    Etiology of ‘sinus headache’. Moving the focus from rhinology to neurology. A systematic review

    ‘Sinus headache and/or facial pain’ (SH) is a common complaint encountered by otorhi-nolaryngologists, neurologists and general practitioners. However, several studies suggested that the majority of those cases may be attributed to primary headaches (i.e., migraine and tension-type headache (TTH). The purpose of this review is to evaluate the etiology of SH. The first part in-cludes cross-sectional studies analyzing the prevalence of respective diagnoses in subjects with SH. The majority of these publications indicate that migraine and TTH are the most prevalent causes of SH, although most of these studies were conducted in a clinical setting. The second part of this review included treatment trials in subjects with SH. The findings from this part of the review show that SH without rhinosinusitis responds well to pharmacotherapy targeted at primary headaches. This observation further supports a neurologic etiology of the majority of SH cases

    Measuring the Public Account- ability of New Modes of Governance

    We present an encompassing research endeavour on the public accountability of new modes of governance in Europe. The aim of this project is to measure the salience, tonality and framing of regulatory bodies and public interest organisations in newspaper coverage and parliamentary debates over the last 15 years. In order to achieve this, we use language technology which is still underused in political science text analyses. Institutionally, the project has emerged from a collaboration between a computational linguistics and a political science department

    Electrical Quality Assurance of the Superconducting Circuits during LHC Machine Assembly

    Based on the LHC powering reference database, all-together 1750 superconducting circuits were connected in the various cryogenic transfer lines of the LHC machine. Testing the continuity, magnet polarity, and the quality of the electrical insulation were the main tasks of the Electrical Quality Assurance (ELQA) activities during the LHC machine assembly. With the assembly of the LHC now complete, the paper reviews the work flow, resources, and the qualification results including the different types of electrical non-conformities

    Four-colour photometry of eclipsing binaries. XLI uvby light curves for AD Bootis, HW Canis Majoris, SW Canis Majoris, V636 Centauri, VZ Hydrae, and WZ Ophiuchi

    CONTEXT: Accurate mass, radius, and abundance determinations from binaries provide important information on stellar evolution, fundamental to central fields in modern astrophysics and cosmology. AIMS: Within the long-term Copenhagen Binary Project, we aim to obtain high-quality light curves and standard photometry for double-lined detached eclipsing binaries with late A, F, and G type main-sequence components, needed for the determination of accurate absolute dimensions and abundances, and for detailed comparisons with results from recent stellar evolutionary models. METHODS: Between March 1985 and July 2007, we carried out photometric observations of AD Boo, HW CMA, SW CMa, V636 Cen, VZ Hya, and WZ Oph at the Str"omgren Automatic Telescope at ESO, La Silla. RESULTS: We obtained complete uvby light curves, ephemerides, and standard uvby\beta indices for all six systems.For V636 Cen and HW CMa, we present the first modern light curves, whereas for AD Boo, SW CMa, VZ Hya, and WZ Oph, they are both more accurate and more complete than earlier data. Due to a high orbital eccentricity (e = 0.50), combined with a low orbital inclination (i = 84.7), only one eclipse, close to periastron, occurs for HW CMa. For the two other eccentric systems, V636 Cen (e = 0.134) and SW CMa (e = 0.316), apsidal motion has been detected with periods of 5270 +/- 335 and 14900 +/- 3600 years, respectively.Comment: Only change is: Bottom lines (hopefully) not truncated anymore.     Excellent Response to OnabotulinumtoxinA: Different Definitions, Different Predictors

    The identification of patients who can benefit the most from the available preventive treatments is important in chronic migraine. We explored the rate of excellent responders to onabotulinumtoxinA in a multicenter European study and explored the predictors of such response, according to different definitions. A pooled analysis on chronic migraineurs treated with onabotulinumtoxinA and followed-up for, at least, 9 months was performed. Excellent responders were defined either as patients with a ≥75% decrease in monthly headache days (percent-based excellent responders) or as patients with <4 monthly headache days (frequency-based excellent responders). The characteristics of excellent responders at the baseline were compared with the ones of patients with a <30% decrease in monthly headache days. Percent-based excellent responders represented about 10% of the sample, whilst frequency-based excellent responders were about 5% of the sample. Compared with non-responders, percent-based excellent responders had a higher prevalence of medication overuse and a higher excellent response rate even after the 1st and the 2nd injection. Females were less like to be frequency-based excellent responders. Chronic migraine sufferers without medication overuse and of female sex may find fewer benefits with onabotulinumtoxinA. Additionally, the excellent response status is identifiable after the first cycle

