    Effects of remote limb ischemic conditioning on muscle strength in healthy young adults: A randomized controlled trial

    Remote limb ischemic conditioning (RLIC) is a clinically feasible method in which brief, sub-lethal bouts of ischemia protects remote organs or tissues from subsequent ischemic injury. A single session of RLIC can improve exercise performance and increase muscle activation. The purpose of this study, therefore, was to assess the effects of a brief, two-week protocol of repeated RLIC combined with strength training on strength gain and neural adaptation in healthy young adults. Participants age 18-40 years were randomized to receive either RLIC plus strength training (n = 15) or sham conditioning plus strength training (n = 15). Participants received RLIC or sham conditioning over 8 visits using a blood pressure cuff on the dominant arm with 5 cycles of 5 minutes each alternating inflation and deflation. Visits 3-8 paired conditioning with wrist extensors strength training on the non-dominant (non-conditioned) arm using standard guidelines. Changes in one repetition maximum (1 RM) and electromyography (EMG) amplitude were compared between groups. Both groups were trained at a similar workload. While both groups gained strength over time (P = 0.001), the RLIC group had greater strength gains (9.38 ± 1.01 lbs) than the sham group (6.3 ± 1.08 lbs, P = 0.035). There was not a significant group x time interaction in EMG amplitude (P = 0.231). The RLIC group had larger percent changes in 1 RM (43.8% vs. 26.1%, P = 0.003) and EMG amplitudes (31.0% vs. 8.6%, P = 0.023) compared to sham conditioning. RLIC holds promise for enhancing muscle strength in healthy young and older adults, as well as clinical populations that could benefit from strength training

    A case-control study of the effectiveness of tissue plasminogen activator on 6 month patients -- reported outcomes and health care utilization

    none5siWe examined the benefit of tissue plasminogen activator (tPA), delivered as part of usual stroke management, on patient-reported outcomes and health care utilization. Using a case control design, patients who received tPA as part of usual stroke management were compared with patients who would have received tPA had they arrived to the hospital within the therapeutic time window. Data were collected from surveys 6 months after stroke using standardized patient-reported outcome measures and questions about health care utilization. Demographic and medical data were acquired from hospital records. Patients were matched on stroke severity, age, race, and gender. Matching was done with 1:2 ratio of tPA to controls. Results were compared between groups with 1-tailed tests because of a directionally specific hypothesis in favor of the tPA group. The tPA (n = 78) and control (n = 156) groups were matched across variables, except for stroke severity, which was better in the control group; subsequent analyses controlled for this mismatch. The tPA group reported better physical function, communication, cognitive ability, depressive symptomatology, and quality of life/participation compared with the control group. Fewer people in the tPA group reported skilled nursing facility stays, emergency department visits, and rehospitalizations after their stroke compared with controls. Reports of other postacute services were not different between groups. Although it is known that tPA reduces disability, this is the first study to demonstrate the effectiveness of tPA in improving meaningful, patientreported outcomes.     The Wide Integral Field Infrared Spectrograph: Commissioning Results and On-sky Performance

    We have recently commissioned a novel infrared (0.91.70.9-1.7 μ\mum) integral field spectrograph (IFS) called the Wide Integral Field Infrared Spectrograph (WIFIS). WIFIS is a unique instrument that offers a very large field-of-view (50^{\prime\prime} x 20^{\prime\prime}) on the 2.3-meter Bok telescope at Kitt Peak, USA for seeing-limited observations at moderate spectral resolving power. The measured spatial sampling scale is 1×1\sim1\times1^{\prime\prime} and its spectral resolving power is R2,500R\sim2,500 and 3,0003,000 in the zJzJ (0.91.350.9-1.35 μ\mum) and HshortH_{short} (1.51.71.5-1.7 μ\mum) modes, respectively. WIFIS's corresponding etendue is larger than existing near-infrared (NIR) IFSes, which are mostly designed to work with adaptive optics systems and therefore have very narrow fields. For this reason, this instrument is specifically suited for studying very extended objects in the near-infrared such as supernovae remnants, galactic star forming regions, and nearby galaxies, which are not easily accessible by other NIR IFSes. This enables scientific programs that were not originally possible, such as detailed surveys of a large number of nearby galaxies or a full accounting of nucleosynthetic yields of Milky Way supernova remnants. WIFIS is also designed to be easily adaptable to be used with larger telescopes. In this paper, we report on the overall performance characteristics of the instrument, which were measured during our commissioning runs in the second half of 2017. We present measurements of spectral resolving power, image quality, instrumental background, and overall efficiency and sensitivity of WIFIS and compare them with our design expectations. Finally, we present a few example observations that demonstrate WIFIS's full capability to carry out infrared imaging spectroscopy of extended objects, which is enabled by our custom data reduction pipeline.Comment: Published in the Proceedings of SPIE Astronomical Telescopes and Instrumentation 2018. 17 pages, 13 figure

