    Evaluation of the Effect of Different Ferrule Designs on Fracture Resistance of Maxillary Incisors Restored with Bonded Posts and Cores

    Introduction: In cases of severe hard tissue loss, 2 mm circumferential ferrule is difficult to achieve. So in these cases we should use different ferrule designs.This in vitro study investigated the effect of different ferrule designs on the fractureresistance of teeth restored with bonded post and cores.Materials and Methods: Forty freshly-extracted central incisors were endodontically treated. The teeth were randomly divided into four groups; group 1 were teeth with 2 mm circumferential ferrule above the CEJ, group 2 were teeth with 2 mm ferrule only on the palatal side of the teeth, group 3 consisted of teeth with 2 mm ferrule only on the facial side and group 4 were teeth with 2 mm ferrule on the palatal and facial side of teeth with interproximal concavities.All teeth were restored with fiber posts and composite cores. The specimen was mounted on a universal testing machine and compressive load was applied to the long axis of the specimen until failure occurred.Results: The fracture resistance was 533.79 ± 232.28 in group 1, 634.75± 133.35 in group 2, 828.90 ±118.27 in group 3 and 678.78± 160.20 in group 4. The post hoc analysis showed statistically significant difference between groups 1 and 3 .Conclusions: The results of this in vitro study showed that facial ferrule increases the fracture resistance of endodontically treated teeth restored with bonded post and cores

    A TWO-STAGE METHOD FOR DAMAGE DETECTION OF LARGE-SCALE STRUCTURES

    ABSTRACT A novel two-stage algorithm for detection of damages in large-scale structures under static loads is presented. The technique utilizes the vector of response change (VRC) and sensitivities of responses with respect to the elemental damage parameters (RSEs). It is shown that VRC approximately lies in the subspace spanned by RSEs corresponding to the damaged elements. The property is leveraged in the first stage of the proposed method by seeking RSEs whose spanned subspace best contains the VRC. Consequently, the corresponding elements are regarded as damage candidates. To alleviate the exploration among RSEs, they are first partitioned into several clusters. Subsequently, discrete ant colony optimization (ACO) is utilized to find the clusters containing the RSEs of damaged elements. In the second stage of the algorithm, damage amounts for the restricted elements are determined using a continuous version of ACO. Two numerical examples are studied. The results illustrate that the method is both robust and efficient for detection of damages in large-scale structures

    A rare disease with a rare presentation: hemi-atrophy caused by fibromuscular dysplasia in a 27 month old girl

    We discuss the case of a 27-month-old girl afflicted with fibromuscular dysplasia. She presented with hemiatrophy of left upper and lower limbs, nail dystrophy, ulcers on the tips of her toes, cold and painful limbs, foot drop, and hypertension. The initial appearance started at 2 months of age and other diagnoses such as complex regional pain syndrome, reflex sympathetic syndrome, vasculitis and coagulation disorders had been considered. Angiography revealed that all the arterial branches of the left lower and upper limbs, from brachial to ulnar and radial, and from iliac and femoral to tibialis arteries were affected. Sural nerve biopsy confirmed the diagnosis. In the follow-up visits until 2 years after the patient’s discharge she did not develop any new problem and her blood pressure was controlled by enalapril and amlodipine

    Association of plasma total testosterone level and metabolic syndrome in adult males

    Introduction: Low testosterone level has strongly been correlated with body fat accumulation and abdominal obesity in men. Objectives: This study aimed to evaluate testosterone level in men with and without metabolic syndrome to determine the relationship between testosterone and metabolic syndrome. Patients and Methods: This case-control study was conducted on 172 cases of metabolic syndrome and 172 participants as a control group in Rasoul Akram hospital, Tehran, Iran. Demographic characteristics, fasting blood sugar (FBS), high-density lipoprotein (HDL), low-density lipoprotein (LDL), cholesterol, triglyceride (TG), and testosterone levels were recorded. SPSS version 21.0 and SAS version 9.1 were used for statistical analysis. Level of significance was considered 0.05. Results: The mean age of the two groups were 45.1 ± 9.3 years and 41.5 ± 11.2 years, respectively. There was a significant difference in serum testosterone levels between both groups and low testosterone levels were associated with metabolic syndrome (P < 0.001). Serum testosterone levels showed a significant negative correlation with age in the metabolic syndrome group (r =-0.16, P = 0.02). The relationship between metabolic syndrome and total plasma testosterone level using logistic regression model showed that, by increasing the total plasma testosterone level, the odds ratio for metabolic syndrome was 0.076 (95 CI: 0.027-0.216; P < 0.001). Conclusion: According to the results, low level of testosterone was related to the presence of metabolic syndrome in adult males. Future studies can investigate diagnostic value of testosterone level in this syndrome. © 2020 The Author(s)

