    Pearl without Oyster; Pregnancy without Uterus!

    Is pregnancy possible after removal of the uterus? Pregnancy is virtually never thought of after hysterectomy. Although rare it is still possible for a reproductive age group woman to conceive after hysterectomy when the tubes and ovaries are left behind. A sperm enter through a fistulous tract in the vaginal vault, fertilize the ovum, implant in the tube and grow. The end result can be catastrophic, if diagnosed late. Fortunately such post-hysterectomy ruptured ectopic gestation is a very rare event; so far only 72 cases have been reported in literature. Here we report a case of ruptured tubal gestation in a 25year old patient after four years of peripartum hysterectomy, who had a successful outcome because of timely diagnosis and early treatment

    Organic acids, produced as intermediates of the tricarboxylic cycle, play a crucial role in the plant primary metabolism and are considered as being ones of the most important quality traits in edible fruits. Even if they are key metabolites in a multitude of cellular functions, little is known about their physiological relevance and regulation. Transgenic tomato (Solanum lycopersicum) plants expressing constitutively a bacterial maleate isomerase, which converts reversibly maleate to fumarate, were generated in order to improve our knowledge about the role of organic acids in the crop and fruit metabolism. Growth and reproduction were affected by the unbalance of tricarboxylic cycle intermediates, as a dwarf phenotype and a flowering delay were observed in the transgenic plants. In addition, a delay in chlorophyll synthesis, a decrease in the numbers of stomata and significant changes in some photosynthetic parameters indicated alterations in central primary metabolism. Postharvest was also impaired, as transgenic fruits showed increased water lost and deterioration, indicating a possible role of the organic acids in cell wall metabolism. Finally, preliminary metabolomics analysis pointed out important changes during fruit ripening in flavor-related metabolites, such as acids and sugars, revealing the importance of organic acids in fruit metabolism. Taken together, these data indicate a pivotal role of tricarboxylic cycle intermediates, such as malate or fumarate, as regulatory metabolites.     The cultivated strawberry (Fragaria x ananassa) is a highly consumed fruit known for its delicate flavour and nutritional characteristics. However, as fruit quality attributes have been lost after years of traditional breeding, new technological tools, such as high throughput metabolomics, are necessary for the identification of factors responsible of these traits. Here we present the metabolomics profiling for the content of primary and secondary metabolites of a 95 F1 individuals strawberry population derived from genotype “1392”, selected for its superior flavour, and “232” (Zorrilla-Fontanesi et al., 2011; Zorrilla-Fontanesi et al., 2012).     Kerr Black Holes are Not Unique to General Relativity

    Considerable attention has recently focused on gravity theories obtained by extending general relativity with additional scalar, vector, or tensor degrees of freedom. In this paper, we show that the black-hole solutions of these theories are essentially indistinguishable from those of general relativity. Thus, we conclude that a potential observational verification of the Kerr metric around an astrophysical black hole cannot, in and of itself, be used to distinguish between these theories. On the other hand, it remains true that detection of deviations from the Kerr metric will signify the need for a major change in our understanding of gravitational physics.Comment: 4 pages, Physical Review Letters in pres

    Isotope geochemistry and petrogenesis of peralkaline Middle Miocene ignimbrites from central Sonora: relationship with continental break-up and the birth of the Gulf of California

    Middle Miocene peralkaline ignimbrites constitute a specific geodynamic marker of the early stage of opening of the Gulf of California, preserved either in central Sonora or the Puertecitos area, in Baja California. Very uniform ages (12-12.5 Ma) obtained on these rocks show that this volcanic episode corresponds to a specific stage in the tectonic evolution of the proto-gulf area. Field observations and slightly different Sr and Nd isotopic signatures support eruptions from several small volume magma batches rather than from a large-volume caldera forming event. Isotopic ratios help to constrain the petrogenesis of the peralkaline liquids by fractional crystallization of transitional basalts in a shallow reservoir, with slight contamination by Precambrian upper crustal material. Less differentiated glomeroporphyritic icelandites erupted at about 11 Ma, mark an increase in the magma production rate and highlight an easier access to the surface, illustrating an advanced stage in the weakening of the continental crust. The tilting of the Middle Tertiary sequences results from a major change in the tectonic regime, from E-W extension giving rise to N-S grabens, to NNW-SSE strike-slip motion that can be related to the transfer of Baja California from North America to the Pacific plate. The location of peralkaline volcanism coincides with the southern edge of the Precambrian crust and the southernmost extension of the California slab window at 12.5 Ma

    Evaluating diverse electronic consultation programs with a common framework.

    BackgroundElectronic consultation is an emerging mode of specialty care delivery that allows primary care providers and their patients to obtain specialist expertise without an in-person visit. While studies of individual programs have demonstrated benefits related to timely access to specialty care, electronic consultation programs have not achieved widespread use in the United States. The lack of common evaluation metrics across health systems and concerns related to the generalizability of existing evaluation efforts may be hampering further growth. We sought to identify gaps in knowledge related to the implementation of electronic consultation programs and develop a set of shared evaluation measures to promote further diffusion.MethodsUsing a case study approach, we apply the Reach, Effectiveness, Adoption, Implementation and Maintenance (RE-AIM) and the Quadruple Aim frameworks of evaluation to examine electronic consultation implementation across diverse delivery systems. Data are from 4 early adopter healthcare delivery systems (San Francisco Health Network, Mayo Clinic, Veterans Administration, Champlain Local Health Integration Network) that represent varied organizational structures, care for different patient populations, and have well-established multi-specialty electronic consultation programs. Data sources include published and unpublished quantitative data from each electronic consultation database and qualitative data from systems' end-users.ResultsOrganizational drivers of electronic consultation implementation were similar across the systems (challenges with timely and/or efficient access to specialty care), though unique system-level facilitators and barriers influenced reach, adoption and design. Effectiveness of implementation was consistent, with improved patient access to timely, perceived high-quality specialty expertise with few negative consequences, garnering high satisfaction among end-users. Data about patient-specific clinical outcomes are lacking, as are policies that provide guidance on the legal implications of electronic consultation and ideal remuneration strategies.ConclusionA core set of effectiveness and implementation metrics rooted in the Quadruple Aim may promote data-driven improvements and further diffusion of successful electronic consultation programs

    Can groundwater secure drinking-water supply and supplementary irrigation in new settlements of North-West Cambodia ?

    Since the end of the Cambodian Civil War in 1998, the population of the Oddar Meanchey province has drastically increased despite the lack of adequate infrastructure, including basic amenities such as drinking-water supply. To improve the access to drinking water, governmental and aid agencies have focussed on drilling shallow boreholes. The use of groundwater for irrigation is also a growing concern to cope with the occasional late arrival of the rainy season or to produce food during the dry season. Since the groundwater resource in the province has not been documented, a 4-year study was undertaken (2011-2014), aiming to estimate the capability of groundwater to supply domestic needs and supplementary irrigation for rice production. Aquifer properties were estimated by combined use of hydrogeological techniques with the geophysical magnetic resonance sounding method. Groundwater storage and recharge were estimated based on new developments in the application of the geophysical method for quantifying specific yield. The median groundwater storage of the targeted sandstone aquifer is 173 mm, the recharge is diffuse and annually ranges from 10 to 70 mm, and the transmissivity is low to medium. Simulations of pumping indicate that the aquifer can easily supply 100 L of drinking water per capita daily, even considering the estimated population in 2030. However, the shallow aquifer can generally not deliver enough water to irrigate paddy fields of several hectares during a 2-month delay in the onset of the monsoon
