    A Study of Diplostomum Parasites Affecting Oreochromis niloticus in Chepkoilel Fish Farm and Two Dams in Eldoret-Kenya

    The Diplostomum parasite completes its life-cycle in fish eating birds, but spends considerable time in the eye vitreous humor of many fresh water fishes. Its infection in fish causes severe ocular pathology, and leads to increased susceptibility to predation. A study on Diplostomum parasites was conducted at Chepkoilel University with an objective of assessing its parasitic indices and effects on O. niloticus. A total of 40 fish were collected from the fish farm and 155 from the dams between November 2010 and January, 2011. Fish were weighed and length measured to ascertain their condition factor. The fish were observed for the Diplostomum parasite to ascertain parasitic indices in the different habitats. The parasite abundance was related to fish size and condition factor to determine parasite’s effect on O. niloticus. Fish were found to have a parasitic prevalence (%) of 100, 84 and 66 in Kerita dam, Kesses dam and Chepkoilel fish farm. Parasite mean intensity was 12, 9 and 14 respectively.The wellbeing of the fish was not compromised by the parasites, as was seen in the correlation between condition factor and parasite abundance. There was a Pearson correlation of P = 0.357, 0.516 and 0.565 in Kerita dam, Kesses dam and Chepkoilel fish farm respectively. The dams and the fish farm did not pose significant difference in the parasitic indices, probably due to their proximity.     Oxacillin resistant Staphlococcus aureus among HIV infected and non-infected Kenyan patients

    Background: Infections due to methicillin resistant S. aureus (MRSA) present global challenges to clinicians since therapeutic options are limited and suboptimal dosing contributes to heightened mortality and increased length of hospital stay particularly among the HIV infected patients.Objectives: To assess the prevalence and relative risk of MRSA infections in HIV infected patients.Design: Cross sectional analytical study.Setting: Kenya Medical Research Institute, Opportunistic Infection Laboratories in Nairobi.Subjects: Four hundred and thirty six male and female patients aged one to 65 years, of whom 220 were HIV-infected and 216 were non-infected.Results: There was 436 male (57.1%) and female (42.9%) respondents.     AbstractBackgroundThe Saudi national asthma protocol (SNAP) for asthma management was released in 1995 in an attempt to streamline asthma management practices in Saudi Arabia and improve the quality of care for asthma. Up to our knowledge, few studies assessed the adherence of Saudi physicians to the national asthma guidelines.ObjectivesThe objective of this present study was to assess the level of adherence of pediatricians and primary care physicians (PCPs) to the current SNAP recommendations and identify barriers to physician adherence.MethodologyThis is a cross-sectional study involving pediatricians and PCPs selected randomly from five major governmental hospitals in Riyadh, Saudi Arabia. Subjects were administered a self-administered questionnaire comprising 24 questions assessing their awareness of SNAP and their level of adherence to the recommendations.ResultsThe response rate was 38% (80/206). Out of most of the physicians who responded, 70% (56) were aware of SNAP, and only 78.2% (n=43) of them had modified their management of asthmatic patients according to the SNAP recommendations. The level of knowledge of the pharmacotherapy and diagnostic parts of the guidelines ranged between 41.5% and 90.7% in the pharmacotherapy part, and 53.7–59.6% in the diagnostic part. The most common barriers to adherence to SNAP were lack of awareness (25.2%), patient non- compliance (18.9%) and lack of resources (13.5%). There was no significant difference in awareness between pediatricians and PCPs (69.2%, 70.7% respectively).ConclusionThis study reveals a substantial gap between the actual care provided by pediatricians and PCPs to asthmatic patients and the recommendations formulated in the Saudi National Asthma Protocol (SNAP). Lack of awareness remains the most common barrier for adherence to the guidelines followed by patient non-compliance. To improve SNAP guideline adherence, tailored interventions that address barriers to adherence need to be implemented

    Tolerance to fusarium verticillioides infection and fumonisin accumulation in maize F1 hybrids and subsequent F2 populations

