    Ten-year survival of ART restorations in permanent posterior teeth

    This study evaluated the 10-year clinical performance of high-viscosity glass-ionomer cement placed in posterior permanent teeth by means of the Atraumatic Restorative Treatment (ART) approach. One operator placed 167 single- and 107 multiple-surface restorations in 43 high-risk caries pregnant women (mean decayed teeth = 9.8 ± 5.5). Examinations were performed at 1-, 2-, and 10-year intervals according to ART criteria. In the last evaluation, the US Public Health Service (USPHS) criteria were also used. After 10 years, 129 restorations (47.1%) were evaluated and achieved a cumulative survival rate of 49.0% (SE 7.2%). The 10-year survival of single- and multiple-surface ART restorations assessed using the ART criteria were 65.2% (SE 7.3%) and 30.6% (SE 9.9%), respectively. This difference was significant (jackknife SE of difference; p < 0.05). Using the USPHS criteria, the 10-year survival of single- and multiple-surface ART restorations were 86.5% and 57.6%, respectively. The primary causes of failure were total loss (9.3%) and marginal defects (5.4%). The survival rates observed, especially for the single-surface restorations, confirm the potential of the ART approach for restoring and saving posterior permanent teeth

    Survival of atraumatic restorative treatment (ART) sealants and restorations: a meta-analysis

    The purpose of this study is to perform a systematic investigation plus meta-analysis into survival of atraumatic restorative treatment (ART) sealants and restorations using high-viscosity glass ionomers and to compare the results with those from the 2005 ART meta-analysis. Until February 2010, four databases were searched. Two hundred four publications were found, and 66 reported on ART restorations or sealant survival. Based on five exclusion criteria, two independent reviewers selected the 29 publications that accounted for the meta-analysis. Confidence intervals (CI) and or standard errors were calculated and the heterogeneity variance of the survival rates was estimated. Location (school/clinic) was an independent variable. The survival rates of single-surface and multiple-surface ART restorations in primary teeth over the first 2 years were 93% (CI, 91–94%) and 62% (CI, 51–73%), respectively; for single-surface ART restorations in permanent teeth over the first 3 and 5 years it was 85% (CI, 77–91%) and 80% (CI, 76–83%), respectively and for multiple-surface ART restorations in permanent teeth over 1 year it was 86% (CI, 59–98%). The mean annual dentine lesion incidence rate, in pits and fissures previously sealed using ART, over the first 3 years was 1%. No location effect and no differences between the 2005 and 2010 survival rates of ART restorations and sealants were observed. The short-term survival rates of single-surface ART restorations in primary and permanent teeth, and the caries-preventive effect of ART sealants were high.     Water sorption and solubility of dentin bonding agents light-cured with different light sources

    Objective: The aim of this study was to compare water sorption (WS) and solubility (WSB) of different dentin bonding agents (DBA) as regards classification and light-activation system. The null hypotheses were: (1) there is no difference among DBA with respect to water sorption and solubility; (2) there is no effect of light-curing source on water sorption and solubility of DBA. Methods: The tested materials were: three-step etch-and-rinse (ScotchBond multi-purpose and Heliobond-control groups), two-step etch-and-rinse (Excite, Adper Single Bond, Adper Single Bond 2), self-etching (Adhse) and all-in-one (Xeno III) systems. For each material, six specimens were prepared (0.8 mm x 5.5 mm) for each light-activation device: halogen lamp (control) or light-emitting diodes (LED). They were transferred to desiccators until a constant mass was obtained (m(1)), subsequently immersed in deionized water until no alteration was obtained (m(2)). Following, the specimens were reconditioned to constant mass in desiccators (m(3)). The volume (V) of each specimen was calculated (mm(3)). WS and WSB were determined using the equations (m(2) - m(3)/V) and (m(3) - m(1)/V), respectively. Data were subjected to one-way analysis of variance and Tukey test (p < 0.05). The influence of photo-activation units was analyzed by t-test. Results: Higher values of WS and WSB were observed for the two-step etch-and-rinse and all-in-one systems. The employed light-activation unit also affected the tested characteristics. Thus, the anticipated null hypotheses were rejected. Conclusions: Simplified systems were more susceptible to water adverse and they may become more prone to degradation over time regardless of the type of light-activation source.     Clinical evaluation of multisurface ART restorations Avaliação clínica de restaurações compostas ou complexas com a técnica do ART

