    Should the diabetics have the internal thoracic artery skeletonized? Assessment of sternal perfusion by scintillography

    OBJECTIVE: To assess, by scintillography, the effect of using bilateral internal thoracic arteries (BITAs) - prepared by two different techniques - on the sternal perfusion. METHODS: 35 patients undergone coronary artery bypass grafting (CABG) were divided into two groups: Group A (18) had both ITA's dissected using skeletonization technique and group B (17) as pedicle preparation. There was no difference in the two groups relating gender, age and demographic characteristics. On the 7th postoperative day the patients underwent bone scintillography. The statistical analysis was performed using the Student's t test with 95% significance. RESULTS: Group A (skeletonized ITA) showed higher perfusion (11.5%) of the sternum as a mean, than Group B (pedicled ITA) patients; however this was not statistically significant (P = 0.127). On the other hand, comparing the diabetic population, seven in each group, there was a marked 47.4% higher perfusion of the sternum in Group A patients (skeletonized ITA) comparing to Group B (pedicled ITA) and this difference reached statistical significance (P = 0.004). CONCLUSION: 1- Sternal perfusion is not affected significantly apart from the dissection technique used for both internal thoracic arteries in the general population when assessed by bone scintillography. 2 - In the diabetic subgroup, a significant preservation of the sternal perfusion was observed in patients undergone skeletonized dissection of the internal thoracic arteries. Although these findings should be confirmed in a greater number of cases, diabetic patients should have the internal thoracic arteries dissected using skeletonization techinque.OBJETIVO: Avaliar o impacto na vascularização do esterno, por cintilografia óssea, da utilização de ambas as artérias torácicas internas (ATIs), preparadas por duas técnicas diferentes. MÉTODOS: Trinta e cinco pacientes coronarianos foram divididos em dois grupos: Grupo A - 18 pacientes tiveram as duas ATIs dissecadas de forma esqueletizada; Grupo B - 17 pacientes tiveram as duas ATIs dissecadas pela técnica pediculada. Não houve diferença nos dois grupos com relação a gênero, idade e características demográficas. Realizou-se cintilografia óssea 7 dias após a cirurgia. A análise estatística foi realizada utilizando-se o teste de t de Student. com significância estabelecida em 95%. RESULTADOS: No grupo A (ATI esqueletizada), o nível de captação do esterno foi de 11,5% mais alto em comparação com a média dos 17 pacientes do grupo B (ATI pediculada), mas essa diferença não foi estatisticamente significante (P = 0,127). Entretanto, a média dos níveis de captação do esterno nos sete pacientes diabéticos do Grupo A (ATI esqueletizada) foi 47,4% mais alta em comparação à média dos sete pacientes diabéticos do grupo B (ATI pediculada), e esta diferença foi estatisticamente significante (P = 0,004). CONCLUSÃO: 1- A forma de dissecção das ATIs não altera de maneira estatisticamente significativa a perfusão esternal, avaliada por cintilografia óssea, no conjunto geral da população estudada. 2- No subgrupo de pacientes diabéticos, observou-se melhor perfusão do esterno nos pacientes submetidos à dissecção esqueletizada.     Grazing prevalence and associations with eating and general psychopathology, body mass index, and quality of life in a middle-income country

    Research from high-income countries has shown that grazing is a common but problematic eating pattern, particularly when associated with a sense of loss of control. However, it is unclear whether these patterns hold globally. Thus, the goal of this study was to extend previous research by examining the prevalence and clinical correlates of compulsive grazing (CG) and non-compulsive grazing (NCG) in a middle-income country. Participants (N = 2297) comprised adult residents from Rio de Janeiro, Brazil. Recruitment of this population-based household survey occurred from September 2019 to February 2020. The short inventory of grazing was used to operationalise grazing subtypes. Chi-square analyses, logistic regression, and univariate tests were conducted using the complex samples procedure. The point prevalence of regular CG was 10.2% (n = 239) and was consistent with high-income countries, while NCG was 29.8% (n = 679) and was less frequent than reported in high-income countries. Additionally, similar to high-income countries, CG was associated with a higher body mass index and higher odds of eating disorders, eating disorder symptomatology, depression, anxiety, and a lower physical and mental health-related quality of life, than no grazing and NCG. Overall, this study demonstrated that grazing patterns in high-income countries extend to middle-income countries

