The Topical Problems of Regional Health Care in the Coverage of Omsk Media
In the modern world, one of the most urgent is the problem of health care, and the media are the leading conductors of medical and preventive knowledge in the field of health care to the general public. Purpose of the study: to reveal the problemthematic originality of materials devoted to health problems in the region, in the sociopolitical edition βArguments and Facts in Omskβ. Research objectives: to study the role of the press in promoting public health problems; analyze media texts on regional health problems; to form an idea of the problem-thematic field of the healthcare sector as an urgent problem of the humanitarian agenda in the practice of modern Russian media; determine on the basis of monitoring and analysis βpain pointsβ of Omsk healthcare. The object of the research is media texts published in the newspaper βAiF in Omskβ. The subject of the research is the problem-thematic originality of publications on health problems at the regional level. Omsk press in the end of XIX β beginning of XX centuries, including specialized one, in comparison with other regional publications has not been studied enough. This factor explains the relevance of the work. The object of the study is βProtocols of the Omsk Medical Societyβ, βMedical and sanitary chronicle of the city of Omskβ. The subject is the genre-thematic specificity and functional originality of Omsk pre-revolutionary medical periodicals. The goal is to reveal the content and genre originality of the pre-revolutionary medical periodicals of Omsk as the administrative center of the Steppe region. To achieve this goal, it is necessary to complete a number of research tasks: to study the history of the organization of the Omsk healthcare system, including its sanitary part; in the course of content analysis to determine the content, genre and functional specificity of the analyzed publications. Several groups of research methods were used in the work: general scientific, special-historical, special methods of journalistic analysis. Issues of the βProtocols of the Omsk Medical Societyβ served as empirical material. The chronological period of the study is 1883β1911
FinTech & New Digital Instruments. Post-Crisis Developments: Russia and Europe
The article examines 2020 post-crisis results and 2021 trends in FinTech regulation development. FinTech, being a relatively new term, has become a completely new industry, which combines rapidly developing technologies and financial products (including digital assets) or services. The year 2020, despite the pandemic and localization (and maybe even more so), seems as significant, if not more so, for market change and further development. The world has changed, and new technologies are vital for successful competition among financial players and even for their survival. Most of the leading international financial centers have focused on regulating FinTech and the use of innovations in classic highly regulated areas. In the article we address the pros and cons of technology regulation and make a comparative analysis of the leading revolutionary trends. The most revolutionary developments have appeared in smaller European countries, which the leaders are forced to follow. Law harmonization has become a natural step forward for Europe to regulate blockchain businesses and to agree on terminology and risk prevention measures for innovation support. The research examines the development and regulation of FinTech in such jurisdictions as Switzerland, Malta, Liechtenstein, Gibraltar and the United States. Spinal dural arteriovenous fistulas: Pathogenesis, clinical manifestations, diagnosis, treatment
The paper describes spinal dural arteriovenous fistulas (SDAVF), the most common type of spinal cord vascular anomalies. SDAVFs account for 60β80% of the spinal cord vascular anomalies. The causes of SDAVFs, the specific features of their hemodynamics, and their classification remain the subject matter of disputes.SDAVFs form in dura mater tissue, on the dorsal surface of radicular cuffs. The pathogenesis of neurological disorders in SDAVF has determined the name Β«venous hypertensive myelopathyΒ», a spinal cord injury occurring in their presence. Pain and paresthesias, cacesthesia (more commonly in their distal parts), and motor disorders as flail legs are observed at the onset of SDAVF in typical cases. On average, 12 to 44 months elapse to establish its diagnosis. In addition of motor and sensory disorders, sphincter impairments and sexual dysfunction are detected in the patients at the time of diagnosis. By this time, most patients have already neurological disability.The paper presents the history of studying SDAVF, the existing classifications of arteriovenous malformations and fistulas, the clinical manifestations of venous hypertensive myelopathy in SDAVF, neuroimaging findings, and treatment options
Regression of lumbar disk herniation
Compression of the spinal nerve root, giving rise to pain and sensory and motor disorders in the area of its innervation is the most vivid manifestation of herniated intervertebral disk. Different treatment modalities, including neurosurgery, for evolving these conditions are discussed. There has been recent evidence that spontaneous regression of disk herniation can regress. The paper describes a female patient with large lateralized disc extrusion that has caused compression of the nerve root S1, leading to obvious myotonic and radicular syndrome. Magnetic resonance imaging has shown that the clinical manifestations of discogenic radiculopathy, as well myotonic syndrome and morphological changes completely regressed 8 months later. The likely mechanism is inflammation-induced resorption of a large herniated disk fragment, which agrees with the data available in the literature. A decision to perform neurosurgery for which the patient had indications was made during her first consultation. After regression of discogenic radiculopathy, there was only moderate pain caused by musculoskeletal diseases (facet syndrome, piriformis syndrome) that were successfully eliminated by minimally invasive techniques

nanometre level of single molecules up several orders of magnitude to longer
length scales in emergent structures of cells, tissues and organisms.
Biological molecules are often characterized by an underlying instability, in
that multiple metastable free energy states exist which are separated by energy
levels of typically just a few multiples of the thermal energy scale of kBT,
where kB is the Boltzmann constant and T the absolute temperature, implying
complex, dynamic inter-conversion kinetics across this bumpy free energy
landscape in the relatively hot, wet environment of real, living biological
matter. The key utility of single-molecule biophysics lies in its ability to
probe the underlying heterogeneity of free energy states across a population of
molecules, which in general is too challenging for conventional ensemble level
approaches which measure mean average properties. Parallel developments in both
experimental and theoretical techniques have been key to the latest insights
and are enabling the development of highly-multiplexed, correlative techniques
to tackle previously intractable biological problems. Experimentally,
technological developments in the sensitivity and speed of biomolecular
detectors, the stability and efficiency of light sources, probes and
microfluidics, have enabled and driven the study of heterogeneous behaviours
both in vitro and in vivo that were previously undetectable by ensemble
methods..
An Agent-Based Model of a Hepatic Inflammatory Response to Salmonella: A Computational Study under a Large Set of Experimental Data

Plos One, 11(8), 39. doi:10.1371/journal.pone.0161131We present an agent-based model (ABM) to simulate a hepatic inflammatory response (HIR) in a mouse infected by Salmonella that sometimes progressed to problematic proportions, known as "sepsis". Based on over 200 published studies, this ABM describes interactions among 21 cells or cytokines and incorporates 226 experimental data sets and/or data estimates from those reports to simulate a mouse HIR in silico. Our simulated results reproduced dynamic patterns of HIR reported in the literature. As shown in vivo, our model also demonstrated that sepsis was highly related to the initial Salmonella dose and the presence of components of the adaptive immune system. We determined that high mobility group box-1, C-reactive protein, and the interleukin-10: tumor necrosis factor-a ratio, and CD4+ T cell: CD8+ T cell ratio, all recognized as biomarkers during HIR, significantly correlated with outcomes of HIR. During therapy-directed silico simulations, our results demonstrated that anti-agent intervention impacted the survival rates of septic individuals in a time-dependent manner. By specifying the infected species, source of infection, and site of infection, this ABM enabled us to reproduce the kinetics of several essential indicators during a HIR, observe distinct dynamic patterns that are manifested during HIR, and allowed us to test proposed therapy-directed treatments. Although limitation still exists, this ABM is a step forward because it links underlying biological processes to computational simulation and was validated through a series of comparisons between the simulated results and experimental studies