    Caregiver Satisfaction with a Video Telehealth Home Safety Evaluation for Dementia

    Family caregivers are vital to telehealth-delivered dementia care. The objective of this mixed methods descriptive study conducted in the VA Bedford Healthcare System was to examine caregiver satisfaction with a video telehealth dementia home safety occupational therapy evaluation. Ten caregivers of Veterans with dementia participated. Ratings of caregiver satisfaction, measured by nine Likert scale items including ability to see and hear, were examined in relation to person and visit-related contextual factors extracted from RA field notes, to develop an in-depth understanding of caregiver experience. Person factors included caregiver age and gender and Veteran cognitive status. Visit-related contextual factors included occurrence  of technical glitches. Caregiver visit satisfaction was overall positive, with exceptions related to technological glitches and the presence of the person with dementia during the visit. Veteran cognitive status appeared to influence caregiver satisfaction. Implications of the study are that proactively addressing technical glitches and incorporating dementia stage-specific approaches may optimize caregivers’ telehealth experience.

    Do ecological characteristics drive the prevalence of Panulirus argus virus 1 (PaV1) in juvenile Caribbean spiny lobsters in a tropical reef lagoon?

    PaV1 is a pathogenic virus found only to infect Caribbean spiny lobsters Panulirus argus, a major fishing resource. P. argus is a benthic mesopredator and has a complex life history, with several ontogenetic habitat changes. Habitat characteristics and species diversity of surrounding communities may have implications for disease dynamics. This is of more concern for juvenile lobsters, which are more susceptible to PaV1 and are far less mobile than adult lobsters. We targeted a population of juvenile P. argus in a reef lagoon in Mexico, where PaV1 was first observed in 2001. Prevalence has been since irregularly assessed, but in 2016 we began a more systematic assessment, with two sampling periods per year (June and November) in three different zones of the reef lagoon. To examine the relationship between PaV1 prevalence and potential ecological determinants, we assessed habitat complexity, cover of different substrates, and invertebrate community composition in all zones during the first four sampling periods (June and November 2016 and 2017). Habitat complexity and percent cover of some substrates varied with zone and sampling period. This was the case for seagrass and macroalgae, which nevertheless were the dominant substrates. The invertebrate community composition varied with sampling period, but not with zone. Probability of infection decreased with increasing lobster size, consistent with previous studies, but was not affected by zone (i.e., variations in ecological characteristics did not appear to be sufficiently large so as to influence prevalence of PaV1). This result possibly reflects the dominance of marine vegetation and suggests that lobsters can be sampled throughout the reef lagoon to assess PaV1 prevalence. Prevalence was higher in only one of seven sampling periods (November 2017), suggesting that the pathogen has leveled off to an enzootic level

    Gas separation membranes made through thermal rearrangement of ortho methoxypolyimides

    Producción CientíficaOrtho-methoxypolyimides were prepared by the classical chemical imidization method and also by azeotropic imidization, using 3,3’-dimethoxybenzidine (DMAB) and hexafluoroisopropylidene diphthalic anhydride (6FDA) as monomers. High molecular weights were achieved by both methods, and the physical properties of the materials were investigated by spectroscopic and thermal analytical techniques. Polymers exhibited excellent thermal properties and film-forming ability, which allowed them to be tested as dense membranes for gas separation after a convenient treatment at high temperature (450 °C) to promote thermal rearrangement (TR). Spectroscopic evidences indicated that the final composition of the TR materials seemed not to correspond to TR-polibenzoxazoles, as it is the case when ortho-hydroxypolyimides are exposed to similar thermal treatments. A detailed study was carried out with the purpose of elucidating the actual mechanism of rearrangement, comparing ortho-hydroxypolyimides and ortho-acetylpolyimides with the ortho-methoxypolyimides obtained in this work. Results led to the conclusion that the chemical nature of the final TR materials attained from ortho-methoxypolyimides is rather complex because imide, cyclolactam and benzoxazole groups seems to be formed under thermal treatment, and the proportion of the different moieties greatly depends on the ortho-substituent, on the synthesis route used to prepare the polyimide and on the time-temperature schedule applied in the thermal rearrangement. The gas permeation properties exhibited by thermally treated ortho-methoxypolyimides compared very well with those of other TR-PBOs reported so far, showing O2 permeability of 94 barrers and CO2 permeability of 540 Barrers.Junta de Castilla y León (programa de apoyo a proyectos de investigación – Ref.
