    Prática Musical Coletiva no isolamento social: relato de experiência do programa de extensão Engenho Musical - Udesc

    Relato de experiência sobre a manutenção das atividades do programa de extensão Engenho Musical (Departamento de Música e Pró-Reitoria de Extensão da UDESC/CEART) durante o período de isolamento/distanciamento social e a consequente suspensão das atividades coletivas presenciais devido à pandemia da Covid-19, referente ao período de março de 2020 à agosto de 2021. A metodologia utilizada contemplou um levantamento bibliográfico de artigos e trabalhos acadêmicos sobre os assuntos referentes à pandemia da Covid-19 e grupos musicais, seguido de discussão crítica sobre a vivência da atividade musical coletiva durante o período citado e a aplicação de uma pesquisa Quantitativa-Qualitativa (questionário com perguntas objetivas e com perguntas abertas dirigido aos integrantes dos grupos musicais do referido programa que se mantiveram atuantes durante a pandemia). Os trabalhos encontrados permitiram discutir o impacto da pandemia nas práticas coletivas musicais e as adaptações que foram necessárias para a manutenção das atividades, possibilitando uma confluência com as ações adotadas no programa Engenho Musical e os relatos de seus integrantes. Os resultados obtidos nesta pesquisa foram satisfatórios quanto à permanência dos integrantes no programa realizando as atividades propostas (reuniões online, gravação de vídeos mosaicos, ensaio coletivo no programa Jamulus, divulgação nas mídias sociais, lives, podcast, composição de obras), mas a dificuldade de acesso aos equipamentos e condições tecnológicas adequadas, aquisição de novas habilidades no uso da tecnologia, mudança de horário de trabalho, exaustão e demais problemas físicos/mentais relatados na pesquisa apontam um ônus indesejável à vida no modo virtual. Foi construído um espaço temporário-emergencial de interação musical e social, numa das poucas possibilidades de prática musical coletiva. A pesquisa apontou impactos importantes nos vários setores da classe artística-musical decorrentes das medidas de contenção do vírus durante a pandemia da Covid-19, ainda sem previsão de retorno presencial totalmente seguro.Experience report about the maintenance of the activities bound to the  Engenho Musical extension program (Music Department and Pro Rectory of Extension from Udesc/Ceart) during the period of social isolation/distancing and the consequent suspension of in-person collective activities due to the Covid-19 pandemic, referring to the period between March 2020 to August 2021. The utilized methodology contemplated a bibliographic survey of academic articles and works about the subjects referring to the Covid-19 pandemic and musical groups, followed by a critical discussion about the collective musical activity experiences during the mentioned period and the application of a quantitative-qualitative research(questionnaire with objective questions and open-ended questions directed to the integrants of the referred program that maintained activity during the pandemic). The collected works allowed a discussion of the impacts of the pandemic on the collective musical practices and the necessary adaptations to the maintenance of the activities, enabling a confluence with the actions adopted in the Engenho Musical extension program and its integrants reports

