    A clinico-radiological and pathological profile of lung cancer patients presented to All India Institute of Medical Sciences (Patna)

    Background: Lung cancer is one of the most common cancers and cause of cancer-related deaths worldwide. The clinicopathologicalprofile of lung cancer has shown marked regional and geographical variation. Majority of the patients have locallyadvanced or disseminated disease at presentation and are not candidates for surgery. Objective: The aim of this study was toevaluate the clinico-radiological and pathological profile of lung cancer patients and difference in histopathology betweensmoker and non-smoker. We also assessed yield of the various diagnostic procedures used for confirmation of lung cancer.Materials and Methods: A total of 30 patients diagnosed between May 1, 2016, and December 31, 2016. The completedemographic profile, smoking status, clinical, radiological, and diagnostic details were recorded in the study. Data were enteredand analyzed using SPSS software. Results: A total of 30 patients (19 male and 11 female) included in our study with mean age of55.26 years. Cough (80%) and dyspnea (80%) were the most common symptom and mass (86%), pleural effusion (53.3%) was themost common radiological presentation of patients. Clubbing and hemoptysis both was found only in 8 out of 30 (26%) patients.Adenocarcinoma (46.6%) was the most common histopathological type followed by squamous cell carcinoma (16.6%) and smallcell carcinoma (13.3%). The majority of patients (60%) presented in Stage 4. Computed tomography guided biopsy had better yieldin compare to ultrasonography guided (80% vs. 70.8%). Bronchoscopic procedure had lowest yield (38.8%). Conclusion: Theclinicopathological profile of lung cancer has changed in last few years, especially in the increase in adenocarcinoma incidence,and now it is the most common cause in both smokers and non-smoker

    Prescription pattern in ischemic heart disease inpatients at B. P. Koirala Institute of Health Sciences: a cross sectional study

    Background: Drug utilization pattern studies seek to screen, evaluate and suggest appropriate modifications in prescription practices. It would help to make patient care rational and cost effective. Objective was to analyze the drug prescribing pattern for treatment of ischemic heart disease (IHD). Methods: A prospective cross-sectional observational study was conducted in patients of IHD admitted in intensive coronary care unit and medicine ward for the period of six months. Data were collected in preformed case record form. The data were analyzed for drug use indicators, demographic parameters, morbidities, pattern of drug use using Microsoft excel 2010. Results: A total of 145 patients were enrolled out of that 89 (61.38%) were males. The mean age was 60.01±12.71 years and majority (26.89%) belonged to age group of 61-70 years. A total of 1208 drugs were prescribed in 145 patients. Most frequently prescribed drugs were antiplatelet group of drugs 100% encounters, followed by hypo-lipidemics (98.62%). Average number of drugs per encounter was 8.33 and percentage of drugs prescribed by generic name was 5.04%.  Conclusions: IHD was more common in males than females. The most commonly prescribed drug classes were anti-platelet drugs followed by hypolipidemic agents

    Background: Mycobacterium w (Mw), an immunomodulator, resulted in better clinical status in severe coronavirus infectious disease 19 (COVID-19) but no survival benefit in a previous study. Herein, we investigate whether Mw could improve clinical outcomes and survival in COVID-19. Materials and Methods: In a multicentric, randomized, double-blind, parallel-group, placebo-controlled trial, we randomized hospitalized subjects with severe COVID-19 to receive either 0.3 mL/day of Mw intradermally or a matching placebo for three consecutive days. The primary outcome was 28-day mortality. The co-primary outcome was the distribution of clinical status assessed on a seven-point ordinal scale ranging from discharged (category 1) to death (category 7) on study days 14, 21, and 28. The key secondary outcomes were the change in sequential organ failure assessment (SOFA) score on days 7 and 14 compared to the baseline, treatment-emergent adverse events, and others. Results: We included 273 subjects (136 Mw, 137 placebo). The use of Mw did not improve 28-day survival (Mw vs. placebo, 18 [13.2%] vs. 12 [8.8%], P = 0.259) or the clinical status on days 14 (odds ratio [OR], 1.33; 95% confidence intervals [CI], 0.79-2.3), 21 (OR, 1.49; 95% CI, 0.83-2.7) or 28 (OR, 1.49; 95% CI, 0.79-2.8) between the two study arms. There was no difference in the delta SOFA score or other secondary outcomes between the two groups. We observed higher injection site reactions with Mw. Conclusion: Mw did not reduce 28-day mortality or improve clinical status on days 14, 21 and 28 compared to placebo in patients with severe COVID-19. [Trial identifier: CTRI/2020/04/024846

