    Contemporary conditions require health professionals both to employ published evidence in their individual practices and as a profession to produce valid evidence of their outcome effectiveness. Heretofore, these two processes of evidence-based practice have often been confounded as one. This theoretical paper separates the two processes into «Evidence-Supported Practice» and «Evidence-Informed Practice.» Each requires a different approach to evidence accumulation and use. Nonetheless, the two processes can and should be interlinked. For external (research) evidence, the research pyramid model values equally the internal and external validity of studies, as both are important for the implementation of external evidence. Furthermore, external evidence must be combined with internal evidence (data generated in the course of interaction with a client) in the decision-making of practitioners. Examples from recent research on occupational therapy practice and literature from several other health professions are cited for illustration. This paper formulates a more comprehensive model for evidence-based practice. From this model follow specific recommendations for practitioners, researchers, and educators in the health professions. Abstract (German) Von den Gesundheitsberufen wird einerseits verlangt, dass jeder einzelne Praktiker wissenschaftsbasiert arbeitet, d.h. publizierte Evidenz in der Praxis anwendet (Evidence Based Practice, EBP). Andererseits mĂŒssen sie als Profession valide Evidenz der Ergebnisse ihres professionellen Handelns produzieren. Diese zwei Dimensionen von EBP werden in der Diskussion oft nicht auseinander gehalten, was zu Verwirrung fĂŒhrt. Dieser theoretische Beitrag differenziert diese beiden Prozesse als “Evidence- Supported Practice” und “Evidence-Informed Practice” und zeigt, wie sie wieder zusammengefĂŒhrt werden können. Zur Bewertung der externen und internen ValiditĂ€t von externer Evidenz wird die Forschungspyramide verwendet. In der Alltagspraxis mĂŒssen externe und interne Evidenzarten verwendet werden.     Where do games of innovation come from? Explaining the persistence of dynamic innovation patterns

    This paper contributes to explaining how and why distinct games of innovation emerge by\ud suggesting that games are nested in innovation systems with persistent innovation dynamics.\ud Dominant lifecycle models focus on how innovation systems transit from an effervescent\ud stage, to product innovation, to process innovation, and so on. They propose specific\ud mechanisms and limiting conditions that affect knowledge production and investment to\ud explain these systematic transitions. Building on these models, we rethink the conditions\ud and mechanisms of innovation to suggest that endogenous renewal cycles can re-create the\ud knowledge and funding necessary to maintain innovation systems for long periods in one\ud stage. We take steps towards developing a theoretical model of innovation dynamics that\ud extends the applicability of lifecycle theories and unifies them with emerging views such as\ud high-velocity innovation and hyper-competition. We also describe three possible types of\ud endogenous renewal cycles, each sustaining a different level of knowledge dynamism and\ud enabling different types of games of innovation

    We present the chemical and biological synthesis of caged phosphoproteins using the in vitro nonsense codon suppression methodology. Specifically, phosphoamino acid analogues of serine, threonine, and tyrosine with a single photocleavable o-nitrophenylethyl caging group were synthesized as the amino acyl tRNA adducts for insertion into full-length proteins. For this purpose, a novel phosphitylating agent was developed. The successful incorporation of these bulky and charged amino acids into the α-subunit of the nicotinic acetyl choline receptor (nAChR) and the vasodilator-stimulated phosphoprotein (VASP) using an in vitro translation system is reported

    Bridging Alone: Religious Conservatism, Marital Homogamy, and Voluntary Association Membership

    This study characterizes social insularity of religiously conservative American married couples by examining patterns of voluntary associationmembership. Constructing a dataset of 3938 marital dyads from the second wave of the National Survey of Families and Households, the author investigates whether conservative religious homogamy encourages membership in religious voluntary groups and discourages membership in secular voluntary groups. Results indicate that couples’ shared affiliation with conservative denominations, paired with beliefs in biblical authority and inerrancy, increases the likelihood of religious group membership for husbands and wives and reduces the likelihood of secular group membership for wives, but not for husbands. The social insularity of conservative religious groups appears to be reinforced by homogamy—particularly by wives who share faith with husbands

    New debris disk candidates: 24 micron stellar excesses at 100 Myr

    Sixty three members of the 100 Myr old open cluster M47 (NGC 2422) have been detected at 24 micron with Spitzer. The Be star V 378 Pup shows an excess both in the near-infrared and at 24 micron (K-[24] = 2.4 mag), probably due to free-free emission from the gaseous envelope. Seven other early-type stars show smaller excesses, K-[24] = 0.6-0.9. Among late-type stars, two show large excesses: P922 - a K1V star with K-[24] = 1.08 pm 0.11 and P1121 - an F9V star with K-[24] = 3.72 pm 0.02. P1121 is the first known main-sequence star showing an excess comparable to that of beta Pic, which may indicate the presence of an exceptionally massive debris disk. It is possible that a major planetesimal collision has occurred in this system, consistent with the few hundred Myr time scales estimated for the clearing of the Solar System.Comment: 14 pages, 3 figures, accepted to the special Spitzer issue of the ApJ

