Recent advances in hepatic transplantation at the University of Pittsburgh.
FK506 undoubtedly improved the survival advantage of hepatic allotransplantation. Hepatic-intestinal and multivisceral transplantation has also become a feasible therapy for patients with combined intestinal and liver failure. With better understanding of the immunologic and metabolic aspects of allo- and xenotransplantation, further clinical attempts to transplant animal organs to humans may be considered with the hope for a better outcome in the very near future
Application of B-splines to determining eigen-spectrum of Feshbach molecules
The B-spline basis set method is applied to determining the rovibrational
eigen-spectrum of diatomic molecules. A particular attention is paid to a
challenging numerical task of an accurate and efficient description of the
vibrational levels near the dissociation limit (halo-state and Feshbach
molecules). Advantages of using B-splines are highlighted by comparing the
performance of the method with that of the commonly-used discrete variable
representation (DVR) approach. Several model cases, including the Morse
potential and realistic potentials with 1/R^3 and 1/R^6 long-range dependence
of the internuclear separation are studied. We find that the B-spline method is
superior to the DVR approach and it is robust enough to properly describe the
Feshbach molecules. The developed numerical method is applied to studying the
universal relation of the energy of the last bound state to the scattering
length. We numerically illustrate the validity of the quantum-defect-theoretic

Risk factors for severe pre-eclampsia and eclamsia in Mulago Hospital, Kampala, Uganda
Objective: To determine the risk factors for severe pre-eclampsia and eclampsia in Mulago Hospital, Kampala, Uganda.Design: A case control study.Setting: Mulago Hospital labour wardSubject: One hundread and fourty three women with severe pre-eclampsia/ eclampsia and 500 women with normal delivery.Results: The predictors of severe pre-eclampsia/eclampsia were: low socio - economic status (OR 7.6, 95%CI 3.9 - 26.9), chronic hypertension (OR 26.9,95% CI 4.3-170.4), family history of hypertension (OR 1.9, 95% CI 1.2-2.9), nulliparity (OR 2.2, 95% CI 1.2-4.3) and delivery of male babies (OR 1.5, 95% CI 1.0 to 2.3).Conclusion: Severe pre - eclampsia is one of the main causes of maternal morbidity and mortality in Mulago hospital. The predictors of severe pre - eclampsia were chronic hypertension, family history of hypertension, low socio - economic status, nulliparity and delivering male babies. Health workers need to identify women at risk and offer them counseling and, those who develop pre - eclampsia be referred and managed inhospitals with expertise and facilities. Those who develop severe pre-eclampsia should be delivered immediately so as to reduce the morbidity and mortality associated with this condition
Spectrotemporal modulation provides a unifying framework for auditory cortical asymmetries
The principles underlying functional asymmetries in cortex remain debated. For example, it is accepted that speech is processed bilaterally in auditory cortex, but a left hemisphere dominance emerges when the input is interpreted linguistically. The mechanisms, however, are contested, such as what sound features or processing principles underlie laterality. Recent findings across species (humans, canines and bats) provide converging evidence that spectrotemporal sound features drive asymmetrical responses. Typically, accounts invoke models wherein the hemispheres differ in time-frequency resolution or integration window size. We develop a framework that builds on and unifies prevailing models, using spectrotemporal modulation space. Using signal processing techniques motivated by neural responses, we test this approach, employing behavioural and neurophysiological measures. We show how psychophysical judgements align with spectrotemporal modulations and then characterize the neural sensitivities to temporal and spectral modulations. We demonstrate differential contributions from both hemispheres, with a left lateralization for temporal modulations and a weaker right lateralization for spectral modulations. We argue that representations in the modulation domain provide a more mechanistic basis to account for lateralization in auditory cortex
The Concept of Culture in Critical Mathematics Education

The final authenticated version is available online at: http://dx.doi.org/10.1007/978-3-319-77760-3A well-known critique in the research literature of critical mathematics education suggests that framing educational questions in cultural terms can encourage ethnic-cultural essentialism, obscure conflicts within cultures and promote an ethnographic or anthropological stance towards learners. Nevertheless, we believe that some of the obstacles to learning mathematics are cultural. ‘Stereotype threat’, for example, has a basis in culture. Consequently, the aims of critical mathematics education cannot be seriously pursued without including a cultural approach in educational research. We argue that an adequate conception of culture is available and should include normative/descriptive and material/ideal dyads as dialectical moments
Genetic diversity of the rain tree (Albizia saman) in Colombian seasonally dry tropical forest for informing conservation and restoration interventions