    The aim of this study was to evaluate the performance of two different glass-ionomer cements: a high-density (Ketac Molar - ESPE) and a resin-modified cement (Fuji VIII - GC) using the Atraumatic Restorative Treatment technique to restore multisurface cavities in permanent teeth. A total of 60 ART restorations (30 with each material) were placed in schoolchildren (9-16 years of age) by two operators. After a period of 6 months, two independent examiners evaluated 59 restorations according to the criteria used in previous ART studies. Data were submitted to McNemar and Fischer tests. The success rate of the treatment was 98.3%. One restoration (Ketac Molar) was replaced by another material and was recorded as failure. The success rates of the restorations were 100% and 96.6% for Fuji VIII and Ketac Molar, respectively. There was no statistically significant difference in the restorations success between baseline and 6 months (p>0.05). In the same way, no significant differences were found between materials, cavity types or operators (p>0.05). The ART approach was highly appropriate and effective in restorations involving two or more tooth surfaces, after 6 months. The results showed a promising performance of the ART technique with both materials.<br>O objetivo deste estudo foi avaliar o comportamento de dois cimentos de ionômero de vidro: um de alta viscosidade (Ketac Molar - ESPE) e um modificado por resina (Fuji VIII - GC) em restaurações envolvendo duas ou mais superfícies dentárias, confeccionadas pela técnica do Tratamento Restaurador Atraumático. Sessenta restaurações (30 com cada material) foram inseridas em estudantes (9-16 anos) por dois operadores. Após 6 meses, dois examinadores independentes avaliaram as restaurações de acordo com os critérios utilizados em trabalhos prévios semelhantes. Os dados foram analisados pelos testes de McNemar e Fischer. A porcentagem de sucesso do tratamento foi de 98,3%. Uma restauração (Ketac Molar) foi substituída por outro material e classificada como falha. As porcentagens de sucesso das restaurações foram de 100% e 96,6% para o Fuji VIII e Ketac Molar, respectivamente. Não houve diferença estatisticamente significante no sucesso das restaurações entre o baseline e 6 meses (p>0,05). Da mesma forma, não houve diferença estatística entre os materiais, tipos de cavidade ou entre operadores.(p>0,05). A técnica ART foi altamente apropriada e efetiva em restaurações envolvendo duas ou mais superfícies, após 6 meses. Os resultados mostraram um comportamento promissor com ambos os materiais

    A wide variety of dental products that are launched on the market becomes the correct selection of these materials a difficult task. Although the mechanical properties do not necessarily represent their actual clinical performance, they are used to guide the effects of changes in their composition or processing on these properties. Also, these tests might help somehow the clinician to choose once comparisons between former formulations and new ones, as well as, with the leading brand, are highlighted by manufactures. This paper presents a review of the most important laboratory tests. In this manner, the knowledge of these tests will provide a critical opinion related to the properties of different dental materials.<br>Uma grande variedade de produtos odontológicos que são lançados no mercado faz da seleção do material uma difícil tarefa. Apesar das propriedades mecânicas não representarem necessariamente o seu real desempenho clínico, os testes são utilizados para orientar os efeitos das alterações das composições do material ou a evolução das suas propriedades. Além disso, estas propriedades podem ajudar o clínico de alguma forma na seleção correta, uma vez que a comparação entre as formulações anteriores e as mais recentes, assim como as líderes de mercado são mais destacadas pelos fabricantes. Este artigo apresenta uma revisão dos testes laboratoriais mais importantes. Desta forma, o conhecimento destes ensaios fornecerá uma opinião crítica relacionada às propriedades dos diferentes materiais dentários

