    When Questions are the Answers

    This is an accessible article published in a magazine that is circulated to all members of the British Association of Social Work. The article draws the attention of practitioners and their managers to developments in what constitutes good practice in supervision.Professional supervision enables social work practitioners to reflect on their performance in relation to organizational goals, but also their own developmental needs. Over recent times, front line managers have been encouraged to incorporate reflective techniques in their supervision, such as those associated with coaching. Among coaching theorists there is a consensus that 'questions are the answers'. Good quality questioning conversations, which are part of a trusting supervisory relationship, may be of benefit to social work practitioners as well as the people they aim to help

    The Lived Experience of Percutaneous Injuries Among US Registered Nurses: A Phenomenological Study

    Thesis advisor: Rosanna F. DeMarcoThe purpose of this study was to understand the lived experience and meanings of percutaneous injury (PI) and its aftermath among US registered nurses. An interpretive phenomenological approach was utilized to carry out the study which included nine percutaneous injury experiences. Van Manen's existential framework was used as a reflective guide. Findings from this study emerged as three essential themes which were common to all participants: being shocked: the potential of a serious or life-threatening infection; needing to know it's going to be okay; and sensing vulnerability. The first theme, being shocked, was identified as the primary mode of living with the sudden occurrence of PI. In the moment of injury, participants' language reflected shock and an immediate consciousness of the potential threat of a serious or life-threatening infection. Nurses' responses were visceral and emotional. All acted on their need to reduce foreign blood contamination and the urgency they felt for immediate care. Needing to know it's going to be okay represented the initial meaning of living in the aftermath of PI as nurses assessed their risk and sought post exposure intervention and caring responses from others. Sensing vulnerability was identified as the secondary mode of living in the aftermath of PI as participants reflected on the fragile nature of health into the future, distinguished between supportive vs. non-supportive relationships in their overall PI experience, and identified the need to be vigilant in the future with respect to their health, life and PI prevention. Together, these three essential themes and their dimensions represent the essence and meanings of percutaneous injury and its aftermath for at least one group of US registered nurses. Findings in this study support the conclusion that the lived experience of PIs and its aftermath imposed a significant psychological burden on nurses. These findings offer a better understanding of the essence and meanings of PI and its aftermath and contribute knowledge to inform nursing education, nursing practice, health policy and future research.Thesis (PhD) — Boston College, 2010.Submitted to: Boston College.     Occupational Therapy on College Campuses: Facilitating Student Success Through Occupation

    Purpose: To explore how occupational therapy services fit into a supported education model on college campuses. Rationale: College students present with many strengths and challenges in college settings. There are limited services for young adults to succeed in college and students with various diagnoses face challenges in the areas of time management, organization, academic skills, and in social areas of college (Orentlicher, & Olson, 2010; Rogers, Kash-MacDonald, Bruker, & Maru, 2010). Higher education is a role emerging area for occupational therapists, and there are some pioneering occupational therapists who have discovered multiple ways to provide services to students who are challenged by aspects of college life. Objectives: Describe the history and current practice of supported education and its fit within the domain of OT. Describe a variety of OT supported education programmes Identify aspects of the supported education programmes that participants could apply to their college or practice setting Format: This workshop will begin by describing the history and models of supported education. The presenters will define six examples of OT programmes on college campuses. Finally, the presenters will encourage participants to share ideas and identify aspects of the various programs that they could apply to their college or practice setting

    Impact and process evaluation of a co-designed ‘Move More, Sit Less’ intervention in a public sector workplace

    BACKGROUND:High levels of sitting are associated with increased risk of adverse health outcomes, including chronic disease. Extensive sitting at work is common, hence organisations should provide options to employees to reduce prolonged sitting. OBJECTIVE:To assess the efficacy and acceptability of a co-designed intervention to increase standing and reduce sitting in a public-sector office. METHODS:Forty-six adults participated in the quasi-experimental study (30 intervention; 16 control). The intervention involved providing sit-stand desks, prompts, workshops, and information emails to assist behavior change. Participants wore a thigh-mounted Actigraph GT3X+ for five working days and responded to an online questionnaire at baseline (BL), 6 (T1) and 13 weeks (T2) post intervention. RESULTS:Inclinometer-measured proportion of time standing increased in the intervention group from 14% (baseline) to 28% (T1) and 27% (T2) (67 minutes more standing over an 8-hour workday). Intervention participants reduced sitting time from 79% (BL) to 63% (T1 and T2), (80 minutes less sitting over an 8-hour workday). The control group showed no changes. The program was highly recommended (94%), and perceived to support behavior change (81%). CONCLUSIONS:This Move More, Sit Less intervention appears to be efficacious and acceptable. Future interventions should be co-designed to ensure culturally appropriate components and higher acceptability

    Self-Collected Mid-Turbinate Swabs for the Detection of Respiratory Viruses in Adults with Acute Respiratory Illnesses

    BACKGROUND: The gold standard for respiratory virus testing is a nasopharyngeal (NP) swab, which is collected by a healthcare worker. Midturbinate (MT) swabs are an alternative due to their ease of collection and possible self-collection by patients. The objective of this study was to compare the respiratory virus isolation of flocked MT swabs compared to flocked NP swabs. METHODS: Beginning in October 2008, healthy adults aged 18 to 69 years were recruited into a cohort and followed up for symptoms of influenza. They were asked to have NP and MT swabs taken as soon as possible after the onset of a fever or two or more respiratory symptoms with an acute onset. The swabs were tested for viral respiratory infections using Seeplex® RV12 multiplex PCR detection kit. Seventy six pairs of simultaneous NP and MT swabs were collected from 38 symptomatic subjects. Twenty nine (38%) of these pairs were positive by either NP or MT swabs or both. Sixty nine (91%) of the pair results were concordant. Two samples (3%) for hCV OC43/HKU1 and 1 sample (1%) for rhinovirus A/B were positive by NP but negative by MT. One sample each for hCV 229E/NL63, hCV OC43/HKU1, respiratory syncytial virus A, and influenza B were positive by MT but negative by NP. CONCLUSIONS: Flocked MT swabs are sensitive for the diagnosis of multiple respiratory viruses. Given the ease of MT collection and similar results between the two swabs, it is likely that MT swabs should be the preferred method of respiratory cell collection for outpatient studies. In light of this data, larger studies should be performed to ensure that this still holds true and data should also be collected on the patient preference of collection methods