    Grazing is associated with ADHD symptoms, substance use, and impulsivity in a representative sample of a large metropolitan area in Brazil

    Grazing is a clinically relevant eating behaviour, especially when it presents with a sense of loss of control (compulsive grazing). There is evidence that other disordered eating patterns are associated with problematic substance use and impulsivity-related conditions, such as attention-deficit/hyperactivity disorder (ADHD). This overlap contributes to higher psychopathology and treatment complications. Less is known about grazing, and most information originates in high-income countries. Hence, we sought to investigate relationships between grazing, tobacco and alcohol use, ADHD, and impulsivity in a large representative sample from Brazil. Data were collected by trained interviewers from adults (N = 2297) through an in-person household survey based on a stratified and clustered probability sample. We found significant associations between compulsive grazing and problematic alcohol use (OR = 3.02, 95% CI: 1.65, 5.53), ADHD (OR = 8.94, 95% CI: 5.11, 15.63), and smoking (OR = 1.67, 95% CI: 1.12, 2.47), with impulsivity contributing to the first two relationships. The substantial association with ADHD suggests that other executive functions may promote disordered eating, possibly expressed through difficulties in adhering to regular meals. Clinically, these findings highlight the importance of assessing problematic eating patterns, such as compulsive grazing, in those presenting with difficulties with substance use or impulsivity, and vice versa

    Este trabalho teve por objetivo analisar as compreensões de professores da Educação Infantil sobre o trabalho pedagógico com alunos com Transtorno Espectro Autista nas escolas municipais de Educação Infantil do município de Jaguarão. A pesquisa foi realizada com seis professoras da Educação Infantil por meio de um questionário com questões relacionadas à definição de autismo, inclusão no ensino regular e formação docente. Nesse contexto foi possível perceber muitas dificuldades na inclusão de crianças autistas no ensino regular, dentre elas omissão do Estado na garantia de estrutura das escolas e falta de formação docente adequada. Diante dos resultados, concluímos que o processo de inclusão é lento, porém já é discutido nas escolas e existem algumas políticas públicas de acesso, entretanto não de permanência das crianças autistas. E ainda que os sujeitos da pesquisa reconhecem uma falha em sua formação para trabalhar com inclusão

    Effect of antibodies to intercellular adhesion molecule type 1 on the protection of distant organs during reperfusion syndrome in rats

    We investigated kidney and lung alterations caused by intercellular adhesion molecule type 1 (ICAM-1) blockade after ischemia and reperfusion of hind limb skeletal muscles. Rats were submitted to ligature of the infrarenal aorta for 6 h. The animals were randomized into three groups of 6 rats each: group I, sacrificed after ischemia; group II, reperfusion for 24 h, and group III, reperfusion for 24 h after receiving monoclonal anti-ICAM-1 antibodies. At the end of the experiment, blood samples were collected for creatinine, lactate dehydrogenase, creatine phosphokinase, potassium, pH and leukocyte counts. Samples were taken from the muscles of the hind limbs and from the kidneys and lungs for histological analysis and measurement of the neutrophil infiltrate by myeloperoxidase staining. The groups did not differ significantly with regard to the laboratory tests. There were no major histological alterations in the kidneys. An intense neutrophil infiltrate in the lungs, similar in all groups, was detected. Myeloperoxidase determination showed that after reperfusion there was significantly less retention of polymorphonuclear neutrophils in the muscles (352 ± 70 vs 1451 ± 235 × 10² neutrophils/mg; P<0.01) and in the kidneys (526 ± 89 vs 852 ± 73 × 10² neutrophils/mg; P<0.01) of the animals that received anti-ICAM-1 before perfusion compared to the group that did not.
