    The effect of online journalism on the freedom of the press: the case of Kuwait

    Online journalism has brought new features of journalism practices for local journalists and forced the expansion of their freedom. The Internet as a whole became the tool for freedom of expression for many suppressed countries, and online journalism became an alternative for press freedom in cyberspace. The diffusion of information enabled more opportunities for freedom of expression and speech prosperity, leading to a higher level of freedom in local press. This research project aims to examine the effect of online journalism on the freedom of the local press in the state of Kuwait. Since mid 1990s, when the Internet was introduced in Kuwait, a new phenomenon of press freedom started to rise. After many decades of relying heavily on local newspapers and controlled radio and TV, many Kuwaitis switched to the Internet to obtain information, news and political analysis. The political dispute of power after the death of Kuwaiti Emir Sheikh Jabber Al-Ahmad Al-Sabah on 15 of January 2006, followed by the public demand to change the electoral constituencies voting system of the National Assembly, and the dissolving of the National Assembly in May 2006 forced many Kuwaitis to go online to get the latest news and analysis regarding the two issues. Kuwaiti online journalism became the source for instant updated information during the disputes. Many local writers praised their work on local press. Mohammad Abdul Qader Al-Jasim, a columnist and former editor in chief for Alwatan local newspaper, in his online Web site ‘Meezan’, provided non-censored detailed analysis of these situations without any restrictions or fear of government interference which was considered as a taboo “red line” no one was permitted to cross. The researcher used three different tools (survey, online content analysis, and interviews) to determine the effect of online journalism on journalist’s practices and the freedom of the press in Kuwait, focusing on the most popular Kuwaiti personal writers’ sites, weblogs and forums. The results show that online journalism affected journalist’s practices but did not replace the traditional practices. The Internet became a source and communication platform for many local journalists. At the same time, online journalism became one of the tools that helped increase the level of freedom in the local press. The language of online journalism took a different direction from the local press with more freedom to write, discuss, and share ideas online with less fear of government retribution. What was considered a taboo “red line” in the local press became an acceptable “green line” online.     Diversity of multi-drug resistant Acinetobacter baumannii population in a major hospital in Kuwait

    Acinetobacter baumannii is one of the most important opportunistic pathogens that causes serious health care associated complications in critically ill patients. In the current study we report on the diversity of the clinical multi-drug resistant (MDR) A. baumannii in Kuwait by molecular characterization. One hundred A. baumannii were isolated from one of the largest governmental hospitals in Kuwait. Following the identification of the isolates by molecular methods, the amplified blaOXA-51-like gene product of one isolate (KO-12) recovered from blood showed the insertion of the ISAba19 at position 379 in blaOXA-78. Of the 33 MDR isolates, 28 (85%) contained blaOXA-23, 2 (6%) blaOXA-24 and 6 (18%) blaPER-1 gene. We did not detect blaOXA-58, blaV IM, blaIMP, blaGES, blaV EB, and blaNDM genes in any of the tested isolates. In three blaPER-1 positive isolates the genetic environment of blaPER-1 consisted of two copies of ISPa12 (tnpiA1) surrounding the blaPER-1 gene on a highly stable plasmid of ca. 140-kb. Multilocus-sequence typing (MLST) analysis of the 33 A. baumannii isolates identified 20 different STs, of which six (ST-607, ST-608, ST-609, ST-610, ST-611, and ST-612) were novel. Emerging STs such as ST15 (identified for the first time in the Middle East), ST78 and ST25 were also detected. The predominant clonal complex was CC2. Pulsed-field gel electrophoresis and MLST defined the MDR isolates as multi-clonal with diverse lineages. Our results lead us to believe that A. baumannii is diverse in clonal origins and/or is undergoing clonal expansion continuously while multiple lineages of MDR A. baumannii circulate in hospital ward simultaneously

    Social Media and the Spiral of Silence: The Case of Kuwaiti Female Students Political Discourse on Twitter

    The theory of the Spiral of Silence (Noelle-Neumann, 1984), explained why the view of a minority is not presented when the majority view dominates the public sphere. For years the theory of the spiral of silence was used to describe the isolation of minority opinions when seeking help from traditional media, which play a significant role in increasing the isolation. The fear of isolation makes many people afraid of exchanging their views face-to-face with others. The main fear comes from identifying the people who hold a minority opinion. However, with the proliferation of social networks people have moved online to exchange their views, whether they hold a minority or a majority opinion, as long as their identities are concealed. Although women are the majority population in many Arab societies, their voices are still considered a minority view. In addition to the effects described in the spiral of silence, there are other obstacles to self-expression. Religion, culture, tradition, and education may have a negative effect, preventing women’s voice being made public. Social networks have helped to promote women’s voices while removing offline obstacles. This paper uses the theory of the spiral of silence in relation to women’s online political participation in Twitter, even though they may not be willing to share their opinions offline (face-to-face), to learn whether the theory is still useful as an account of online relationships. The results show that the spiral of silence does not explain the behavior of women, either face-to-face or online

