    "Enhancing Perinatal Outcomes In Gestational Diabetes Mellitus: A Midwifery Facilitator Based Approach"

    Introduction: Gestational diabetes mellitus, or GDM, is a metabolic disorder that affects both expectant mothers and their developing fetuses. It increases the chance of several pregnancy and delivery complications. Babies born to women with gestational diabetes mellitus (GDM) are at risk for perinatal death, fetal macrosomia, low blood sugar, birth trauma, and congenital malformations. This paper explores the possibility of enhancing perinatal outcomes in GDM by applying a midwifery facilitator-based approach. Objectives: This study's primary objective is to assess the impact of a midwife facilitator-based approach on the management of gestational diabetes mellitus (GDM) and its associated complications in expectant mothers and their babies. Our objective is to create a midwifery facilitator approach model that provides recommendations for clinical practice. Methods: A prospective cohort study involving 111 mothers compared standard care with care led by midwives. The intervention group was chosen based on their preferences, and they received full support, which included education, lifestyle modifications, and assistance for mothers. A certified diabetes educator led the classes, which covered a variety of subjects like multiple pregnancies, chronic illnesses, and absences. Group 1 comprised 62 mothers who attended classes and follow-ups, while 49 mothers in Group 2 did not attend any of them. Consistency and low turnover were assured. Results: The Midwifery Facilitator-Based Approach, which considers psychological, emotional, and physical factors, fully manages GDM. Strict diabetes management, nutritional counseling, promoting a healthy diet, and aiding in stress reduction are all made possible by these midwives. They empower mothers with GDM to take care of themselves and allow informed decision-making, potentially improving perinatal outcomes and the general well-being of GDM-affected families. Conclusion: A promising strategy for improving perinatal outcomes in GDM is the facilitator-based approach used by midwives. It offers a treatment plan that is holistic in nature, taking into account the relationships between various aspects of health. By integrating midwifery into GDM management, we can enhance mothers' empowerment and comprehension, which will ultimately benefit moms and their infants

    An Advanced Caching Solution to Cluster Storage Environment

    Clustered storage is the deployment of multiple data servers working together to improve reliability, capacity and performance. Clustering divides workloads to every storage server to control and monitor workload transfer and file access between servers without taking into account of the physical location of the file. Solid State Drives (SSD) can be considered as a more sophisticated version of a USB memory stick since the memory stick does not have any moving part associated with it and moreover, data is stored in microchips. In this paper, we give an overview of an advanced caching solution to improve IO and application performance by using flash storage in cluster storage environment. It is a cluster storage solution with two highly scalable servers with optimizations to ensure fast service failovers and deploying one or two solid state drives as the cache devices for faster and better performance. The software supports write-back caching policy where both read and write requests on hot regions of drivesare cached. With write-back, write requests to the hot regions are acknowledged immediately after it is written to the cache device and this (dirty) data will be flushed to back-end virtual drive in the background. Flushing of dirty data will be performed by the flush manager of the software under different scenarios like amount dirty data reaches a threshold, IO activity during a time interval is low etc. The solution effectively harnesses the flash storage performance potential by retaining only frequently accessed data in flash for quick retrieval.     Random amplified polymorphic DNA (RAPD) markers reveal genetic diversity in bael (Aegle marmelos Correa) genotypes of Andaman Islands, India

    The present study evaluated genetic variability of superior bael genotypes collected from different parts of Andaman Islands, India using fruit characters and random amplified polymorphic DNA (RAPD) markers. Genomic DNA extracted from leaf material using cetyl trimethyl ammonium bromide (CTAB) method was subjected to polymerase chain reaction (PCR) with 12 polymorphic primers. A total of 476 polymorphic loci were identified with mean value of 39.66 bands per primer and 63.99% polymorphism. Application of unweighted pair group method using arithmetic average cluster analysis generated three genotypic groups. ‘Bael-5’ and ‘Bael-8’ were most similar genotypes whereas ‘Bael-7’ and ‘Bael-1’ were extreme divergent. The clusters based on molecular data were not in agreement with the morphological traits in most of the cases as low level of correlation was observed between the classification methods based on fruit characteristics and RAPD markers. The bael genotypes were found to have considerable genetic variability, demonstrating the importance of RAPD markers to analyse each genotype in a collection in order to efficiently maintain the germplasm collection for genetic improvement of bael.Keywords: Aegle, Bael, random amplified polymorphic DNA (RAPD) and genetic diversity.African Journal of Biotechnology Vol. 12(42), pp. 6055-606

    STRATEGIES FOR FACIAL NERVE PROTECTION DURING PAROTID GLAND SURGERY, A PROSPECTIVE STUDY: USING THE TRAGAL POINTER, POSTERIOR BELLY OF DIGASTRIC AND MASTOID TIP AS LANDMARKS FOR FACIAL NERVE TRUNK IDENTIFICATION IN PAROTID SURGERY.

