Whistleblowing in Science: in the Lion's Den
With the growing emphasis on scientific integrity as a way to curb research misconduct, the “moral obligation to act” when a researcher observes a breach of integrity is becoming stronger. However, it takes a lot of courage to blow the whistle as it can have an enormous impact on the whistleblower’s career and personal well-being – and that of colleagues and researchers associated with the perpetrator. Therefore, it is important to be cognizant of the stressful process that accompanies the act of whistleblowing, to provide clear and accessible procedures to report misconduct, and to support whistleblowers throughout the process. Furthermore, it is essential that appropriate whistleblower protection measures are in place and enforced. Based on our review, we argue that an obligation to report scientific misconduct would currently do more harm than good
Effects of long-acting bronchodilators and placebo on histamine-induced asthma symptoms and mild bronchusobstruction
SummaryStudy objectiveAccurate perception of airway caliber remains an important issue in asthma management. The way bronchodilation is perceived is partly related to the perception of the efficacy of bronchodilators in relieving complaints. In the present study, we compared the effects of salmeterol, formoterol and placebo on relief of histamine-induced asthma symptoms and mild bronchusobstruction.MethodsIn this randomized controlled, double blind study, 30 asthmatics were challenged with histamine until forced expiratory volume in 1s (FEV1) fell with ⩾20%. Subjects received salmeterol, formoterol or placebo after the histamine provocation. Pulmonary function (FEV1) and asthma symptoms (Asthma Symptom Checklist, Borg Dyspnea Scale) were assessed 5 and 20min later.ResultsFEV1 improved significantly more in the salmeterol and formoterol group than in the placebo group (P<0.001, P<0.001 and P<0.05, respectively). Salmeterol and formoterol were not different with regard to the pulmonary function recovery. No significant differences were found between the effects of salmeterol, formoterol and placebo on any of the symptom responses at the different time points.ConclusionsWe conclude that after a histamine-induced mild bronchusobstruction, a similar asthma symptom recovery occurred when inhaling salmeterol, formoterol or placebo, despite better recovery of pulmonary function in the active drug conditions

Symptoomperceptie in astma
Meer dan de helft van de astmapatiënten neemt de toestand van hun luchtw egen weinig accuraat waar. Vanuit medische hoek zijn daarvoor reeds een veelheid aan verklaringen gesuggereerd en onderzocht, recent werd zelfs aangetoond dat problemen met symptoomperceptie gelinkt kunnen zijn aan b epaalde hersendefecten. Deze medische verklaringen volstaan echter niet om de discrepantie tussen de luchtwegpathologie en de intensiteit van de klachten te verklaren: psychologische factoren spelen een belangrijke r ol. In dit doctoraat werden twee mogelijke psychologische verklaringen v oor slechte symptoomperceptie onderzocht: enerzijds de mogelijkheid dat subjectieve klachten kunnen zijn aangeleerd, anderzijds de mogelijkheid dat specifieke automatische aandachtsprocessen de perceptie van astmasym ptomen beïnvloeden. Aangeleerde klachten. Om CO2-inhalatie als experimentele indu ctie van respiratoire symptomen te kunnen gebruiken bij astmapatiënten, onderzochten we in eerste instantie of de rebreathing techniek veilig kon worden gebruikt bij astmapatiënten (n = 18). We vonden geen nadelige fysiologische effecten, ook niet na 3 adembeurten. Wel vonden we een ve rhoogde rapportering van klachten bij basislijnmeting en een dal ing van klachten na de eerste rebreathing beurt (= CO2-trial). We int erpreteerden deze laatste bevinding als een effect van opluchting. Vervolgens gebruikten we de rebreathing techniek in een differentieel co nditionerings-design bij 20 astmapatiënten en 20 gezonde deelnemers. All e deelnemers verwachtten èn rapporteerden mee r klachten tijdens de testfase bij een (placebo-)puffer die voordien met CO2-inhalatie en dus milde respiratoire klachten samenging da n bij een puffer die samenging met zuurstof en dus geen klachten, zond er dat fysiologische veranderingen dit patroon verklaarden. Binnen de gr oep