    Measurement of inclusive charged-particle jet production in pp and p-Pb collisions at sNN=5.02\sqrt{s_{\rm NN}}=5.02 TeV

    International audienceMeasurements of inclusive charged-particle jet production in pp and p-Pb collisions at center-of-mass energy per nucleon-nucleon collision sNN=5.02\sqrt{s_{\rm NN}} = 5.02 TeV and the corresponding nuclear modification factor RpPbchjetR_{\rm pPb}^{\rm ch\,jet} are presented, using data collected with the ALICE detector at the LHC. Jets are reconstructed in the central rapidity region ηjet<0.5|\eta_{\rm jet}| < 0.5 from charged particles using the anti-kTk_{\rm T} algorithm with resolution parameters R=0.2R = 0.2, 0.3, and 0.4. The pTp_{\rm T}-differential inclusive production cross section of charged-particle jets, as well as the corresponding cross-section ratios, are reported for pp and p-Pb collisions in the transverse momentum range 10<pT,jetch<14010 < p^{\rm ch}_{\rm T,jet} < 140 GeV/cc and 10<pT,jetch<16010 < p^{\rm ch}_{\rm T,jet} < 160 GeV/cc, respectively, together with the nuclear modification factor RpPbchjetR_{\rm pPb}^{\rm ch\,jet} in the range 10<pT,jetch<14010 < p^{\rm ch}_{\rm T,jet} < 140 GeV/cc. The analysis extends the pTp_{\rm T} range of the previously-reported charged-particle jet measurements by the ALICE Collaboration. The nuclear modification factor is found to be consistent with one and independent of the jet resolution parameter with the improved precision of this study, indicating that the possible influence of cold nuclear matter effects on the production cross section of charged-particle jets in p-Pb collisions at sNN=5.02\sqrt{s_{\rm NN}} = 5.02 TeV is smaller than the current precision. The obtained results are in agreement with other minimum bias jet measurements available for RHIC and LHC energies, and are well reproduced by the NLO perturbative QCD POWHEG calculations with parton shower provided by PYTHIA8 as well as by JETSCAPE simulations

They are described within uncertainties by perturbative QCD calculations employing the fragmentation fractions of beauty quarks to baryons measured at forward rapidity by the LHCb Collaboration. The bb{\rm b\overline{b}} production cross section per unit of rapidity at midrapidity, estimated from these measurements, is dσbb/dyy<0.5=83.1±3.5(stat.)±5.4(syst.)3.2+12.3(extrap.)μ{\rm d}\sigma_{\rm b\overline{b}}/{\rm d}y|_{|y|<0.5} = 83.1 \pm 3.5 (\mathrm{stat.}) \pm 5.4(\mathrm{syst.}) ^{+12.3}_{-3.2} (\mathrm{extrap.})\,\mub. The baryon-to-meson ratios are computed to investigate the hadronization mechanism of beauty quarks. The non-prompt Λc+/D0\Lambda^+_{\rm c}/{\rm D^0} production ratio has a similar trend to the one measured for the promptly produced charmed particles and to the p/π+/\pi^+ and Λ/KS0\Lambda/{\rm K^0_S} ratios, suggesting a similar baryon-formation mechanism among light, strange, charm, and beauty hadrons. The pTp_{\rm T}-integrated non-prompt Λc/D0\Lambda_{\rm c}/{\rm D^0} ratio is found to be significantly higher than the one measured in e+^+e^- collisions