    Compressive and diametral tensile strength of glass ionomer cements Resistência à compressão e à tração diametral de cimentos de ionômero de vidro

    The aim of this study was to compare, in different periods of time, the compressive and diametral tensile strength of a traditional high viscous glass ionomer cement: Fuji IX (GC Corporation), with two new Brazilian GIC's: Vitro-Molar (DFL) and Bioglass R (Biodinamica), all indicated for the Atraumatic Restorative Treatment (ART) technique. Fifteen disk specimens (6.0mm diameter x 3.0mm height) for the diametral tensile strength (DTS) test and fifteen cylindrical specimens (6.0mm diameter x 12.0mm height) for the compressive strength (CS) test were made of each GIC. Specimens were stored in deionized water at 37&ordm; C and 100% of humidity in a stove until testing. Five specimens of each GIC were submitted to CS and DTS test in each period, namely 1 hour, 24 hours and 7 days. The specimens were tested in a testing machine (Emic) at a crosshead speed of 1.0mm/min for CS and 0.5mm/min for the DTS test until failure occurred. The data were submitted to two-way ANOVA and Tukey tests (alpha=0.05). The mean CS values ranged from 42.03 to 155.47MPa and means DTS from 5.54 to 13.72 MPa, with test periods from 1h to 7 days. The CS and DTS tests showed no statistically significant difference between Fuji IX and Vitro Molar, except for CS test at 1-hour period. Bioglass R had lowest mean value for CS of the cements tested. In DTS test Bioglass R presented no statistically significant differences when compared with all others tested GICs at 1-hour period and Bioglass R presented no difference at 24-hour and 7-day periods when compared to Vitro-Molar. Further studies to investigate other physical properties such as fracture toughness and wear resistance, as well as chemical composition and biocompatibility, are now needed to better understand the properties of these new Brazilian GIC's.<br>Comparou-se a Resistência à Compressão (RC) e à Tração Diametral (TD) de um cimento de ionômero de vidro de alta viscosidade [Fuji IX (GC Corporation)] e de dois novos cimentos Brasileiros [Vitro Molar (DFL) e Bioglass R (Biodinamica)], recentemente lançados no mercado, ambos indicados para o Tratamento Restaurador Atraumático (ART), em diferentes períodos de tempo. Foram confeccionados quinze corpos-de-prova com 6,0 mm de diâmetro x 3,0 mm de altura para o teste de TD e quinze com 6,0 mm de diâmetro e 12,0 mm de altura para o teste de RC, para cada ionômero a ser testado. Os corpos-de-prova foram armazenados em recipientes plásticos, com água deionizada, e mantidos em estufa a 37&ordm;C e 100% de umidade, até a realização dos testes. Cinco corpos-de-prova de cada material foram submetidos aos testes de TD e RC em cada período de tempo: 1-hora, 24-horas e 7-dias, em uma máquina de testes universal (EMIC - DL 500) a uma velocidade de 1,0 mm/min para RC e 0,5mm/min para TD. Os dados obtidos foram submetidos aos testes ANOVA a dois critérios e Tukey (á=0,05). Os valores médios de RC e TD variaram de 42,03 a 155.47 MPa e de 5,54 a 13,72 MPa, respectivamente para os períodos analisados. O Fuji IX e o Vitro Molar não apresentaram diferenças em relação aos testes de RC e TD, exceto para RC no período de 1-hora. O Bioglass R apresentou os menores valores de RC dos cimentos testados. Na TD o Bioglass R não apresentou diferença em relação aos outros cimentos testados no período de 1-hora e não foi diferente do Vitro-Molar nos períodos de 24-horas e 7-dias. Mais estudos são necessários para avaliar outras propriedades mecânicas desses novos cimentos de ionômero de vidro brasileiros, tais como: tenacidade e desgaste, bem como composição química e biocompatibilidade
