Relation structure-fonction de l'iris de patients présentant un syndrome de Horner

Contexte: L'apparition des symptômes oculaires sont observés lors d'une atteinte des afférences oculaires sympathique. Ce syndrome a le nom de Claude-Bernard-Horner(SH). Il est caractérisé par la coexistence de trois signes cliniques- un myosis, un ptôsis et une pseudo-énophtalmie, due respectivement à une paralysie du dilatateur de l'iris, du muscle de Müller et du rétracteur de la paupière inférieure. Ces signes sont liés à la réduction du fonctionnement des muscles à cause d'une dénervation prolongée. On peut observer la pathologie du muscle dilatateur de l'iris comme le retard de redilatation de la pupille dans l'ombre après un flash (Dilation lag) et on peut la confirmer par l'enregistrement sur le pupillographe. Il peut exister des autres signes comme l'anhidrose, vasodilatation de l'hémiface homolatérale, l'hétérochromie irienne avec éclaircissement de la couleur du côté atteint. Ce signe est bien connu lors des CBH congénitaux. Mais existe des rares cas chez des patients avec CBH acquis. Ce phénomène ainsi que l'atrophie secondaire d'iris peuvent être liés à une diminution du nombre d'axones sympathiques. De plus, Il n'y a pas de donnée sur la comparaison de la durée de «dilation lag » avec la durée de la maladie. Objectifs: 1.Déterminer la corrélation entre l'ancienneté de la maladie et l'importance de la « dilation lag » 2.Comparer la relation structuro-fonctionnelle des yeux chez les patients avec SH et les yeux des sujets sains, ainsi que deux yeux du même patient avec SH. 3.Mesurer l'épaisseur del'iris chez les personnes avec SH acquise pour voir probablement des signes d'atrophie et hypochromie secondaire. Méthode: Une revue de la littérature concernant l'influence de la pathologie de la chaîne sympathique sur les relations structuro-fonctionnelles des yeux sera faite. La population cible comprend deux groupes. Le premier, des patients avec « dilation lag », présentant ou non une hypochromie et atrophie de l'iris. Ils vont être recrutés parmi des patients avec syndrome d'Horner de l'Hôpital Ophtalmique Jules-Gonin-10-15 personnes. Le deuxième, 10-15 sujets en « bonne santé » en âge comparable. L'invitation sera transmise par lettre ou par appel téléphonique. Pour analyser et mesurer la structure de l'iris seront utiliser: Lampe à fente Refractomètre Pupillographe OCT - Optical Coherence Tomography Afin de reconstruire l'évolution des patients, leurs dossiers seront utiliser. Résultats escomptés : Nous espérons voir : Une prolongation de « dilation lag » avec la durée de la maladie Une atrophie d'iris chez les patients avec dénervation sympathique chronique. Et probablement une hypochromie d'iris. Valeur escomptée: Ce travail permettra de mieux connaître l'impact de la pathologie de la chaîne sympathique sur les changements structurels et fonctionnels du segment antérieur de l'oeil. Il aidera à mieux comprendre les processus physiopathologiques dans le SH

ANALYSIS OF FOREIGN EXPERIENCE OF INTRODUCTION OF THE SCHOOL DRESS CODE

The aim of this article is to analyze the international discourse on policy issues of school dress code; to identify problems relevant to the national system of education, and proposed ways of their solution. Methods. The methods involve summarizing of the content of the leading internationalsources regarding issues regarding school education and health; a systematic analysis of sociological researchResults. The polemic aspects to the requirements for the appearance of the students are considered. These requirements affect many important aspects and the principles of functioning of an educational system and all society in general: the observance of the constitutional rights of a child, including freedom of expression on grounds of gender; tolerance policy of intra school and state policy in the light of migration processes; problems of leveling of social and economic stratification of children, etc. The advantages and disadvantages of existing approaches to the development requirements in the countries of Europe, Asia, Africa, and North America are analysed.Scientific novelty. The influence of policy of a school dress-code on children’s behavior and health, relationships in multinational school groups, identification and safety of school students is proved. The ambiguity of information expressiveness and  ignificance of garments of clothing and other elements of appearance of modern school students is disclosed: symbols of belonging to a religious and/or national culture, a well-known fashion brand, a modern informal/political trend, as well as the severity of gender differences in the design of school uniforms. The correlation between the wording of the requirements of the dress code and the effectiveness of its implementation from the point of view of minimization of internal stresses in the school team is defined.Practical significance. Based on the systemized proposals for quality improvement of the socio-cultural environment of educational institutions, it will be possible to improve the efficiency of interaction of participants of an educational processЦель статьи – проанализировать аспекты международного дискурса по поводу политики школьного дресс-кода; выявить связанные с ним проблемы, актуальные для отечественной системы образования, и показать возможные пути их решения. Методика и методы – обобщение содержания зарубежных публикаций по вопросам школьного образования и здравоохранения; системный анализ социологических исследований, касающихся дресс-кода.Результаты. Рассмотрены полемизируемые во многих странах требования к внешнему виду учащихся. Эти требования затрагивают многие важные аспекты и принципы функционирования системы образования и всего общества в целом: соблюдение конституционных прав ребенка, в том числе на свободу самовыражения по национальному и гендерному признакам; толерантность внутришкольной и государственной политики в свете миграционных процессов; проблемы нивелирования социально-экономического расслоения детей и др. Разобраны достоинства и недостатки существующих в различных государствах Европы, Азии, Африки, Северной Америки подходов к разработке требований к школьному дресс-коду. Научная новизна. Доказано влияние политики школьного дресс-кода на культуру поведения и здоровье детей, взаимоотношения в многонациональных школьных коллективах, идентификацию и обеспечение безопасности школьников. Вскрыта неоднозначность информационной выразительности и знаковости предметов одежды и других элементов внешнего вида современных школьников: символов принадлежности к религиозной и/или национальной культуре, известному модному бренду, современному неформальному/политическому течению; а также степени выраженности гендерного различия в дизайне школьной формы. Обозначена взаимосвязь между формулировками требований дресс-кода и эффективностью его внедрения с точки зрения минимизации внутреннего напряжения в школьном коллективе. Практическая значимость. Систематизированы предложения по повышению качества социокультурной среды образовательных учреждений, на основе которых возможно укрепление позитивного взаимодействия участников образовательного процесса

In this work the determination of the parameters of endogenous intoxication in patients herpes associated multiforme exudative erythema in the context of the severity of the disease compared with patients with exudative erythema multiforme other etiologies and patients with recurrent herpes simplex. Management of wastewater from landfill of inorganic fiberglass

ArticleSustainability is one of the key factors in smart environment management and include the reduction of environment footprint. The waste and wastewater management plans are aimed on actions to reduce the amount of waste and environmental pollution. This includes collection of waste, logistics, storage, processing or valorisation and also treatment. The aim of this research was to evaluate environmental pollution risk and to demonstrate one of the wastewater management schemes to reduce the pollution level. Evaluation of the better management scheme was performed in one of the landfills in Latvia, where fiberglass waste and other inorganic waste is stored. Onsite evaluation results demonstrated the need to develop a better wastewater management scheme of inorganic fiberglass landfill. After that, laboratory–scale experiments for conventional coagulation and biodegradation tests have been performed for efficient management