    Mindfulness based interventions in multiple sclerosis: a systematic review

    <b>Background</b> Multiple sclerosis (MS) is a stressful condition; depression, anxiety, pain and fatigue are all common problems. Mindfulness based interventions (MBIs) mitigate stress and prevent relapse in depression and are increasingly being used in healthcare. However, there are currently no systematic reviews of MBIs in people with MS. This review aims to evaluate the effectiveness of MBIs in people with MS.<p></p> <b>Methods</b> Systematic searches were carried out in seven major databases, using both subject headings and key words. Papers were screened, data extracted, quality appraised, and analysed by two reviewers independently, using predefined criteria. Study quality was assessed using the Cochrane Collaboration risk of bias tool. Perceived stress was the primary outcome. Secondary outcomes include mental health, physical health, quality of life, and health service utilisation. Statistical meta-analysis was not possible. Disagreements were adjudicated by a third party reviewer.<p></p> <b>Results</b> Three studies (n = 183 participants) were included in the final analysis. The studies were undertaken in Wales (n = 16, randomised controlled trial - (RCT)), Switzerland (n = 150, RCT), and the United States (n = 17, controlled trial). 146 (80%) participants were female; mean age (SD) was 48.6 (9.4) years. Relapsing remitting MS was the main diagnostic category (n = 123, 67%); 43 (26%) had secondary progressive disease; and the remainder were unspecified. MBIs lasted 6–8 weeks; attrition rates were variable (5-43%); all employed pre- post- measures; two had longer follow up; one at 3, and one at 6 months. Socio-economic status of participants was not made explicit; health service utilisation and costs were not reported. No study reported on perceived stress. All studies reported quality of life (QOL), mental health (anxiety and depression), physical (fatigue, standing balance, pain), and psychosocial measures. Statistically significant beneficial effects relating to QOL, mental health, and selected physical health measures were sustained at 3- and 6- month follow up.<p></p> <b>Conclusion</b> From the limited data available, MBIs may benefit some MS patients in terms of QOL, mental health, and some physical health measures. Further studies are needed to clarify how MBIs might best serve the MS population.<p></p&gt

    Are German patients burdened by the practice charge for physician visits ('Praxisgebuehr')? A cross sectional analysis of socio-economic and health related factors

    <p>Abstract</p> <p>Background</p> <p>In 2004, a practice charge for physician visits ('Praxisgebuehr') was implemented in the German health care system, mainly in order to reduce expenditures of sickness funds by reducing outpatient physician visits. In the statutory sickness funds, all adults now have to pay € 10 at their first physician visit in each 3 month period, except for vaccinations and preventive services. This study looks at the effect of this new patient fee on delaying or avoiding physician visits, with a special emphasis on different income groups.</p> <p>Methods</p> <p>Six representative surveys (conducted between 2004 and 2006) of the Bertelsmann Healthcare Monitor were analysed, comprising 7,769 women and men aged 18 to 79 years. The analyses are based on stratified analyses and logistic regression models, including a focus on the subgroup having a chronic disease.</p> <p>Results</p> <p>Two results can be highlighted. First, avoiding or delaying a physician visit due to this fee is seen most often among younger and healthier adults. Second, those in the lowest income group are much more affected in this way than the better of. The multivariate analysis in the subgroup of respondents having a chronic disease shows, for example, that this reaction is reported 2.45 times more often in the lowest income group than in the highest income group (95% CI: 1.90–3.15).</p> <p>Conclusion</p> <p>The analyses indicate that the effects of the practice charge differ by socio-economic group. It would be important to assess these effects in more detail, especially the effects on health care quality and health outcomes. It can be assumed, however, that avoiding or delaying physician visits jeopardizes both, and that health inequalities are increasing due to the practice charge.</p

    Currents issues in cardiorespiratory care of patients with post-polio syndrome

    ABSTRACT Post-polio syndrome (PPS) is a condition that affects polio survivors years after recovery from an initial acute attack of the poliomyelitis virus. Most often, polio survivors experience a gradual new weakening in muscles that were previously affected by the polio infection. The actual incidence of cardiovascular diseases (CVDs) in individuals suffering from PPS is not known. However, there is a reason to suspect that individuals with PPS might be at increased risk. Method A search for papers was made in the databases Bireme, Scielo and Pubmed with the following keywords: post polio syndrome, cardiorespiratory and rehabilitation in English, French and Spanish languages. Although we targeted only seek current studies on the topic in question, only the relevant (double-blind, randomized-controlled and consensus articles) were considered. Results and Discussion Certain features of PPS such as generalized fatigue, generalized and specific muscle weakness, joint and/or muscle pain may result in physical inactivity deconditioning obesity and dyslipidemia. Respiratory difficulties are common and may result in hypoxemia. Conclusion Only when evaluated and treated promptly, somE patients can obtain the full benefits of the use of respiratory muscles aids as far as quality of life is concerned
