A Novel Method for Vibrotactile Proprioceptive Feedback Using Spatial Encoding and Gaussian Interpolation
Objective: The bidirectional communication between the user and the prosthesis is an important requirement when developing prosthetic hands. Proprioceptive feedback is fundamental to perceiving prosthesis movement without the need for constant visual attention. We propose a novel solution to encode wrist rotation using a vibromotor array and Gaussian interpolation of vibration intensity. The approach generates tactile sensation that smoothly rotates around the forearm congruently with prosthetic wrist rotation. The performance of this scheme was systematically assessed for a range of parameter values (number of motors and Gaussian standard deviation). Methods: Fifteen able-bodied subjects and one individual with congenital limb deficiency used vibrational feedback to control the virtual hand in the target-achievement test. Performance was assessed by end-point error and efficiency as well as subjective impressions. Results: The results showed a preference for smooth feedback and a higher number of motors (8 and 6 versus 4). With 8 and 6 motors, the standard deviation, determining the sensation spread and continuity, could be modulated through a broad range of values (0.1 - 2) without a significant performance loss (error: ∼ 10%; efficiency: ∼ 30%). For low values of standard deviation (0.1-0.5), the number of motors could be reduced to 4 without a significant performance decrease. Conclusion: The study demonstrated that the developed strategy provided meaningful rotation feedback. Moreover, the Gaussian standard deviation can be used as an independent parameter to encode an additional feedback variable. Significance: The proposed method is a flexible and effective approach to providing proprioceptive feedback while adjusting the trade-off between sensation quality and the number of vibromotors
Antiblastic Treatment, for Solid Tumors, during Pregnancy: A Crucial Decision:
Cancer is the second leading cause of death during the reproductive years complicating between 0.02% and 0.1% of pregnancies. The incidence is expected to rise with the increase in age of childbearing. The most common types of pregnancy-associated cancers are: cervical cancer, breast cancer, malignant melanoma, Hodgkin's lymphoma, non-Hodgkin's lymphoma and ovarian cancer. The relatively rare occurrence of pregnancy-associated cancer precludes conducting large, prospective studies to examine diagnostic, management and outcome issues. The treatment of pregnancy-associated cancer is complex since it may be associated with adverse fatal effects. In pregnant patients diagnosed with cancer during the first trimester, treatment with multidrug anti-cancer chemotherapy is associated with an increased risk of congenital malformations, spontaneous abortions or fetal death, and therefore, should follow a strong recommendation for pregnancy termination. Second and third trimester exposure is not associated with teratogenic effect but increases the risk of intrauterine growth retardation and low birth weight. There are no sufficient data regarding the teratogenicity of most cytotoxic drugs. Almost all chemotherapeutic agents were found to be teratogenic in animals and for some drugs only experimental data exist. Moreover, no pharmacokinetic studies have been conducted in pregnant women receiving chemotherapy in order to understand whether pregnant women should be treated with different doses of chemotherapy. Hannes Prosthesis Control Based on Regression Machine Learning Algorithms
The quality of life for upper limb amputees can be greatly improved by the adoption of poly-articulated myoelectric prostheses. Typically, in these applications, a pattern recognition algorithm is used to control the system by converting the recorded electromyographic activity (EMG) into complex multi-degrees of freedom (DoFs) movements. However, there is currently a trade-off between the intuitiveness of the control and the number of active DoFs. We here address this challenge by performing simultaneous multi-joint control of the Hannes system and testing several state-of-the-art classifiers to decode hand and wrist movements. The algorithms discriminated multi-DoF movements from forearm EMG signals of 10 healthy subjects reproducing hand opening-closing, wrist flexion-extension and wrist pronation-supination. We first explored the effect of the number of employed EMG electrodes on device performance through the classifiers optimization in terms of F1Score. We further improved classifiers by tuning their respective hyperparameters in terms of the Embedding Optimization Factor. Finally, three mono-lateral amputees tested the optimized algorithms to intuitively and simultaneously control the Hannes system. We found that the algorithms performances were similar to that of healthy subjects, particularly identifying the Non-Linear Regression classifier as the ideal candidate for prosthetic applications
Lahar events in the last 2000 years from Vesuvius eruptions – Part 2: Formulation and validation of a computational model based on a shallow layer approach
In this paper we present a new model for the simulation of lahars based on the depth-averaged code IMEX-SfloW2D with new governing and constitutive equations introduced to better describe the dynamics of lahars. A thorough sensitivity analysis is carried out to identify the critical processes (such as erosion and deposition) and parameters (both numerical and physical) controlling lahar runout using both synthetic and real case topographies. In particular, an application of the model to a syn-eruptive lahar from a reference size eruption from Somma–Vesuvius, affecting the Campanian Plain (southern Italy), described in Di Vito et al. (2024), is used in this work for the sensitivity analysis. Effects of erosion and deposition are investigated by comparing simulations with and without these processes. By comparing flow thickness and area covered by the flow and their evolution with time, we show that the modelling of both the processes is important to properly simulate the effects of the bulking and debulking as well as the associated changes in rheology. From a computational point of view, the comparisons of simulations obtained for different numerical grids (from 25 to 100 m), scheme order, and grain size discretization were useful to find a good compromise between resolution and computational speed. The companion paper by Sandri et al. (2024) shows an application of the presented model for probabilistic volcanic hazard assessment for lahars from Vesuvius deposits in the Neapolitan area.</p
The archaeological record from gruta El Manzano and their implicances for Nodpatagonia archaeology
En este trabajo se presentan los resultados de los análisis realizados sobre los materiales del sitio arqueológico Gruta de El Manzano, localizado sobre el río Grande, en el Departamento de Malargüe, al sur de la provincia de Mendoza. Utilizando la información generada, se discuten aspectos de la funcionalidad del sitio, los cambios a través del tiempo y se destaca su importancia para la discusión de los temas de la arqueología del norte de Patagonia. Los recientes fechados radiocarbónicos ubican al inicio de las ocupaciones en más de 8.000 años AP y confirman la existencia del hiatus regional para el Holoceno medio.
