The development of a multilingual tool for facilitating the primary-specialty care interface in low resource settings: the MSF tele-expertise system
In 2009, Médecins Sans Frontières (MSF) started a pilot trial of store-and-forward telemedicine to support field workers. One network was operated in French and one in English; a third, Spanish network was brought into operation in 2012. The three telemedicine pilots were then combined to form a single multilingual tele-expertise system, tailored to support MSF field staff. We conducted a retrospective analysis of all telemedicine cases referred from April 2010 to March 2014. We also carried out a survey of all users in December 2013. A total of 1039 referrals were received from 41 countries, of which 89% were in English, 10% in French, and 1% in Spanish. The cases covered a very wide range of medical and surgical specialties. The median delay in providing the first specialist response to the referrer was 5.3 h (interquartile range 1.8, 16.4). The survey was sent to 294 referrers and 254 specialists. Of these, 224 were considered as active users (41%). Out of the 548 users, 163 (30%) answered the survey. The majority of referrers (79%) reported that the advice received via the system improved their management of the patient. The main concerns raised by referrers and specialists were the lack of support or promotion of system at headquarters' level and the lack of feedback about patient follow-up. Because of the size of the MSF organization, it is clear that there is potential for further organizational adoption
The Fear of God in Early Modern Political Theory
A group's collective identity is a complex phenomenon which is always difficult to delineate and understand, but however one does so, historical antecedents must be a crucial element. This year's "Presidential Address" explores this important question, which was raised in an earlier presidential discourse. On that occasion, Robert Craig Brown noted that "historical knowledge is an essential component of a nation's sense of cultural identity. ' ' Professor Wallot elucidates this theme: without a concept of what you have been, you cannot know who you are, or what you can be. As one of the characters in Joy Kogama's novel Obasan observes, "you are your history. If you cut off any of it, you're an amputee. ' 'Professor Wallot sets out to explore this problem by examining the Lower Canadian identity between roughly 1780 and 1815, in order to place the colony within the context of the culture of the North Atlantic world. Though Quebec/Lower Canada has often been portrayed as a closed society, relatively homogenous in its attitudes, cut off from its intellectual roots, and somewhat unsympathetic to new ideas, study of aspects of its culture suggest otherwise. The colony had access to contemporary international thought, in all of its variety, and was more than a passive observer in the clash of ideas and the rhythms of cultural change then current in Europe. In arriving at these conclusions, the author presents a two-part defence; in the first part of his paper, he examines the means of cultural diffusion, the role of printed materials in the formation of attitudes and the rapidity with which European ideas were transferred to Quebec. He concludes that, when one removes the time required to transmit these ideas, the colony was aware of, and deeply involved in, the intellectual cross-currents of the North Atlantic world.The author then proceeds to test the validity of this point by examining three quite different aspects of public culture: the discussion aroused by the fear of overpopulation and consequent impoverishment; the banking system and money, and finally, parliamentary theory and practice. In each of these fields, Professor Wallot concludes, the colony's cultural élite, at the very least, was aware of, and responsive to, recent European thought. In a society which boasted nearly universal literacy, this conclusion suggests a culture far more up-to-date than previous work would lead us to expect
Teledermatology in low-resource settings: the MSF experience with a multilingual tele-experise platform
Introduction: In 2010, Médecins Sans Frontières (MSF) launched a tele-expertise system to improve the access to specialized clinical support for its field health workers. Among medical specialties, dermatology is the second most commonly requested type of tele-expertise. The aim of the present study was to review all MSF teledermatology cases in the first 4 years of operation. Our hypothesis was that the review would enable the identification of key areas for improvement in the current MSF teledermatology system. Methods: We carried out a retrospective analysis of all dermatology cases referred by MSF field doctors through the MSF platform from April 2010 until February 2014. We conducted a quantitative and qualitative analysis based on a survey sent to all referrers and specialists involved in these cases. Results: A total of 65 clinical cases were recorded by the system and 26 experts were involved in case management. The median delay in providing the first specialist response was 10.2 h (IQR 3.7–21.1). The median delay in allocating a new case was 0.96 h (IQR 0.26–3.05). The three main countries of case origin were South Sudan (29%), Ethiopia (12%), and Democratic Republic of Congo (10%). The most common topics treated were infectious diseases (46%), inflammatory diseases (25%), and genetic diseases (14%). One-third of users completed the survey. The two main issues raised by specialists and/or referrers were the lack of feedback about patient follow-up and the insufficient quality of clinical details and information supplied by referrers. Discussion: The system clearly delivered a useful service to referrers because the workload rose steadily during the 4-year study period. Nonetheless, user surveys and retrospective analysis suggest that the MSF teledermatology system can be improved by providing guidance on best practice, using pre-filled referral forms, following-up the cases after teleconsultation, and establishing standards for clinical photography