    Absorbing customer knowledge: how customer involvement enables service design success

    Customers are a knowledge resource outside of the firm that can be utilized for new service success by involving them in the design process. However, existing research on the impact of customer involvement (CI) is inconclusive. Knowledge about customers’ needs and on how best to serve these needs (articulated in the service concept) is best obtained from customers themselves. However, codesign runs the risk of losing control of the service concept. This research argues that of the processes of external knowledge, acquisition (via CI), customer knowledge assimilation, and concept transformation form a capability that enables the firm to exploit customer knowledge in the form of a successful new service. Data from a survey of 126 new service projects show that the impact of CI on new service success is fully mediated by customer knowledge assimilation (the deep understanding of customers’ latent needs) and concept transformation (the modification of the service concept due to customer insights). However, its impact is more nuanced. CI exhibits an “∩”-shaped relationship with transformation, indicating there is a limit to the beneficial effect of CI. Its relationship with assimilation is “U” shaped, suggesting a problem with cognitive inertia where initial learnings are ignored. Customer knowledge assimilation directly impacts success, while concept transformation only helps success in the presence of resource slack. An evolving new service design is only beneficial if the firm has the flexibility to adapt to change

    The AURORA Study: A Longitudinal, Multimodal Library of Brain Biology and Function after Traumatic Stress Exposure

    Adverse posttraumatic neuropsychiatric sequelae (APNS) are common among civilian trauma survivors and military veterans. These APNS, as traditionally classified, include posttraumatic stress, postconcussion syndrome, depression, and regional or widespread pain. Traditional classifications have come to hamper scientific progress because they artificially fragment APNS into siloed, syndromic diagnoses unmoored to discrete components of brain functioning and studied in isolation. These limitations in classification and ontology slow the discovery of pathophysiologic mechanisms, biobehavioral markers, risk prediction tools, and preventive/treatment interventions. Progress in overcoming these limitations has been challenging because such progress would require studies that both evaluate a broad spectrum of posttraumatic sequelae (to overcome fragmentation) and also perform in-depth biobehavioral evaluation (to index sequelae to domains of brain function). This article summarizes the methods of the Advancing Understanding of RecOvery afteR traumA (AURORA) Study. AURORA conducts a large-scale (n = 5000 target sample) in-depth assessment of APNS development using a state-of-the-art battery of self-report, neurocognitive, physiologic, digital phenotyping, psychophysical, neuroimaging, and genomic assessments, beginning in the early aftermath of trauma and continuing for 1 year. The goals of AURORA are to achieve improved phenotypes, prediction tools, and understanding of molecular mechanisms to inform the future development and testing of preventive and treatment interventions

    Mutations in SPAG1 Cause Primary Ciliary Dyskinesia Associated with Defective Outer and Inner Dynein Arms

    Primary ciliary dyskinesia (PCD) is a genetically heterogeneous, autosomal-recessive disorder, characterized by oto-sino-pulmonary disease and situs abnormalities. PCD-causing mutations have been identified in 20 genes, but collectively they account for only ∌65% of all PCDs. To identify mutations in additional genes that cause PCD, we performed exome sequencing on three unrelated probands with ciliary outer and inner dynein arm (ODA+IDA) defects. Mutations in SPAG1 were identified in one family with three affected siblings. Further screening of SPAG1 in 98 unrelated affected individuals (62 with ODA+IDA defects, 35 with ODA defects, 1 without available ciliary ultrastructure) revealed biallelic loss-of-function mutations in 11 additional individuals (including one sib-pair). All 14 affected individuals with SPAG1 mutations had a characteristic PCD phenotype, including 8 with situs abnormalities. Additionally, all individuals with mutations who had defined ciliary ultrastructure had ODA+IDA defects. SPAG1 was present in human airway epithelial cell lysates but was not present in isolated axonemes, and immunofluorescence staining showed an absence of ODA and IDA proteins in cilia from an affected individual, thus indicating that SPAG1 probably plays a role in the cytoplasmic assembly and/or trafficking of the axonemal dynein arms. Zebrafish morpholino studies of spag1 produced cilia-related phenotypes previously reported for PCD-causing mutations in genes encoding cytoplasmic proteins. Together, these results demonstrate that mutations in SPAG1 cause PCD with ciliary ODA+IDA defects and that exome sequencing is useful to identify genetic causes of heterogeneous recessive disorders