    The new technologies of broadcasting, sports coverage, sports casters, and sports analysis, especially in Europe, have attracted many local sports players and fans to enjoy and imitate famed European players. The globalization of football (soccer) has affected sports culture in Kuwait. In-depth interviews with 17 interviewees including sports academics, experts, practitioners, coaches, sports players, fans, and sports reporters revealed that the English Premier League not only entertained the fans in Kuwait but also affected their popular culture behavior and local football league performance and attendance. The English Premier League also affected fandom lifestyle and expenditure through expensive sports subscriptions or even traveling to Europe to attend football matches.Output Status: Forthcoming/Available Onlin

    Achieving accurate estimates of fetal gestational age and personalised predictions of fetal growth based on data from an international prospective cohort study: a population-based machine learning study.

    Background: Preterm birth is a major global health challenge, the leading cause of death in children under 5 years of age, and a key measure of a population's general health and nutritional status. Current clinical methods of estimating fetal gestational age are often inaccurate. For example, between 20 and 30 weeks of gestation, the width of the 95% prediction interval around the actual gestational age is estimated to be 18-36 days, even when the best ultrasound estimates are used. The aims of this study are to improve estimates of fetal gestational age and provide personalised predictions of future growth. Methods: Using ultrasound-derived, fetal biometric data, we developed a machine learning approach to accurately estimate gestational age. The accuracy of the method is determined by reference to exactly known facts pertaining to each fetus-specifically, intervals between ultrasound visits-rather than the date of the mother's last menstrual period. The data stem from a sample of healthy, well-nourished participants in a large, multicentre, population-based study, the International Fetal and Newborn Growth Consortium for the 21st Century (INTERGROWTH-21st). The generalisability of the algorithm is shown with data from a different and more heterogeneous population (INTERBIO-21st Fetal Study). Findings: In the context of two large datasets, we estimated gestational age between 20 and 30 weeks of gestation with 95% confidence to within 3 days, using measurements made in a 10-week window spanning the second and third trimesters. Fetal gestational age can thus be estimated in the 20-30 weeks gestational age window with a prediction interval 3-5 times better than with any previous algorithm. This will enable improved management of individual pregnancies. 6-week forecasts of the growth trajectory for a given fetus are accurate to within 7 days. This will help identify at-risk fetuses more accurately than currently possible. At population level, the higher accuracy is expected to improve fetal growth charts and population health assessments. Interpretation: Machine learning can circumvent long-standing limitations in determining fetal gestational age and future growth trajectory, without recourse to often inaccurately known information, such as the date of the mother's last menstrual period. Using this algorithm in clinical practice could facilitate the management of individual pregnancies and improve population-level health. Upon publication of this study, the algorithm for gestational age estimates will be provided for research purposes free of charge via a web portal.     Interhospital Transfer Before Thrombectomy Is Associated With Delayed Treatment and Worse Outcome in the STRATIS Registry (Systematic Evaluation of Patients Treated With Neurothrombectomy Devices for Acute Ischemic Stroke).

    BACKGROUND: Endovascular treatment with mechanical thrombectomy (MT) is beneficial for patients with acute stroke suffering a large-vessel occlusion, although treatment efficacy is highly time-dependent. We hypothesized that interhospital transfer to endovascular-capable centers would result in treatment delays and worse clinical outcomes compared with direct presentation. METHODS: STRATIS (Systematic Evaluation of Patients Treated With Neurothrombectomy Devices for Acute Ischemic Stroke) was a prospective, multicenter, observational, single-arm study of real-world MT for acute stroke because of anterior-circulation large-vessel occlusion performed at 55 sites over 2 years, including 1000 patients with severe stroke and treated within 8 hours. Patients underwent MT with or without intravenous tissue plasminogen activator and were admitted to endovascular-capable centers via either interhospital transfer or direct presentation. The primary clinical outcome was functional independence (modified Rankin Score 0-2) at 90 days. We assessed (1) real-world time metrics of stroke care delivery, (2) outcome differences between direct and transfer patients undergoing MT, and (3) the potential impact of local hospital bypass. RESULTS: A total of 984 patients were analyzed. Median onset-to-revascularization time was 202.0 minutes for direct versus 311.5 minutes for transfer patients ( CONCLUSIONS: In this large, real-world study, interhospital transfer was associated with significant treatment delays and lower chance of good outcome. Strategies to facilitate more rapid identification of large-vessel occlusion and direct routing to endovascular-capable centers for patients with severe stroke may improve outcomes.
