Hipotonia dializacyjna u chorego leczonego przewlekłymi hemodializami - trudny przypadek kliniczny
Hipotonia śróddializacyjna stanowi częste powikłanie
dializoterapii wpływające istotnie na rokowanie pacjentów
leczonych hemodializami. Prezentowany przypadek
kliniczny opisuje pacjenta z częstymi epizodami
hipotonii śróddializacyjnej wynikającymi zarówno
z braku współpracy chorego (istotne klinicznie przewadnianie
w okresie między dializami), miażdżycy,
jak i niedostatecznej reakcji na stymulację autonomicznego
układu nerwowego prowadzącej do braku
odpowiedzi hemodynamicznej na spadek ciśnienia
krwi. Zastosowane działania terapeutyczne: edukacja chorego, zmniejszenie temperatury płynu dializacyjnego
do 36oC, profilowanie stężenia sodu (od 146
do 133 mmol/l) i ultrafiltracji oraz dializowanie
w rytmie 4 razy w tygodniu przyniosły znaczną poprawę
kliniczną pomimo niepełnej, ale lepszej niż
na początku leczenia, współpracy chorego.
Ze względu na wieloczynnikowość problemu hipotonii
śróddializacyjnej leczenie pacjentów cierpiących
na to powikłanie musi być wielotorowe, a warunkiem
jego powodzenia jest współpraca chorych
z zespołem leczącym

Electronic measurement of medication adherence in patients with heart failure
Wstęp. Niewydolność serca (HF) jest istotnym problemem klinicznym i socjoekonomicznym zarówno w Polsce, jak i na świecie. Pomimo rozwoju nowoczesnej i skutecznej farmakoterapii, brakuje obiektywnych danych na temat stosowania się do zaleceń lekarskich dotyczących przyjmowania leków przez pacjentów. Celem poniższej pracy była ocena regularności przyjmowania leków i jej związku ze zmiennymi demograficznymi i klinicznymi u pacjentów z HF.
Materiały i metody. Przeprowadzono prospektywne badanie kohortowe na populacji 25 pacjentów z rozpoznaną HF. Przyjmowanie leków oceniano przez 30 kolejnych dni za pomocą elektronicznego systemu monitorowania przyjmowania leków (MEMS) — elektronicznej nakrętki zakładanej na pojemnik z wybranym lekiem, pozwalającej na cyfrowy pomiar dokładnego momentu przyjęcia leku stosowanego w HF. Na podstawie uzyskanych danych pacjentów sklasyfikowano jako przestrzegających lub nieprzestrzegających zaleceń oraz oceniono wpływ zmiennych klinicznych i demograficznych na regularność przyjmowania leków.
Wyniki. Uzyskano pełne wyniki z urządzeń MEMS od 22 pacjentów. Mediana wieku pacjentów wynosiła 70 lat (IQR = 14), średnia frakcja wyrzutowa lewej komory wynosiła 33% ± 12. Średni odsetek prawidłowo przyjętych dawek wynosił 89% ± 17. Jako nieregularnie przyjmujących leki zostało sklasyfikowanych 27% pacjentów (n = 6). Byli oni istotnie młodsi (54 vs. 71 lat; p = 0,015), mieli niższą frakcję wyrzutową lewej komory (24 vs. 36%; p = 0,04) i byli częściej hospitalizowani z powodu ostrej HF (83 vs. 19%; p = 0,011).
Wnioski. W krótkoterminowej obserwacji znaczący odsetek pacjentów z HF nie przyjmował leków zgodnie z zaleceniami. Zidentyfikowana w niniejszym badaniu populacja pacjentów częściej nieprzestrzegających zaleceń lekarskich wymaga intensywniejszej i ukierunkowanej interwencji ze strony systemu opieki zdrowotnej. Opracowanie skutecznej interwencji i zogniskowanie jej na powyższej podgrupie ma szansę poprawić rokowanie chorych.Introduction. Heart failure (HF) is a significant clinical and socioeconomic problem both in Poland and around the world. However, objective data on the level of adherence in the era of improved medical therapy is lacking. Therefore, the aim of the study was to investigate the level of medication adherence and its association with demographical and clinical variables in patients with HF.
Material and methods. We have conducted a prospective cohort study of 25 patients with diagnosed HF. Medication adherence was measured for 30 consecutive days using the Medication Event Monitoring System (MEMS) — an electronic cap attached to the medication container, allows to record the exact moment of taking the measured medicine. Based on the acquired data, patients were classified as adherent or non-adherent using an evidence-based cut-point. In addition to adherence measurement, patients’ demographic and clinical information was collected.
Results. Twenty-two patients provided full results from the MEMS devices. The median age of the patients was 70 years (interquartile range =14), and the mean left ventricular ejection fraction was 33% ± 12. The mean percentage of correct doses was 89% ± 17. Twenty-seven percent of patients (n = 6) were classified as non-adherent. Patients classified as non-adherent were significantly younger (54 vs. 71 years; p = 0.015), had a lower left ventricular ejection fraction (24 vs. 36%; p = 0.04), and were more frequently enrolled after HF hospitalization (83 vs. 19%; p = 0.011).
< p style="text-align: justify;" > Conclusions. In the short-term observation, a significant proportion of patients with HF were found to be non-adherent. In our study, we identified a population with an increased risk of non-adherence. Those patients require the implementation of more intensive and targeted healthcare system-based interventions in order to improve their prognosis