Los materiales analizados muestran importantes cambios en las tendencias de los mismos, especialmente hacia la segunda mitad del Holoceno tardío, donde los diferentes indicadores sugieren diferencias en relación a la movilidad, el uso de la fauna, los recursos vegetales y la incorporación de tecnología cerámica.In this paper we present the results of the analysis carried out with the materials from Gruta de El Manzano archaeological site, located beside Grande River, in Malargüe, southern Mendoza province.
Using the new information, we discuss the site function, changes trough time, and remark his importance for the northern Patagonia discussion. The last radiocarbon data place the beginning of the occupation in more than 8.000 years BP and confirm the existence of the mid Holocene regional hiatus. SARS-CoV-2 Breakthrough Infections: Incidence and Risk Factors in a Large European Multicentric Cohort of Health Workers.
Background: The research aimed to investigate the incidence of SARS-CoV-2 breakthrough infections and their determinants in a large European cohort of more than 60,000 health workers.
Methods: A multicentric retrospective cohort study, involving 12 European centers, was carried out within the ORCHESTRA project, collecting data up to 18 November 2021 on fully vaccinated health workers. The cumulative incidence of SARS-CoV-2 breakthrough infections was investigated with its association with occupational and social-demographic characteristics (age, sex, job title, previous SARS-CoV-2 infection, antibody titer levels, and time from the vaccination course completion).
Results: Among 64,172 health workers from 12 European health centers, 797 breakthrough infections were observed (cumulative incidence of 1.2%). The primary analysis using individual data on 8 out of 12 centers showed that age and previous infection significantly modified breakthrough infection rates. In the meta-analysis of aggregated data from all centers, previous SARS-CoV-2 infection and the standardized antibody titer were inversely related to the risk of breakthrough infection (p = 0.008 and p = 0.007, respectively).
Conclusion: The inverse correlation of antibody titer with the risk of breakthrough infection supports the evidence that vaccination plays a primary role in infection prevention, especially in health workers. Cellular immunity, previous clinical conditions, and vaccination timing should be further investigated
Epidemiological patterns of asbestos exposure and spatial clusters of incident cases of malignant mesothelioma from the Italian national registry
Abstract
BACKGROUND:
Previous ecological spatial studies of malignant mesothelioma cases, mostly based on mortality data, lack reliable data on individual exposure to asbestos, thus failing to assess the contribution of different occupational and environmental sources in the determination of risk excess in specific areas. This study aims to identify territorial clusters of malignant mesothelioma through a Bayesian spatial analysis and to characterize them by the integrated use of asbestos exposure information retrieved from the Italian national mesothelioma registry (ReNaM).
METHODS:
In the period 1993 to 2008, 15,322 incident cases of all-site malignant mesothelioma were recorded and 11,852 occupational, residential and familial histories were obtained by individual interviews. Observed cases were assigned to the municipality of residence at the time of diagnosis and compared to those expected based on the age-specific rates of the respective geographical area. A spatial cluster analysis was performed for each area applying a Bayesian hierarchical model. Information about modalities and economic sectors of asbestos exposure was analyzed for each cluster.
RESULTS:
Thirty-two clusters of malignant mesothelioma were identified and characterized using the exposure data. Asbestos cement manufacturing industries and shipbuilding and repair facilities represented the main sources of asbestos exposure, but a major contribution to asbestos exposure was also provided by sectors with no direct use of asbestos, such as non-asbestos textile industries, metal engineering and construction. A high proportion of cases with environmental exposure was found in clusters where asbestos cement plants were located or a natural source of asbestos (or asbestos-like) fibers was identifiable. Differences in type and sources of exposure can also explain the varying percentage of cases occurring in women among clusters.
CONCLUSIONS:
Our study demonstrates shared exposure patterns in territorial clusters of malignant mesothelioma due to single or multiple industrial sources, with major implications for public health policies, health surveillance, compensation procedures and site remediation programs