Psychometric properties of the Generalised Anxiety Disorder Dimensional Scale in an Australian sample
The Generalised Anxiety Disorder Dimensional Scale is a new measure of generalised anxiety disorder developed to assist clinicians in the dimensional assessment of generalised anxiety disorder by the Diagnostic and Statistical Manual (Fifth Edition) Anxiety, Obsessive-Compulsive Spectrum, Posttraumatic, and Dissociative Disorder Work Group. This study aims to evaluate the psychometric properties of the scale in an Australian community sample. A sample of 293 Australians (72.7% female) aged between 18 and 73 (M = 28.31 years; SD = 12.11 years) was recruited. Participants completed the Generalised Anxiety Disorder Dimensional Scale, as well as related measures used to assess convergent and discriminant validity. A small proportion of the sample (n = 21) completed the scale a second time to assess test-retest reliability. The scale demonstrated a unidimensional factor structure, good internal consistency (Cronbach’s α = .94), good test-retest reliability (ICC = .85), good convergent validity with the Generalised Anxiety Disorder– 7 item (rs = .77), and discriminant validity with the Panic Disorder Severity Scale–Self Report (rs = .63). The scale appears to be a reliable and valid measure of generalised anxiety disorder symptomology for use in the Australian population
Exploring the effects of cigarette smoking on inflammatory bowel disease using Mendelian randomisation
Background. Previous observational evidence has suggested an association between smoking and inflammatory bowel disease (IBD). Methods. We used observational techniques followed by Mendelian randomisation (MR) to explore if smoking is a causal factor in the development of IBD and its subtypes.Results. In those who have ever smoked, we observed increased risk of IBD, and, in current smokers, we observed increased risk of Crohn’s disease and decreased risk of Ulcerative Colitis. However, our MR analyses found little evidence that smoking affects the development of IBD. Conclusion. Overall, our results suggest that smoking does not causally influence the risk of IBD
Perspectives of statistician, microbiologist, and clinician stakeholders on the use of microbiological outcomes in randomised trials of antimicrobial stewardship interventions
Microbiological data are used as indicators of infection, for diagnosis, and the identification of antimicrobial resistance in trials of antimicrobial stewardship interventions. However, several problems have been identified in a recently conducted systematic review (e.g., inconsistency in reporting and oversimplified outcomes), which motivates the need to understand and improve the use of these data including analysis and reporting. We engaged key stakeholders including statisticians, clinicians from both primary and secondary care, and microbiologists. Discussions included issues identified in the systematic review and questions about the value of using microbiological data in clinical trials, perspectives on current microbiological outcomes reported in trials, and alternative statistical approaches to analyse these data. Various factors (such as unclear sample collection process, dichotomising or categorising complex microbiological data, and unclear methods of handling missing data) were identified that contributed to the low quality of the microbiological outcomes and the analysis of these outcomes in trials. Whilst not all of these factors would be easy to overcome, there is room for improvement and a need to encourage researchers to understand the impact of misusing these data. Coordinated progression through two subtranscriptomes underlies the tachyzoite cycle of Toxoplasma gondii
BACKGROUND: Apicomplexan parasites replicate by varied and unusual processes where the typically eukaryotic expansion of cellular components and chromosome cycle are coordinated with the biosynthesis of parasite-specific structures essential for transmission. METHODOLOGY/PRINCIPAL FINDINGS: Here we describe the global cell cycle transcriptome of the tachyzoite stage of Toxoplasma gondii. In dividing tachyzoites, more than a third of the mRNAs exhibit significant cyclical profiles whose timing correlates with biosynthetic events that unfold during daughter parasite formation. These 2,833 mRNAs have a bimodal organization with peak expression occurring in one of two transcriptional waves that are bounded by the transition into S phase and cell cycle exit following cytokinesis. The G1-subtranscriptome is enriched for genes required for basal biosynthetic and metabolic functions, similar to most eukaryotes, while the S/M-subtranscriptome is characterized by the uniquely apicomplexan requirements of parasite maturation, development of specialized organelles, and egress of infectious daughter cells. Two dozen AP2 transcription factors form a series through the tachyzoite cycle with successive sharp peaks of protein expression in the same timeframes as their mRNA patterns, indicating that the mechanisms responsible for the timing of protein delivery might be mediated by AP2 domains with different promoter recognition specificities. CONCLUSION/SIGNIFICANCE: Underlying each of the major events in apicomplexan cell cycles, and many more subordinate actions, are dynamic changes in parasite gene expression. The mechanisms responsible for cyclical gene expression timing are likely crucial to the efficiency of parasite replication and may provide new avenues for interfering with parasite growth
