    Peningkatan angka kecelakaan dapat menyebabkan trauma yang mendekati bentuk endemik. Pasien dengan kecelakaan yang berat dapat diikuti dengan trauma multi organ diantaranya cidera tulang belakang. Seseorang dengan cidera tulang belakang akan memasuki dunia baru, dimana muncul masalah yang kompleks baik fisik maupun psikologis. Orang yang mengalami psikologi shock akan terjadi gangguan dalam tingkah laku, suasana hati pikiran dan kognitif, hal ini akan mempengaruhi konsep dirinya. Masalah lain yang berpengaruh terhadap konsep dirinya yaitu adanya kelumpuhan baik tetraplegi, para plegi dan klinikal syndrome. Penelitian ini bertujuan untuk mengetahui gambaran diri, ideal diri, harga diri, identitas diri, penampilan peran serta gangguan konsep diri yang paling menonjol dialami oleh pasien cidera tulang belakang.Penelitian ini menggunakan metode cross sectional dan jenisnya deskriptif yang dilaksanakan pada pasien cidera tulang belakang di Bangsal Dahlia Rumah Sakit Ortopedi Prof. DR. R. Soeharso Surakarta. Alat ukur yang digunakan adalah yang sudah pernah digunakan oleh peneliti sebelumnya yaitu Imron (2000) dalam peneliti ini dimodifikasi menjadi favorable dan unfavorable, jumlah 46 butir soal, jawaban diberi skor dengan skala Linkert, kemudian dikategorikan jelek, kurang, cukup dan baik menurut Sugiono.Hasil penelitian mengungkapkan bahwa sebagian besar pasien dengan gambaran diri yang kurang (47%), kategori yang jelek dan baik dengan prosentase yang sama untuk ideal diri (37%), sebagian besar harga dirinya cukup (47%) dan baik (40%), identitas diri pasien cukup (33%) dan baik (57%) serta prosentase yang sama yaitu 33% untuk penampilan peran pasien cukup dan baik. Gangguan konsep diri yang paling banyak dialami oleh pasien adalah gambaran diri. Gambaran secara umum adalah cukup positif berada dalam rentang adaptif. Sebagai saran agar adanya komunikasi yang baik dari berbagai pihak, saling menghargai, memberikan peran yang sesuai sehingga stressor yang ada tidak menjadi faktor predisposisi dan presipitasi yang dapat mempengaruhi konsep dirinya

    Hormone Receptor Status in Breast Cancer and its Relation to Age and Other Prognostic Factors

    Background Increasing evidence shows the importance of young age, estrogen receptor (ER), progesterone receptor (PR) status, and HER-2 expression in patients with breast cancers. Patients and methods We organized an analytic cross-sectional study of 105 women diagnosed with breast cancer who have been operated on between 2008 to 2010. We evaluated age, size, hormone receptor status, HER-2 and P53 expression as possible indicator of lymph node involvement. Results There is a direct correlation between positive progesterone receptor status and being younger than 40 ( P < 0.05). Also, compared with older women, young women had tumors that were more likely to be large in size and have higher stages ( P < 0.05). Furthermore patients with negative progesterone receptor status were more likely to have HER-2 overexpression ( P < 0.05). The differences in propensity to lymph node metastasis between hormone receptor statuses were not statically significant. Conclusions Although negative progesterone receptor tumors were more likely to have HER-2 overexpression, it is possible that higher stage and larger size breast cancer in younger women is related to positive progesterone receptor status

    Prevalence of antimicrobial resistance in Helicobacter pylori isolates from Iran

    Helicobacter pylori, a gram negative bacterium is capable of being resistant to a wide spectrum of antimicrobial drugs. The prevalence of antibiotic resistance in H. pylori strains differs amongst distinct geographical areas and has increased worldwide. Therefore, information concerning the prevalence of antimicrobial-resistant H. pylori strains is important in predicting therapeutic response. In this study, drug susceptibility of H. pylori in patients was investigated in Laleh hospital, Tehran, Iran from 2007 - 2008. 104 antral biopsies of patients with non ulcer dyspepsia and peptic ulcer were cultured. Susceptibility patterns were determined by disk diffusion method. Minimum inhibitory concentration (MIC) was performed for resistant isolates of H. pylori. In our study, 51.5% of clinical isolates showed resistance to metronidazole. All of the isolates were sensitive to other antibiotic disks including clarithromycin, amoxicillin, tetracycline and furazolidone. MIC was determined as 16 μg/ml in 5.8% and 32 μg/ml in 94.1% of isolates for metronidazole resistance isolates. The results indicated that the major drug resisted by H. pylori is metronidazole and it should be considered when recommending drugs to patients in this region.Keywords: Helicobacter pylori, antimicrobial resistance, MICAfrican Journal of Biotechnology Vol. 9(36), pp. 5962-5965, 6 September, 201

