    Molar band re-use and decontamination: a survey of specialists

    Objective: To determine the pattern of use and re-use of orthodontic molar bands, and examine infection control measures in a sample of UK orthodontists. Design: Questionnaire survey. Subjects and methods: Questionnaires were sent to 204 individuals selected at random from the UK Specialist Orthodontist list. Follow-up questionnaires were sent to those that had not replied within 8 weeks. An overall response rate of 74.5% was achieved. Main outcome measures: Orthodontic band use and re-use and cross-infection control. Results: The reported rates of pre-sterilization cleaning and sterilization of orthodontic instruments were 92 and 100%, respectively. Of the respondents, 90% were using bands for molar teeth with the remainder routinely used bonded attachments. Most clinicians (95%) using bands routinely re-used them after being tried-in with 5% discarding them. Pre-sterilization cleaning of re-used molar bands was carried out by 92% of respondents who reclaimed bands. Sterilization of these bands was then carried out by most specialists apart from 2. Conclusions: The majority of UK specialist orthodontists who responded to the questionnaire are adhering to universal precautions for cross-infection control and are carrying out approved decontamination procedures. The majority are also reusing orthodontic bands that have been tried in the mouth, but found to be the wrong size. The great diversity of reported procedures for decontamination of instruments and bands suggest that more research is required to provide guidelines into the most effective method

    Smallholder farmers’ perspectives on advisory extension services: a case study of the Gamo Communities of Southern Ethiopia

    This is a case study-based research project investigating the status of Advisory Extension Services in southern Ethiopia. The goal was to determine whether available service provisions meet the requirements of smallholder farmers and enabled them to improve their farming practices and livelihoods. A combination of an exploratory inductive approach and mixed methods was used (e.g., questionnaire survey, focus group discussions, key informant interviews). Participants included members of farming households, and agents, experts, and providers working in the agricultural rural sector. The key findings suggested that limited access to resources and unpredictable environmental conditions were stifling smallholder farmer innovation and livelihoods. Service provisions should be better tailored to local conditions, provide greater resource access, and work more closely with farmers. The development and implementation of service provision should involve a wide range of institutions and farmers throughout the process. Local community- and farmer-based organisations are especially important, and can work alongside innovative and talented farmers to enable more effective dissemination of information. Agricultural rural development and service provision should focus greater attention on the views and perspectives of farmers from a range of areas with differing socio-demographic and agro-ecological characteristics for comparative analysis

    This position paper, directed at Reichsführer-SS Heinrich Himmler, from the Reich Association of Dowsing addresses the situation of dowsing and dowsing research and offers proposals for official actions. The Association emphasises that a scientific explanation is urgently needed, but much progress had been made despite a crippling lack of resources. They cite evidence of dowsing successes (tens of thousands of cases) and note that failures occur in every profession and in dowsing they are not disproportionate. They call attention to the obstacles of wild dowsers, not connected with the Association, as well as the hostility of the science community which influenced national and regional authorities. They note that practical professions such as engineering are friendlier to dowsing as they are more concerned with practicality than theory. The paper cites recent support from scientists and politicians, in particular Rudolf Hess, Hermann Göring and Wilhelm Keppler. They also note that Göring assured them that Hitler was not against dowsing. The proposals include 1) a ban on wild dowsing - any activity not conducted within the scope of the Reich Association for Dowsing; 2.) Police and legal recognition of the Association so that all paid dowsers must work under its auspices; 3.) Work to revoke the prohibition on dowsing in many agencies; 4.) Work to stop attacks on dowsing from the science community; 5.)     Non-invasive Ischaemia Testing in Patients With Prior Coronary Artery Bypass Graft Surgery: Technical Challenges, Limitations, and Future Directions

