    A field study in Benin to investigate the role of mosquitoes and other flying insects in the ecology of Mycobacterium ulcerans

    Background Buruli ulcer, the third mycobacterial disease after tuberculosis and leprosy, is caused by the environmental mycobacterium M. ulcerans. There is at present no clear understanding of the exact mode(s) of transmission of M. ulcerans. Populations affected by Buruli ulcer are those living close to humid and swampy zones. The disease is associated with the creation or the extension of swampy areas, such as construction of dams or lakes for the development of agriculture. Currently, it is supposed that insects (water bugs and mosquitoes) are host and vector of M. ulcerans. The role of water bugs was clearly demonstrated by several experimental and environmental studies. However, no definitive conclusion can yet be drawn concerning the precise importance of this route of transmission. Concerning the mosquitoes, DNA was detected only in mosquitoes collected in Australia, and their role as host/vector was never studied by experimental approaches. Surprisingly, no specific study was conducted in Africa. In this context, the objective of this study was to investigate the role of mosquitoes (larvae and adults) and other flying insects in ecology of M. ulcerans. This study was conducted in a highly endemic area of Benin. Methodology/Principal Findings Mosquitoes (adults and larvae) were collected over one year, in Buruli ulcer endemic in Benin. In parallel, to monitor the presence of M. ulcerans in environment, aquatic insects were sampled. QPCR was used to detected M. ulcerans DNA. DNA of M. ulcerans was detected in around 8.7% of aquatic insects but never in mosquitoes (larvae or adults) or in other flying insects. Conclusion/Significance This study suggested that the mosquitoes don't play a pivotal role in the ecology and transmission of M. ulcerans in the studied endemic areas. However, the role of mosquitoes cannot be excluded and, we can reasonably suppose that several routes of transmission of M. ulcerans are possible through the world

    Effectiveness of insecticidal nets on uncomplicated clinical malaria : a case-control study for operational evaluation

    Background: In a context of large-scale implementation of malaria vector control tools, such as the distribution of long-lasting insecticide nets (LLIN), it is necessary to regularly assess whether strategies are progressing as expected and then evaluate their effectiveness. The present study used the case-control approach to evaluate the effectiveness of LLIN 42 months after national wide distribution. This study design offers an alternative to cohort study and randomized control trial as it permits to avoid many ethical issues inherent to them. Methods: From April to August 2011, a case-control study was conducted in two health districts in Benin; Ouidah-Kpomasse-Tori (OKT) in the south and Djougou-Copargo-Ouake (DCO) in the north. Children aged 0-60 months randomly selected from community were included. Cases were children with a high axillary temperature (>= 37.5 degrees C) or a reported history of fever during the last 48 h with a positive rapid diagnostic test (RDT). Controls were children with neither fever nor signs suggesting malaria with a negative RDT. The necessary sample size was at least 396 cases and 1188 controls from each site. The main exposure variable was "sleeping every night under an LLIN for the 2 weeks before the survey" (SL). The protective effectiveness (PE) of LLIN was calculated as PE = 1 - odds ratio. Results: The declared SL range was low, with 17.0 and 27.5 % in cases and controls in the OKT area, and 44.9 and 56.5 % in cases and controls, in the DCO area, respectively. The declared SL conferred 40.5 % (95 % CI 22.2-54.5 %) and 55.5 % (95 % CI 28.2-72.4 %) protection against uncomplicated malaria in the OKT and the DCO areas, respectively. Significant differences in PE were observed according to the mother's education level. Conclusion: In the context of a mass distribution of LLIN, their use still conferred protection in up to 55 % against the occurrence of clinical malaria cases in children. Social factors, the poor use and the poor condition of an LLIN can be in disfavour with its effectiveness. In areas, where LLIN coverage is assumed to be universal or targeted at high-risk populations, case-control studies should be regularly conducted to monitor the effectiveness of LLIN. The findings will help National Malaria Control Programme and their partners to improve the quality of malaria control according to the particularity of each area or region as far as possible

    Prevention and control of malaria and sleeping sickness in Africa: Where are we and where are we going?

    The International Symposium on Malaria and Human African Trypanosomiasis: New Strategies for their Prevention & Control was held 7-8 October, 2010 in Cotonou, Benin with about 250 participants from 20 countries. This scientific event aimed at identifying the gaps and research priorities in the prevention and control of malaria and sleeping sickness in Africa and to promote exchange between North and South in the fields of medical entomology, epidemiology, immunology and parasitology. A broad range of influential partners from academia (scientists), stakeholders, public health workers and industry attempted the meeting and about 40 oral communications and 20 posters were presented by phD students and internationally-recognized scientists from the North and the South. Finally, a special award ceremony was held to recognize efforts in pioneer work conducted by staff involved in the diagnostic of the Sleeping illness in West Africa with partnership and assistance from WHO and Sanofi-Aventis group

    Recognizing the equity implications of restoration priority maps

    A growing number of studies seek to identify global priority areas for conservation and restoration. These studies often produce maps that highlight the benefits of concentrating such activity in the tropics. However, the potential equity implications of using these prioritization exercises to guide global policy are less often explored and articulated. We highlight those equity issues by examining a widely publicized restoration priority map as an illustrative case. This map is based on a prioritization analysis that sought to identify places where restoration of agricultural land might provide the greatest biodiversity and carbon sequestration benefits at the lowest cost. First, we calculate the proportion of agricultural land in countries around the world that the map classifies as a top 15% restoration priority. A regression analysis shows that this map prioritizes restoration in countries where displacing agriculture may be most detrimental to livelihoods: countries that are poorer, more populated, more economically unequal, less food secure, and that employ more people in agriculture. Second, we show through another regression analysis that a similar pattern appears sub-nationally within the tropics: 5 km x 5 km parcels of land in the tropics that are less economically developed or more populated are more likely to be top 15% restoration priorities. In other words, equity concerns persist at a subnational scale even after putting aside comparisons between the tropics and the Global North. Restorative activity may be beneficial or harmful to local livelihoods depending on its conceptualization, implementation, and management. Our findings underline a need for prioritization exercises to better attend to the risks of concentrating potentially negative livelihood impacts in vulnerable regions. We join other scholars calling for greater integration of social data into restoration science

