Breast feeding practices and views among diabetic women: a retrospective cohort study
Objective:
to explore the pattern and experiences of breast-feeding practices among diabetic women.
Design:
retrospective cohort study using maternal records and postal questionnaires in a Baby-Friendly hospital.
Participants:
diabetic mothers including women with gestational diabetes, and type 1 and 2 diabetes mellitus.
Findings:
from the total group of respondents, 81.9% intended to breast feed. The actual breast feeding rates were 81.9% at birth, 68.1% at 2 weeks and 28.7% at 6 months postpartum. Major themes that were identified from women's experiences included information and advice, support vs. pressure, classification and labelling, and expectations.
Conclusions:
more than two-thirds of the diabetic women intended to breast feed and actually did breast feed in this study. For both the total study population and the type 1 and 2 diabetics alone, more than half were still breast feeding at 2 weeks postpartum, and approximately one-third were still breast feeding at 6 months postpartum.
Implications for practice:
structured support, provided for women through Baby-Friendly initiatives, was appreciated by the diabetic women in this study. The extent to which this support influenced the highly successful breast feeding practices in this group of women needs focused investigation. The need for a delicate balancing act between pressure and advice in order to prevent coercion was noted.</p
Rectenna system design
The function of the rectenna in the solar power satellite system is described and the basic design choices based on the desired microwave field concentration and ground clearance requirements are given. One important area of concern, from the EMI point of view, harmonic reradiation and scattering from the rectenna is also designed. An optimization of a rectenna system design to minimize costs was performed. The rectenna cost breakdown for a 56 w installation is given as an example
Structure of Micro-instabilities in Tokamak Plasmas: Stiff Transport or Plasma Eruptions?
Solutions to a model 2D eigenmode equation describing micro-instabilities in
tokamak plasmas are presented that demonstrate a sensitivity of the mode
structure and stability to plasma profiles. In narrow regions of parameter
space, with special plasma profiles, a maximally unstable mode is found that
balloons on the outboard side of the tokamak. This corresponds to the
conventional picture of a ballooning mode. However, for most profiles this mode
cannot exist and instead a more stable mode is found that balloons closer to
the top or bottom of the plasma. Good quantitative agreement with a 1D
ballooning analysis is found provided the constraints associated with higher
order profile effects, often neglected, are taken into account. A sudden
transition from this general mode to the more unstable ballooning mode can
occur for a critical flow shear, providing a candidate model for why some
experiments observe small plasma eruptions (Edge Localised Modes, or ELMs) in
place of large Type I ELMs.Comment: 11 pages, 3 figure
Making sense of joint commissioning: three discourses of prevention, empowerment and efficiency
Background:
In recent years joint commissioning has assumed an important place in the policy and practice of English health and social care. Yet, despite much being claimed for this way of working there is a lack of evidence to demonstrate the outcomes of joint commissioning. This paper examines the types of impacts that have been claimed for joint commissioning within the literature.
Method:
The paper reviews the extant literature concerning joint commissioning employing an interpretive schema to examine the different meanings afforded to this concept. The paper reviews over 100 documents that discuss joint commissioning, adopting an interpretive approach which sought to identify a series of discourses, each of which view the processes and outcomes of joint commissioning differently.
Results:
This paper finds that although much has been written about joint commissioning there is little evidence to link it to changes in outcomes. Much of the evidence base focuses on the processes of joint commissioning and few studies have systematically studied the outcomes of this way of working. Further, there does not appear to be one single definition of joint commissioning and it is used in a variety of different ways across health and social care. The paper identifies three dominant discourses of joint commissioning – prevention, empowerment and efficiency. Each of these offers a different way of seeing joint commissioning and suggests that it should achieve different aims.