    Coronary artery bypass graft (CABG) surgery effectively relieves symptoms and improves outcomes. However, patients undergoing CABG surgery typically have advanced coronary atherosclerotic disease and remain at high risk for symptom recurrence and adverse events. Functional non-invasive testing for ischaemia is commonly used as a gatekeeper for invasive coronary and graft angiography, and for guiding subsequent revascularisation decisions. However, performing and interpreting non-invasive ischaemia testing in patients post CABG is challenging, irrespective of the imaging modality used. Multiple factors including advanced multi-vessel native vessel disease, variability in coronary hemodynamics post-surgery, differences in graft lengths and vasomotor properties, and complex myocardial scar morphology are only some of the pathophysiological mechanisms that complicate ischaemia evaluation in this patient population. Systematic assessment of the impact of these challenges in relation to each imaging modality may help optimize diagnostic test selection by incorporating clinical information and individual patient characteristics. At the same time, recent technological advances in cardiac imaging including improvements in image quality, wider availability of quantitative techniques for measuring myocardial blood flow and the introduction of artificial intelligence-based approaches for image analysis offer the opportunity to re-evaluate the value of ischaemia testing, providing new insights into the pathophysiological processes that determine outcomes in this patient population

    The impact of androgen receptor polymorphism and parental ethnicity on semen quality in young men from Latvia

    Recent studies on young men from the general population have demonstrated geographic and ethnic differences in semen quality. The aim of this study was to investigate whether reported ethnic differences in semen quality might be associated with the maternally derived CAG and GGN polymorphisms in the androgen receptor gene or paternal ethnicity. In total 114 military conscripts from Latvia were included in the study. Information on maternal and parental ethnicity was collected by questionnaires. CAG and GGN repeats were analysed by direct sequencing of leukocyte DNA. Men with Latvian mothers (n = 83) had marginally shorter CAG repeat length (21.6 ± 2.9) as compared with those with non-Latvian mothers (22.9 ± 3.2, n = 31), not reaching statistical significance (p = 0.053). Sperm concentration did not differ significantly between these two groups (76 ± 59 and 70 ± 52, p = 0.9 respectively). In contrast, significantly higher sperm concentration and total sperm count were found in men with Latvian fathers (n = 77) as compared with men with non-Latvian fathers (n = 37) (80 ± 61 vs. 62 ± 48, p = 0.035, for sperm concentration and 225.7 ± 209 vs. 158.4 ± 134.4, p = 0.002, for total sperm count respectively). CAG repeat length did not correlate with any semen parameters in the whole population. However, GGN repeat length correlated with semen volume: men with GGN > 23 presented with higher semen volume (3.2 ± 2.1) as compared with men with GGN = 23 (2.6 ± 1.3, p = 0.04) or GGN < 23 (2.0 ± 1.2, p = 0.006). We conclude that GGN repeat length has an impact on semen volume, whereas differences in sperm numbers are associated with the paternal ethnicity.Peer reviewe

    Management of orthodontic emergencies in primary care – self-reported confidence of general dental practitioners

    Objective: To determine general dental practitioners’ (GDPs) confidence in managing orthodontic emergencies. Design: Cross-sectional study. Setting: Primary dental care. Subjects and methods: An online survey was distributed to dentists practicing in Wales. The survey collected basic demographic information and included descriptions of ten common orthodontic emergency scenarios. Main outcome measure Respondents’ self-reported confidence in managing the orthodontic emergency scenarios on a 5‑point Likert scale. Differences between the Likert responses and the demographic variables were investigated using chi-squared tests. Results: The median number of orthodontic emergencies encountered by respondents over the previous six months was 1. Overall, the self-reported confidence of respondents was high with 7 of the 10 scenarios presented scoring a median of 4 indicating that GDPs were ‘confident’ in their management. Statistical analysis revealed that GDPs who saw more orthodontic emergencies in the previous six months were more confident when managing the presented scenarios. Other variables such as age, gender, geographic location of practice and number of years practising dentistry were not associated with self reported confidence. Conclusions: Despite GDPs encountering very few orthodontic emergencies in primary care, they appear to be confident in dealing with commonly arising orthodontic emergency situations