    Background: In order to decrease the likelihood of facial nerve paralysis and address postoperative issues, our goal was to share our experience with parotid surgery. Methods: A 2 year research involving 40 patients with parotid edoema was conducted. Prior to the procedure, a cautious clinical assessment, an examination of the patient's medical history, and a facial nerve evaluation were carried out. These 40 patients (24 men and 16 women) who had parotid edoema underwent parotidectomy using techniques for an antegrade strategy over a period of two years. Results: Each of the 40 patients had a superficial parotidectomy. Thirteen patients (32.5%) exhibited impairments in facial loss of nerve mobility, with seven having HB II (17.5%), four having HB Scale III (10%), and six having HB IV (15%). The majority of patients (18/30 patients) in this study showed significant recovery between four to six months following the procedure of surgery (mean time for recovery: 7 months), and patients affected had recovered in the time duration of 11 months of parotidectomy. Conclusion: To reduce the likelihood of transient facial nerve paresis, the following safety measures were used in this investigation “vertical retraction” to lower the possibility of traction injuries. Bipolar diathermy was performed after the nerve trunk was located, and surgical sutures (5/0 polyglactin) were used to create hemostasis. With a sensitivity of 67% and a specificity of 13%, FNAC remains the investigation of choice. The numerical outcomes of the many parameters that were evaluated show that they are consistent with research from previous studies and the medical literature. Recommendation: The techniques discussed in the study if employed can significantly reduce the risk of facial nerve dysfunction. Thus, while performing Parotid gland surgery the technique should be employed

    Perceived Stress, Quality of Life and Pregnancy Outcomes Among Women with Hypertensive Disorders of Pregnancy (HDP) And Role of Midwives in It: A Systematic Review

    Hypertensive disorders during pregnancy are one of the leading causes of maternal & newborn death across the world. Many of the hypertensive antenatal women often end of with admission to ICU due to severe pre-eclampsia and eclampsia, or delivering dead babies of term pregnancy, even if live baby delivered, they mostly admitted to NICU due to respiratory distress and prematurity. Hence, they need continuous educational intervention and follow up by healthcare personnel. And a midwife is the right person for this. Midwives play an important role in encouraging pregnant mothers to adhere to treatment through various education and follow up resulting in safe pregnancy outcome among these mothers. But in Indian scenario in many places the role of midwives are often ignored while managing pregnancy complications. Therefore, current paper aimed to explore the expanded role of midwives in identifying the affected quality of life and its impact on pregnancy outcome among women with HDPs. The empirical research papers of various countries were searched through the electronic databases like Pro-Quest, Embase, Research Gate, Science direct, PUBMED, MEDLINE, CINAHL-EBSCO, web of science, Google Scholar etc and the references were also searched of those primary articles. A total of 15 out of 275 studies including both RCTs, Systematic & narrative review papers were analysed. In this paper 4 relevant points were analyzed, like- perceived stress, quality of life (QOL), pregnancy outcomes and role of midwife in GHTN management in rural India. The research papers on pregnancy outcomes as well as stress and QOL secondary to gestational outcome were plenty, but very less studies were retrieved on role of midwives. Mother’s with GHTN has more stress regarding the fetal wellbeing which ultimately affects the quality of life. Interventional studies revealed reduction in stress and improved perinatal outcome secondary to some integrated midwifery management approach. More research on addressing quality of life and exploring the stressors and interventional programs by midwifes are recommended to bring solution to this problem. The result of this review can be incorporated into nursing research to empower the women with gestational hypertension to have a positive pregnancy experience

    Tonsillitis is one of the commonest infectious illness suffered by pediatric age group. It has high incidence in developing nations like India. Its recurrent attack results in hypertrophy of tonsils, difficulty in swallowing, sleep problems resulting in poor nutrition and poor school performance as well. Recurrent tonsillitis may also result in late complications like rheumatic heart diseases. Conventional systems of medicine usually advise surgical removal of tonsils to treat its recurrent infections. Ayurveda describes a similar condition, Tundikeri which have same clinical manifestations like recurrent tonsillitis and also advised its medicinal treatment. Ayurveda have ample treasure of herbal medicine which can be used in the management of recurrent tonsillitis. This article reviews various classical texts as well as recent experimental and clinical studies to find out herbal medicine useful in recurrent tonsillitis and revalidate their use to reduce necessity of tonsillectomy