Constraining the degree of the dominant mode in QQ Vir
We present early results of the application of a method which uses multicolor
photometry and spectroscopy for \ell discrimination. This method has been
successfully applied to the pulsating hot subdwarf Balloon 090100001. Here we
apply the method to QQ Vir (PG1325+101). This star was observed
spectroscopically and photometrically in 2008. Details on spectroscopy can be
found in Telting et al. (2010) while photometry and preliminary results on \ell
discrimination are provided here. The main aim of this work was to compare the
value of the \ell parameter derived for the main mode in QQ Vir to previously

Determination of physical parameters of the eclipsing binary V729 Cyg
We report new BVRI photometric observations of an eclipsing, massive binary V729 Cyg taken between 2008 and 2011. We performed light curve modeling of the new data and those available in the literature using the Wilson-Devinney code. The best fit was obtained for a contact configuration, similarly to results derived previously by other investigators. However, a huge temperature difference of about 10 000K - 12 000K was derived, inconsistent with theoretical calculations. Ruling out a possibility of V729 Cyg being a semi-detached system harbouring an accretion disk, we determined physical parameters of components
Inhibitors of sodium-glucose transport protein 2: A new multidirectional therapeutic option for heart failure patients
Several mechanisms have been suggested to explain positive cardiovascular effects observed in studies with sodium-glucose co-transporter 2 (SGLT2) inhibitors. The reduction in glucose reabsorption in proximal tubuli induced by SGLT2 inhibitors increases urinary glucose and sodium excretion resulting in increased osmotic diuresis and consequently in decreased plasma volume, followed by reduced preload. In addition, the hemodynamic effects of SGLT2 inhibition were observed in both hyper and euglycemic patients. Due to the complex and multidirectional effects induced by SGLT2 inhibitors, this originally antidiabetic group of drugs has been successfully used to treat patients with heart failure as well as for subjects with chronic kidney disease. Moreover, their therapeutic potential seems to be even broader than the indications studied to date

On the search for the right definition of heart failure with preserved ejection fraction
The definition of heart failure with preserved ejection fraction (HFpEF) has evolved from a clinically based “diagnosis of exclusion” to definitions focused on objective evidence of diastolic dysfunction and/or elevated left ventricular filling pressures. Despite advances in our understanding of HFpEF pathophysiology and the development of more sophisticated imaging modalities, the diagnosis of HFpEF remains challenging, especially in the chronic setting, given that symptoms are provoked by exertion and diagnostic evaluation is largely conducted at rest. Invasive hemodynamic study, and in particular — invasive exercise testing, is considered the reference method for HFpEF diagnosis. However, its use is limited as opposed to the high number of patients with suspected HFpEF. Thus, diagnostic criteria for HFpEF should be principally based on non-invasive measurements. As no single non-invasive variable can adequately corroborate or refute the diagnosis, different combinations of clinical, echocardiographic, and/or biochemical parameters have been introduced. Recent years have brought an abundance of HFpEF definitions. Here, we present and compare four of them: 1) the 2016 European Society of Cardiology criteria for HFpEF; 2) the 2016 echocardiographic algorithm for diagnosing diastolic dysfunction; 3) the 2018 evidence-based H2FPEF score; and 4) the most recent, 2019 Heart Failure Association HFA-PEFF algorithm. These definitions vary in their approach to diagnosis, as well as sensitivity and specificity. Further studies to validate and compare the diagnostic accuracy of HFpEF definitions are warranted. Nevertheless, it seems that the best HFpEF definition would originate from a randomized clinical trial showing a favorable effect of an intervention on prognosis in HFpEF