    The major problem of this study is how anxiety is reflected in the major character‟s personality. The aim of this study is to analyze David Richard Ellis’s Final Destination 4 based on the structural elements and the development of personality of the major character based on Psychoanalytic Approach. This study focuses on the major character, namely Nick Obanon. This research is qualitative research. The data of the study are text and image taken from two data sources: primary and secondary. The primary data source is the Final Destination movie directed by David Richard Ellis released in 2009. While the secondary data sources are other materials taken from books, journals, and internet related to the study. Both data are collected through library research and analyzed by descriptive analysis. The study comes to the following conclusions. First, based on the structural analysis of each elements, it shows that the character and characterization, casting, plot, setting, point of view, theme, mise-en scene, cinematography, sound, and editing are related to each other and form a unity. Second, based on the psychoanalytic perspective that the problems of the major character cause the conflict of his mental condition. It happens when there is contradiction between Id, Ego, and Superego and it brings him into anxiety

    Nitazoxanide and doxycycline sensitivity among metronidazole resistant helicobacter pylori isolates from patients with gastritis

    Background: Antibiotic therapy should be done based on resistance characteristics of Helicobacter pylori strains to commonly prescribed antibiotics in areas with higher resistance rates. Objectives: This study examined antibacterial activity of nitazoxanide and doxycycline against clinical H. pylori isolates showing different metronidazole resistance levels. Methods: A total of 122 patients, who underwent endoscopy were enrolled in this study from 3 hospitals of Tehran, during November 2014 to July 2015. Helicobacter pylori isolates were obtained from gastric biopsies of the patients after culture in specific culture medium and characterization by both biochemical and molecular methods. Antimicrobial susceptibility to metronidazole was detected using the agar dilution method and minimum inhibitory concentrations of nitazoxanide and doxycycline were determined for metronidazole resistant strains. Results: From a total of 122 gastric biopsy specimens, 55 H. pylori strains were recovered (45). Thirty-three of these strains (60.0) were resistance to metronidazole. MIC50 and MIC90 values for metronidazole were 32 and 64 µg/mL, respectively. MIC50 and MIC90 values for doxycycline and nitazoxanide were measured as 4 and �8 µg/mL, and 8 and �32 µg/mL, respectively. Conclusions: Dominance of high level metronidazole resistance H. pylori strains among the studied patients questioned its usefulness for first-line therapy in Iran. Nitazoxanide and doxycycline showed superior activity against H. pylori strains in comparison to metronidazole, which should be considered for alternative therapies. © 2018, Kowsar Medical Publishing Company.     Takayasu's arteritis associated with Crohn's disease: A case report

    Introduction: The simultaneous presence of Takayasu's arteritis and Crohn's disease in a patient seems to be rare. To our knowledge, no patient with the combination of Crohn's disease and Takayasu's arteritis has been reported from our region. Case presentation: Herein we present the case of a 22-year-old Iranian woman previously diagnosed as Crohn's disease and who had subsequently developed Takayasu's arteritis. Conclusion: Clinical suspicion, proper imaging, and consideration of the differential diagnosis are important for the correct diagnosis and management of patients with this coincidence. © 2008 Daryani et al; licensee BioMed Central Ltd

    Hypercalcemia and huge splenomegaly presenting in an elderly patient with B-cell non-Hodgkin's lymphoma: a case report

    <p>Abstract</p> <p>Introduction</p> <p>Hypercalcemia is the major electrolyte abnormality in patients with malignant tumors. It can be due to localized osteolytic hypercalcemia or elaboration of humoral substances such as parathyroid hormone-related protein from tumoral cells. In hematological malignancies, a third mechanism of uncontrolled synthesis and secretion of 1-25(OH)<sub>2</sub>D<sub>3 </sub>from tumoral cells or neighboring macrophages may contribute to the problem. However, hypercalcemia is quite unusual in patients with B-cell non-Hodgkin's lymphoma.</p> <p>Case presentation</p> <p>An 85-year-old Caucasian woman presented with low grade fever, anorexia, abdominal discomfort and fullness in her left abdomen for the last six months. She was mildly anemic and complained of fatigability. She had huge splenomegaly and was hypercalcemic. After correction of her hypercalcemia, she had a splenectomy. Microscopic evaluation revealed a malignant lymphoma. Her immunohistochemistry was positive for leukocyte common antigen, CD20 and parathyroid hormone-related peptide.</p> <p>Conclusion</p> <p>Immunopositivity for parathyroid hormone-related peptide clearly demonstrates that hypersecretion of a parathyroid hormone-like substance from the tumor had led to hypercalcemia in this case. High serum calcium is seen in only seven to eight percent of patients with B-cell non-Hodgkin's lymphoma, apparently due to different mechanisms. Evaluation of serum parathyroid hormone-related protein and 1-25(OH)<sub>2</sub>D<sub>3 </sub>can be helpful in diagnosis and management. It should be noted that presentation with hypercalcemia has a serious impact on prognosis and survival.</p