astmapatiënten vonden we het leereffect vooral bij patiënten die vee l hyperventilatieklachten rapporteerden in het dagelijkse leven. <I >Aandacht. Een eerste studie met de dot-probe taak toonde aan dat zow el astmapatiënten (n = 32) als gezonde personen (n = 16) hun aandacht ri chtten op astma-woorden, zonder dat er een verschil tussen de groepen wa s. Binnen de astmagroep vonden we een effect van negatieve affectiviteit (NA): hoog-NA patiënten hadden verhoogde aandacht voor de astmawoorden wanneer het andere woord in het woordpaar een neutraal woord was. Dit wa s niet het geval voor laag-NA patiënten. Een mogelijke verklaring voor het ontbreken van verschillen tussen astma patiënten en de controlegroep ligt in het feit dat de semantische stimul i uit de dot-probe taak te weinig appeleren aan de sensorieel-somatische ervaring die astma is. Om die reden gebruikten we verder een ander para digma: deelnemers luisterden naar 2 geluidsopnamen, één waarop een rustige, normale ademhaling te horen was, de andere met een snellere ade mhaling met duidelijk piepen. Tijdens deze geluidsfragmenten werden kort e toontjes aangeboden, waarop deelnemers zo snel mogelijk moesten reager en. We vonden in een eerste studie met dit paradigma dat alle deelnemers z owel de astmapatiënten (n = 25) als de controlegroep (n = 20) trager reageerden wanneer de toontjes in hetzelfde oor werden aangebo den als de opname van de ademhaling, vergeleken met toontjes in het ande re oor. In een volgende studie met 30 astmapatiënten en een controlegroe p van patiënten met andere aandoeningen (n = 50), konden we deze bevindi ngen echter niet repliceren. We vonden enkel een effect van het type ade mhaling, niet meer van congruentie, ondanks een aantal methodologische a anpassingen om de power van het design te verhogen. Als onderdeel v an de procedure van de laatste studie over aandachtsprocessen verzamelde n we gegevens over de symptoomperceptie van astmapatiënten (n = 30) tijd ens een histamineprovocatie. Door de longfunctiegegevens te vergelijken met de klachten die de patiënten rapporteerden wanneer hun longfunctie d aalde, konden we voor elke patiënt zijn of haar sensitiviteit van symp toomperceptie berekenen. Afzonderlijke analyses voor emotioneel-affectie ve en sensorieel-fysiologische klachten toonden aan dat de sensitiviteit van symptoomperceptie samenhing met het angstniveau bij aanvang van de provocatie en dat de emotionele symptomen een prominente plaats innamen in de klachtenrapporteringen.Catastroferend denken over astma. All e astmapatiënten die deelnamen aan de beschreven studies vulden een vrag enlijst in die peilde naar catastroferende gedachten in verband met astm a. De vragenlijst was gemodeleerd naar de Pain Catastrophizing Scale (PC S-DV). De schaal voor astma bleek uitstekende psychometrische kwaliteite n te bezitten. Bovendien bleek uit de resultaten van de hogervermelde st udies dat catastroferend denken een belangrijke rol speelt in de symptoo mperceptie van astmapatiënten en dat catastroferende gedachten over astm a samenhangen met het rapporteren van meer klachten.EERST i

When randomness is intrinsic, formalize it
Competition for research funds has, in the recent decade, become hypercompetitive. Commonly, to determine which proposals receive funding, a system of peer review is used, which is broadly accepted, easily understood and broadly trusted among researchers. It is often considered the best system in use, but it suffers from important shortcomings and adaptations to overcome these shortcomings have small and often short-lived effects. Hence, the preference for peer review does not mean it necessarily outperforms all other systems. In fact, it is time for an open discussion about alternative allocation mechanisms. Random allocation of research funding may be a viable alternative to the current peer review system. In particular the “organized randomness” of a modified lottery is interesting, combining the benefits of randomization with some of the most valuable aspects of peer review. Still, many questions remain and this is certainly not a plea to allocate all research funds using lotteries without further research. But we need to be prepared to consider alternatives, even though they are not perfect, and modified lotteries should be part of the solution