Conclusions:
There is a lack of clarity not only in terms of what joint commissioning has been demonstrated to achieve but even in terms of what it should achieve. Joint commissioning is far from a clear concept with a number of different potential meanings. Although this ambiguity can be helpful in some ways in the sense that it can bring together disparate groups, for example, if joint commissioning is to be delivered at a local level then more specificity may be required in terms of what they are being asked to deliver
The joint large-scale foreground-CMB posteriors of the 3-year WMAP data
Using a Gibbs sampling algorithm for joint CMB estimation and component
separation, we compute the large-scale CMB and foreground posteriors of the
3-yr WMAP temperature data. Our parametric data model includes the cosmological
CMB signal and instrumental noise, a single power law foreground component with
free amplitude and spectral index for each pixel, a thermal dust template with
a single free overall amplitude, and free monopoles and dipoles at each
frequency. This simple model yields a surprisingly good fit to the data over
the full frequency range from 23 to 94 GHz. We obtain a new estimate of the CMB
sky signal and power spectrum, and a new foreground model, including a
measurement of the effective spectral index over the high-latitude sky. A
particularly significant result is the detection of a common spurious offset in
all frequency bands of ~ -13muK, as well as a dipole in the V-band data.
Correcting for these is essential when determining the effective spectral index
of the foregrounds. We find that our new foreground model is in good agreement
with template-based model presented by the WMAP team, but not with their MEM
reconstruction. We believe the latter may be at least partially compromised by
the residual offsets and dipoles in the data. Fortunately, the CMB power
spectrum is not significantly affected by these issues, as our new spectrum is
in excellent agreement with that published by the WMAP team. The corresponding

Joint Bayesian component separation and CMB power spectrum estimation
We describe and implement an exact, flexible, and computationally efficient
algorithm for joint component separation and CMB power spectrum estimation,
building on a Gibbs sampling framework. Two essential new features are 1)
conditional sampling of foreground spectral parameters, and 2) joint sampling
of all amplitude-type degrees of freedom (e.g., CMB, foreground pixel
amplitudes, and global template amplitudes) given spectral parameters. Given a
parametric model of the foreground signals, we estimate efficiently and
accurately the exact joint foreground-CMB posterior distribution, and therefore
all marginal distributions such as the CMB power spectrum or foreground
spectral index posteriors. The main limitation of the current implementation is
the requirement of identical beam responses at all frequencies, which restricts
the analysis to the lowest resolution of a given experiment. We outline a
future generalization to multi-resolution observations. To verify the method,
we analyse simple models and compare the results to analytical predictions. We
then analyze a realistic simulation with properties similar to the 3-yr WMAP
data, downgraded to a common resolution of 3 degree FWHM. The results from the

Far-ultraviolet imaging of the Hubble Deep Field-North: Star formation in normal galaxies at z < 1
We present far-ultraviolet (FUV) imaging of the Hubble Deep Field-North (HDF-N) taken with the Solar Blind Channel of the Advanced Camera for Surveys (ACS SBC) and the FUV MAMA detector of the Space Telescope Imaging Spectrograph onboard the Hubble Space Telescope. The full WFPC2 deep field has been observed at 1600 Å. We detect 134 galaxies and one star down to a limit of FUV_(AB) ~ 29. All sources have counterparts in the WFPC2 image. Redshifts (spectroscopic or photometric) for the detected sources are in the range 0 < z < 1. We find that the FUV galaxy number counts are higher than those reported by GALEX, which we attribute at least in part to cosmic variance in the small HDF-N field of view. Six of the 13 Chandra sources at z < 0.85 in the HDF-N are detected in the FUV, and those are consistent with starbursts rather than active galactic nuclei. Cross-correlating with Spitzer sources in the field, we find that the FUV detections show general agreement with the expected L_(IR)/L_(UV) versus β relationship. We infer star formation rates (SFRs), corrected for extinction using the UV slope, and find a median value of 0.3 M_☉ yr^(-1) for FUV-detected galaxies, with 75% of detected sources having SFR < 1 M_☉ yr^(-1). Examining the morphological distribution of sources, we find that about half of all FUV-detected sources are identified as spiral galaxies. Half of morphologically selected spheroid galaxies at z < 0.85 are detected in the FUV, suggesting that such sources have had significant ongoing star formation in the epoch since z ~ 1