Grain infection by these fungi occurs before harvest and selection of parental lines resistant to fumonisin accumulation for breeding purposes is the most effective and environmentally friendly control strategy for F. verticillioides. This study intended to evaluate F1 hybrids and F2 breeding populations in Kenya for improved resistance to FER and fumonisin contamination. Trials were artificially inoculated and FER severity, F. verticillioides accumulation, and fumonisin contamination were determined. Inheritance of resistance was also determined in the F1 hybrids. CML444 × MIRTC5, R119W × CKL05015, and CML444 × CKL05015 exhibited little to no FER and had the least fungal and fumonisin contamination, respectively. Inbred lines CML495, CKL05015, and P502 had negative, significant general combining ability (GCA) estimates for F. verticillioides colonization and fumonisin contamination, but positive, significant GCA estimates for 1,000-kernel weight, respectively. The genotype × environment interaction was the main source of variation observed in the F2 populations with R119W × CKL05015 and CML444 × CKL05015 being the most tolerant to fungal and fumonisin contamination in Kiboko and MIRTC5 × CML495 the most tolerant in Katumani.Publisher's versio

    Mapping Opuntia stricta in the arid and semi-arid environment of Kenya using sentinel-2 imagery and ensemble machine learning classifiers

    Globally, grassland biomes form one of the largest terrestrial covers and present critical social–ecological benefits. In Kenya, Arid and Semi-arid Lands (ASAL) occupy 80% of the landscape and are critical for the livelihoods of millions of pastoralists. However, they have been invaded by Invasive Plant Species (IPS) thereby compromising their ecosystem functionality. Opuntia stricta, a well-known IPS, has invaded the ASAL in Kenya and poses a threat to pastoralism, leading to livestock mortality and land degradation. Thus, identification and detailed estimation of its cover is essential for drawing an effective management strategy. The study aimed at utilizing the Sentinel-2 multispectral sensor to detect Opuntia stricta in a heterogeneous ASAL in Laikipia County, using ensemble machine learning classifiers. To illustrate the potential of Sentinel-2, the detection of Opuntia stricta was based on only the spectral bands as well as in combination with vegetation and topographic indices using Extreme Gradient Boost (XGBoost) and Random Forest (RF) classifiers to detect the abundance. Study results showed that the overall accuracies of Sentinel 2 spectral bands were 80% and 84.4%, while that of combined spectral bands, vegetation, and topographic indices was 89.2% and 92.4% for XGBoost and RF classifiers, respectively. The inclusion of topographic indices that enhance characterization of biological processes, and vegetation indices that minimize the influence of soil and the effects of atmosphere, contributed by improving the accuracy of the classification. Qualitatively, Opuntia stricta spatially was found along river banks, flood plains, and near settlements but limited in forested areas. Our results demonstrated the potential of Sentinel-2 multispectral sensors to effectively detect and map Opuntia stricta in a complex heterogeneous ASAL, which can support conservation and rangeland management policies that aim to map and list threatened areas, and conserve the biodiversity and productivity of rangeland ecosystems

    Implementation of a comprehensive program including psycho-social and treatment literacy activities to improve adherence to HIV care and treatment for a pediatric population in Kenya

    BACKGROUND: To achieve good clinical outcomes with HAART, patient adherence to treatment and care is a key factor. Since the literature on how to care for pediatric HIV patients is limited, we describe here adherence interventions implemented in our comprehensive care program in a resource-limited setting in Kenya. METHODS: We based our program on factors reported to influence adherence to HIV care and treatment. We describe, in detail, our program with respect to how we adapted our clinical settings, implemented psycho-social support activities for children and their caregivers and developed treatment literacy for children and teenagers living with HIV/AIDS. RESULTS: This paper focused on the details of the program, with the treatment outcomes as secondary. However, our program appeared to have been effective; for 648 children under 15 years of age who were started on HAART, the Kaplan-Meier mortality survival estimate was 95.27% (95%CI 93.16-96.74) at 12 months after the time of initiation of HAART. CONCLUSION: Our model of pediatric HIV/AIDS care, focused on a child-centered approach with inclusion of caregivers and extended family, addressed the main factors influencing treatment adherence. It appeared to produce good results and is replicable in resource-limited settings
