    Sensitivity of single and multiple cosmic ray neutrons to the surrounding medium in a lead-free monitor

    In 1981-1985 the neutron component of cosmic rays was recorded, the effect of cosmic ray multiplication in lead being disregarded. The recording device consisted of neutron counters placed in a polyethylene retarder (polyethylene tubes with wall thickness of 2 cm). The device registered neutrons formed directly on the surface or not deep underground; the intensity of neutrons depended on the chemical composition of the substance. The neutron component was also measured in the Moscow Canal, Belomor-Baltic Canal, and in the Atlantic Ocean. The time variation of 5 minute data of the intensity obtained in the Belomor-Baltic Canal and in the Atlantic Ocean relative to the mean value in the open sea (in %) is presented

    The rotational velocity of the sdOB primary of the eclipsing binary system LB 3459 (AA Dor)

    We present an analysis of the rotational velocity of the primary of LB 3459 based on 107 new high-resolution and high-S/N ESO VLT UVES spectra. 105 of them cover a complete orbital period (0.26 d) of this binary system. We have determined an orbital period of P = 22600.702 +/- 0.005 sec, a radial velocity amplitude of A_1 = 39.19 +/- 0.05 km/sec, and T_0 = 2451917.152690 +/- 0.000005. From simulations of the He II 4686A line profile (based on NLTE model atmosphere calculations), we derive v_rot = 47 +/- 5 km/sec. We present an animation which shows the orbital movement of the binary system, its synthetic lightcurve, and compares the phase-dependent variation of the predicted with the observed He II 4686A line profile. The radius of the cool component is almost the same size like Jupiter but its mass is about 70 times higher than Jupiter's mass. Thus, from its present mass (M_2 = 0.066 M_sun), the secondary of LB 3459 lies formally within the brown-dwarf mass range (0.013 - 0.08 M_sun).     The results of the spatial distribution of cosmic ray density, gradients, and anisotropy obtained on the basis of the numerical solution of the anisotropic diffusion equation with an account of solar wind velocity change depending on the latitudinal angle theta of the form U=u sub oe sup alpha theta and the diffusion coefficient depending on the spatial coordinates and the particle rigidity are presented. It is shown that the increase of the solar wind velocity and the diffusion coefficient with heliolatitude leads to gradient distributions that are in accord with experimental data observed in space. The results of the energetic spectrum of 11 and 22-year cosmic ray variations obtained with an account of direction of the general magnetic field of the Sun are presented are given

    The latitudinal behavior of intensities and multiplicities was registered by the neutron monitor 2 NM and the lead-free neutron monitor 3 SND (slow-neuron detector) in the equator-Kaliningrad line in the Atlantic Ocean. Coupling coefficients for 3 SND show the sensitivity of this detector to primary particles of cosmic rays of energies on the average lower than for 2 NM. As multiplicities increase, the coupling coefficients shift towards higher energies

    During cosmic ray propagation in interplanetary space there appear characteristic cosmic-ray intensity scintillations which are due to charged particle scattering on random inhomogeneities of the interplanetary magnetic field. The power spectra of cosmic ray scintillations on the Earth during some intervals from 1977 to 1982 (for quiet periods, for solar flares and Forbush decreases due to power shock waves) have been calculated from five-minute, one and two-hour values of the cosmic-ray intensity measured by the scintillator supertelescope IZMIRAN. The spectra were estimated by the methods of spectral analysis and by autoregressive methods which mutually control each other and make it possible not only to analyze scintillation powers at distinguished frequencies, but also to determine the behavior of spectrum slopes in some frequency ranges

    Splicing mutation analysis reveals previously unrecognized pathways in lymph node-invasive breast cancer.

    Somatic mutations reported in large-scale breast cancer (BC) sequencing studies primarily consist of protein coding mutations. mRNA splicing mutation analyses have been limited in scope, despite their prevalence in Mendelian genetic disorders. We predicted splicing mutations in 442 BC tumour and matched normal exomes from The Cancer Genome Atlas Consortium (TCGA). These splicing defects were validated by abnormal expression changes in these tumours. Of the 5,206 putative mutations identified, exon skipping, leaky or cryptic splicing was confirmed for 988 variants. Pathway enrichment analysis of the mutated genes revealed mutations in 9 NCAM1-related pathways, which were significantly increased in samples with evidence of lymph node metastasis, but not in lymph node-negative tumours. We suggest that comprehensive reporting of DNA sequencing data should include non-trivial splicing analyses to avoid missing clinically-significant deleterious splicing mutations, which may reveal novel mutated pathways present in genetic disorders

In 1996, the Royal College of Physicians and Surgeons Canada responded to the evolving roles and obligations of medical specialists by implementing a framework of core competencies called the “CanMEDS Roles” which define surgeons as medical experts, communicators, collaborators, managers, health advocates, scholars and professionals.     Budget impact of adding ivabradine to standard of care in patients with chronic systolic heart failure in the United States