    Realizing Inclusive SAI: Contextualizing indicators to better evaluate gender and intergenerational inequity in SAI processes and outcomes - Cases from Southern and Western Africa

    Despite increasing sustainable agricultural intensification (SAI) investments, indicators for detecting gender and intergenerational inequities in SAI costs and benefits sharing often remain overgeneralized, theoretical, or locally irrelevant. We examine the relative value of, and how to, customize standard SAI indicators to detect such inequities in specific socio-cultural contexts to enhance data collection for evidence-based decision making in fostering gender/youth inclusive SAI. Using focus-group discussions and key informant interviews among farmers and diverse government, NGO, private sector, and academic stakeholders in two districts in Malawi and three in Ghana, we assess the perceived roles, differentiated needs/ priorities of men, women and youth, and the sharing of SAI burdens and benefits within farming households. We investigate what context-appropriate questions to ask, to whom, and how, to collect reliable information on indicators of SAIinvestment inequities. Results illuminate context-specific, gendered and intergenerational factors shaping access to and ownership of productive resources, household decision making, SAI participation, and appropriateness of selected indicators. Combining farmers’ and local field-expert’ perspectives offers practical insights for customizing inequity indicators. Findings highlight advantages of local contextualization of SAI indicators, including insights on appropriate data-collection approaches that challenge orthodox survey/quantitative methods for detecting and assessing gender/age inequities to foster inclusive SAI

    A decision makers’ guide to equitable sustainable agricultural intensification

    In many parts of the world, there is a clear need for investment in agriculture to counteract low yields and food insecurity. Focusing only on short-term production gains, however, through technologies such as improved seeds, irrigation, fertilizers and pesticides, increases risks to the environment and human health. Assessing the sustainability of agricultural intensification must go beyond simply finding economical ways to preserve agriculture’s natural resource base and reduce environmental harm from agriculture. The process of sustainable agricultural intensification (SAI) has to also be inclusive and move towards social equity if it is to be truly sustainable. There are many tools for assessing agriculture through an environmental or economic lens, but relatively few that use social criteria. This leaves a gap as more SAI projects and investments aim to achieve equitable benefits across gender and age lines. This guide provides decision-makers with data collection tools to assess gender and youth inequities associated with changes during SAI. These tools were developed and refined following workshops, field work and interviews with decisionmakers in Ghana and Malawi. In agricultural research, important social data often comes from large-scale household surveys that need significant investment of time and money. This guide focused on non-survey data collection tools, many of which originate from participatory learning and action, for two reasons: participatory tools encourage reflection by participants to increase stakeholder equity, and they are often better matched to the resource requirements and time constraints of those involved. Tools are presented based on their ability to provide information about three identified risks to equity from the SAI process: (i) unequal increases in workload, (ii) unequal access to and use of agricultural resources and (iii) inequitable impacts from changes in technologies and markets. For each tool, an overview explains how the tool relates to SAI. Then, the steps needed to facilitate use are presented, followed by special considerations for effective implementation. The guide supports decision-makers in choosing appropriate data collection tools and in effectively using the information. To make the choice of tool easier, information is provided on affordability, timeliness and human resource requirements for each. Also considered is each tool’s ability to assess potential technologies ex ante, so decision-makers can adapt them before implementation to better foster gender and youth equity. Finally, a number of examples of decision-making tools are presented with how to use the data collected to inform more inclusive SAI. The goal is to enhance the capacity of decision-makers to make a robust analysis of the distribution of benefits and burdens resulting from SAI investments. Decision-makers are encouraged to apply the tools within a community-driven gendertransformative process that aims to change the norms that perpetuate social inequities, by simultaneously influencing household, community, market and political domains

    Gender- and youth-sensitive data collection tools to support decision making for inclusive sustainable agricultural intensification

    Open Access Article; Published online: 15 Sep 2020To achieve equitable sustainable agricultural intensification (SAI), it is essential to understand differential access and control over agricultural resources by women and youth, and to assess how intensification interacts with gendered and age-dependent relationships. Existing packages for assessing women’s empowerment in agriculture tend to be large-scale surveys that do not provide timely results, nor are they easily integrated into a gender-transformative process. This paper applies concepts from Kabeer on gender analysis and empowerment to evaluate promising tools available for assessing inequities in SAI and supporting a gender-transformative approach. We interviewed decision makers in Malawi and Ghana to understand their needs and practices for equityanalysis. We evaluated, adapted and tested tools to detect inequities from SAI. Our results demonstrate the suitability of tools to decision makers’ needs for ex-ante assessment and early detection of disparities. We synthesize information from the testing and adaptation of tools about the resources required, the equity issues they can reveal and their potential role in a gender-transformative approach. The use of the tools needs to be part of an inclusive and culturally specific process for identifying gaps and facilitating a more equitable sharing of SAI responsibilities and benefits through iterative cycles of action and learning
