    The Reaction 7Li(pi+,pi-)7B and its Implications for 7B

    The reaction 7Li(pi+,pi-)7B has been measured at incident pion energies of 30-90 MeV. 7Li constitutes the lightest target nucleus, where the pionic charge exchange may proceed as a binary reaction to a discrete final state. Like in the Delta-resonance region the observed cross sections are much smaller than expected from the systematics found for heavier nuclei. In analogy to the neutron halo case of 11Li this cross section suppression is interpreted as evidence for a proton halo in the particle-unstable nucleus 7B.Comment: 4 pages, 4 figure

    The value of clinical judgement analysis for improving the quality of doctors' prescribing decisions

    Background Many initiatives are taken to improve prescribing decisions. Educational strategies for doctors have been effective in at least 50% of cases. Some reflection on one's own performance seems to be a common feature of the most effective strategies. So far, such reflections have mainly focused on the observed outcomes of the doctors' decisions, i.e. on what doctors do in practice. Studies in other fields have shown that another form of feedback based on the analysis of judgements may be useful as well. Objectives The objectives of the study were to discuss the principles underlying clinical judgement analysis, give examples of its use in the medical context, and discuss its potential for improving prescribing decisions. Results Clinical judgement analysis can look behind the outcome of a decision to the underlying decision process. Carefully constructed or selected case material is required for this analysis. Combining feedback on outcomes with feedback based on clinical judgement analysis offers doctors insight both in what they do, and why or when they do it. It may reveal determinants of decision making which are not available through unaided introspection. Interventions using this combination of feedback for improving doctors' prescribing behaviour have been (partly) successful in 4 cases and unsuccessful in one case. Conclusions Clinical judgement analysis gives doctors a structured reflection on the decision-making policy, and can help them to improve their future decisions. It may be especially useful for groups of doctors who try to work towards a consensus policy. The approach is not very helpful when simple decision rules are appropriate

    Search for the Hypothetical pi -> mu x Decay

    The KARMEN collaboration has reported the possible observation of a hitherto unknown neutral and weakly interacting particle x, which is produced in the decay pi -> mu + x with a mass m(x) = 33.9 MeV. We have searched for this hypothetical decay branch by studying muons from pion decay in flight with the LEPS spectrometer at the piE3 channel at PSI and find branching ratios BR(pi- to mu- anti-x) < 4e-7 and BR(pi+ to mu+ x) < 7e-8 (95\% C.L.). Together with the limit BR > 2e-8 derived in a recent theoretical paper our result would leave only a narrow region for the existence of x if it is a heavy neutrino.Comment: 10 pages, TeX (uses epsf), 3 Postscript figures uu-encode

    Perspectives about living on the horns of dilemmas: An analysis of gender factors related to superintendent decision-making and problem-solving

    The major focus of this paper is a gender-based analysis of school superintendent decision-making and problem-solving as well as an investigation of contemporary leadership dilemmas. The findings are based on responses from 258 superintendents of K-12 school districts in Delaware, Maryland, New Jersey, New York, and Pennsylvania collected over a period of three years (2009-2011). The researchers also conducted 18 comprehensive qualitative “face-to-face” interviews with self-selected superintendents who responded to the quantitative survey. The intended outcome of this article is for education policy makers, professors, and practitioners to comprehensively examine the extent and degree of various dilemmas confronting the Mid-Atlantic Region school superintendent sample and to evaluate the decision-making and problem-solving approaches used by them. The study results that are presented will serve as valuable references to not only individual superintendents but also to university administrator preparation professors and to state administrator licensure agencies because it is important for all aspiring superintendents to know the various issues associated with education leadership and the personal and professional dilemmas that they need to be prepared to face as they embark on a career to improve schooling in the United States

    Medication Adherence Affects Treatment Modifications in Patients With Type 2 Diabetes

    Background: Low rates of treatment modification in patients with insufficiently controlled risk factors are common in type 2 diabetes. Although adherence problems are often mentioned in surveys as a reason for not intensifying treatment, observational studies have shown inconclusive results. Objective: To assess how medication adherence affects treatment modifications for hypertension and hyperglycemia in patients with type 2 diabetes. Methods: This was a cohort study of 11,268 primary care patients with type 2 diabetes in the Netherlands. Inclusion criteria were diagnosis before 2007, >= 1 prescription to antihypertensive or glucose-regulating medication in the preceding 6 months, and a systolic blood pressure level >= 140 mm Hg or glycosylated hemoglobin >= 7% in 2007. Patients on maximal treatment were excluded. Treatment modifications as observed from prescriptions were classified as none, dose increase, dose decrease, class switch, class addition, or class discontinuation. Refill adherence was assessed as medication possession ratio or length of last gap between refills. We performed multilevel multinomial regression analysis to test for associations. Results: We included 4980 diabetic patients with elevated blood pressure and 2945 diabetic patients with elevated glycosylated hemoglobin levels. Patients with lower adherence for antihypertensive drugs were more likely to have those medications discontinued (odds ratio [OR] for every 10% lower medication possession ratio =1.22; 95% CI, 1.11-1.33) or the dose decreased (OR = 1.14; CI 1.01-1.28). For glucose-regulating medication, dose increases (OR = 0.92; 95% CI, 0.85-0.98) and medication additions (OR = 0.90; 95% CI, 0.82-0.99) were less likely in patients with lower adherence levels. Conclusions: Low adherence inhibits the intensification of glucose-regulating but not antihypertensive medication in type 2 diabetic patients with insufficiently controlled risk factors in the Netherlands.     Evidence for the tongue of ionization under northward interplanetary magnetic field conditions

    [1] The activities of the International Ionospheric Tomography Community open up new possibilities of simultaneously imaging the large-scale spatial structure of the ionosphere in different longitude sectors. In the study, tomography receiver chains in Scandinavia and Greenland were used to provide a wide view of the plasma density structure in the winter, magnetic postnoon sector under conditions of stable, positive interplanetary magnetic field B z component. The spatial distributions of the plasma are discussed in light of a high-latitude plasma convection pattern pertinent to the conditions, which is supported by DMSP flow measurements. The observations are consistent with a tongue of dayside photoionization being drawn antisunward by the convection pattern to form an arc of enhanced plasma density around the periphery of the polar cap

    Search for Light Gauge Bosons of the Dark Sector at the Mainz Microtron

    A new exclusion limit for the electromagnetic production of a light U(1) gauge boson {\gamma}' decaying to e^+e^- was determined by the A1 Collaboration at the Mainz Microtron. Such light gauge bosons appear in several extensions of the standard model and are also discussed as candidates for the interaction of dark matter with standard model matter. In electron scattering from a heavy nucleus, the existing limits for a narrow state coupling to e^+e^- were reduced by nearly an order of magnitude in the range of the lepton pair mass of 210 MeV/c^2 < m_e^+e^- < 300 MeV/c^2. This experiment demonstrates the potential of high current and high resolution fixed target experiments for the search for physics beyond the standard model.Comment: 4 pages, 7 figure
