Renland Ice Cap (Greenland) through the Holocene to assess whether influxes always elicit consistent responses or novel responses. We measured sedimentary algal pigments in two proximal lakes, snow-fed Raven and glacier- and snow-fed Bunny Lake, and diatom community structure and turnover in Bunny Lake. Diatom data were not available in Raven Lake. We also modeled lake-level change in Bunny Lake to identify how glacial meltwater may have altered diatom habitat availability through time. Through a series of glacier advances and retreats over the Holocene, the algal response in Bunny Lake was relatively constant until approximately 1015 yr BP, after which there were major changes in sedimentary algal remains. Algal pigment concentrations sharply declined, and diatom species richness increased. Diatom community structure underwent three reorganizations. Until 1015 yr BP, assemblages were dominated by Pinnularia braunii and Aulacoseira pffaffiana. However, approximately 1015–480 yr BP, these species declined and Tabellaria flocculosa and Hannaea arcus became a significant component of the assemblage. Approximately 440 yr BP, A. pfaffiana increased along with species indicating elevated nitrogen. In contrast, the algal pigment records from nearby snow-fed Raven Lake showed different and minimal change through time. Our results suggest that changes in the magnitude and composition of meltwater in our two study lakes were unique over the last 1000 yr BP and elicited a non-linear threshold response absent during other periods of glacier advance and retreat.