Albizia saman is a multipurpose tree species of seasonally dry tropical forests (SDTFs) of Mesoamerica and northern South America typically cultivated in silvopastoral and other agroforestry systems around the world, a trend that is bound to increase in light of multimillion hectare commitments for forest and landscape restoration. The effective conservation and sustainable use of A. saman requires detailed knowledge of its genetic diversity across its native distribution range of which surprisingly little is known to date. We assessed the genetic diversity and structure of A.saman across twelve representative locations of SDTF in Colombia, and how they may have been shaped by past climatic changes and human influence. We found four different genetic groups which may be the result of differentiation due to isolation of populations in preglacial times. The current distribution and mixture of genetic groups across STDF fragments we observed might be the result of range expansion of SDTFs during the last glacial period followed by range contraction during the Holocene and human‐influenced movement of germplasm associated with cattle ranching. Despite the fragmented state of the presumed natural A. saman stands we sampled, we did not find any signs of inbreeding, suggesting that gene flow is not jeopardized in humanized landscapes. However, further research is needed to assess potential deleterious effects of fragmentation on progeny. Climate change is not expected to seriously threaten the in situ persistence of A. saman populations and might present opportunities for future range expansion. However, the sourcing of germplasm for tree planting activities needs to be aligned with the genetic affinity of reference populations across the distribution of Colombian SDTFs. We identify priority source populations for in situ conservation based on their high genetic diversity, lack or limited signs of admixture, and/or genetic uniqueness

Metformin therapy in a hyperandrogenic anovulatory mutant murine model with polycystic ovarian syndrome characteristics improves oocyte maturity during superovulation
<p>Abstract</p> <p>Background</p> <p>Metformin, an oral biguanide traditionally used for the treatment of type 2 diabetes, is widely used for the management of polycystic ovary syndrome (PCOS)-related anovulation. Because of the significant prevalence of insulin resistance and glucose intolerance in PCOS patients, and their putative role in ovulatory dysfunction, the use of metformin was touted as a means to improve ovulatory function and reproductive outcomes in PCOS patients. To date, there has been inconsistent evidence to demonstrate a favorable effect of metformin on oocyte quality and competence in women with PCOS. Given the heterogeneous nature of this disorder, we hypothesized that metformin may be beneficial in mice with aberrant metabolic characteristics similar to a significant number of PCOS patients. The aim of this study was to gain insight into the <it>in vitro </it>and <it>in vivo </it>effects of metformin on oocyte development and ovulatory function.</p> <p>Methods</p> <p>We utilized metformin treatment in the transgenic <it>ob/ob </it>and <it>db/db </it>mutant murine models which demonstrate metabolic and reproductive characteristics similar to women with PCOS. Results: Metformin did not improve <it>in vitro </it>oocyte maturation nor did it have an appreciable effect on <it>in vitro </it>granulosa cell luteinization <it>(</it>progesterone production) in any genotype studied. Although both mutant strains have evidence of hyperandrogenemia, anovulation, and hyperinsulinemia, only <it>db/db </it>mice treated with metformin had a greater number of mature oocytes and total overall oocytes compared to control. There was no observed impact on body mass, or serum glucose and androgens in any genotype.</p> <p>Conclusions</p> <p>Our data provide evidence to suggest that metformin may optimize ovulatory performance in mice with a specific reproductive and metabolic phenotype shared by women with PCOS. The only obvious difference between the mutant murine models is that the <it>db/db </it>mice have elevated leptin levels raising the questions of whether their response to metformin is related to elevated leptin levels and/or if a subset of PCOS women with hyperleptinemia may be responsive to metformin therapy. Further study is needed to better define a subset of women with PCOS that may be responsive to metformin.</p
Nonthermal Hard X-ray Emission and Iron Kalpha Emission from a Superflare on II Pegasi
We report on an X-ray flare detected on the active binary system II~Pegasi
with the Swift telescope. The trigger had a 10-200 keV luminosity of
2.2 erg s-- a superflare, by comparison with energies of
typical stellar flares on active binary systems. The trigger spectrum indicates
a hot thermal plasma with T180 K. X-ray spectral analysis
from 0.8--200 keV with the X-Ray Telescope and BAT in the next two orbits
reveals evidence for a thermal component (T80 K) and Fe K 6.4
keV emission. A tail of emission out to 200 keV can be fit with either an
extremely high temperature thermal plasma (TK) or power-law
emission. Based on analogies with solar flares, we attribute the excess
continuum emission to nonthermal thick-target bremsstrahlung emission from a
population of accelerated electrons. We estimate the radiated energy from
0.01--200 keV to be erg, the total radiated energy over
all wavelengths erg, the energy in nonthermal electrons above 20
keV erg, and conducted energy erg. The
nonthermal interpretation gives a reasonable value for the total energy in
electrons 20 keV when compared to the upper and lower bounds on the thermal
energy content of the flare. This marks the first occasion in which evidence
exists for nonthermal hard X-ray emission from a stellar flare. We investigate
the emission mechanism responsible for producing the 6.4 keV feature, and find
that collisional ionization from nonthermal electrons appears to be more
plausible than the photoionization mechanism usually invoked on the Sun and

NCI-MATCH Arms N & P: Phase II study of PI3K beta inhibitor GSK2636771 in patients (pts) with cancers (ca) with PTEN mutation/deletion (mut/del) or PTEN protein loss
Background: The NCI-MATCH trial is the largest national study (1173 sites) for ptswith relapsed/ refractory solid tumors, lymphomas and myeloma, which assigns tar-geted therapies based on individual tumor molecular alterations detected using theadapted Oncomine AmpliSeq panel (143 genes) and immunohistochemistry (IHC).We hypothesized that patients with PTEN-deficient cancers enrolled to Arms N and Pmay benefit from treatment with the PI3K beta-selective inhibitor GSK2636771.
