    Catch-Up Growth Following Fetal Growth Restriction Promotes Rapid Restoration of Fat Mass but Without Metabolic Consequences at One Year of Age

    BACKGROUND: Fetal growth restriction (FGR) followed by rapid weight gain during early life has been suggested to be the initial sequence promoting central adiposity and insulin resistance. However, the link between fetal and early postnatal growth and the associated anthropometric and metabolic changes have been poorly studied. METHODOLOGY/PRINCIPAL FINDINGS: Over the first year of post-natal life, changes in body mass index, skinfold thickness and hormonal concentrations were prospectively monitored in 94 infants in whom the fetal growth velocity had previously been measured using a repeated standardized procedure of ultrasound fetal measurements. 45 infants, thinner at birth, had experienced previous FGR (FGR+) regardless of birth weight. Growth pattern in the first four months of life was characterized by greater change in BMI z-score in FGR+ (+1.26+/-1.2 vs +0.58 +/-1.17 SD in FGR-) resulting in the restoration of BMI and of fat mass to values similar to FGR-, independently of caloric intakes. Growth velocity after 4 months was similar and BMI z-score and fat mass remained similar at 12 months of age. At both time-points, fetal growth velocity was an independent predictor of fat mass in FGR+. At one year, fasting insulin levels were not different but leptin was significantly higher in the FGR+ (4.43+/-1.41 vs 2.63+/-1 ng/ml in FGR-). CONCLUSION: Early catch-up growth is related to the fetal growth pattern itself, irrespective of birth weight, and is associated with higher insulin sensitivity and lower leptin levels after birth. Catch-up growth promotes the restoration of body size and fat stores without detrimental consequences at one year of age on body composition or metabolic profile. The higher leptin concentration at one year may reflect a positive energy balance in children who previously faced fetal growth restriction

Source data for the figures are available as Supplementary Data 2.BACKGROUND: Monogenic insulin resistance (IR) includes lipodystrophy and disorders of insulin signalling. We sought to assess the effects of interventions in monogenic IR, stratified by genetic aetiology. METHODS: Systematic review using PubMed, MEDLINE and Embase (1 January 1987 to 23 June 2021). Studies reporting individual-level effects of pharmacologic and/or surgical interventions in monogenic IR were eligible. Individual data were extracted and duplicates were removed. Outcomes were analysed for each gene and intervention, and in aggregate for partial, generalised and all lipodystrophy. RESULTS: 10 non-randomised experimental studies, 8 case series, and 23 case reports meet inclusion criteria, all rated as having moderate or serious risk of bias. Metreleptin use is associated with the lowering of triglycerides and haemoglobin A1c (HbA1c) in all lipodystrophy (n = 111), partial (n = 71) and generalised lipodystrophy (n = 41), and in LMNA, PPARG, AGPAT2 or BSCL2 subgroups (n = 72,13,21 and 21 respectively). Body Mass Index (BMI) is lowered in partial and generalised lipodystrophy, and in LMNA or BSCL2, but not PPARG or AGPAT2 subgroups. Thiazolidinediones are associated with improved HbA1c and triglycerides in all lipodystrophy (n = 13), improved HbA1c in PPARG (n = 5), and improved triglycerides in LMNA (n = 7). In INSR-related IR, rhIGF-1, alone or with IGFBP3, is associated with improved HbA1c (n = 17). The small size or absence of other genotype-treatment combinations preclude firm conclusions. CONCLUSIONS: The evidence guiding genotype-specific treatment of monogenic IR is of low to very low quality. Metreleptin and Thiazolidinediones appear to improve metabolic markers in lipodystrophy, and rhIGF-1 appears to lower HbA1c in INSR-related IR. For other interventions, there is insufficient evidence to assess efficacy and risks in aggregated lipodystrophy or genetic subgroups.Wellcome TrustWellcome Trus

    El futuro del mundo es el futuro de los jóvenes. Estos estudiantes dejan volar su imaginación para adivinar una salida al futuro: En la Universidad Centroamericana de Nicaragua se ha introducido en el primer año de estudios de todas las carreras, la asignatura de Cultura, Ciencia y Mundo actual. Se pretende con ella colocamos ante los retos que la realidad cambiante del mundo y la particular situación de Centroamérica y Nicaragua nos hacen sentir de un modo más acuciante. La crisis ecológica, el nuevo orden mundial, la depauperización creciente de grandes mayorías del planeta, la brecha creciente entre los países ricos y bs países pobres, la crisis de los partidos políticos... son algunos de los temas que hemos abordado. La sensación de los alumnos, ante la complejidad y enormidad de los problemas, ante unas tendencias a corto plazo más bien negativas, ante la falta de recetas y de proyectos, ha sido hasta cierto punto paralizante y angustiosa. Si al peso de la vida cotidiana le añadimos las estadísticas y la falta de perspectivas no es para menos. Recuerdo, por ejemplo, que un alumno que llegó tarde al examen de la asignatura me dijo como justificación que lo acababan de atracar bajando del bus. Ante mi mirada de incredulidad se levantó la camisa y me dijo: "Mire, mire, aquí está la herida, nadie me ayudó". Si para sobrevivir es importante no perder el sentido de realidad no lo es menos no perder el sentido de lo posible: "Un acontecimiento y una verdad posibles no son iguales a un acontecimiento y una verdad reales, y el primero contiene un fuego, una perspectiva, una voluntad de construir que, lejos de temer la realidad, la trata simplemente como una tarea y una invención perpetuas" (Robert Musil, El hombre sin cualidades). Quizás por todo eso, cuando tuvimos que escoger uno de los trabajos que bajo el título "Alternativas de futuro" tenían que redactar los alumnos, nos decidimos por aquel que más imaginación y sentido del humor derrochaba frente a los que a fuerza de repetir los datos y la gravedad de los problemas nos daban pocas alas para construir el futuro (Jorge Corominas)