    Synoptic tool for reporting of hematological and lymphoid neoplasms based on World Health Organization classification and College of American Pathologists checklist

    <p>Abstract</p> <p>Background</p> <p>Synoptic reporting, either as part of the pathology report or replacing some free text component incorporates standardized data elements in the form of checklists for pathology reporting. This ensures the pathologists make note of these findings in their reports, thereby improving the quality and uniformity of information in the pathology reports.</p> <p>Methods</p> <p>The purpose of this project is to develop the entire set of elements in the synoptic templates or "worksheets" for hematologic and lymphoid neoplasms using the World Health Organization (WHO) Classification and the College of American Pathologists (CAP) Cancer Checklists. The CAP checklists' content was supplemented with the most updated classification scheme (WHO classification), specimen details, staging as well as information on various ancillary techniques such as cytochemical studies, immunophenotyping, cytogenetics including Fluorescent In-situ Hybridization (FISH) studies and genotyping. We have used a digital synoptic reporting system as part of an existing laboratory information system (LIS), CoPathPlus, from Cerner DHT, Inc. The synoptic elements are presented as discrete data points, so that a data element such as tumor type is assigned from the synoptic value dictionary under the value of tumor type, allowing the user to search for just those cases that have that value point populated.</p> <p>Results</p> <p>These synoptic worksheets are implemented for use in our LIS. The data is stored as discrete data elements appear as an accession summary within the final pathology report. In addition, the synoptic data can be exported to research databases for linking pathological details on banked tissues.</p> <p>Conclusion</p> <p>Synoptic reporting provides a structured method for entering the diagnostic as well as prognostic information for a particular pathology specimen or sample, thereby reducing transcription services and reducing specimen turnaround time. Furthermore, it provides accurate and consistent diagnostic information dictated by pathologists as a basis for appropriate therapeutic modalities. Using synoptic reports, consistent data elements with minimized typographical and transcription errors can be generated and placed in the LIS relational database, enabling quicker access to desired information and improved communication for appropriate cancer management. The templates will also eventually serve as a conduit for capturing and storing data in the virtual biorepository for translational research. Such uniformity of data lends itself to subsequent ease of data viewing and extraction, as demonstrated by rapid production of standardized, high-quality data from the hemopoietic and lymphoid neoplasm specimens.</p

    Atypical presentation of colon adenocarcinoma: a case report

    <p>Abstract</p> <p>Introduction</p> <p>Adenocarcinoma of the colon is the most common histopathological type of colorectal cancer. In Western Europe and the United States, it is the third most common type and accounts for 98% of cancers of the large intestine. In Uganda, as elsewhere in Africa, the majority of patients are elderly (at least 60 years old). However, more recently, it has been observed that younger patients (less than 40 years of age) are presenting with the disease. There is also an increase in its incidence and most patients present late, possibly because of the lack of a comprehensive national screening and preventive health-care program. We describe the clinicopathological features of colorectal carcinoma in the case of a young man in Kampala, Uganda.</p> <p>Case presentation</p> <p>A 27-year-old man from Kampala, Uganda, presented with gross abdominal distension, progressive loss of weight, and fever. He was initially screened for tuberculosis, hepatitis, and lymphoma, and human immunodeficiency virus/acquired immunodeficiency syndrome infection. After a battery of tests, a diagnosis of colorectal carcinoma was finally established with hematoxylin and eosin staining of a cell block made from the sediment of a liter of cytospun ascitic fluid, which showed atypical glands floating in abundant extracellular mucin, suggestive of adenocarcinoma. Ancillary tests with alcian blue/periodic acid Schiff and mucicarmine staining revealed that it was a mucinous adenocarcinoma. Immunohistochemistry showed strong positivity with CDX2, confirming that the origin of the tumor was the colon.</p> <p>Conclusions</p> <p>Colorectal carcinoma has been noted to occur with increasing frequency in young adults in Africa. Most patients have mucinous adenocarcinoma, present late, and have rapid disease progression and poor outcome. Therefore, colorectal malignancy should no longer be excluded from consideration only on the basis of a patient's age. A high index of suspicion is important in the diagnosis of colorectal malignancy in young African patients.</p