Methods: Eligibility: relapsed/refractory ca, good end-organ function, and ECOG PS ≤ 1. Pts were screened for molecular alterations by centralized testing on fresh tumor biopsy and had deleterious PTEN mut/del without loss of expression (Arm N) or complete loss of cytoplasmic and nuclear PTEN staining on IHC (Arm P), and no other aberrations activating the PI3K/MTOR and MAPK pathways (mut in PIK3CA, PIK3R1, BRAF, KRAS, AKT1, TSC1/2, mTOR, RHEB, NF2, NRAS, HRAS). Pts received GSK2636771 400mg/day (28-days cycles). RECIST 1.1 overall response rate (ORR) was the primary endpoint.
Results: Of 59 enrolled pts, 56 were eligible and received treatment. Of 22 pts with PTEN mut/del (Arm N: 6 uterine, 2 breast, 2 prostate, 2 head/neck ca, 10 other), all are off treatment as of analysis (14 disease progression, 4 for adverse events [AEs], 4 other). One pt (4.5%) with prostate ca (PTEN deletion, MPRSS2-ERG fusion) attained a partial response (-42%). Of 7 (32%) pts with stable disease (SD), 2 had SD \u3e 6 months (uterine leiomyosarcoma; endometrial carcinoma). Of 34 pts with loss of PTEN protein by IHC (Arm P: 7 prostate, 6 breast, 3 squamous anal ca, 2 cholangiocarcinoma, 16 other), all are off treatment as of analysis (26 disease progression, 4 for AE, 4 other). Of 9 (37.5%) pts with SD, 3 had SD \u3e 6 months (prostate cancer; squamous bladder cancer, squamous anal cancer). Median progression-free survival was 1.8 months for both arms. Gr ≥ 3 treatment-related (tr) reversible toxicities were experienced by 30% (7) and 20% (7) of pts in arms N and P, respectively. No tr Gr 5 toxicities were observed in either arm.
Conclusions: Single agent GSK2636771 has very modest activity in ca with PTEN gene mutation/deletion and/or PTEN protein loss
A Systematic Review of Digital Storytelling as Psychotherapy for People with Mental Health Needs
Storytelling is used in many cultures as an important way to communicate historical messages of lived experiences intergenerationally. Past studies indicated that storytelling is an effective tool in education and mental health, but evidence of the therapeutic use of digital storytelling is scarce. This review therefore explored available literature evidence of the use of digital storytelling media as mental health therapy to identify knowledge gaps for a further Secret Story Network role-playing game intervention study. Based on some key search terms and a set of inclusion and exclusion criteria, 14 full-text articles were systematically selected through searches of mainly EBSCOhost that connected seven databases, including AMED, BNI, CINAHL, EMBASE, EMCARE, Medline, and APA PsycInfo. The studies reviewed suggested a tactical focus on adolescents and adults older than 18 years and more females than men. Ten digital storytelling media interventions were found in 11 sources, but only two studies on older adults with dementia had a therapeutic intervention framework. Qualitative and mixed-methods reported in nine sources were shown to be the common study methodologies. The evidence extracted also revealed six criteria for classifying storytelling types, and the purposes, effects, benefits, and uses of digital storytelling indicated a general assumption that digital storytelling interventions have therapeutic, educational, social, and psychological effects. However, evidence suggests that while digital storytelling may significantly reduce symptoms of depression (p < .05), its effects on other mental health symptoms are inconclusive. Thus, further research into the psychotherapeutic effect of digital storytelling is necessary. Five implications for future research are discussed