    BACKGROUND: Heart failure (HF) costs 21billionannuallyindirecthealthcarecosts,80OBJECTIVE:ToestimatethebudgetimpactofivabradinefromaU.S.commercialpayerperspective.METHODS:Abudgetimpactmodelestimatedthepermemberpermonth(PMPM)impactofintroducingivabradinetoexistingformulariesbycomparingareferencescenario(SoC)andanewdrugscenario(ivabradine+SoC)inhypothetical1millionmembercommercialandMedicareAdvantageplans.Inbothscenarios,U.S.claimsdatawereusedforthereferencecumulativeannualratesofhospitalizations(HF,nonHFcardiovascular[CV],andnonCV),andhospitalizationrateswereadjustedusingSHIFTdata.ThemodelcontrolledformortalityriskusingSHIFTandU.S.lifetabledata,andhospitalizationcostswereobtainedfromU.S.claimsdata:HFrelated=21 billion annually in direct health care costs, 80% of which is directly attributable to hospitalizations. The SHIFT clinical study demonstrated that ivabradine plus standard of care (SoC) reduced HF-related and all-cause hospitalizations compared with SoC alone. OBJECTIVE: To estimate the budget impact of ivabradine from a U.S. commercial payer perspective. METHODS: A budget impact model estimated the per-member-per month (PMPM) impact of introducing ivabradine to existing formularies by comparing a reference scenario (SoC) and a new drug scenario (ivabradine + SoC) in hypothetical 1 million-member commercial and Medicare Advantage plans. In both scenarios, U.S. claims data were used for the reference cumulative annual rates of hospitalizations (HF, non-HF cardiovascular [CV], and non-CV), and hospitalization rates were adjusted using SHIFT data. The model controlled for mortality risk using SHIFT and U.S. life table data, and hospitalization costs were obtained from U.S. claims data: HF-related = 37,507; non-HF CV = 28,951;andnonCV=28,951; and non-CV = 17,904. The annualized wholesale acquisition cost of ivabradine was 4,500,withbaselineuseforthisnewdrugat2RESULTS:BasedontheapprovedU.S.indication,approximately2,000commerciallyinsuredpatientsfroma1millionmembercommercialplanwereeligibletoreceiveivabradine.IvabradineresultedinaPMPMcostsavingsof4,500, with baseline use for this new drug at 2%, increasing 2% per year. RESULTS: Based on the approved U.S. indication, approximately 2,000 commercially insured patients from a 1 million-member commercial plan were eligible to receive ivabradine. Ivabradine resulted in a PMPM cost savings of 0.01 and 0.04inyears1and3ofthecoremodel,respectively.Afterincludingtheacquisitionpriceforivabradine,themodelshowedadecreaseintotalcostsinthecommercial(0.04 in years 1 and 3 of the core model, respectively. After including the acquisition price for ivabradine, the model showed a decrease in total costs in the commercial (991,256 and 474,499,respectively)andMedicarepopulations(474,499, respectively) and Medicare populations (13,849,262 and 4,280,291,respectively)inyear1.Thisdecreasewasdrivenbyivabradinesreductioninhospitalizationrates.Forthecoremodel,theestimatedpharmacyonlyPMPMinyear1was4,280,291, respectively) in year 1. This decrease was driven by ivabradine’s reduction in hospitalization rates. For the core model, the estimated pharmacy-only PMPM in year 1 was 0.01 for the commercial population and $0.24 for the Medicare Advantage population. CONCLUSIONS: Adding ivabradine to SoC led to lower average annual treatment costs. The negative PMPM budget impact indicates that ivabradine is an affordable option for U.S. payers

    Higher comfort temperature preferences for anthropometrically matched Chinese and Japanese versus White-Western-Middle-European individuals using a personal comfort / cooling system

    Purpose: To investigate potential differences in preferred Personal Comfort Systems (PCS) settings of Japanese and Han-Chinese versus white-western-middle-Europeans. Method: A series of five experiments with similar methodology is reported that allowed participants to self-select their preferred PCS outlet air temperature in a warm controlled climatic chamber setup with and without solar radiation. Test groups were matched for age, height, weight, body-surface-area and body-mass-index to remove the influence of these confounding factors on the results. Participants were first exposed to solar radiation (exp-1-4; simulating glazed building without proper shading or a car) before starting to control the outlet temperature of the PCS, or (exp-5, simulating warm building) were exposed to a warm room temperature and immediately could control the PCS. Ethnicity effects were studied through the chosen preferred PCS outlet temperatures and the microclimate temperature close to the participants’ chest. Results: In all experiments, Asian groups selected a PCS outlet temperature significantly higher, on average by 5 °C, leading to a 1.9 °C higher microclimate temperature at chest level. While absolute selected temperatures of the PCS differed between experiments, related to different designs of the PCS and climate conditions, no interaction between ethnicity and experiment was present. Conclusions: Despite removing important confounding factors that could explain earlier observed differences between Asian and white western middle-European ethnicities tested, a substantial, consistently higher thermal preference temperature of the PCS was found in the two Asian groups. This has implications for the design parameters of PCS for use in offices or air-conditioning systems in cars