    Search for jet quenching effects in high-multiplicity pp collisions at s\sqrt{s} = 13 TeV via di-jet acoplanarity

    International audienceThe ALICE Collaboration reports a search for jet quenching effects in high-multiplicity (HM) proton-proton collisions at s\sqrt{s} = 13 TeV, using the semi-inclusive azimuthal-difference distribution Δφ\Delta\varphi of charged-particle jets recoiling from a high transverse momentum (high-pT,trigp_{\mathrm{T,trig}}) trigger hadron. Jet quenching may broaden the Δφ\Delta\varphi distribution measured in HM events compared to that in minimum bias (MB) events. The measurement employs a pT,trigp_{\mathrm{T,trig}}-differential observable for data-driven suppression of the contribution of multiple partonic interactions, which is the dominant background. While azimuthal broadening is indeed observed in HM compared to MB events, similar broadening for HM events is observed for simulations based on the PYTHIA 8 Monte Carlo generator, which does not incorporate jet quenching. We elucidate the origin of the broadening by comparing biases induced by HM selection in the data and simulations, and discuss its implications for the study of jet quenching in small collision systems

    Prompt and non-prompt J/ψ/\psi production at midrapidity in Pb-Pb collisions at sNN\sqrt{s_{\mathrm{NN}}} = 5.02 TeV

    International audienceThe transverse momentum (pTp_{\rm T}) and centrality dependence of the nuclear modification factor RAAR_{\rm AA} of prompt and non-prompt J/ψ/\psi, the latter originating from the weak decays of beauty hadrons, have been measured by the ALICE collaboration in Pb-Pb collisions at sNN\sqrt{s_{\mathrm{NN}}} = 5.02 TeV. The measurements are carried out through the e+e{\rm e}^{+}{\rm e}^{-} decay channel at midrapidity (y|y| 5 GeV/cc, which becomes stronger with increasing collision centrality. The results are consistent with similar LHC measurements in the overlapping pTp_{\rm T} intervals, and cover the kinematic region down to pTp_{\rm T} = 1.5 GeV/cc at midrapidity, not accessible by other LHC experiments. The suppression of prompt J/ψ/\psi in central and semicentral collisions exhibits a decreasing trend towards lower transverse momentum, described within uncertainties by models implementing J/ψ/\psi production from recombination of c and c\overline{\rm c} quarks produced independently in different partonic scatterings. At high transverse momentum, transport models including quarkonium dissociation are able to describe the suppression for prompt J/ψ/\psi. For non-prompt J/ψ/\psi, the suppression predicted by models including both collisional and radiative processes for the computation of the beauty-quark energy loss inside the quark-gluon plasma is consistent with measurements within uncertainties

    International audienceThe ALICE Collaboration reports the measurement of semi-inclusive distributions of charged-particle jets recoiling from a high transverse momentum (high pTp_{\rm T}) hadron trigger in proton-proton and central Pb-Pb collisions at sNN=5.02\sqrt{s_{\rm NN}} = 5.02 TeV. A data-driven statistical method is used to mitigate the large uncorrelated background in central Pb-Pb collisions. Recoil jet distributions are reported for jet resolution parameter R=0.2R=0.2, 0.4, and 0.5 in the range 7<pT,jet<1407 < p_{\rm T,jet} < 140 GeV/c/c and trigger-recoil jet azimuthal separation π/2<Δφ<π\pi/2 < \Delta\varphi < \pi. The measurements exhibit a marked medium-induced jet yield enhancement at low pTp_{\rm T} and at large azimuthal deviation from Δφπ\Delta\varphi\sim\pi. The enhancement is characterized by its dependence on Δφ\Delta\varphi, which has a slope that differs from zero by 4.7σ\sigma. Comparisons to model calculations incorporating different formulations of jet quenching are reported. These comparisons indicate that the observed yield enhancement arises from the response of the QGP medium to jet propagation