    Measuring Servitization Progress and Outcome:The Case of ‘Advanced Services’

    The purpose of this paper is to establish a framework for assessing the progress and outcome of a manufacturer’s transformation towards becoming a provider of ‘advanced services’ – a complex bundling of products and services, whereby manufacturers offer capabilities and outcomes instead of products alone. ‘Advanced services’ represent the most complex offering in the current servitization trend among manufacturers. However, current performance measures lack the breadth and focus to assess progress or outcomes, and so support research and practice of organisational transformation efforts required. To address this gap the paper investigates how a manufacturer’s efforts to become an ‘advanced services’ provider can be comprehensively measured, and develops a framework for assessing the transformation journey towards becoming an ‘advanced services’ provider. The research method is based on (1) a systematic literature review process to create a comprehensive set of service-related performance measures that are available to assess a manufacturer’s servitization efforts, followed by (2) an engagement with an expert panel to synthesise the identified measures and create a set of ‘advanced services’ performance measures. The proposed framework is presented as a scorecard that can be used in practice to assess the progress and outcome of a manufacturer’s transformation towards becoming a provider of ‘advanced services’

    Evolution of hepatitis B virus surface gene and protein among Iranian chronic carriers from different provinces

    Background and Objectives: Iranian chronic HBV carrier�s population has shown a unique pattern of genotype D distribution all around the country. The aim of this study was to explore more details of evolutionary history of carriers based on structural surface proteins from different provinces. Materials and Methods: Sera obtained from 360 isolates from 12 Different regions of country were used for amplification and sequencing of surface proteins. A detailed mutational analysis was undertaken. Results: The total ratio for Missense/Silent nucleotide substitutions was 0.96. Sistan and Kermanshah showed the lowest rate of evolution between provinces (P = 0.055). On the other hand, Khorasan Razavi and Khoozestan contained the highest ratio (P = 0.055). The rest of regions were laid between these two extremes. Azarbayjan and Guilan showed the highest proportion of immune epitope distribution (91.3 and 96, respectively). Conversely, Sistan and Tehran harbored the least percentage (66.6 and 68.8, respectively). Kermanshah province contained only 5.2, whereas Isfahan had 54.5 of B cell epitope distribution. In terms of T helper epitopes, all provinces showed a somehow homogeneity: 22.58 (Fars) to 46.6 (Khuzestan). On the other hand, distribution of substitutions within the CTL epitopes showed a wide range of variation between 6.6 (Khuzestan) and 63 (Kermanshah). Conclusion: Further to low selection pressure found in Iranian population, the variations between different regions designate random genetic drift within the surface proteins. These finding would have some applications in terms of specific antiviral regimen, design of more efficient vaccine and public health issues. © 2015, Tehran University of Medical Science.     Assessment of the genetic risks of a metallic alloy used in medical implants

    The use of artificial implants provides a palliative or permanent solution for individuals who have lost some bodily function through disease, an accident or natural wear. This functional loss can be compensated for by the use of medical devices produced from special biomaterials. Titanium alloy (Ti-6Al-4V) is a well-established primary metallic biomaterial for orthopedic implants, but the toxicity of the chemical components of this alloy has become an issue of concern. In this work, we used the MTT assay and micronucleus assay to examine the cytotoxicity and genotoxicity, respectively, of an extract obtained from this alloy. The MTT assay indicated that the mitochondrial activity and cell viability of CHO-K1 cells were unaffected by exposure to the extract. However, the micronucleus assay revealed DNA damage and an increase in micronucleus frequency at all of the concentrations tested. These results show that ions released from Ti-6Al-4V alloy can cause DNA and nuclear damage and reinforce the importance of assessing the safety of metallic medical devices constructed from biomaterials
